Objective Wearable activity trackers hold potential as a research tool to increase physical activity. However, long-term wearable adherence is low among users, which may be due to the limited understanding of the factors related to use and relation to health behavior theory. The purpose of this study was to qualitatively explore the perceptions of wearables among active adult users and non-users. Findings will inform potential barriers and facilitators for the adherence and adoption of wearables through the application of the Self-Determination Theory. Methods Six focus groups were conducted and equally stratified to wearable users ( n = 10) and non-users ( n = 10). Data were audio recorded, transcribed, and analyzed using an iterative approach creating first-level codes. This was followed by developing second-level codes that allowed for generating themes. Results For users, the wearables’ feedback provided them with validation, a sense of achievement, and other-determined motivation. Users appreciated the functionality of wearables, particularly with simpler and newer models. They also reported improvements in health behaviors . While both users and non-users had a general positive feeling towards wearables, they held similar concerns about cost, guilt, dependency, and accuracy. Non-users were unique in their concerns for materialism and functionality (i.e. ease of use and charge) associated with wearables. They also seemed to be more intrinsically motivated to be physically active by relying less on external sources of motivation as potentially provided by wearables. Conclusions Findings show that while both adult users and non-users held positive perceptions of wearables and concerns for feelings of guilt and dependency, widespread adoption and adherence may be prevented by differences in motivation for physical activity and concerns for cost, materialism, and functionality.

Wearable activity trackers (wearables) embed numerous behaviour change techniques (BCTs) that have previously been shown to increase adult physical activity (PA). With few children and adolescents achieving PA guidelines, it is crucial to explore ways to increase their PA. This systematic review examined the acceptability, feasibility, and effectiveness of wearables and their potential mechanisms of action for increasing PA in 5 to 19-year-olds. A systematic search of six databases was conducted, including data from the start date of each database to December 2019 (PROSPERO registration: CRD42020164506). Thirty-three studies were included. Most studies (70%) included only adolescents (10 to 19 years). There was some—but largely mixed—evidence that wearables increase steps and moderate-to-vigorous-intensity PA and reduce sedentary behaviour. There were no apparent differences in effectiveness based on the number of BCTs used and between studies using a wearable alone or as part of a multi-component intervention. Qualitative findings suggested wearables increased motivation to be physically active via self-monitoring, goal setting, feedback, and competition. However, children and adolescents reported technical difficulties and a novelty effect when using wearables, which may impact wearables’ long-term use. More rigorous and long-term studies investigating the acceptability, feasibility, and effectiveness of wearables in 5 to 19-year-olds are warranted.

BACKGROUND Wearable activity trackers offer the opportunity to increase physical activity through continuous monitoring. Viewing tracker use as a beneficial health behavior, we explored the factors that facilitate and hinder long-term activity tracker use, applying the transtheoretical model of behavior change with the focus on the maintenance stage and relapse. OBJECTIVE The aim of this study was to investigate older adults’ perceptions and uses of activity trackers at different points of use: from nonuse and short-term use to long-term use and abandoned use to determine the factors to maintain tracker use and prevent users from discontinuing tracker usage. METHODS Data for the research come from 10 focus groups. Of them, 4 focus groups included participants who had never used activity trackers (n=17). These focus groups included an activity tracker trial. The other 6 focus groups (without the activity tracker trial) were conducted with short-term (n=9), long-term (n=11), and former tracker users (n=11; 2 focus groups per user type). RESULTS The results revealed that older adults in different tracker use stages liked and wished for different tracker features, with long-term users (users in the maintenance stage) being the most diverse and sophisticated users of the technology. Long-term users had developed a habit of tracker use whereas other participants made an effort to employ various encouragement strategies to ensure behavior maintenance. Social support through collaboration was the primary motivator for long-term users to maintain activity tracker use. Short-term and former users focused on competition, and nonusers engaged in vicarious tracker use experiences. Former users, or those who relapsed by abandoning their trackers, indicated that activity tracker use was fueled by curiosity in quantifying daily physical activity rather than the desire to increase physical activity. Long-term users saw a greater range of pros in activity tracker use whereas others focused on the cons of this behavior. CONCLUSIONS The results suggest that activity trackers may be an effective technology to encourage physical activity among older adults, especially those who have never tried it. However, initial positive response to tracker use does not guarantee tracker use maintenance. Maintenance depends on recognizing the long-term benefits of tracker use, social support, and internal motivation. Nonadoption and relapse may occur because of technology’s limitations and gaining awareness of one’s physical activity without changing the physical activity level itself.

Wearable activity trackers can motivate older adults to engage in the recommended daily amount of physical activity (PA). However, individuals may not maintain their use of the trackers over a longer period. To investigate the attitudes of activity tracker adoption and their effects on actual PA performance, we conducted a three-month study. We gave activity trackers to 16 older adults and assessed attitudes on activity tracker adoption through a survey during the study period. We extracted participants’ PA measures, step counts, and moderate and vigorous physical activity (MVPA) times. We observed significant differences in adoption attitudes during the three different periods (χ2(2, 48) = 6.27, p < 0.05), and PA measures followed similar decreasing patterns (F(83, 1357) = 12.56, 13.94, p < 0.00001). However, the Pearson correlation analysis (r = 0.268, p = 0.284) and a Bland–Altman plot indicated a bias between two PA measures. Positive attitudes at the initial stage did not persist through the study period, and both step counts and length of MVPA time showed waning patterns in the study period. The longitudinal results from both measures demonstrated the patterns of old adults’ long-term use and adoption. Considering the accuracy of the activity tracker and older adults’ athletic ability, MVPA times are more likely to be a reliable measure of older adults’ long-term use and successful adoption of activity trackers than step counts. The results support the development of better activity tracker design guidelines that would facilitate long-term adoption among older adults.

Background Wearable activity trackers and social media have been identified as having the potential to increase physical activity among adolescents, yet little is known about the perceived ease of use and perceived usefulness of the technology by adolescents. Objective The aim of this study was to use the technology acceptance model to explore adolescents’ acceptance of wearable activity trackers used in combination with social media within a physical activity intervention. Methods The Raising Awareness of Physical Activity study was a 12-week physical activity intervention that combined a wearable activity tracker (Fitbit Flex) with supporting digital materials that were delivered using social media (Facebook). A total of 124 adolescents aged 13 to 14 years randomized to the intervention group (9 schools) participated in focus groups immediately post intervention. Focus groups explored adolescents’ perspectives of the intervention and were analyzed using pen profiles using a coding framework based on the technology acceptance model. Results Adolescents reported that Fitbit Flex was useful as it motivated them to be active and provided feedback about their physical activity levels. However, adolescents typically reported that Fitbit Flex required effort to use, which negatively impacted on their perceived ease of use. Similarly, Facebook was considered to be a useful platform for delivering intervention content. However, adolescents generally noted preferences for using alternative social media websites, which may have impacted on negative perceptions concerning Facebook’s ease of use. Perceptions of technological risks included damage to or loss of the device, integrity of data, and challenges with both Fitbit and Facebook being compatible with daily life. Conclusions Wearable activity trackers and social media have the potential to impact adolescents’ physical activity levels. The findings from this study suggest that although the adolescents recognized the potential usefulness of the wearable activity trackers and the social media platform, the effort required to use these technologies, as well as the issues concerning risks and compatibility, may have influenced overall engagement and technology acceptance. As wearable activity trackers and social media platforms can change rapidly, future research is needed to examine the factors that may influence the acceptance of specific forms of technology by using the technology acceptance model. BACKGROUND In China, for most diabetic patients, wearable activity trackers are an innovative product that has not yet been generally accepted and used. Many studies have proved personality traits to affect patients' self-management behavior with chronic diseases, the willingness to adopt medical services, and willingness to use mobile healthcare equipment. OBJECTIVE This study aimed to investigate the influence of personality traits of type 2 diabetes patients on the use intention of the intelligent wearable device -- wearable activity trackers METHODS A self-designed questionnaire and Chinese Big Five Personality Inventory brief version (CBF-PI-B) were used to investigate type 2 diabetes mellitus (T2DM) patients from 22 community health service stations affiliated to 4 community health service centers in Tongzhou District and Shunyi District of Beijing. RESULTS A total of 728 cases were included in this study. Monofactor analysis indicated differences between patients with different genders in neuroticism, individual innovation, and behavioral intention, differences between patients with different ages in neuroticism, openness, individual innovation, and privacy concerns, differences between patients with different degree of education in openness, extraversion, perceived ease of use, the individual innovation and social image, and differences between patients with different income levels in openness, extraversion, perceived ease of use and individual innovation. The structural equation showed that perceived ease of use, perceived usefulness, social image, and privacy concern had direct effects on behavioral intention (β=0.158, 0.223, 0.151, -0.657). Neuroticism had a positive impact on social image and privacy concerns (β=0.144, 0.154). Conscientiousness and openness had a positive influence on perceived ease of use (β=0.147, 0.142). Agreeableness showed negative impacts on both perceived ease of use and privacy concerns (β=-0.108, -0.251). Openness and extroversion both had a positive impact on individual innovation (β=0.149, 0.180) while openness generated a positive influence on social image (β=0.189). CONCLUSIONS Different personality traits had influences on individual innovation, privacy concerns and social image, and indirectly affect patients’ intention to use wearable activity trackers. Therefore, personalized guidance and health education could be carried out according to different personality traits when using intelligent wearable devices in the intervention of patients with type 2 diabetes.

Background: There has been an explosion in the use of wearable activity trackers (WATs), but we do not fully understand who wears them and why. This study’s purpose was to describe the characteristics of WAT users and to compare current and former users. Materials and Methods: A variety of internet-based resources (eg, Craigslist, Facebook) were used to recruit current and former WAT users. Respondents completed a web-based survey, where they provided information on sociodemographic characteristics, health, physical activity behavior, and about their WAT use. Results: Of the 2826 respondents who gave informed consent, 70.8% (n = 2002) met inclusion criteria for this analysis. Respondents ranged from 18 to 81 years old (mean 32.9 ± 12.2 standard deviation) with 73.8% women. Most were current WAT users (68.7%), and the average length of WAT use overall was 9.3 ± 9.7 months. On average, current users wore the device for 3.7 months longer than former users. Compared to current users, former users had a lower body mass index (1.2 kg/m2 less), reported fewer medical conditions, shared data from their device less often, and received the device as a gift more frequently. Conclusions: Current and former users varied in their reasons for using a WAT and how they used their device. Differences identified between these groups support further exploration of associations between WAT users’ profiles and their physical activity behavior.

Former student-athletes (SAs) experience unique barriers to maintaining their physical activity, such as loss of team support, less motivation without specific goals, and identity-related changes. Informed by a self-determination theory framework, the authors developed a 6-week Pilates-based intervention to support the physical and psychological wellness of SAs by fostering self-determined motivation and basic psychological needs satisfaction as they make the transition to physically active alumni. In this case study, the authors outline the development and implementation of the program with final-year SAs (N = 12) at a Division III institution. Feasibility was demonstrated through high adherence and positive participant feedback suggesting they valued their experiences in the program and felt more confident in pursuing new forms of physical activity beyond college sports. To extend this type of programming at other institutions, the authors recommend that sport psychology professionals consider unique institutional barriers and opportunities for supporting SAs in their transition to meaningful lifetime activity.

Background: In Denmark, most children are not sufficiently physically active and only a few interventions have been found to increase long-term physical activity among overweight and obese children. The aim of our study was to investigate if children are physically active in correspondence to Danish recommendations after attending a multicomponent-overnight camp. Methods: A questionnaire was developed to estimate children’s physical activity level and behavior and investigate how transport, economy, availability, time, motivation, and knowledge about physical activity affect children’s physical activity level and behavior. Results: In this study, 60.9% of the children did vigorous physical activity (VPA) minimum 30 minutes 3 times per week up to 3 years after camp. Most children were physically active at a sports club (44.3%) and only 5.7% of the children did not participate in physical activity. Parental physical activity and child motivation toward physical activity were significantly (P < .05) associated with children doing VPA. Conclusion: Our findings suggest that 60.9% of children who attended camp engage in VPA after camp, which compared with a recent Danish study, is more frequent than children who did not attend camp. Further investigations are needed to determine the long-term health effects in children attending interventions such as multicomponent-overnight camps.
