Background. Microbiological disorders in the genital tract predispose to the development of chronic pelvic diseases and subsequently worsen the prognosis for reproductive function, causing infertility, miscarriage. Aims: study of the microbiocenosis of the vaginal tract and its role in initiating inflammatory process in endometrium in women with reproductive disorders. Materials and methods. The study included 50 patients with reproductive disorders with histological confirmation of the diagnosis of chronic endometritis and 50 patients with reproductive disorders without chronic endometritis. Mi- crobiological studies of the biotope of the vagina were carried out according to the guidelines. The counting of immune cells was carried out in peripheral blood with flow cytofluorimeter. Methods of mathematical statistics implemented in the application package Statistica 6.1 (StatSoft Inc., USA) were used in the study. Used criteria were Student’s t-test and Fisher criterion with a significance level less than 5 % (p ≤ 0.05). Results and conclusions. Conditionally pathogenic microflora in women with chronic endometritis in the cervical ca- nal is represented by candidiasis, herpesviruses, ureaplasma and facultative-anaerobically positive microorganisms. Candida, obligate-anaerobic gram-negative, and facultative anaerobic gram-positive microorganisms predominated in the endometrium. The secondary immunodeficiency state is characterized by suppressor activity of T-lymphocytes; the decrease in mi- crobicide activity of blood, pronounced inhibition of the humoral link of the immune system. Violations of the balance of the indigenous and conditionally pathogenic microflora accompanied by a secondary immunodeficiency may be the initiator of chronic inflammation in the endometrium.

The article highlights modern ideas about the role of the endometrium in the genesis of reproductive failures. A crucial place of chronic endometritis in the structure of the endometrial infertility factor in endometriosis is noted. Given the frequent association of chronic endometritis and endometriosis, common features of these diseases have been identified. The characteristic of endometrial microbiota in infertility, chronic endometritis and endometriosis is given. An extremely important area of scientific research is precisely the interaction between the microbiota of the endometrium and its immunity, and not just the confirmation of the presence of microorganisms in the endometrium. The modern aspects of the etiology and pathogenesis of chronic endometritis and endometriosis are described. The main pathogenetic determinants of chronic endometritis are the failure of adaptation mechanisms to the constant effects of microbes and, as a consequence, the decrease and inferiority of the immune response and the development of autoimmune reactions. The modern paradigm for the development and progression of endometriosis involves the presence of bacterial contamination of the endometrium and the abdominal cavity, which in turn becomes a trigger for genetically-epigenetic modifications of cells, launching different cascades that underlie reproductive function disorders. The pathogenetically significant relationship between chronic endometritis and endometriosis is highlighted. It was revealed that chronic endometritis can be both a cause and a consequence of endometriosis. Disorders of the endometrial receptivity in chronic endometritis associated with endometriosis, their importance in the implementation of endometrial infertility are described. The important role of the microbiome in the formation of reproductive disorders is noted.

The review deals with the main role of chronic endometritis in the genesis of disorders of the female reproductive function and reveals the etiologic risk factors for the development and starting mechanism of chronic inflammatory processes in the endometrium, features of the pathogenesis and clinical picture of chronic endometritis. The work shows the importance of diagnosis of chronic endometritis with immunohistochemical method, reasonability of immunohistochemical research for the assessment of receptor expression and expressiveness of local immunity in the endometrium, the necessity of detection of fertility alpha-2-microglobulin (AMGF) protein for prediction of reproductive disorders.

Objective: Fatigue associated with malignant conditions and their treatments is a disabling condition. This trial assessed the anti-fatigue effects of melatonin coadministration during adjuvant treatment of patients with the breast cancer. Material and Methods: Patients with breast cancer were randomly assigned to receive melatonin or placebo during adjuvant chemotherapy and radiotherapy. Thirty-seven patients were randomly enrolled in each group. The mean ages of patients in the intervention and control groups were 50.47 ± 10.79 and 46.05 ± 10.55 years, respectively ( P = .223). The intervention group received oral melatonin (18 mg/day) from 1 week before until 1 month after the adjuvant radiotherapy. The level of fatigue was assessed before and after intervention using Brief Fatigue Inventory (BFI) in both groups. To analyze data, the Student’s t-test and the Chi-square test were used at a significance level of P ≤ .05. Results: The BFI score was similar before the intervention in both groups, however, after the intervention, it was significantly lower in the melatonin group ( P < .001). Moreover, the frequency of severe fatigue in the melatonin group was significantly lower than in the placebo group after intervention (42.1% vs 83.3%, P < .001). Conclusion: Coadministration of melatonin during adjuvant chemotherapy and radiotherapy of women with breast cancer decreased the levels of fatigue associated with the malignant condition and its treatments.

ABSTRACT Objective: To correlate low back pain caused by fatty degeneration, visualized by magnetic resonance imaging (MRI), with sex and age. Methods: With a timeframe of 2015 to 2020, data on sex and age of fifty symptomatic patients with low back pain were collected from medical records and correlated with MRI analyses showing the occurrence of fatty infiltration in the paraspinal and erector muscles. Three trained observers, using the classification of Goutallier et al. (1994), analyzed five lumbar levels and the degree of injury. The Kolmogorov-Smirnov, Shapiro Wilk, Student's t, Mann-Whitney and Spearman correlation tests were used, all with a 5% significance level. Results: Among the cases, the L5-S1 level was found in all the images, with a prevalence of 46% of grades 1 and 2 and 24% in the five lumbar levels. Severity was present in a small portion (8%) of the injuries. There was a predominance of 78% women, with significantly more severe injuries in relation to the number of levels (p < 0.001) and injuries of a higher degree (p < 0.001). The age range was 14 to 38 years (mean = 26.70 ± 5.70 years), showing a significant and moderate correlation between the number of levels reached and a greater degree of degeneration (p < 0.001). Conclusion: Fatty degeneration as a cause of low back pain was significantly more severe in women in terms to the number of levels and the higher degree of injury. The number of levels and the severity of fatty degeneration were not correlated with the patient's age. Level of evidence II; Retrospective Study.

Aim: To investigate the percentage of correctness (PC) of the Nasal Index (NI) in human skeletons for determination of sex, ancestry and estimation of age in the Brazilian population. Methods: This was a cross-sectional study of 173 human skulls belonging to the Piracicaba Dental School. 93 skeletons were males and 80 females; 34 were aged up to 39 years, 56 between 40 and 59 years, and 83 60 years or older; 96 were from white individuals, 49 were from mixed-race and 28 from black individuals. High-precision digital caliper was used to measure nasal height (NH=ANS-nasion) and the maximum nasal width (NW) values, which were applied into the formula NI=NW/NHx100. The data were submitted to discriminant analysis and Student’s t test with equal variances, Mann-Whitney, F (ANOVA), Tukey and Kruskal Wallis, 5% significance level. Results: Dominant nasal type in the Brazilian population was the mesorine. Males showed nasal height and width values greater than those of females, with statistically significant differences in all measurements (p≤0.021) and PC of 76.6%. Similar nasal measurements were found regardless of age (p>0.05), with PC of 41.7%. Ancestral analysis revealed that black individuals have greater nasal width (26.35) and nasal index (53.67) than white ones (24.60 and 49.25), while mixed-race individuals showed intermediate values (25.36 and 52.13) (p<0.05). Nevertheless, these measurements presented an estimated PC of 54.3%. Conclusion: The Nasal Index can be better used for sex determination than for estimation of age and ancestry in the Brazilian population, as the latter showed intermediate and low percentage of correctness, respectively.

ABSTRACT Objectives: to identify the use of religious/spiritual coping in informal caregivers of children with cleft lip and/or palate, dysphagic, powered exclusively by probe. Methods: descriptive and cross-sectional study, including 30 informal caregivers. For data collection, a Sociodemographic Questionnaire and the Brief Religious/Spiritual Coping Scale were used. For statistical analysis, ANOVA, Student’s t and Spearman’s correlation tests were used, with a significance level of 5%. Results: the use of religious/spiritual coping was high among participants (mean=3.71), with a predominance of positive (mean=3.30) compared to negative (mean=1.88). It was evident that the greater the reported importance of religiosity/spirituality in the participants’ lives, the greater the positive coping (p=0.001). Conclusions: informal caregivers used positive religious/spiritual coping as a way of coping with care demands related to the child’s health condition. These findings point to the importance of including spirituality/religiosity as health indicators.

Statement of the problem. Based on the analysis of research by Russian authors, the article outlines the problem of the formation of emotional decentration in preschool children and the importance of play in this process. The purpose of the article is to identify the relationship between the formation of a role-playing game and the formation of emotional decentration. The research methodology is based on the cultural and historical concept of L.S. Vygotsky, as well as the theory of the activity by A.N. Leontiev; in the course of the study, the following methodology was applied: analysis of theoretical sources on formation of the emotional sphere of a preschooler and development of role-playing games; standardized observation of the progress of specially organized games; ascertaining experiment to determine the level of emotional decentration in preschoolers; formative experiment; qualitative and quantitative data analysis; comparative analysis of data using the statistical Student’s t-test. The significance level is p = 0.05. Research results. In the course of the study, our hypothesis was confirmed: such conditions for role-playing games as providing preschoolers with the opportunity to create and transform the play space with the help of unformed object material, adults acceptance of the position as a play partner contribute to the internal and external dynamics of the game, which, in turn, affects the progress in the development of emotional decentration in preschool children. Conclusion. In this paper, the features of emotional development of preschool age were described, the concept of emotional decentration from the point of view of different authors was considered, and the process of the development of play in preschool age was described. A scheme has been developed to determine the level of emotional decentration in preschoolers. In this scheme, three levels of formation of emotional decentration in children 4-6 years old are identified (relative to three criteria as indicators of emotional decentration): high, medium and low. A diagnostic game procedure “Wishes of a Fairy-Tale Character” has been developed, which can be used to identify the level of formation of emotional decentration. A scheme for analyzing the internal and external dynamics of a role-playing game has been developed.

Objectives: identify the incidence and characterize pressure injuries in an adult intensive care unit regarding the occurrence, locations and risk factors, and verify whether there is an association between these and the appearance of the injuries. Method: observational, cohort, prospective study, developed in an intensive care unit of a tertiary hospital, from October to December 2019. The population consists of adults on the first day of admission to the unit, without pressure injury at admission. Participants were monitored during hospitalization, sociodemographic and clinical variables, and risk assessment of developing a pressure injury, skin assessment and Braden scale were collected daily. Pearson’s chi-square tests and student’s t-test were used to assessing the relationship between variables and injuries. For the analyzes, a significance level (α) of 5% was considered. Results: 40 participants were included, 20% had pressure injuries with a predominance of stages 1 and 2; the main affected sites were the sacral region followed by the calcaneus. The average hospital stay was 23.38 days for the injured group and 5.77 days for the non-injured group; time showed a significant relationship with the appearance of lesions (p = 0.002). Conclusion: it was concluded that the most affected site was the sacral region and grade 1 was the most frequent staging; length of stay was the variable that influenced the appearance of injuries.
