Burnout in frontline ambulance staff

Background: Staff retention is a significant issue for ambulance services across the globe. Exploratory research, although minimal, indicates that stress and burnout, in particular, influence attrition within the paramedic profession. These need to be understood if their impact on retention is to be addressed. Aims: To determine the presence of and contributory factors for burnout in the ambulance service to inform recommendations for positive change. Methods: A two-phased survey approach was adopted using an adapted Maslach Burnout Inventory and Copenhagen self-assessment burnout questionnaire, to measure levels of burnout, depersonalisation (cynicism) and personal achievement. Open-ended questions explored factors that influenced these. Demographic and comparative analysis identified trends and thematic analysis was carried out on the qualitative data. Results: Ninety-four per cent of ambulance staff in this study (n=382) reported a sense of personal achievement within their professional role; however, more than 50% were experiencing varying levels of burnout with 87% displaying moderate or high levels of depersonalisation towards their work. Causes of stress were complex: themes attributed were a perceived lack of management support, the public's misuse of the ambulance service, involuntary overtime and a poor work-life balance. Conclusions: Burnout poses a genuine threat to retention in the ambulance service and needs addressing. Proactive screening, better communication between practice staff and management and access to counselling services are recommended. This problem of burnout is beginning to be acknowledged but further evidence is needed to understand it in more depth in order for effective solutions to be developed.

The link between dairy farm systems and cost of environmental compliance is not always clear. A survey of Waikato dairy farmers was conducted to establish the real (non-modelled) cost of compliance with environmental regulation in the region. Quantitative and qualitative data were gathered to improve understanding of compliance costs and implementation issues for a range of Waikato farm systems. The average oneoff capital cost of compliance determined through a survey approach was $1.02 per kg milksolids, $1490 per hectare and $403 per cow. Costs experienced by Waikato farmers have exceeded average economic farm surplus for the region in the past 5 years. As regulation increases there are efficiencies to be gained through implementing farm infrastructure and farm management practice to best match farm system intensity. INTRODUCTION: Small-Medium Enterprises (SMEs) play a vital role in economic development of Indonesia.  To support its economic development, the government is obliged to generate revenue.  In order to generate more revenue, Indonesian government implemented self- assessment system in compliance with tax obligations.  Most of the SMEs are compliant while some are not that aware of it because of lack of socialization.  The aim of this study is to comprehend the effect of implementing self-assessment system on tax compliance.   METHOD:  This study focused on SMEs in Lembang, West Java, Indonesia.  It was exploratory research based on data were gathered from West Java Regency Office.  Questionnaires were distributed to 70 (based on Slovin formula) respondents from the total population of 244.   RESULTS: The result showed that the correlation of implementation of self-assessment system on tax compliance is weak, thus, the findings showed that implementation of self-assessment system has no significant influence on tax compliance.         DISCUSSION AND RECOMMENDATION: Implementation of self-assessment system has no significant influence on tax compliance.  There are several reasons for this, such as, most of the people around the district of Lembang are not yet fully aware about its implementation. Also, it is not well socialized by the tax authorities from the Revenue Offices.  It lacks personnel to do the socialization.  The stated reasons were affirmed during the interview with the Head of SMEs located in West Bandung Regency.   Implementation of the Self - Assessment System is worth to grab by SMEs in the district of Lembang, Bandung, Indonesia. It is highly recommended to socialize again the implementation and assign personnel that can extend time and effort to socialization.                Keywords:  SMEs, revenue, self-assessment, socialization, compliance

The socio-economic characterisation of resettled small-holder farmers under the Fast Track Land Reform Programme (FTLRP) in Zimbabwe has blind spots in relation to the emergent transformative social policy features such as ‘social cohesion’, ‘cooperation’, ‘protection’ and ‘accumulation’ which are equally important among land beneficiaries. Using the Sangwe farm, this article departs from the conventional use of the political economy, sustainable livelihoods, human rights-based and neo-patrimonial approaches to experiment with the transformative social policy approach. Using both quantitative and qualitative data in an exploratory research design, the article shows that viewed from the transformative social policy approach, the FTLRP was neither a resounding success nor a complete disaster. The programme actually produced mixed results. The article thus, recommends the use of in-depth, ideologically free and neutral approaches in its analysis so as to reveal its detailed outcomes. Additional studies in which existing land reform policies can be considered in the collective efforts of improving the transformative agenda of the FTLRP across the country are needed.

Objective: To determine burnout prevalence in Canadian radiology residency and identify contributing factors. Materials and Methods: A prospective 57-item survey, including the 22-item Maslach Burnout Inventory-Health Sciences Survey, was sent to all Canadian radiology residents, with a total resident population of 359. The association between emotional exhaustion (EE), depersonalization (DP), and personal achievement (PA) scores with items in the survey was performed. Continuous data were evaluated using the Student t test for comparing the means between the 2 groups or the analysis of variance test for comparing the means between at least 3 groups. Spearman correlation coefficient was performed when evaluating ordinal categorical data. Results: Response rate is 40.1% (n = 144); 50.7% of residents demonstrate high EE, 48.6% demonstrate high DP, and 35.9% demonstrate low PA. Being unhappy with residency and with radiology as a career is associated with burnout ( P < .001). Age, sex, marital status, and children have no impact on burnout. More hours worked is associated with higher EE ( P = .025) and DP ( P = .004). In all, 47.2% residents experienced intimidation or harassment. Feeling unsupported by staff radiologists is associated with higher EE ( P < .001), higher DP ( P = .001), and lower PA ( P = .008). In all, 45.1% of residents have poor work–life balance, and those residents demonstrate higher EE ( P < .001), higher DP ( P = .006), and lower PA ( P = .01). In all, 25% of residents identify poor education-service balance in their residency, and those residents have higher EE ( P < .001), higher DP ( P = .042), and lower PA ( P = .005). Conclusion: This study demonstrates significant burnout in Canadian radiology residents with major contributory factors identified.

Abstract Background patient self-assessment using electronic tablet could improve the quality of assessment of older Emergency Department(ED) patients. However, the acceptability of this practice remains unknown. Objective to compare the acceptability of self-assessment using a tablet in the ED to a standard assessment by a research assistant (RA), according to seniors and their caregivers. Design randomised crossover pilot study. Setting The Hôpital de l’Enfant-Jésus (CHU de Québec–Université Laval) (2018/05–2018/07). Subjects (1) ED patients aged ≥65, (2) their caregiver, if present. Methods participants’ frailty, cognitive and functional status were assessed with the Clinical Frailty scale, Montreal Cognitive Assessment, and Older American Resources and Services scale and patients self-assessed using a tablet. Test administration order was randomised. The primary outcome, acceptability, was measured using the Treatment Acceptability and Preferences (TAP) scale. Descriptive analyses were performed for sociodemographic variables. TAP scores were adjusted using multivariate linear regression. Thematic content analysis was performed for qualitative data. Results sixty-seven patients were included. Mean age was 75.5 ± 8.0 and 55.2% were women. Adjusted TAP scores for RA evaluation and patient self-assessment were 2.36 and 2.20, respectively (P = 0.08). Patients aged ≥85 showed a difference between the TAP scores (P < 0.05). Qualitative data indicates that this might be attributed to the use of technology. Data from nine caregivers showed a 2.42 mean TAP score for RA evaluation and 2.44 for self-assessment. Conclusions our results show that older patients believe self-assessment in the ED using an electronic tablet as acceptable as a standard evaluation by a research assistant. Patients aged ≥85 find this practice less acceptable.

In view of the negative impact of pain on the quality of life of cancer patients, identifying and stimulating the use of effective strategies to minimize these painful feelings is highly relevant for care. The aim of this study was to identify painful experiences of adolescents with cancer and to get to know their strategies for pain relief. This is an exploratory research, using qualitative data analysis. Semistructured interviews were held with nine adolescents with cancer, who reported on acute, recurring and chronic, physical and emotional painful experiences. To relieve these pains, they described pharmacological and non-pharmacological strategies, including: distraction, presence of relatives, bed positioning, cooperation to accomplish procedures and keeping up positive thinking. Therefore, it is essential for health professionals to know available evidence for pain relief and to develop skills to articulate this knowledge with their professional experience and with the patients' own strategies.

Abstract Aim Core Surgical Training (CST) programs are associated with a 60% risk of burnout and failure to progress. This study aimed to assess the influence of a novel Enhanced Stress and Resilience Training (ESRT) course delivered alongside a focused induction at the start of CST in a single UK Statutory Education Body. Method All CST1s were invited to participate in a six-week ESRT course teaching mindfulness-based exercises to develop tools to deal with stress at work and burnout. Primary outcome measures were compliance with ESRT programme and degree of burnout measured using Maslach Burnout Inventory (MBI) scoring. Results Of 43 boot camp attendees, 38 trainees completed questionnaires, with 24 choosing to participate (63.2%; male 13, female 11, median age 28 yr.) Burnout was identified in 23 trainees (60.5%) with no evident difference in baseline MBI scores between participants or not (p = 0.77, median 4 [0-11] vs. 4 [1-11]). MBI scores were comparable before and after ESRT in the intervention cohort (p = 0.63, median 4 [0-11] vs. 4 [1-10]). Qualitative data reflected challenges delivering ESRT because of arduous and inflexible clinical on-call rotas and time pressures related to academic curriculum demands. Despite these challenges, 22 (91.6%) considered the course valuable, 14 (58.3%) continued to use the techniques in daily clinical work there was and unanimous support to continue the program in future. Conclusion ESRT was feasible and further research is warranted to determine its medium-term efficacy.

This article examines men’s involvement in an institutional gender equity award scheme and how their self-concept as allies develops over time. It draws specifically on a subset of qualitative data from the four men participating in a study involving in-depth interviews with university staff involved in the self-assessment team of one Australian institution’s Science in Australia Gender Equality (SAGE) Athena SWAN pilot. Data related to the men’s experiences is the article’s focus. Key themes from the data include: 1) men’s motivations for engagement; 2) men’s self-understandings as ‘champions for change’ 3) the barriers/risks associated with male championship; and 4) men’s evolving perceptions and critiques of the male champions model. Findings show that men demonstrated personal growth and increased awareness through their participation in the pilot. Yet, their frustration with how equity and diversity was managed in their organisational context highlights pitfalls in the concept of a male ‘champion’. This article provides timely guidance for institutions seeking to engage allies in gender equity initiatives.

The topic of alcohol consumption and conflicts has been explored in several context not only because consumers do not have a good consumption culture, but also because alcohol consumption often leads to social violence (Nasse, 2018; Nasse, Ouédraogo & Diop, 2019). Most of the studies on the topic are done in other contexts. Little research on the subject has been conducted in Burkina Faso. Therefore, this research aims to explore the concepts of alcohol consumption and conflicts in the context. Burkina Faso is a country where religion plays a significant role. However, alcohol consumption is increasing (Sow, 2005), even among consumers who are believers, but ignorance in the sector of marketing seems to be a barrier that favors unethical consumption. The research is a mixed approach. The qualitative data collection is performed with a voice recorder by the means of some semi-structured interviews. Then, the qualitative data is transcribed by hand. The quantitative data is collected by the means of a questionnaire. The qualitative data and the quantitative data are performed using sphinx IQ software. The results show that social class moderates the relationship between alcohol consumption and conflicts.

This paper focuses on a new approach for facilitating the participation of stakeholders in a process of strategic planning known as open strategic planning (OSP). OSP is recognised through three characteristics: inclusiveness, transparency, and the use of information technology (IT) tools. Drawing on the theoretical foundations of OSP, the research explores moderating factors impacting the relationship between these characteristics and OSP effectiveness by referring to qualitative data obtained from two open strategic planning projects. A secondary aim was to examine how stakeholders interpret the effectiveness of OSP in an organisational setting. Results indicate various moderating factors (level of trust, IT literacy, and diversity of participants) impacted the relationship between the characteristics of OSP and strategic planning effectiveness. The study formulates eight propositions, each is discussed in relation to the existing literature on strategic planning effectiveness. This paper is significant as it is the first exploratory research linking openness and strategic planning outcomes.
