Category: Ankle, Sports Introduction/Purpose: Ankle injuries can often have profound implication in the potential careers of National Collegiate Athletic Association (NCAA) athletes. Accordingly, a more thorough characterization of these injuries and insight into injury etiology is warranted. In addition, a more comprehensive understanding will allow proper education of athletes when injuries do occur. Here we review the incidence and effect of ankle injuries on NCAA athletes and their athletic season. Methods: Ankle injuries across 16 collegiate sports played by men and women from 2004-05 to 2013-14 academic years were surveyed from the NCAA Injury Surveillance Program (NCAA-ISP). Ankle injury rates per 10,000 athlete-exposures (IR), operative rate, annual injury rate trends, re-injury rates, in-season status (pre/in/post-season) at time of injury, and time loss distributions were perused. In effort to include both male and female atheletes equally, a sub-group analysis of contact sports played by both genders (C-BG) was performed to determine if there was a significant difference in risk when compared to those athletes that did not play contact sports. These sports included basketball, soccer, lacrosse, and ice hockey. Types of ankle injuries (soft tissues or bony) were also sub-classified and counted. Results: Over the course of 10 years, there were 14,080 ankle injures identified (IR=11.9). There were 8,978 (IR=12.1) injuries in males and 5,102 (IR=11.7) injuries in females. The rate of injuries that occurred in C-BG cohort (IR=14.5) was greater than that of the total group of noncontact participants (IR=7.6). When comparing injury rates between all contact sports(IR=14.1) and all noncontact sports (IR=7.6) the difference was found to be statistically significant(p <0 .0001). In the C-BG group, 1.4% of injuries were operative (N=93). The overall mean time loss for C-BG was 10.9 days when excluding patients who had season ending injuries. Across all atheletes, the top three contact sports with season ending ankle injuries were football, basketball, and lacrosse. Importantly,16.8% of all injuries were classified as re-injuries. Conclusion: Ankle injuries represent a common and broad spectrum of injuries in collegiate athletes. A majority of ankle injuries occurred during the regular season with sprains, strains, contusions, and fractures representing the most common types of injuries. Contact sports tend to have higher rates of ankle injuries than noncontact sports. As the consequence of index ankle injury appears to be higher predisposition to reinjury, a better understanding of the kinds of ankle injuries and their respective causes may help elucidate trends useful in the development of various prevention strategies.

Background: Ice hockey is a high-speed contact sport in which athletes are prone to many different injuries. While past studies have examined overall injury rates in ice hockey, foot and ankle injuries among collegiate ice hockey players have yet to be analyzed. Purpose/Hypothesis: The purpose of this study was to elucidate the epidemiology of foot and ankle injuries among collegiate ice hockey players utilizing data from the National Collegiate Athletic Association (NCAA) Injury Surveillance Program. We hypothesized that male ice hockey players would sustain more injuries compared with female ice hockey players and that the injuries sustained would be more severe. Study Design: Descriptive epidemiology study. Methods: Data on all foot and ankle injuries sustained during the academic years 2004 through 2014 were obtained from the NCAA Injury Surveillance Program. Injury rates, rate ratios (RRs), and injury proportion ratios were reported with 95% CIs. Results: Over the study period, the overall rate of foot and ankle injuries for men was higher than that for women (413 vs 103 injuries, respectively; RR, 4.01 [95% CI, 3.23-4.97]). Injury rates were highest during the regular season for both men (358 injuries; RR, 64.78 [95% CI, 58.07-71.49]) and women (89 injuries; RR, 38.37 [95% CI, 30.40-46.35]) compared with the preseason or postseason. The most common injury in men was a foot and/or toe contusion (22.5%), while women most commonly sustained a low ankle sprain (31.1%). For men, foot and/or toe contusions accounted for the most non–time loss (≤24 hours ) and moderate time-loss (2-13 days) injuries, while high ankle sprains accounted for the most severe time-loss (≥14 days) injuries. For women, foot and/or toe contusions accounted for the most non–time loss injuries, low ankle sprains accounted for the most moderate time-loss injuries, and high ankle sprains accounted for the most severe time-loss injuries. Conclusion: Foot and ankle injuries were frequent among collegiate ice hockey players during the period studied. For men, contusions were the most commonly diagnosed injury, although high ankle sprains resulted in the most significant time lost. For women, low ankle sprains were the most common and resulted in the most moderate time lost. These findings may direct future injury prevention and guide improvements in ice skate design.

This study examines the incidence and effect of foot injuries on NCAA athletes and their athletic season. Methods: Foot injuries across 16 sports among men and women during the 2004-05 to 2013-14 academic years were analyzed from the NCAA Injury Surveillance Program (NCAA-ISP). Three common orthopaedic foot injuries were sub-analyzed, including fifth metatarsal injuries (5MT), Lisfranc injuries, and turf toes. Multiple variables were reported including injury rates per 100,000 athlete-exposures (IR), operative rate, annual injury rate trends, re-injury rates, in-season status (pre/in/post-season) at time of injury, and time loss distributions were compiled and calculated. Results: A total of 3718 foot injuries were identified over 10-year period. 4.4% of all injuries were operative. Overall foot injury rate was comparable between male (IR=31.1) and female athletes (IR=32.4); however, the operative injury rate was greater in male compared to female (IR=2.0 vs. 0.6) athletes. The top three operative injuries were 5MT (45.1%), stress fractures (12.2%), and Lisfranc injuries (7.9%). 10.8% of turf toes are re-injury, and average time loss was 7.0 days. 43% of 5MT injuries were operative, and 19.2% were re-injuries. 40.6% of 5MT injuries were season-ending with an average time loss of 36.5 days. 33% of Lisfranc injuries were operative, and 7.5% were re-injuries. 45.9% of Lisfranc injuries were season-ending with an average time loss of 25.9 days. Conclusion: Foot injuries are among the most common form of injuries that occur in NCAA athletics. In addition, these injuries can sometimes lead to significant loss of playing time and may even result in the end of the athlete’s season or career. Certain NCAA sports such as men’s football and basketball may more likely predispose patients to season ending injuries and require operative fixation. Close examination of the kinds of foot injuries and their respective mechanisms may help elucidate trends useful in the development of various prevention strategies.

Abstract Background Ice hockey injuries epidemiology is still poorly understood and very few studies are focused on it, especially about professional players. Methods Our prospective study collected all injuries occurring on ice during practice and games over 7 years (2006–2013) in a professional hockey team playing in the 1st division championship in Switzerland. Results During the 7 seasons, we recorded a total of 525 injuries and 190 injuries with time loss (TL). Mean injuries incidence was 5.93 (95% CI 5.28 to 6.27) injuries/1000 h/player and with time loss 2.14 (95% CI 1.79 to 2.39) injuries/1000 h/player. The lower limb was the most affected part of the body, with a total of 40.4% of all injuries, mostly knee Medial Collateral Ligament tear and muscle adductors/abdominal sprain. For the upper limb, shoulder was the most affected joint with mostly acromioclavicular sprain and shoulder dislocation. Forwards had a significant (p < 0.05) higher risk than defensemen for knee Medial Collateral Ligament (MCL) tear. There was no significant difference in the concussion risk between forwards and defensemen, but defensemen had a significant higher risk (p < 0.05) to have a more severe concussion. Conclusion This study provides a better understanding about professional ice hockey epidemiology, which is still insufficiently researched and understood. We also found some significant risk factors, being a forward for knee MCL tear, being a defensemen for concussion severity. Concussion program prevention seems to be effective but it is crucial to continue the follow up of concussion on long term and expand the surveillance system to all the League.

Background: Injuries to the upper extremity among collegiate athletes are reported to account for approximately 20% of all injuries; however, little is known about the proportion of these injuries that require surgery. Purpose/Hypothesis: The purpose of this study was to examine all shoulder injuries that required a surgical intervention and were recorded in the National Collegiate Athletic Association (NCAA) Injury Surveillance Program (ISP). We hypothesized that contact would be the mechanism causing injuries most at risk for needing surgery and that dislocations would be the injuries most likely to require an operative intervention. Study Design: Cross-sectional study; Level of evidence, 3. Methods: Injury surveillance data between 2009-2010 and 2013-2014 for operative collegiate shoulder injuries and their associated sport exposures were analyzed. Results: A total of 185 operative shoulder injuries occurred over 3,739,004 athlete-exposures (AEs), for an overall incidence of 0.49 per 10,000 AEs. The sports with the highest incidence of operative injuries were men’s football (1.31/10,000 AEs), men’s wrestling (1.14/10,000 AEs), men’s ice hockey (0.60/10,000 AEs), women’s gymnastics (0.44/10,000 AEs), and men’s swimming (0.41/10,000 AEs). Men were significantly more likely than women to sustain operative injuries for all sports combined. Of the injuries that required surgical treatment, superior labrum from anterior to posterior (SLAP) tears (46.4%), other non-SLAP glenoid labrum tears (46.2%), anterior shoulder dislocations (33.3%), and posterior shoulder dislocations (30.0%) were seen most often. There was no significant difference in injury proportion ratios (IPRs) for injuries requiring surgery when comparing contact versus noncontact mechanisms of injury (IPR, 1.0 [95% CI, 0.6-1.6]). The incidence of operative injuries sustained during competition was significantly higher compared with during practice. Conclusion: The sports with the highest incidence of operative shoulder injuries were men’s football, men’s wrestling, men’s ice hockey, and women’s gymnastics. Operative shoulder injuries were more likely to occur during competition. SLAP tears, other non-SLAP glenoid labrum tears, and anterior shoulder dislocations had the highest incidence of requiring surgery. Athletes sustaining these injuries, along with their coaches and medical providers, may benefit from identifying collegiate sport participants who are at highest risk for sustaining an operative injury. This may assist in planning medical care and setting expectations, which may be critical to a young athlete’s career.

Little has been studied regarding the incidence and implications of Achilles tendon ruptures in this patient population. Better characterization of the factors commonly found in athletes who rupture their Achilles may provide clues to aid in their prevention. Methods: Achilles injuries across 16 sports among NCAA men and women during the 2004-05 to 2013-14 academic years were analyzed using the NCAA Injury Surveillance Program (NCAA-ISP). Achilles tendon rupture rates per 100,000 athlete-exposures (IR), operative rate, annual injury rate trends, re-injury rates, mechanism of injury, in-season status (pre/in/post-season) and time loss distributions were compiled and calculated. A sub-analysis of contact sports and sports played by both genders (C-BG) was performed to determine if there were significant differences in risks in patients who played in contact sports. Results: N=255 Achilles tendon injuries were identified over 10 academic years (IR: 2.17). The injury rate was higher in males compared to females (IR=2.33 vs. 1.89 respectively). Achilles injuries were most common in Men’s Basketball (IR=4.26), Soccer (IR=3.06), and Football (IR=2.69). The top three women’s sports with Achilles injury were Gymnastics (IR=16.73), Basketball (IR=3.32), and Soccer (IR=1.81). Thirty-three injuries were operative (13.1%) and 14.9% (N=38) were season-ending injuries. The average time loss was 10.65 days when excluding patients who had season/career ending injuries. Reinjury rate was 11.0% (N=28). 61.2% of all injuries occurred during the regular season (N=156) with 36.0% (N=92) and 2.7% (N=7) occurring in pre- and post- season, respectively. There was a significantly greater number of injuries in contact (N=198) versus non-contact sports (N=50) (p<0.001). Conclusion: Achilles tendon ruptures can be devastating injuries in professional and collegiate athletes. In our study, nearly 15% of all NCAA Achilles tendon ruptures resulted in season ending injuries or significant time loss and over 13% of injuries required operative management with a majority of injuries occurring during practice. In addition, a significantly higher proportion of athletes who played a contact sport had Achilles injuries. Better understanding of what circumstances more often tend to result in Achilles injuries can help establish prevention strategies.

Background: Ice hockey has significant workload demands. Research of other sports has suggested that decreased rest between games as well as an increased workload may increase the risk of injuries. Purpose: To evaluate whether condensed game schedules increase the frequency and severity of injuries in the National Hockey League (NHL). Study Design: Descriptive epidemiology study. Methods: Data were obtained from publicly available online sources on game schedules and injuries for all NHL teams for the 2005-2006 through 2018-2019 seasons. Injury rates (per team per game) and the proportion of severe and nonsevere injuries were determined. The game-spacing analysis assessed the risk of injuries in relation to the number of days between games played (range, 0-≥6 days). The game-density analysis assessed the risk of injuries in relation to the number of games played within 7 days (range, 1-5 games). Results were assessed by analysis of variance, the post hoc Tukey test, and the chi-square test of distribution. Results: The game-spacing analysis included 33,170 games and 7224 injuries, and a significant group difference was found ( P = 1.44×10–5), with the post hoc test demonstrating an increased risk of injuries when games were spaced with <1 day of rest. There was no significant difference in the ratio of severe to nonsevere injuries. The game-density analysis included 33,592 games and 10,752 injuries, and a significant group difference was found ( P = 8.22×10–48), demonstrating an increased risk of injuries with an increased number of games in all conditions except for the comparison between 4 versus 5 games in 7 days. There was also a significant difference in injury severity ( P = .008), indicating that the least dense condition had a higher ratio of severe to nonsevere injuries compared with the other game-density conditions. Finally, the game-density analysis was repeated after excluding games played with <1 day of rest, and the finding of increased injury rates with increasingly condensed schedules remained significant ( P = 9.52×10–46), with significant differences between all groups except for the comparison between 1 versus 2 games in 7 days. Conclusion: We found that a condensed schedule and <1 day of rest between games were associated with an increased rate of injuries in the NHL. These findings may help in the design of future game schedules.

AbstractRink Hockey is a high-speed low-contact sport with a widely recognized injury potential. Played with a hard ball by players wearing little or no protective gear, the rates of concussions and head injuries are of high interest. In this study, we reviewed and investigated injuries sustained by 108 German National League rink hockey players. We conducted an epidemiological cross-sectional study to determine sport-specific injuries and injury patterns in female and male rink hockey players. Data were collected by a standardized questionnaire. A total of 108 players participated in the study. The combined rate of injuries were 9.4/1000 athlete exposures. There was no significant difference between injury rates during games and practice. A sex-specific difference was not detected. Head injuries were the most frequent type (20.8% of all injuries). Concussions made up 7% of all head injuries with a rate of 0.76/1000 athlete exposure. Ball contact was the cause for 31% of injuries, while player contact generated 26.2% of all injuries. High rates of ball-contact-related injuries resulted in time loss and medical consultations. Head injuries were frequent, however did not result in significant time loss as compared to other injuries. The use of protective gear should be recommended.

Epidemiologic studies have identified differences in concussion incidence between the sexes. However, few authors to date have updated injury rates (IRs) and time loss between male and female concussed athletes.Context: To examine sex differences in IRs and time loss in concussed National Collegiate Athletic Association (NCAA) athletes.Objective: Descriptive epidemiologic study.Design: National Collegiate Athletic Association athletics.Setting: A total of 1702 concusssed NCAA athletes, consisting of 903 females and 779 males participating in soccer, basketball, ice hockey, lacrosse, softball, or baseball over a 5-year period from 2004–2005 through 2008–2009.Patients or Other Participants: Using the NCAA Injury Surveillance Program, athletic trainers reported concussions, athlete-exposures (AEs), and time loss across 10 NCAA sports. An IR is the number of injuries in a particular category divided by the number of AEs in that category.Main Outcome Measure(s): During the study period, 1702 concussions were reported during 4 170 427 AEs for an overall total of 5.47 per 10 000 AEs. In sex-comparable sports, females had a 1.4 times higher overall concussion IR than males (IRs = 4.84 and 3.46, respectively), with greater rates in women's baseball/softball, basketball, ice hockey, and soccer than men. Female soccer and basketball players also displayed more time loss after concussion compared with male basketball and soccer players.Results: Female athletes sustained a higher rate of concussion and, in all sports except lacrosse, had greater time loss from concussion than male athletes. Additional research is needed on sex differences in time loss after concussions.Conclusions:

Objective: This study aimed to investigate the static and dynamic balance in basketball students with an ankle injury history. Methods: Subjects of this study were 36 elementary school basketball male students who participated in this study purposefully and voluntarily. Subjects were divided into two groups of 18 people with an ankle injury and the control group without ankle injury. To measure the static balance from the stork test and the dynamic balance from the star test was used. Descriptive statistics (mean and standard deviation) and inferential statistics (Shapiro-Wilk test and independent t-test) were used to analyze the results. Results: There was a significant difference between the static balance of basketball students and the control group (P=0.001), but no significant difference was found between basketball studentschr('39') dynamic balance with ankle injuries and the control group (P=0.0887). Conclusion: Based on research results, it can be concluded that lower limb injuries, especially ankle injuries they can influence postural control and static balance of basketball students and caused some functional disability. Also recommended coaches during the sports era of basketball players to design specific exercises to strengthen ankle muscles and ligaments and use static balance exercises not to witness this type of injury.

Background: Ice hockey is a physically demanding sport where athletes are susceptible to a variety of injuries. Several studies reported the overall injury rates in ice hockey; however, there is a paucity of information on upper extremity (UE) injuries among collegiate ice hockey players. Purpose: To describe the epidemiology of UE injuries among collegiate male and female ice hockey players with NCAA (National Collegiate Athletic Association) injury surveillance data from 2004-2005 to 2013-2014. Study Design: Descriptive epidemiology study. Methods: Data were obtained from the NCAA Injury Surveillance Program for all UE injuries sustained during the academic years 2004-2005 to 2013-2014. Injury rates, rate ratios (RRs), and injury proportion ratios were reported with 95% CIs. Results: During the 10 years studied, the overall rate of UE injuries for men was higher than that for women (236 vs 125 injuries per 100,000 athlete-exposures [AEs]; RR, 1.89; 95% CI, 1.67-2.15). UE injuries sustained during either pre- or postseason were approximately 3 times higher for men than for women (preseason: 149 vs 53 per 100,000 AEs; RR, 2.83; 95% CI, 1.69-4.74; postseason: 143 vs 49 per 100,000 AEs; RR, 2.91; 95% CI, 1.33-6.38). The overall injury rate was highest during the regular season (men: 257 per 100,000 AEs; 95% CI, 242-272; women: 143 per 100,000 AEs; 95% CI, 126-160). Additionally, the injury rate for men and women was higher during competition than practice (men: 733 vs 83 per 100,000 AEs; 95% CI, 687-780 and 75-92; women: 303 vs 64 per 100,000 AEs; 95% CI, 259-348 and 52-76). The most common injury observed was acromioclavicular joint sprain (men, 29.1%; women, 13.8%). For both groups, acromioclavicular joint injuries accounted for most non–time loss, moderate time loss (2-13 days), and severe time loss (≥14 days) injuries. Conclusion: Men and women sustained a significant number of UE injuries playing collegiate ice hockey during the period studied, with acromioclavicular joint sprain being the most common UE injury and the one that most frequently led to significant time loss. These data may provide insight for future injury prevention and guide improvements in training.
