Abstract Background Vaccination against COVID-19 continues apace, but side-effects, both common and severe, continue to be reported. We report here the first published case of COVID-19 vaccine-related encephalitis. Case presentation A young woman presented with acute neuropsychiatric symptoms following recent ChAdOx1 nCoV-19 vaccination. Extensive investigation did not identify alternative causes. Conclusions This difficult case is here described, including presentation, investigation, and management. Further study on neuropsychiatric side-effects of COVID-19 vaccination, including investigation as to whether there may be a causal link, is required.

Abstract Objectives Vipadika (Palmo plantar Psoriasis) is a Kshudra Kushta (minor skin disorder) of Vatakapha origin (body humors like vata and kapha) characterized by Sphutana (cracks) and Teevra Vedana (severe pain) in Pani (palm) and Pada (sole). Based on the symptoms it can be correlated to Palmo Plantar Psoriasis which is a variant of psoriasis which affects the skin of the palms and soles with the features of hyperkeratotic, pustular, or mixed morphologies. The treatment methods for Palmo plantor psoriasis causes severe side effects due to which an evident shift from modern treatments to ayurvedic treatment line. Case presentation A 15 year old, female, with complaint of cracks and dryness over sole of both feet with pain for the last 12 and half years. The case was managed with Padanimajjana (immersion of foot). Padanimajjana is a modified form of Avagaha Sweda (switz bath) where the affected part of the foot is dipped in medicated liquid. Here Mahisha Gritha (Buffaloe ghee) is used for Padanimajjana. Conclusions Mahisha Gritha Padanimajjana shows significant improvement in number of cracks, dryness and pain within short period of time. This is a very simple, economical, and effective treatment for Vipadika.

Abstract IntroductionAs more novel COVID-19 vaccines are being rolled out in a frantic pace globally, any complication that might be related to COVID-19 vaccines should be highlighted, especially since COVID-19 vaccines are relatively new, and side effects may yet to be fully elucidated. We report a case of a healthy 18-year-old male who presented with new onset focal seizures 5 days after receiving 1st dose of Oxford/AstraZeneca COVID-19 vaccine. Case PresentationThe patient was treated with intravenous phenytoin and oral levetiracetam 250mg twice daily with no further events. There was no documented fever. CT venogram and EEG were unremarkable. MRI brain revealed generalised atrophy including mild bilateral hippocampal atrophy with no evidence of sclerosis. There was no predilection for seizures identified from the patient’s history. The patient was discharged the following day on levetiracetam and advised to proceed with the 2nd dose of Oxford/AstraZeneca COVID-19 vaccination in 3 months’ time.ConclusionSeizures following COVID-19 vaccination have only been reported in a handful of cases. COVID-19 vaccination could lower seizure threshold, or unmask an underlying predisposition for epilepsy. As most COVID-19 vaccines worldwide are given in 2 doses, clinicians should consider maintaining patients on anti-seizure drugs if vaccination was thought to be a provoking factor.

Introduction. Takotsubo syndrome is a reversibile cardiomyopathy that often occurs during an emotional stress and is more frequent in post-menopausal women. Case presentation. We describe a clinic case of a 42 years old woman, admitted to our emergency unit because of a thoracic pain. The ECG showed ST-T elevation in V1- V4, the echocardiography revealed akinesia of apical segment of left ventricle. Coronary arteries were free from significant stenosis and ventriculography showed akinesia of apical segment of left ventricle. In the following days, ECG evolved toward a normalization of ST-T segment with appearance of negative T wave in V2-V6, and also the echocardiography showed a normalized LV wall motion. The patient was discharged and ECG and echocardiography were normal at two months follow up. Conclusion. In this case report, a young woman during puerperium presented with a Takotsubo syndrome. We speculate that the high Prolactin level and her emotional state contributed to the clinical manifestation of the syndrome.

Background: The Novel Oral Anticoagulants (NOACs), despite numerous benefits, such as the ease of use and less drug involvement, provide extensive adverse effects. One of the most significant, but rare side effects of them in women is severe and dangerous bleeding. Case presentation: In this study, we reported a case of severe vaginal bleeding (manometric hemorrhage) in a woman receiving rivaroxaban to prevent pulmonary thrombosis. Conclusion: The oral anticoagulant rivaroxaban could present a rare adverse effect on women.

ABSTRACTFebrile infection-related epilepsy syndrome is characterized by super refractory status epilepticus that is resistant to conventional antiepileptic drugs.This case report critically analyses the treatment options adopted in a hospital to manage this syndrome. Despite the aggressive efforts that weretaken, the patient succumbed to the complications of the illness and side effects of the treatment strategies adopted. This shows that the treatmentoptions currently available are in adequate, so an intensive research in the area of pathogenesis of status epilepticus is required to frame treatmentstrategies that can bring out better outcomes.Keywords: Antiepileptics, Febrile infection-related epilepsy syndrome, Status epilepticus.

AbstractObjectivesKeratosis pilaris (KP) is the condition of the skin with extensive keratin follicular plugging. It may be associated with the erythema. The upper arm extensor area, shoulders, back of neck and thighs, as well as face and the upper trunk are the areas of presentation. Available medications for KP give only symptomatic relief, while some produce serious side effects. There is no proven universal treatment for the disease that can provide complete recovery. Āyuṛveda management of KP is not yet reported.Case presentationA 26-year-old male patient, presented with main complaints started with papular lesions over his right shoulder, chest and upper back along and later with pustular lesions in the past 2 weeks. The condition was associated with redness, mild swelling and itching. The case was diagnosed as Keratosis pilaris based on its presentation, site, and pathogenesis. Also by analyzing the extent of vitiation of doṣas (morbidities), the Vata kapha pitta hara line of treatment was adopted, which was accomplished in two phases i. e. Śodhana Cikitsa and Śamana Cikitsa.ConclusionBoth internal and external treatments along with diet restrictions were found effective in arresting the pathogenesis and recovery in a short period. All the symptoms associated with the condition were completely cured with no signs of re-occurrence.

Introduction: Circumcision is the most common surgery in male infants in Islamic countries. Therefore, knowledge of its side effects and appropriate therapies is essential. Case Presentation: In this case report, we describe a 12-day neonate with glans ischemia, two days after circumcision. We admitted the neonate and treated him with oral pentoxifylline and antibiotic for three days. Conclusions: Thus, pentoxifylline can be implemented as a treatment in post-circumcision ischemia of the glans penis in neonates.

Introduction: Coronavirus disease 2019 (COVID-19) has been identified as an emerging disease in 2019. Complications of the disease remain unknown, and extensive research has been conducted to investigate it. Due to the way the disease is transmitted through ACE2, the virus affects many organs. So far, no cases of hepatic encephalopathy following the virus have been reported. Case Presentation: A 3-year-old boy referred to Ali-Ibn-Abitaleb Hospital in Zahedan, Iran was hospitalized with icterus and decreased level of consciousness. After examinations and evaluation, the patient was diagnosed with hepatic encephalopathy following COVID-19. Unfortunately, despite the treatments, the patient was finally expired. Conclusions: To the best of our knowledge, this is the first case report demonstrating COVID-19 related hepatic encephalopathy. This case is a useful reminder for doctors to be much more vigilant and cautious about possible side effects of COVID-19.

Abstract Background Although M. pneumoniae (M. pneumoniae) infections have been associated with various extrapulmonary manifestations, there have been very few documented cases of thrombotic events in pediatrics, and none to our knowledge with such extensive involvement as the patient described here. We aim to contribute to the urgency of discovering the mechanism of the coagulopathy associated with M. pneumoniae infections. Case presentation This 10-year-old boy was admitted after 2 weeks of fever, sore throat, worsening cough, and progressive neck and back pain. During hospitalization, he developed clots in several different organs: bilateral pulmonary emboli, cardiac vegetations, multiple splenic infarcts, and deep venous thromboses in three of four extremities. He was treated with long-term antibiotics and anticoagulation, and fully recovered. Conclusions This is the first case known to us of a child with an extensive number of thrombotic events in multiple anatomic sites associated with M. pneumoniae infection. The mechanism by which M. pneumoniae infection is related to thrombotic events is not fully understood, but there is evidence that the interplay between the coagulation pathways and the complement cascade may be significant. This patient underwent extensive investigation, and was found to have significant coagulopathy, but minimal complement abnormalities. By better understanding the mechanisms involved in complications of M. pneumoniae infection, the clinician can more effectively investigate the progression of this disease saving time, money, morbidity, and mortality.

Introduction: Blepharoplasty, which is performed for either functional or cosmetic purposes, is one of the most common surgeries around the world. This procedure can be performed under either local or general anesthesia. It depends on the surgical indication, patient’s demand, surgeon’s plan, and need for auxiliary operations. It has been proved that both local and general anesthesia have different complications. Hypnosis is a skill by which hypnotherapists can make changes in perceptual experience and can be used instead of anesthetic drugs on some occasions. The present study was conducted to survey the effectiveness of hypnosis used as an alternative to anesthesia procedures. Case Presentation: A 54-year-old woman was scheduled for bilateral upper lid cosmetic blepharoplasty. Hypnotic anesthesia was conducted owing to her fear of anesthesia and drug-related side effects. Hypnotic anesthesia was accomplished by a hypnotherapist who was an anesthesiologist, and the operation was done without any surgical complications. Conclusions: Hypnosis may be a good alternative to local anesthesia and lead to a reduction in analgesics consumption.
