Suicide is the leading cause of death among offenders in correctional facilities in many parts of the world. While epidemiological data have described the extent and scope of the problem, no research has documented the experiences and insights of health professionals who are tasked with providing care to suicidal offenders in South African correctional centres. It is within this context that we set out to document the experiences of a group of health professionals working in two correctional facilities in South Africa. We were interested in learning from them about their perceptions of the factors that contribute to suicidal behaviour among offenders and their suggestions for suicide prevention. In-depth semi-structured interviews were conducted with a group of health professionals ( N = 10). Data were analysed using thematic analysis and an inductive approach was adopted for coding the data. The participants describe a number of structural and contextual factors which they believe make correctional centres in South Africa unsafe, thus increasing the risk of suicidal behaviour among offenders and hindering suicide prevention efforts within the two correctional centres where data were collected. These factors include high levels of psychopathology, trauma, substance abuse, violence, gangsterism, overcrowding, inadequate mental health care resources, and problems relating to stigma. These findings highlight human rights and mental health issues facing offenders and draw attention to conditions within correctional centres that need attention as part of any national suicide prevention strategy in South Africa.

Youth suicidal behaviour poses a significant public health concern. Mental health care professionals working in schools have an important role to play in youth suicide prevention initiatives, although little is known of the experiences of this group of professionals in low and middle income countries (LMIC’s). The aim of this study was to explore the experiences of mental health professionals working in South African schools and to document their insights, attitudes and beliefs regarding youth suicidal behaviour. In-depth semi-structured interviews were conducted with seven school-based mental health care professionals and data were analysed using Thematic Analysis. Participants reported that they relied on a reactive strategy by responding to youths who were in crisis. They were challenged by a lack of support from faculty staff, a lack of access to resources, and heavy caseloads. Findings highlight the need for a proactive and collaborative approach to suicide prevention among mental health care professionals, teachers and parents in South African schools and improved training and supervision.

The literature on sexual abuse indicates low rates of inquiry by mental health professionals. This study explores early career psychologists’ experiences of inquiry into their clients’ sexual abuse histories. Twelve Australian psychologists participated in semi-structured interviews with transcripts analyzed using thematic analysis. The vast majority of participants reported that they did not routinely inquire about sexual abuse with barriers including not knowing what to do, discomfort, stigma, and fear of negative outcomes. Participants asserted that their university training in sexual abuse inquiry was inadequate. Findings emphasize the need for the development of an evidence-based framework for sexual abuse training.

Orientation: Understanding the nature and challenges of making the transition from a functional role to a general management role in South African organisations.Research purpose: The objective of this study was to gain insight into the obstacles that affect the transition from functional to general management and identify steps that may be taken to overcome these challenges.Motivation for the study: One of the most difficult crossroads for a manager is making the shift from being a functional specialist to becoming a general manager. New competencies and behaviours are required, as well as a more strategic mind set. If the transition is not made successfully, the manager and the organisation suffer.Research design, approach and method: A qualitative design was used consisting of in-depth, semi-structured interviews, with 19 senior business leaders who had successfully made the transition. The interviews were used to gather insights into the challenges they faced during their transitions, and how these were overcome.Main findings: To make the transition successfully, functional managers need to gain relevant experience to prepare them for the broader scope of a general management role. They need to develop appropriate skills, attitudes and personal characteristics. Mentoring is an effective development process. Newly appointed general managers need to learn to let go of control while maintaining ownership, build relationships and strike the right balance between strategic thinking and execution. There are unique aspects of being a general manager in South Africa, such as dealing with Black Economic Empowerment and challenges of race and identity, given the country’s history.Practical and managerial implications: Specific interventions are suggested which are directed at both aspiring general managers and organisations seeking to assist middle managers to make the transition to general managers.Contribution: This study contributes to knowledge concerning the skills and attributes required by potential general managers, and the practical steps to be taken by South African organisations to facilitate the development of general managers.

PurposeThe study aims to deepen the understanding of why risk management principles are circumvented, thereby contributing to transgressions in public procurement for South African state-owned enterprises (SOEs). A deeper understanding of why risk management principles are circumvented is especially important in South Africa, given the high social, economic and environmental risks to which national and major SOEs are exposed in the procurement process.Design/methodology/approachThe study uses a qualitative design, based on detailed semi-structured interviews with 19 participants comprising management advisors, forensic investigators and auditors to explore why risk management principles are circumvented by South Africa SOEs.FindingsThe results of the study indicate that the tone that is set at political and executive level plays an important role in determining compliance with risk management principles by lower-level staff. Intense levels of political influence at SOEs are the main reason behind risk management systems being undermined.Originality/valueThe current study is one of the first explorations of why transgressions in public procurement continue to be evident despite risk management reforms being adopted by South Africa public sector. The research responds to the call for more studies on why reforms in South Africa public sector are not reducing transgression in public procurement. The study provides primary evidence on the importance of political and executive leadership in influencing the effectiveness of risk management reforms in the public sector.

Purpose The purpose of this paper is to describe the barriers and facilitators to an effective transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS). It also presents a new entry into considering how the transition can be improved. Design/methodology/approach Insights into the transition from CAMHS to AMHS were gathered through eight semi-structured interviews with mental health professionals. Two methods of data analysis were employed to explore the emerging themes in the data and the observed deficit approach to organisational development. Findings The findings identified a vast volume of barriers in comparison to facilitators to the transition. Adolescents who transition from CAMHS to AMHS initially experience difficulty adapting to the differences in the services due to the short duration of the transition period. However, despite the established barriers to the transition, adolescents tend to adapt to the differences between the services. Findings also showed a negative framing towards the transition amongst the mental health professionals which resembles a deficit approach to organisational development. Originality/value This paper explores mental health professionals’ perspectives regarding the transition in Dumfries and Galloway, Scotland. The transition is increasingly recognised as an area in health care that requires improvement. This research provides a new way to consider the transition by exploring the perceived deficit approach to organisational development in the services.

Close scrutiny of legal precedents and psychological literature has revealed significant differences in the views of legal and mental health professionals regarding the major criteria used in custody decisions. This article carries the investigation further and considers empirically the responses to the criteria of these two groups of professionals in South Africa. Findings show subtle but significant differences between them, particularly with regard to the ‘child’ dimension of the ‘best interests’ concept. The implications for mental health professionals in their evaluation of custody cases, and in their giving of testimony, are underscored.

BackgroundAlthough GPs provide care to many patients with severe and persistent mental illness, the role and skillset of the GP in this space are contested. Patients are less satisfied with GP care of mental health than physical health issues.AimTo explore patient expectations and experiences of GP roles in their mental health, and identify opportunities for improving mental health care in general practice.Design & settingPatient participants were recruited from community mental health clinics in Brisbane, Australia.MethodIndividual semi-structured interviews were conducted with a convenience sample of patients. Interviews were audio-recorded and transcribed professionally. The authors conducted an inductive thematic analysis, attending to participant vulnerability and reflexivity.Results16 interviews were conducted by one author (RW), average duration 29 minutes. Three overarching themes were identified: being heard; being known; and being safe. Participants greatly valued ‘good GPs’ who were able to detect early signs of relapse, and with whom they came to feel heard, known and safe over time. Experiences of perfunctory, hurried care and avoidance of mental health issues were also reported. Many participants were uncertain whether GP training in mental health was sufficient to keep them safe. Patients may suspect negative attitudes to mental illness in GPs who actively engage predominantly with their physical health.ConclusionSome GPs play central roles in patients’ mental healthcare. Barriers for others need further exploration, and may include time, confidence and/or expertise. Findings challenge GPs to engage more actively and effectively with these patients in theirgeneral practice consultations.

Inconsistent political realities are associated with mental health issues such as hopelessness, anxiety, and depression. The psychological impact of Brexit is clearly an important and timely issue, but hitherto has been understudied. This study uses a critical realist approach to qualitatively explore the lived experiences of British Citizens living in Luxembourg during the Brexit era. The study carried out semi-structured interviews with 8 British nationals aged 18-65. An experientially-focused Thematic Analysis was conducted, exploring two main themes: Loss (with psychological and broader social implications) and Integration (contrasting the mover's community with the receiving community). This study demonstrates the psychological impact of Brexit and highlights the urgency for future researchers and mental health practitioners alike – both in the UK and overseas – to consider the human consequences associated with political upheaval. ABSTRACT Physicians are not immune to changes that accompany aging, including decreases in physical and cognitive health and abilities. Many are calling for, or turning to, screening senior physicians for decrements in their ability to provide safe care. Our purpose was to determine the acceptability and feasibility of a pilot screening process, which evaluates the physical, mental and cognitive health of senior physicians. The screening process was developed by the University of California, San Diego, Physician Assessment and Clinical Education Program. The screen included: mental health screening (PHQ-9, GAD-7, and substance abuse screen), cognitive health screening (MicroCog™ and Montreal Cognitive Assessment [MoCA©]) and physical health screening (medical history review and physical examination). Qualitative semi-structured interviews were conducted post-screening. Thirty senior physicians participated in the pilot process, including post-screening interviews. Eight (27%) participants were judged to “require”/“may require” further evaluation after cognitive assessment. No physicians were found to have physical or mental health issues that would prevent them from practicing competently. Interviews revealed that participants felt the screening process was a positive experience that was effective, acceptable, efficient and relevant to their practice. The results of this pilot study indicate that screening physical, mental and cognitive health is considered both feasible and acceptable to senior physicians. This is important as screening the health and cognition of senior physicians is integral to the national discussion related to regulation and patient safety.

IntroductionSelf-harm and suicidal behaviour represent major global health problems, which account for significant proportions of the disease burden in low-income and middle-income countries, including Ghana. This review aims to synthesise the available and accessible evidence on prevalence estimates, correlates, risk and protective factors, the commonly reported methods and reasons for self-harm and suicidal behaviour in Ghana.Methods and analysisWe will conduct a systematic review reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement (2009) recommendations. Regional and global electronic databases (African Journals OnLine, African Index Medicus, APA PsycINFO, Global Health, MEDLINE and PubMed) will be searched systematically up to December 2021 for observational studies and qualitative studies that have reported prevalence estimates, correlates, risk and protective factors, methods and reasons for self-harm and suicidal behaviour in Ghana. The electronic database searches will be supplemented with reference harvesting and grey literature searching in Google Scholar and ProQuest Dissertations & Theses Global for postgraduate dissertations. Only records in English will be included. The Mixed Methods Appraisal Tool (2018) will be used to assess the methodological quality of included studies. Meta-analysis or narrative synthesis or both will be used, contingent on the extent of heterogeneity across eligible observational studies.Ethics and disseminationConsidering that this is a systematic review of accessible and available literature, we will not seek ethical approval. On completion, this review will be submitted to a peer-reviewed journal, be disseminated publicly at (mental) health conferences with focus on self-harm and suicide prevention. The important findings would also be shared with key national stakeholder groups in Ghana: Ghana Association for Suicide Prevention, Ghana Mental Health Authority, Ghana Psychological Association, Centre for Suicide and Violence Research, Accra and the Parliamentary Select Committee on Health.Prospero registration numberCRD42021234622.
