Diffuse Otitis Externa: Clinical and Microbiologic Findings in the Course of a Multicenter Study on a New Otic Solution

This report reviews the literature on the epidemiology, pathogenesis and bacteriology of diffuse otitis externa. Four departments of otolaryngology undertook identical studies of these factors, as well as the efficacy and safety of two similar antibiotic-corticosteroid formulations; one a suspension and the other a clear solution. A total of 239 patients, or 283 ears, were studied, utilizing objective clinical as well as correlative bacteriologic criteria. A high degree of clinical and bacteriologic efficacy was demonstrated by the medications irrespective of the formulation, infecting organism, patient's age, length of disease history, severity, or geographic location. There was no statistical difference between either formulation, both achieving a clinical efficacy rate of 97% and a bacteriologic efficacy rate of 83%. Drug related side effects occurred in 1.9% of the patients given the solution and in 1.1% of those given the suspension.

Objective.To report the efficacy and safety of tocilizumab (TCZ) for giant cell arteritis (GCA).Methods.A retrospective multicenter study that included 34 patients receiving TCZ for GCA.Results.TCZ was effective in all but 6 patients, who still had mild symptoms. Mean glucocorticoid dose was tapered. One patient died and 3 patients had to stop TCZ therapy because of severe adverse events. Twenty-three patients stopped treatment; 8 of these experienced relapses after a mean of 3.5 ± 1.3 months.Conclusion.TCZ is effective in GCA. However, side effects occur. Whether this treatment has only a suspensive effect remains to be determined.

Summary Anti-vaccination sentiment exists worldwide and Japan is no exception. Health professionals publish pro-influenza vaccination messages online to encourage proactive seeking of influenza vaccination. However, influenza vaccine coverage among the Japanese population is less than optimal. The contents of pro- and anti-influenza vaccination websites may contribute to readers’ acceptance of one or the other position. We aimed to use a text-mining method to examine frequently appearing content on websites for and against influenza vaccination. We conducted online searches in January 2017 using two major Japanese search engines (Google Japan and Yahoo! Japan). Targeted websites were classified as ‘pro’, ‘anti’ or ‘neutral’ depending on their claims, with author(s) classified as ‘health professionals’, ‘mass media’ or ‘laypersons’. Text-mining analysis was conducted, and statistical analysis was performed using a chi-squared test. Of the 334 websites analyzed, 13 content topics were identified. The three most frequently appearing content topics on pro-vaccination websites were vaccination effect for preventing serious cases of influenza, side effects of vaccination, and efficacy rate of vaccination. The three most frequent topics on anti-vaccination websites were ineffectiveness of influenza vaccination, toxicity of vaccination, and side effects of vaccination. The main disseminators of each topic, by author classification, were also revealed. We discuss possible tactics of online influenza vaccination promotion to counter anti-vaccination websites.

Background: Acne vulgaris is characterised by comedones, papules, pustules and nodules occurring in a sebaceous distribution. Topical treatments, such as adapalene and benzoyl peroxide, are popular in mild to moderate acne vulgaris. This study was aimed to compare the efficacy and safety of adapalene with benzoyl peroxide in the patients of mild to moderate acne vulgaris.Methods: We planned a randomized, open-labelled, prospective study to compare the efficacy and side effects of adapalene and benzoyl peroxide in acne patients. A total of 100 patients with mild to moderate acne vulgaris were included in the study. They were randomly divided into 2 groups with 50 patients in each group. One group was given 0.1% adapalene gel and the other group received 2.5% benzoyl peroxide gel. Efficacy was assessed as reduction in the lesion counts, whereas for safety, the side effects like dryness, burning, irritation, erythema were recorded during the treatment. Total duration of the study was 3 months.Results: The study revealed that adapalene was more effective than benzoyl peroxide in treating non-inflammatory and inflammatory lesions of acne vulgaris, and there was a statistically significant difference found between the groups (p≤0.05) in efficacy. Adapalene was also found to be comparatively safer than benzoyl peroxide because the patients treated with adapalene had lesser side effects than those treated with benzoyl peroxide.Conclusions: Our study concludes a better efficacy and safety of adapalene than benzoyl peroxide in the treatment of mild to moderate acne vulgaris.

Implantable contraception, despite some side effects and contraindications, has shown itself not only as a prolonged method with high efficacy and safety, but also as a method with a high degree of compliance. This suggests that this method of protection is easy to use for any patient, which means that its potential for contraceptive reliability will be realized to the highest degree.

BACKGROUND: keloid is a benign overgrowth of brotic tissue and are aesthetically unacceptable to the patients. Management is difcult and pose a challenge for physicians. This study compares the efcacy and safety of intralesional triamcinolone acetonide, intralesional 5-uorouracil and combination of both in management of keloids. METHOD: Total 60 patients, randomly divided into three groups: A, B, C were subjected to intralesional Triamcinolone acetonide, 5- Fluorouracil and their combination respectively, every 3weeks for 18 weeks and assessed with Vancouver Scoring Scale. RESULTS:Improvement seen in all parameters, maximum with combination of triamcinolone and 5-uorouracil. Major side effects were hypopigmentation with intralesional triamcinolone; pain and hyperpigmentation with intralesional 5- uorouracil and mild pain with combination of two. CONCLUSION: Combination of 5-uorouracil and triamcinolone acetonide seemed to be more effective with lesser side effects than the other two.

Vitiligo, an autoimmune disease, is a depigmentation skin disorder characterized by hypopigmentation spots and patches that are difficult to treat. The purpose of this study is to analyze the clinical efficacy of simvastatin in the treatment of vitiligo. From December 2016 to October 2019, five vitiligo patients from Peking Union Medical College Hospital were treated with simvastatin and tacrolimus. For simvastatin, the first three patients (patient 1, patient 2, and patient 3) began to take 40 mg/day, and the latter two patients began to take 20 mg/day. From week 5, patients 1 and 2 received 20 mg simvastatin once a day. And the 0.1% tacrolimus ointment was used topically. The response was measured using the Vitiligo European Task Force (VETF) scoring system at baseline and at the fourth and eighth weeks of treatment. The results of the VETF score showed that three of the patients achieved significant clinical efficacy. There was no significant improvement in the other two cases. No serious acute or chronic side effects were observed. In conclusion, our results suggest that conventional oral simvastatin is safe, although it may not be effective in the treatment of vitiligo.

Abstract Abstract 4651 Romiplostim and eltrombopag, the first thrombopoietic receptor-agonists (TPO-RAs) with demonstrated efficacy against immune thrombocytopenia (ITP) in prospective controlled studies, were recently authorized in Europe for adults with chronic ITP. Their efficacies and safety profiles were evaluated in 46 ITP patients who sequentially received both drugs. Thirty-five patients first received romiplostim (romiplostim group), whereas eltrombopag (eltrombopag group) was the first TPO-RA for 11 patients. All but 2 patients had previously received corticosteroids and IVIg, and 70% had been given rituximab without responding. The splenectomy rate was 50% (23/46). Patients were switching from one to the other for the following reasons: lack of efficacy for half of them, platelet-count fluctuations for 11, side effects for 4 and 8 patients' preferences. For 50% to 80% of the patients, switching from 1 TPO-RA to the other effectively impacted the platelet count, with the disappearance of its fluctuations in 54%, and side effects resolved in 100%. In 80% of the patients, the 2 TPO-RAs achieved similar response patterns. Our results confirmed that switching from 1 TPO-RA to the other could be beneficial in clinical practice for patients with severe chronic ITP who failed to respond or experienced adverse events(s) to the first. Background: Glucocorticoids exert a wide range of physiological effects. They  effectively control various inflammatory and autoimmune diseases and play an important role in organ transplantation. Glucocorticoids are associated with unfavorable side effects that restrict their utilization. The most undesirable side effects are related to the transactivation of target genes. On the other hand, the transrepression of the genes is also responsible for the anti-inflammatory and immunomodulator of glucocorticoids. Principal Findings: The separation between these two processes through alteration in glucocorticoid receptor resulting in a compound with similar GCs benefit activity with fewer side effects. This review will discuss the molecular mechanism and summarizes the most common compounds and their beneficial effect in preclinical experiments. Conclusion: Several compounds that possess this feature are tested in preclinical experiments with promising results. These compounds are expected to be a better alternative to GCs drugs in the management of different diseases with a high degree of effectiveness.

The article presents the results of studies of efficacy and safety of androgen therapy for hypogonadism, metabolic syndrome and type 2diabetes mellitus in men. In the study was used testosterone undecanoate therapy which causes reduction of obesity, a decrease in theseverity of the other components of the metabolic syndrome, improving glycemic profile without significant side effects. Nevertheless,this therapy requires monitoring and management for risk factors.

The paper outlines the author's experience on the efficacy and safety of androgen therapy of metabolic syndrome and obesity in men with hypogonadism. The study used testosterone undecanoate therapy that reduced body fat and the severity of the other components of the metabolic syndrome, improved sexual function without causing severe side effects. A number of patients on therapy observed suppression of spermatogenesis, which prevents the use of the drug in the reproductive rehabilitation.
