Diagnosis of Carpal Tunnel Syndrome: The Role of RANTES

Carpal tunnel syndrome (CTS) is regarded as most frequent entrapment neuropathy in upper extremities with high prevalence in females. It is a multi-factorial disease that had signs and symptoms due to compression of median nerve at the carpal tunnel in wrist, although may be asymptomatic. Immune system has been recognized as the main factor of neuropathic pain. Regulated on Activation, Normal T cell Expressed and Secreted (RANTES) is among the cytokines that revealed a dysregulated pattern of neuropathy specific cytokine profile in CTS and, in human, it is encoded by CCL5 gene. Sixty-four patients suffering from CTS were enrolled in this controlled cross sectional study. For all, nerve conduction study was performed to prove the diagnosis of CTS and to classify its severity. The mean age of all patients was 44.9±7.8 years, 89.1 % were female, 40.7% had mild type CTS and 40.7 % had right side CTS. 5 ml blood was obtained from each participant to measure the level of serum RANTES using specific ELISA kit. There was significant high RANTES levels in CTS patients when compared with its reference value. We conclude that RANTES level increases in patients with carpal tunnel syndrome and can be used as a predictor for its diagnosis despite its proposed neuroprotective role.

Carpal Tunnel Syndrome frequently experienced by workers whose daily activities always use repetitive movements on the wrist. Carpal Tunnel Syndrome is a syndrome that can actually be prevented because there is no risk factor that can’t be changed except the age factor. Previous research has shown a fairly high prevalence about 42%-93% in factory workers. This study aims to determine the factors that affect the occurence of Carpal Tunnel Syndrome in women workers warehouse tobacco. This is an analytical observational study with cross sectional study design which using 50 samples who met inclusion and exclusion criteria. Instrument in this research is clinical questionnaire to diagnose CTS and Phalen test. Spearman correlation test result obtained long working p=0,036,  r=0,298 and pain intensity p= <0,01, r=0,635 which means there is significant correlation with moderate strength between length of work with CTS and strong correlation between the intensity of pain with CTS. The aim of this article was to investigate the knowledge, management, and clinical practice of Italian physiotherapists concerning patients with carpal tunnel syndrome (CTS). A national cross-sectional survey consisted of 24 questions was administered from December 2019 until February 2020. A Chi-squared independence test was run to study any difference between subgroups of the sample and responses to the questionnaire. Five hundred and eight respondents completed the survey. Most respondents (n = 225/508; 44.3%) are under 29 years old, female (n = 256/508; 50.4%) and have been working as physiotherapists for less than 5 years (n = 213/508; 41.9%). Most of respondents correctly knows about the cause (n = 455/508, 89.6%), main signs and symptoms of CTS (n = 415/508, 81.70%) and administer education, manual therapy, myofascial techniques and therapeutic exercises (n = 457/508, 89.88%). Three hundred and sixty-four (71.68%) respondents were aware of the influence of psychosocial factors on the patient’s outcomes. The survey showed greater adherence to evidences by physiotherapists holding a master’s degree. The results are mostly comparable with other surveys structured all over the world on the same topic. Italian physiotherapists management of the CTS was not always in line with current evidence. Interventions such as education, manual therapy, therapeutic exercise, nerve and tendon glide techniques are widely used, while the orthotic is only offered by half of the sample.

Abstract This is a descriptive cross-sectional study designed to analyze the prevalence and severity of carpal tunnel syndrome (CTS) in addition to its association with clinical, biochemical, and imaging variables in a cohort of patients with acromegaly. Data collection was performed by entering them in our database and we subsequently analyzed in the SPSS system. Of the 71 patients registered with acromegaly, 68 met the inclusion criteria. Among the results, 97.8% presented CTS evidenced by electroconduction. Of these, 43 (63.1%) showed moderate involvement, followed by 17 severe cases (25%) and 6 mild cases (8.8%). The level of GH in serum, the years of delay in diagnosis, and the size of the adenoma were related to the presence and severity of CTS (p less than 0.05). The multivariate analysis showed that severe grade had a significant association with these variables compared with moderate and mild grade.

Objective: Measure the intratunnel pressure of carpal tunnel in patients who underwent surgical treatment for carpal tunnel syndrome in Hanoi Medical University Hospital Patients and method: Prospective cross-sectional study of 33 carpal tunnel syndrome patients (61 hands) treated surgically at Hanoi Medical University Hospital. Measure the preoperative intratunnel pressure with manometer using needle catheter with 3 positions: neutral position, 90º of flexion and 90º of extension. Results: the average intratunnel pressure was 13.87 ±1.56 mmHg, it increased when wrist at 90º of flexion and 90º of extension (27.60 ± 2.53 and 34.76 ± 3.25, respectively). Conclusion: There is a high intratunnel pressure situation in the carpal tunnel syndrome.

PROFILE OF PATIENT CARPAL TUNNEL SYNDROME IN THE NEUROLOGY CLINIC SANGLAH GENERAL HOSPITAL IN DENPASARABSTRACTIntroduction: Carpal tunnel syndrome (CTS) is the entrapment of the median nerve at the carpal tunnel.Aims: To determine the profile of patients with carpal tunnel syndrome in the Neurology Clinic Sanglah GeneralHospital in Denpasar.Methods: This is a descriptive, cross sectional study. Data was collected during a6-month period from April 2016 to September 2016.Results: Among 25 subjects, 76% were female with mean age 48+10,133 years old, housewives (40%), with working hours of more than 8 hours per day. Most subjects had symptoms on the right hand 68% in the form of tingling sensation (96%), pain (36%), numbness (48%), and hand weakness (24%). Most subject has positive Phalen test (80%), Tinnel (80%), and Flick (76%) and small number had tenar muscle atrophy (28%), impaired sensibility (32%), and decrease in muscle strength (24%). As many as 68% of subjects had distal sensoric latency of median nerve recorded on the 4th digit>3.5msec, while 72% showed ≥0.5 difference of distal sensoric latency between the median nerve and ulnar nerve, while8% showed no response.Discussion: Most patients were female, with dominant complaint was tingling, positive Phalen and Tinnel test, and68% ENMG result shown CTS.Keywords: Carpal tunnel syndrome, electroneuromyography, patient profileABSTRAKPendahuluan: Sindrom terowongan karpal (STK) adalah penjepitan N. Metode: Penelitian potong lintang terhadap pasien dengan STK yang berobat ke poliklinik saraf RSUP Sanglah,Denpasar secara konsekutif pada bulan April-September 2016.Hasil: Dari 25 subjek, 76% adalah perempuan dengan rerata usia 48+10,133 tahun, ibu rumah tangga (40%) dengan durasi kerja lebih dari 8 jam perhari. Mayoritas keluhan terjadi pada tangan kanan (68%), berupa kesemutan (96%), nyeri (36%), rasa tebal (48%), dan kelemahan otot  tangan (24%). Sebagian besar hasil positif pada tes Phalen (80%), Tinnel (80%), dan Flick (76%), serta sebagian kecil mengalami atrofi otot tenar (28%), gangguan sensibilitas (32%), dan kelemahan otot tangan (24%). Latensi distal sensorik N. Medianus pada perekaman jari IV terutama >3,5ms (68%), perbedaan latensi distal sensorik N. Medianus dan Ulnaris ≥0,5ms (72%), dan tidak ada respons 8%.Diskusi: Sebagian besar sampel adalah perempuan, gejala yang dominan berupa kesemutan, tes Phalen dan Tinnel yang positif, serta hasil ENMG 68% menderita STK.Kata kunci: Elektroneuromiografi, profil pasien, sindrom terowongan karpal

Carpal tunnel syndrome (CTS) is one of the most commonly reported nerve entrapment syndromes. There are almost 10 million cases present in India alone. It was also found women were more commonly affected compared to men. Higher prevalence rates of CTS can be found in certain occupational groups, for example, the construction workers. The present study was a cross-sectional study conducted among IT professionals (primarily females). The questionnaire consisted of 7-10 questions and was given to IT professionals. Different populations can be covered online. The sample size of this survey was 100. People seem to have a sufficient amount of knowledge on CTS. However, there is more on CTS that they should be made aware. 70% of people are aware of the carpal tunnel syndrome, but only around 50 % are aware of more information on CTS. Thus more research and studies must be further continued on this topic as it is a prevalent problem and millions of people are affected by it, and many might still be unaware of the problem in the world.

ABSTRAK Carpal Tunnel Syndrome (CTS) merupakan salah satu gangguan pada lengan tangan akibat penyempitan terowongan karpal sehingga terjadi penekanan terhadap nervus medianus. Gerakan tangan saat menggunakan keyboard dan mouse dalam bermain game online memiliki tingkat repetisi tinggi pada jari tangan sehingga dapat berpengaruh terhadap kejadian CTS. Penelitian ini bertujuan untuk mengetahui proporsi dan faktor risiko CTS pada siswa SMA pemain game online di Kota Denpasar. Penelitian kuantitatif analitik dengan rancangan cross-sectional study. Responden berjumlah 70 siswa dengan teknik pengambilan sampel purposive sampling. Hasil menunjukan proporsi CTS responden sebesar 31,43% dengan kategori ringan. Seluruh variabel karakteristik individu yang mencakup usia (p=0,25) dan jenis kelamin (p=0,50) tidak memiliki hubungan yang bermakna dengan kejadian CTS, sedangkan variabel pola bermain, perangkat bermain, dan repetitive motion (p=0,00) terbukti memiliki hubungan bermakna dengan kejadian CTS. Pola bermain merupakan faktor risiko yang paling berpengaruh terhadap kejadian CTS pada siswa pemain game online dengan OR= 23,74. Oleh karena itu diperlukan adanya dukungan pemerintah berupa program preventif - rehabilitatif bagi pecandu game untuk mengedukasi siswa dan orangtua terkait dampak game online.   Kata Kunci: Carpal Tunnel Syndrome, Siswa, Game Online
