Background The World Health Organization (WHO) recommends one ultrasound scan before 24 weeks gestation as part of routine antenatal care (WHO 2016). We explored influences on provision and uptake through views and experiences of pregnant women, partners, and health workers. Methods We undertook a systematic review (PROSPERO CRD42021230926). We derived summaries of findings and overarching themes using metasynthesis methods. We searched MEDLINE, CINAHL, PsycINFO, SocIndex, LILACS, and AIM (Nov 25th 2020) for qualitative studies reporting views and experiences of routine ultrasound provision to 24 weeks gestation, with no language or date restriction. After quality assessment, data were logged and analysed in Excel. We assessed confidence in the findings using Grade-CERQual. Findings From 7076 hits, we included 80 papers (1994–2020, 23 countries, 16 LICs/MICs, over 1500 participants). We identified 17 review findings, (moderate or high confidence: 14/17), and four themes: sociocultural influences and expectations; the power of visual technology; joy and devastation: consequences of ultrasound findings; the significance of relationship in the ultrasound encounter. Providing or receiving ultrasound was positive for most, reportedly increasing parental-fetal engagement. However, abnormal findings were often shocking. Some reported changing future reproductive decisions after equivocal results, even when the eventual diagnosis was positive. Attitudes and behaviours of sonographers influenced service user experience. Ultrasound providers expressed concern about making mistakes, recognising their need for education, training, and adequate time with women. Ultrasound sex determination influenced female feticide in some contexts, in others, termination was not socially acceptable. Overuse was noted to reduce clinical antenatal skills as well as the use and uptake of other forms of antenatal care. These factors influenced utility and equity of ultrasound in some settings. Conclusion Though antenatal ultrasound was largely seen as positive, long-term adverse psychological and reproductive consequences were reported for some. Gender inequity may be reinforced by female feticide following ultrasound in some contexts. Provider attitudes and behaviours, time to engage fully with service users, social norms, access to follow up, and the potential for overuse all need to be considered.

The World Health Organization (WHO) states that maternal mortality worldwide due to complications during pregnancy and childbirth in 2017 is estimated at around 810 cases. Between 2000 and 2017 there was a decline in the ratio of MMR around the world by 38%. WHO also states that 94% of global maternal deaths occur in low and middle income countries. In Indonesia, maternal deaths due to complications from pregnancy or childbirth every year are estimated at 20,000 mothers died from five million births. Delivery assistance by trained health workers in health facilities can be an effort to reduce MMR and IMR. In addition, awareness of pregnant women is also important for the importance of having a pregnancy with a health worker. This community service aims to increase the knowledge of pregnant women about anemia and to detect early pregnancy complications by laboratory examinations. The target of this activity is 15 pregnant women. The dedication activity is conducting educational activities, laboratory examinations in Tambakreja Village, Cilacap Regency. Based on the results of this activity it was concluded that there was an increase in knowledge of pregnant women about anemia and found 2 pregnant women experiencing anemia from 15 pregnant women and urine examination found all negative pregnant women

Sub-Saharan Africa, has the highest child mortality rate in the world (World Health Organization [WHO], 2016). However, there is a paucity of current systematic reviews on the impact of essential newborn care interventions in Africa. Therefore, the aim of this systematic review was to summarize evidence about the impact of essential newborn care interventions in Africa. Numerous databases were searched to retrieve articles that reported interventions in newborn care in Africa. The search was limited to the English language and to articles published between 2007 and 2017. Nine articles were selected for inclusion in this systematic review. Overall, these papers demonstrated an increase in performance of health workers (between 8 and 400%) following a test of knowledge, while health workers practical performance increased by 34%. Moreover, neonatal mortality was reduced by 45%, while perinatal mortality was reduced by 30%. Training healthcare workers is one of the most effective ways of improving newborn care and neonatal survival in Africa. However, there is a need for additional evidence to support this, because none of the reviewed studies assessed the impact of training by examining variables such as trainees' satisfaction with training, the knowledge and skills developed, and the health outcomes achieved.

The global adult lifetime risk of maternal mortality is 1 in 180; in Pakistan, it is 1 in 170; in developed regions, 1 in 4,900 (Alkema et al., 2016; Filippi, Chou, Ronsmans, Graham, & Say, 2016; World Health Organization [WHO], 2015). The differences in maternal mortality between developed and developing countries are mainly due to the quality of antenatal care (ANC) available in the two groups of countries. The purpose of this study was to assess the structural and procedural quality of ANC services provided and to assess satisfaction levels of women receiving ANC services in two large hospitals in Islamabad, Pakistan. A cross-sectional survey was conducted at the hospitals' outpatient maternal and child health clinics, with a random sample of 138 women. The overall quality of ANC was rated as good (61%), average (17.5%), or poor (17.5%). The findings suggest a need to cultivate quality of care at public health facilities, train health workers in communication skills, and build technical capacity by continuing education and supportive supervision to train health-care providers to follow standard protocols for provision of quality ANC services.

Objectives: Despite the growing interest and efforts by government to make popular use of antenatal care (ANC) services in Nigeria as recommended by the World Health Organization, high level of infant and maternal mortality remains a major public health challenge facing the country. Dissatisfaction toward ANC services among pregnant women may be attributed to low level of awareness. This study assessed the level of awareness and satisfaction of ANC services among pregnant women in Lagos state, Nigeria. Material and Methods: The study adopted a survey research design. A multistage sampling technique was utilized to recruit participants for this study. A validated questionnaire was used for data collection and data were analyzed using both descriptive and inferential statistics. Ethical approval was obtained from Babcock University Health Research Ethics Committee with approval no: BUHREC543/17. Approval was also obtained from health service commission and in the six general hospitals used for the study. Informed consent was taken and respondents were reassured of the privacy and confidentiality of the information obtained. Results: The results showed that most of the respondents (85.6%) were in their reproductive years, that is, ages 23–37. The results showed that the level of awareness had a significant influence on pregnant women’s satisfaction with the services (β = 0.460, F(1,1313) = 351.499, R2 = 0.211, P < 0.05). The level of awareness of ANC services was high (M = 4.31, SD = 1.01) on a scale of 5. Conclusion: The study concluded that awareness of ANC services positively impacts pregnant women’s satisfaction with the services in Lagos state. Efforts should be made to improve the level of awareness of pregnant mothers to achieve greater satisfaction with ANC services in Lagos state.

Kematian maternal menurut World Health Organization (WHO) mencapai 585.000 jiwa setiap tahun. Faktor predisposisi yang menyebabkan hiperemesis gravidarum antara lain paritas, usia kehamilan, riwayat kehamilan, umur ibu, pekerjaan, pendidikan, riwayat penyakit ibu, psikologis, pengetahuan, dan sosial budaya. Tujuan penelitian ini untuk mengetahui karakteristik ibu hamil yang mengalami hiperemesis gravidarum di Rumah Sakit Bhayangkara Palembang tahun 2017. Dengan menggunakan metode deskriptif yaitu untuk mendeskripsikan variabel paritas, usia kehamilan, riwayat kehamilan, umur ibu, pekerjaan, dan pendidikan ibu hamil yang mengalami hiperemesis gravidarum di Rumah Sakit Bhayangkara Palembang tahun 2017. Populasi dalam penelitian ini adalah semua ibu hamil yang mengalami hiperemesis gravidarum yang pernah dirawat di Rumah Sakit Bhayangkara Palembang tahun 2017 yang berjumlah 17 orang. Sampel penelitian diambil dengan menggunakan metode Porpusive Sampling. Hasil penelitian menunjukkan paritas resiko tinggi lebih sedikit (11,8%) dibandingkan paritas resiko rendah (88,2%). Usia kehamilan Trimester I lebih banyak (82,4%) dibandingkan Trimester II (17,6%). Ibu dengan molahidatidosa dan gemeli lebih sedikit (23,5%) dibandingkan dengan ibu yang tidak pernah mengalami molahidatidosa dan gemeli (76,5%). Umur resiko tinggi lebih sedikit (29,4%) dibandingkan dengan resiko rendah (70,6%). Ibu yang bekerja lebih sedikit (41,2%) dibandingkan dengan yang tidak bekerja (58,8%). Ibu yang berpendidikan tinggi lebih banyak (64,7%) dibandingkan dengan yang berpendidikan rendah (35,3%). Diharapkan kepada petugas kesehatan  dapat memberikan informasi dan penyuluhan tentang hiperemesis gravidarum dan bagi peneliti selanjutnya agar meneliti dengan metode yang berbeda serta sampel yang lebih banyak.   ABSTRACT Maternal mortality according to the World Health Organization (WHO) reach 585,000 inhabitants every year. Predisposing factors that cause hiperemesis gravidarum, among others, gestational age, parity, maternal age, pregnancy history, employment history, education, maternal diseases, psychological, social, and cultural knowledge. The purpose of this research is to know the characteristics of pregnant women who experience hiperemesis gravidarum Bhayangkara Hospital in Palembang year 2017. By using descriptive method to describe the variable parity, gestational age, pregnancy history, age, occupation, and education of pregnant women who experience hiperemesis gravidarum Bhayangkara Hospital in Palembang year 2017. The population in this research is all the pregnant women who experience hiperemesis gravidarum ever hospitalized Bhayangkara Palembang 2017 year totalling 17 people. Research samples taken using Porpusive method of Sampling. The results showed fewer high risk parity (11.8%) than low risk parity (88.2%). Pregnancy Trimester I more (82.4%) than a Trimester II (17.6%). Mom with molahidatidosa and fewer gemeli (23.5%) compared with mothers who've never experienced molahidatidosa and gemeli (76.5%). High risk age less (29.4%) compared to the low risk (70.6%). Mothers work less (41.2%) compared to not working (58.8%). Mothers who are highly educated more (64.7%) compared to the low educated (35.3%). Expected to health workers can provide information and guidance about the hiperemesis gravidarum and for subsequent researchers in order to examine different methods and samples more.

Abstract Background Antenatal care (ANC) is crucial for the health of the mother and unborn child as it delivers highly effective health interventions that can prevent maternal and newborn morbidity and mortality. In 2002, the World Health Organization (WHO) recommended a minimum of four ANC visits for a pregnant woman with a positive pregnancy during the entire gestational period. Tanzania has sub-optimal adequate (four or more) ANC visits, and the trend has been fluctuating over time. An understanding of the factors that have been contributing to the fluctuating trend over years is pivotal in increasing the proportions of pregnant women attaining adequate ANC visits in Tanzania. Methods The study used secondary data from Tanzania Demographic Health Survey (TDHS) from 2004 to 2016. The study included 17976 women aged 15-49 years. Data were analyzed using Stata version 14. Categorical and continuous variables were summarized using descriptive statistics and weighted proportions. A Poisson regression analysis was done to determine factors associated with adequate ANC visits. To determine factors associated with changes in adequate ANC visits among pregnant women in Tanzania from 2004 to 2016, multivariable Poisson decomposition analysis was done. Results The overall proportion of women who had adequate ANC visits in 2004/05, 2010 and 2015/16 was 62, 43 and 51% respectively. The increase in the proportion of women attaining adequate ANC from 2010 to 2015/16 was mainly, 66.2% due to changes in population structure, thus an improvement in health behavior. While 33.8% was due to changes in the mother’s characteristics. Early initiation of first ANC visit had contributed 51% of the overall changes in adequate ANC attendance in TDHS 2015/16 survey. Conclusion Early ANC initiation has greatly contributed to the increased proportion of pregnant women who attain four or more ANC visits overtime. Interventions on initiating the first ANC visit within the first twelve weeks of pregnancy should be a priority to increase proportion of women with adequate ANC visit.

The basic component of the new World Health Organization (WHO) antenatal care model prescribes reduced number of clinic visits and limited investigations for low-risk pregnant women. The objectives of this study were to determine the proportion of pregnant women seeking antenatal care in a Nigerian teaching hospital who qualify for the basic component and to document difficulties that may arise with the classifying form. In December 2004, 234 pregnant women who had initiated antenatal care were enrolled for the study. Using the classifying form, 157 (67%) were eligible for the basic component, 41 (18%) for special care, but 36 (15%) women could not be classified. Those that did not know the birth weight of their last babies accounted for most (89%) of the unclassified group. The WHO antenatal care model was the most appropriate and relevant method for our hospital where a large percentage (67%) of prenatal women were eligible for the basic component. However, we consider that the classifying form should be adapted to accommodate all pregnant women.

Background Sustainable Development Goals (SDG) has set the target to reduce premature mortalities from non-communicable diseases (NCDs) by one-third. One of the ways to achieve this is through strengthening the countries’ implementation of the World Health Organization Framework Convention on Tobacco Control (WHO FCTC). Community health workers (CHWs) involvement has shown promising results in the prevention of NCDs. This systematic review is aimed at critically evaluating the available evidence on the effectiveness of involving CHWs in smoking cessation. Materials and methods We systemically searched PubMed and CENTRAL up to September 2019. We searched for published interventional studies on smoking cessation interventions using the usual care that complemented with CHWs as compared to the usual or standard care alone. Our primary outcome was abstinence of smoking. Two reviewers independently extracted data and assessed study risks of bias. Result We identified 2794 articles, of which only five studies were included. A total of 3513 smokers with 41 CHWs were included in the studies. The intervention duration range from 6 weeks to 30 months. The studies used behavioral intervention or a combination of behavioral intervention and pharmacological treatment. Overall, the smoking cessation intervention that incorporated involvement of CHWs had higher smoking cessation rates [OR 1.95, 95% CI (1.35, 2.83)]. Significant smoking cessation rates were seen in two studies. Conclusion Higher smoking cessation rates were seen in the interventions that combined the usual care with interventions by CHWs as compared to the usual care alone. However, there were insufficient studies to prove the effectiveness. In addition, there was high heterogeneity in terms of interventions and participants in the current studies.

Abstract Purpose To describe indicators used for the assessment of antenatal care (ANC) quality worldwide under the World Health Organization (WHO) framework and based on a systematic review of the literature. Data sources Searches were performed in MEDLINE, SciELO, BIREME and Web of Science for eligible studies published between January 2002 and September 2016. Study selection Original articles describing women who had received ANC, any ANC model and, any ANC quality indicators were included. Data extraction Publication date, study design and ANC process indicators were extracted. Results of data synthesis Of the total studies included, 69 evaluated at least one type of ANC process indicator. According to WHO ANC guidelines, 8.7% of the articles reported healthy eating counseling and 52.2% iron and folic acid supplementation. The evaluation indicators on maternal and fetal interventions were: syphilis testing (55.1%), HIV testing (47.8%), gestational diabetes mellitus screening (40.6%) and ultrasound (27.5%). Essential ANC activities assessment ranged from 26.1% report of fetal heart sound, 50.7% of maternal weight and 63.8% of blood pressure. Regarding preventive measures recommended by WHO, tetanus vaccine was reported in 60.9% of the articles. Interventions performed by health services to improve use and quality of ANC care, promotion of maternal and fetal health, and the number of visits to the ANC were evaluated in 65.2% of the studies. Conclusion Numerous ANC content indicators are being used to assess ANC quality. However, there is a need to use standardized indicators across countries and efforts to improve quality evaluation.

ABSTRAK Menurut World Health Organization (WHO) menunjukkan bahwa kematian akibat Tetanus Neonatorum di Negara-negara berkembang adalah 135 kali  lebih tinggi daripada Negara maju. Tahun 2011 dilaporkan kasus Tetanus di seluruh dunia sebanyak 14.132 kasus dengan kematian terjadi pada 61.000 ibu hamil. Di Indonesia, masih ditemukan banyak  kasus tetanus neonatorum terutama daerah  dengan cakupan persalinan oleh  tenaga kesehatan yang rendah. Tujuan penelitian mengetahui hubungan sikap, dukungan keluarga dan peran petugas kesehatan dengan pemberian  imunisasi tetanus       toxoid     pada ibu hamil di Puskesmas Sekip Palembang Tahun 2018. Penelitian jenis penelitian adalah bersifat kuantitatif dengan menggunakan metode survey analitik dengan pendekatan cross sectional. Subjek penelitiannya adalah semua ibu hamil K1 yang datang ke Puskesmas Sekip menggunakan kuisioner dengan jumlah 67 responden. Hasil penelitian didapatkan ibu hamil dengan sikap positif sudah mengerti akan tujuan/manfaat dan efek samping mengenai pemberian imunisasi Tetanus Toxoid pada ibu hamil sehingga ibu hamil cenderung  bersikap positif. ibu hamil dan orang orang yang berhubungan dekat dengan ibu hamil yang memberikan dukungan positif cenderung dilakukan imunisasi. Peran petugas kesehatan yang memiliki tugas untuk meningkatkan kesehatan masyarakat, termasuk juga petugas kesehatan imunisasi Tetanus Toksoid yang memiliki tugas untuk mengajak ibu hamil cenderung melakukan imunisasi agar terhindar dari penyakit infeksi tetanus yang dapat menyebabkan kematian bagi ibu dan bayi yang tidak melakukan  imunisasi Tetanus selama kehamilan. The research objective was to find out the relationship between attitudes, family support and the role of health workers by giving tetanus toxoid immunization to pregnant women at Palembang Sekip Health Center in 2018. The research was quantitative in nature using analytical survey method with cross sectional approach. The research subjects were all K1 pregnant women who came to the Sekip Health Center using a questionnaire with a total of 67 respondents. The results showed that pregnant women with a positive attitude already understood the purpose / benefits and side effects of giving Tetanus Toxoid immunization to pregnant women so that pregnant women tended to be positive. pregnant women and people who are in close contact with pregnant women who provide positive support tend to be immunized. The role of health workers who have the duty to improve public health, including health workers Tetanus Toksoid immunization who has the task of inviting pregnant women to do immunizations to avoid tetanus infection which can cause death for mothers and infants who do not do Tetanus immunization during pregnancy.  Keywords: Tetanus Toxoid immunization, attitude, family support, role of health workers
