Abstract Background Health care workers (HCWs) are among the high-risk groups in contracting and dying from COVID-19. World Health Organization estimates that over 10,000 HCWs in Africa have been infected with COVID-19 making it a significant occupational health hazard to HCWs. In Ghana, over 100 HCWs have already been infected and dozen others died from the virus. Acceptability and uptake of the COVID-19 vaccine is therefore critical to promote health and safety of HCWs as the country battles out of a third wave of the pandemic. Objective The study sought to ascertain the correlates of HCWs likelihood of participating in a COVID-19 vaccine trial and accepting the vaccine when given the opportunity. Methods The study was a web-based cross-sectional survey among HCWs (n = 1605) in all sixteen (16) administrative regions in Ghana. Data were analyzed with STATA statistical analysis software (version 14). Chi-square (X2) and Fisher’s exact tests were used to test for differences in categorical variables; bivariate probit regression analysis with Average Marginal Effect (AME) was employed to ascertain the determinants of HCWs’ likelihood of participating in a COVID-19 vaccine trial and taking the vaccine. Results It was found that 48% of HCWs will participate in a COVID-19 vaccine trial when given the opportunity; 70% will accept the COVID-19 vaccine; younger HCWs (AME = 0.28, SE = 0.16, p < 0.1), non-Christians (AME = 21, SE = 0.09, p < 0.05) and those who worked in faith-based health facilities (AME = 18, SE = 0.07, p < 0.05) were more likely to participate in a COVID-19 vaccine trial. Female HCWs (AME = − 11, SE = 0.04, p < 0.05) and those with lower educational qualification were less likely to accept a COVID-19 vaccine (AME = − 0.16, SE = 0.08, p < 0.1). Reasons cited for unwillingness to participate in a COVID-19 vaccine trial or uptake the vaccine were mainly fear, safety concerns, mistrust, uncertainty, spiritual and religious beliefs. Conclusions Acceptance of the COVID-19 vaccine appear to be high among HCWs; conversely, willingness to volunteer for the vaccine trial was low. Continuous targeted and integrated public health education for HCWs will enhance vaccine acceptability to promote safety and population health in the global south as Ghana intensifies efforts to produce COVID-19 vaccines locally.

Background The World Health Organization has warned against a dramatic impact of COVID'19 in subSaharan Africa unless adequate response strategies are implemented. Whatever the strategy, the role of health care workers is pivotal. We undertook this study to assess knowledge of COVID'19 and perception of the response to the pandemic among the staff of a regional hospital in charge of COVID'19 patients in West Cameroon. Methods We used a convenience non probabilistic sampling method to carry out a survey with a self administered questionnaire from April 14, 2020 to April 29, 2020 at the Bafoussam Regional Hospital (BRH). All the staff was invited to participate. Statistical analyses were done using Microsoft Excel 2010 and Epi Info version 7.1.5.2 software. Results Response rate was 76.1% (464/610). Mean age (SD) and average work experience (SD) were 35.0 (8.9) and 8.4 (7.4) years respectively. Sex ratio (M/F) was 101/356. Nursing and midwifery staff (56.8%) and in patients units (49.94%) were predominant. Knowledge on origin and transmission of SARS'CoV'2 was poor but knowledge of clinical signs and the role of laboratory tests were good. 53.2% of respondents said all therapeutic regimens are only supportive and only a third of them trusted drugs recommended by health authorities. For 36.9% of respondents, herbal remedies can prevent/cure COVID'19. 70% of staffs felt they were not knowledgeable enough to handle COVID'19 cases. 85.6% of respondents thought the BRH had insufficient resources to adequately respond to COVID'19 and 55.6% were dissatisfied with its response to the pandemic (weaknesses: medicines/technologies (74.5%), service delivery (28.1%), human resource (10.9%)). 68% of staff felt insufficiently protected on duty and 76.5% reported that the pandemic significantly reduced non-COVID-19 services. 85.5% said they complied with preventive measures while in the community. For 44% of respondents Cameroonian regulations on COVID'19 corpses should be made more culture sensitive. 51.2% of respondents were against vaccine trial in their community. Conclusion Knowledge of COVID'19 was poor and perception of the response to the pandemic was unfavorable. Key words Health care workers ; Bafoussam ; Knowledge ; Perception ; COVID'19

Abstract Background: Health care workers (HCWs) are at increased risk of influenza infection. Hence, the rationale for immunization to protect them and their patients particularly during the co-circulation of influenza and coronavirus disease 2019 (COVID-19). This study aims to assess knowledge, attitudes, and practices among HCWs towards the influenza vaccine and to identify the determinants of their willingness to vaccinate. Methods: Using an online survey, a cross-sectional study, was conducted in Lebanon between 14th and 28th October 2020 among HCWs. Descriptive statistics were reported using frequency and percentages for categorical variables. Multivariable logistic regression was carried out to identify the factors associated with influenza vaccine uptake. Adjusted odds ratio and their 95% confidence intervals were reported. Results: A total of 560 HCWs participated in the survey of which 72.9% were females. The majority were nurses (63.2%), married (66.1%) and aged between 30-49 years (53.9%). Nearly half of them had a good level of knowledge (≥80%). As perceived by HCWs, the availability of a sufficient quantity of vaccine will be the biggest challenge. Benefits of vaccination in enhancing patient safety, and avoiding co-infection by influenza and COVID-19 were well-acknowledged by HCWs. 32.1% of HCWs have received the influenza vaccine during the last year and 80.2% were willing to vaccinate for the current year. The influenza uptake was higher among HCWs who have received influenza vaccine in the past season (OR = 6.812, CI (3.045-15.239)), having a fair health status (OR = 3.117, CI (1.345-7.220)), good knowledge (OR=3.305, CI (1.155-9.457)), positive attitude (OR=2.305, CI (0.921-5.571)), low perception of barriers (OR= 4.130, CI (1.827-9.334)) and high perception of benefits (OR=6.264, CI (2.919-13.442) and was lower in single and divorced (OR=0.527, CI (0.284-0.978)). ConclusionAddressing misconceptions unveiled in this study through adapted training, along with economic interventions are needed to improve influenza vaccination coverage among HCWs.

Influenza vaccination is the most effective way of influenza prevention. The vaccination rate is low worldwide. In Hungary, the vaccine is free of charge to health care workers and, therefore, the low vaccination rate is unaccountable. Aims: In this study, the authors wanted to explore those factors which influence the refusal of vaccination. Methods: The Health Science Center of Debrecen University has about 4000 employees. The authors adjusted a questionnaire with 45 questions and sent it to 525 randomly selected health care workers, 294 of whom responded (response rate, 56%). The Epiinfo software was used for statistical evaluation. Results: The respondents strongly agreed that the vaccine is free and easy to obtain at the workplace. Official recommendations of the occupational health, the Medical Association of Hungary and advice of the family doctors failed to influence the decision. However, a significant impact of communication with family members, friends and colleagues on the decision was documented. Conclusions: The results indicate that the most important tool in decision making of influenza vaccination is the internal communication, but this effect is not a permanent one. International data show highly variable vaccination rates (between 2.1% and 82%). A better vaccination rate (98% or above) may be achieved with a mandatory influenza vaccination program among health care workers. Orv. Hetil., 2012, 153, 505–513.

Abstract Background Antenatal care (ANC) provided by a trained health care provider is important for monitoring pregnancy thereby reducing potential risks for the mother and child during pregnancy and delivery. The World Health Organization (WHO) recommends at least four ANC visits to all pregnant women. While the proportion of women who attend at least one ANC in low-income countries is high, most pregnant women start their first ANC attendance very late. In Tanzania only 24% of pregnant women start their first ANC attendance before the fourth month of pregnancy. While factors for the utilization of antenatal care in general have been widely studied, there is paucity of studies on the factors affecting timing of the first ANC attendance. This study aimed to understand individual, community, and health system factors that lead to the delay in seeking ANC services among pregnant women in Ilala Municipal in Dar es Salaam region, Tanzania. Methods A qualitative exploratory study, using in-depth interviews with 20 pregnant women and five health care workers was conducted in three different health facilities in Dar es Salaam Tanzania. Thematic analysis approach was used to analyse the data. Results Individual perceptions of antenatal care, past experience with pregnancy, fear of pregnancy disclosure, and socio-cultural beliefs were the key individual and social factors for late ANC attendance. Shortage of trained health care workers, lack of spouse’s escort and health providers’ disrespect to pregnant women were the main health system barriers to early ANC attendance. Conclusions This study concludes that community members should be sensitized about the importance of early ANC attendance. Additionally, while spouse’s escort policy is important for promoting PMTCT, the interpretation of the policy should not solely be left to the health providers. District and regional health officials should provide correct interpretation of this policy.

AbstractSince the coronavirus disease 2019 (COVID-19) identified in Wuhan, Hubei, China in December 2019, it has been characterized as a pandemic by World Health Organization (WHO). It was reported that asymptomatic persons are potential sources of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. We present an outbreak among health-care workers incited by a doctor who cared a patient with COVID-19 in a Hospital in Wuhan, Hubei, China, which indicates existence of super-spreader even during incubation period.

Chronic obstructive pulmonary disease (COPD) is becoming an increasing problem for health care workers. The World Health Organization predicts that in the year 2020, this disease will be the fifth most prevalent disease worldwide, up from 12th place, and it will be the third most common cause of death, up from sixth place in 1997 (1). Hospitalization and mortality rates for COPD continue to rise in Canada (2). Therefore, the burden of COPD on Canadians, the health care system and physicians is obvious.

The acceptance or otherwise of the COVID-19 vaccine by health care workers can influence the uptake of COVID-19 vaccines among the general population as they are a reliable source of health information. In this study, we sought to determine the acceptability of COVID-19 vaccines among health care workers in Ghana. Using a cross-sectional design, we collected data from 234 health care workers through a self-administered online survey from 16 January to 15 February 2021. Descriptive, bivariate and multivariate analyses using binary logistic regression were performed using STATA version 15. The results showed that 39.3% of health care workers had the intention of receiving the COVID-19 vaccine. Factors such as sex, category of health care workers, relative being diagnosed with COVID-19, and trust in the accuracy of the measures taken by the government in the fight against COVID-19 proved to be significant predictors of the acceptability of the COVID-19 vaccine. Concerns about the safety of vaccines and the adverse side effects of the vaccine were identified as the main reasons why health care workers would decline uptake of the COVID-19 vaccine in Ghana. The self-reported low intention of health care workers to accept the COVID-19 vaccine in Ghana requires the urgent call of the Government of Ghana and other stakeholders to critically address health care workers' concerns about the safety and adverse side effects of COVID-19 vaccines, as this would increase vaccine uptake. Interventions must also take into consideration sex and the category of health care workers to achieve the desired results. Nigeria is making effort to address the child mortality burden by increasing vaccine coverage rates, yet the vaccine coverage falls short of 90% target. Scaling up of new and under-used vaccines to 90% coverage could save more than 600,000 Nigerian children. Healthcare givers knowledge of vaccine used for immunization is essential to increase the vaccine uptake rates. This study assesses the knowledge of routine, underutilized and future vaccines among health workers. A cross sectional descriptive study was done among health care workers at a tertiary health facility in Nigeria. Using a pre-tested semi structured interviewer administered questionnaire, 220 respondents were selected by cluster sampling technique. The health workers’ knowledge of these vaccines was assessed using a scale developed for the study. Data collected were analyzed using SPSS version 22.The mean age of the respondents was 31.9 + 5.7 years . Doctors and nurses formed more than half of the respondents, 51.8% (n=114). About three quarters of respondents 72.3% (n=159) had good knowledge of vaccines used in routine immunizations. Knowledge of under-utilized and future vaccine was low with less than a fifth 18.6% (n=41) and one tenth 9.1% (n=20) having good knowledge scores respectively. Similarly, poor perception scores of future vaccines were observed in 90.1% (n=218) of the respondents. Only marital status was associated with knowledge of underutilized vaccines (p<0.05) and no association was observed between other sociodemographic variables and knowledge of these vaccines (P>0.05). The awareness level of health care workers on routine immunization is high. Their knowledge and perception of under-utilized and future vaccines were however low. There is, therefore, need for more training and retraining of health care workers on the vaccines.

BACKGROUND The COVID-19 pandemic continues to ravage and burden hospitals around the world. The epidemic started in Wuhan, China, and was subsequently recognized by the World Health Organization as an international public health emergency and declared a pandemic in March 2020. Since then, the disruptions caused by the COVID-19 pandemic have had an unparalleled effect on all aspects of life. OBJECTIVE With increasing total hospitalization and intensive care unit admissions, a better understanding of features related to patients with COVID-19 could help health care workers stratify patients based on the risk of developing a more severe case of COVID-19. Using predictive models, we strive to select the features that are most associated with more severe cases of COVID-19. METHODS Over 3 million participants reported their potential symptoms of COVID-19, along with their comorbidities and demographic information, on a smartphone-based app. Using data from the >10,000 individuals who indicated that they had tested positive for COVID-19 in the United Kingdom, we leveraged the Elastic Net regularized binary classifier to derive the predictors that are most correlated with users having a severe enough case of COVID-19 to seek treatment in a hospital setting. We then analyzed such features in relation to age and other demographics and their longitudinal trend. RESULTS The most predictive features found include fever, use of immunosuppressant medication, use of a mobility aid, shortness of breath, and severe fatigue. Such features are age-related, and some are disproportionally high in minority populations. CONCLUSIONS Predictors selected from the predictive models can be used to stratify patients into groups based on how much medical attention they are expected to require. This could help health care workers devote valuable resources to prevent the escalation of the disease in vulnerable populations.
