Recently, the world literature has been actively discussing the effect of circadian rhythm and sleep disturbances on human metabolic health. One of the most socially significant causes of circadian disorders is the shift work schedule. Shift work schedules began to occur more often due to the high demand for flexibility and labor productivity in modern society. Shift work is characteristic of medical personnel, law enforcement officials, rescue services, transport, the media and others. Shift workers make up about 17% of all workers in Europe. Globally, approximately 2.5 billion people work in shifts. Shift work was identified as an important professional risk. The connection between work with rotation of day and night shifts with overweight and obesity, with circadian rhythms and sleep disorders was proved. The review systematizes information regarding the role of the shift chart in the development of obesity and the metabolic syndrome, and considers mechanisms that mediate its effect on the regulation of energy balance.

Abstract Background Approximately 17% of the European Union workforce is engaged in shift work. Shift work has been associated with a number of chronic conditions, including obesity and obesity-related metabolic diseases. The aim of this study was to explore the dietary and lifestyle behaviours of shift workers with a healthy vs. overweight/obese body mass index (BMI). Methods A cross-sectional study was conducted on 1080 shift workers using a 15-min, telephone-administered questionnaire developed from qualitative research on Irish shift workers and national dietary intake data. Demographic and work-related factors, as well as dietary and lifestyle behaviours were recorded. BMI was calculated using self-reported height and weight. Univariate and multivariate logistic regression methods were used to analyze data according to BMI category. Results Over 40% of shift workers were classified as overweight or obese. Multivariate analysis indicated that being male [P < 0.001, aOR = 2.102, 95% CI (1.62–2.73)] and middle- or older-aged were independently associated with overweight and obesity [P < 0.001, aOR = 2.44 95% CI (1.84–3.24) and P < 0.001, aOR = 2.9 95% CI (1.94–4.35), respectively]. Having a medium–high consumption of fried foods was independently associated with overweight and obesity [aOR = 1.38, 95% CI (1.06–1.8)]. Conclusions Similar to the general population, overweight and obesity were strongly associated with male sex and middle- or older-age. Male shift workers may benefit from targeted dietary and lifestyle advice specifically focused on limiting fried foods to help protect against overweight and obesity.

Shift work, especially if it involves night work and/or weekend work, may represent disadvantages for workers and their families at the level of family, social life, and health. The present study evaluates the impacts of three work shifts (morning, afternoon and night) on sleep, family and social life as well as satisfaction with work schedules. In total, 373 Portuguese textile workers participated in this study; all were men. The results indicated that night shift workers had greater sleep disturbances and less satisfaction with their work schedules. On the other hand, morning shift workers were most satisfied with family and social life in association with their work schedules. Based on the results, implications of the practice have been reflected.

ObjectivesOccupational factors, particularly night-shift work, are attracting growing interest as a possible determinant of metabolic syndrome (MetS). This study aimed to determine the association between night-shift work and MetS, and assess whether sleep quality is a mediating factor.MethodsA cross-sectional study was conducted among Malaysian manufacturing workers, aged 40–65 years old. They completed a self-administered questionnaire on sociodemographics, lifestyle and family history, and the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Waist circumference, blood pressure, fasting blood sugar, triglycerides and high-density lipoprotein levels were measured. Baron and Kenny’s method, Sobel test and multiple mediation models with bootstrapping were used to determine whether the PSQI global score or its components mediated the association between night-shift work and MetS.ResultsOf the 494 participants, 177 (36%) worked night shift and 51% were men. The prevalence of MetS was 37%. Night-shift work was independently associated with a twofold increase in the risk of MetS (adjusted OR: 1.92, 95% CI 1.24 to 2.97). However, the association between night-shift work and MetS did not appear to be modified by sex. Night-shift workers also reported significantly poorer sleep quality, longer sleep latency, shorter sleep duration, sleep disturbances and daytime dysfunction. Robust mediation analysis nonetheless showed that neither PSQI global score nor its components mediated the association between night-shift work and MetS.ConclusionEarly screening and management of MetS and the development of programmes to improve sleep quality should be carried out among night-shift workers. Future research should investigate other modifiable mediators linking night-shift work and MetS.

The aim of this study was to explore the effect of work related problem on shift workers’ attitude, aptitude and job satisfaction. A total of 240 shift workers in five Ferro-alloy industries of Orissa working in rotating three shift systems were participated in this study. The findings indicated that shift work has major adverse impact on psychological, social, family and conjugal life of shift worker. It has also seen that the shift work schedule curtails leisure activities, affects sleep and causes various health problems.

The present review has the objective of summarising chronobiological aspects of shift work and obesity. There was a systematic search in PubMed databases, using the following descriptors: shift work; obesity; biological clock. Shift work is extremely frequent in several services and industries, in order to systematise the needs for flexibility of the workforce, necessary to optimise productivity and business competitiveness. In developing countries, this population represents a considerable contingent workforce. Recently, studies showed that overweight and obesity are more prevalent in shift workers than day workers. In addition, the literature shows that shift workers seem to gain weight more often than those workers submitted to a usual work day. In conclusion, there is considerable epidemiological evidence that shift work is associated with increased risk for obesity, diabetes and CVD, perhaps as a result of physiological maladaptation to chronically sleeping and eating at abnormal circadian times. The impact of shift work on metabolism supports a possible pathway to the development of obesity and its co-morbities. The present review demonstrated the adverse cardiometabolic implications of circadian misalignment, as occurs chronically with shift workers.

Night shift work is associated with risk of overweight and obesity. In night shift workers, short sleep duration combined with circadian misalignment may contribute to altered food intake regulation, favoring positive energy balance and weight gain. Prior work investigating food intake in shift workers has suffered methodologically due to reliance on subjective self-report for dietary assessment. No study has yet been done to examine the impact of night shift work on food intake in real-life shift workers using objective measures. Female day (n = 12) and night (n = 12) shift workers from a hospital setting participated in a laboratory-based objective food intake assessment. Participants entered the laboratory in the fasted state after awakening from the sleep episode following a final work shift, and underwent an ad libitum 14-item test meal buffet to objectively quantify food choice/intake. Sleep duration (measured via wrist-accelerometry) during the sleep episode before laboratory assessment was significantly longer in day vs. night workers (373.9 ± 127.5 vs. 260.6 ± 102.9 min, p = 0.03). No significant group difference was observed in calories consumed during the test meal (943.08 ± 469.55 vs. 878.58 ± 442.68 kcal, p = 0.74). When expressed as percent of energy consumed, day workers had higher protein consumption vs. night workers (16.03 ± 5.69 vs. 11.82 ± 4.05%; p = 0.05). To our knowledge, this is the first laboratory-based behavioral assessment of food choice/intake in actual night and day shift workers. Although not studied here, work by others has linked protein intake to satiety. This may be a potential pathway placing shift workers at risk for overweight and obesity.

Main funding source(s): Fundação para a Ciência e Tecnologia (FCT) and Fundo Social Europeu (FSE) Introduction Modern societies function 24h/day which accounts for shift work (SW) encompassing already more than 20% of the European workforce. While the relationship between blood pressure (BP) and cardiovascular events is a continuous, hypertension (HTN) remains the major preventable cause of cardiovascular disease (CVD) in our continent. SW has been associated with sleep disturbances, circadian misalignment and unhealthy behaviours, possibly conducting to several chronic diseases. Therefore, we aimed to determine if shift workers have higher BP values and/or HTN risk. Methods This systematic review was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A literature search was performed on MEDLINE, EMBASE and Cochrane electronic databases. Included studies reported HTN diagnosis using current thresholds and/or BP values, in both shift workers and in a control group of day workers. SW was categorized in 4 subgroups, focused mostly on night work. Pooled mean difference and 95% Confidence Intervals (95%IC) were estimated for continuous outcomes, Systolic BP (SBP) and diastolic BP (DBP), and pooled Odds Ratio and 95%IC were determined for HTN risk. The Newcastle-Ottawa Quality Assessment Scale was used for quality evaluation of included studies. Results Forty-five independent studies were included engaging 117252 workers. Of these, 41 studies were included in the meta-analysis for SBP, 39 for DBP and 14 for HTN. Most studies provided cross-sectional data (n = 42), were developed in Asia (n = 21), in an industrial setting (n = 25) and included only men (n = 26). Most studies were rated as satisfactory quality (n = 23) with confounder adjustment being the weakest parameter. We found a statically significant increase for both systolic and diastolic BP among permanent night workers, 2.52mmHg [0.75–4.29] and 1.77mmHg [0.39–3.15] respectively. Amongst rotational shift workers, both with and without night work, we found a significant increase only for SBP, namely 1.28 mmHg [0.18–2.39] and 0.65 mmHg [0.07–1.22]. For HTN risk, none of SW types showed significant differences. Conclusions Certain groups of shift workers may be at special risk for increased BP. This is the first review assessing the impact of SW specifically on BP values. Although the increases were generally modest, these may play an important role among already susceptible individuals exposed overtime. The effect of SW was more consistent for SBP, which has a major impact on CVD risk. Hence, occupational health services should embrace a holistic CVD preventive approach including atypical risk factors such as SW, with active monitoring across the lifespan of more vulnerable workers.

This study aimed to determine the impact of night-shift work on health-related quality of life (HRQoL), and to assess whether sleep quality was a mediating factor.DesignA cross-sectional study.Setting11 manufacturing factories in Malaysia.Participants177 night-shift workers aged 40–65 years old were compared with 317 non-night-shift workers.Primary and secondary outcomesParticipants completed a self-administered questionnaire on socio-demographics and lifestyle factors, 12-item Short Form Health Survey V.2 (SF-12v2) and the Pittsburgh Sleep Quality Index (PSQI). The Baron and Kenny’s method, Sobel test and multiple mediation model with bootstrapping were applied to determine whether PSQI score or its components mediated the association between night-shift work and HRQoL.ResultsNight-shift work was associated with sleep impairment and HRQoL. Night-shift workers had significantly lower mean scores in all the eight SF-12 domains (p<0.001). Compared with non-night-shift workers, night-shift workers were significantly more likely to report poorer sleep quality, longer sleep latency, shorter sleep duration, sleep disturbances and daytime dysfunction (p<0.001). Mediation analyses showed that PSQI global score mediated the association between night-shift work and HRQoL. ‘Subjective sleep quality’ (indirect effect=−0.24, SE=0.14 and bias corrected (BC) 95% CI −0.58 to −0.01) and ‘sleep disturbances’ (indirect effect=−0.79, SE=0.22 and BC 95% CI −1.30 to −0.42) were mediators for the association between night-shift work and physical well-being, whereas ‘sleep latency’ (indirect effect=−0.51, SE=0.21 and BC 95% CI −1.02 to −0.16) and ‘daytime dysfunction’ (indirect effect=−1.11, SE=0.32 and BC 95% CI −1.86 to −0.58) were mediators with respect to mental well-being.ConclusionSleep quality partially explains the association between night-shift work and poorer HRQoL. Organisations should treat the sleep quality of night-shift workers as a top priority area for action to improve their employees’ overall wellbeing.

AbstractShift work is associated with circadian disruption, disturbs sleep and social life, and modifies disease risk factors, and thus can potentially contribute to various chronic diseases including cardiovascular diseases. The aim of the study was to follow cardiovascular risk in white collar workers under shift work. Cardiovascular risk was studied in sound-engineering staff in sound-recording production. The study encompassed 168 employees (46% males and 54% females) working under shift work of age 48.04 ± 8.8 years and length of service 21.6 ± 4.6 years. Our data show that 31.1% of the investigated employees were hypertensive, of which 22.6% had preliminary physician’s diagnosis of arterial hypertension, showing comparatively high rates of undiagnosed hypertension, higher in males in comparison to females. The TC and HDL-C were slightly higher with the females, while the triglycerides and TC/HDL-C ratio with the males. High rates of smoking, physical inactivity, overweight and obesity were found with both genders, slightly higher with the females. The preventive approach incorporating regular medical surveillance of shift workers and health promotion covering both life style factors and shift work organization is needed for tackling CVD in shift workers for better cardiovascular health.

Shift work disorder (SWD), which is characterized by insomnia and excessive sleepiness related with shift work, is one of the most common health problems in shift workers. Shift work disorder causes insomnia, fatigue, worse work performance, an increased likelihood of accidents, and a poor quality of life. In addition, SWD is associated with decreased productivity and increased economic costs. The correct management of SWD is important to prevent sleep disturbances and maintain work performance in shift workers. To diagnose and evaluate SWD, it is necessary to take detailed medical histories, assess the severity of sleep disturbances, and evaluate shift workers’ sleep using a sleep diary and actigraphy. The work-fitness evaluation should include recommendations on how shift workers can reduce their sleep disturbances and increase work performance, as well as the assessment of work performance. This paper reviews previous research on the evaluation, diagnosis, and management of SWD and summarizes the work-fitness evaluation of SWD.
