AbstractObjectivethis study was conducted to determine the distribution of high-risk human papillomavirus (HR-HPV) genotypes in women in the general population of three regions of Burkina Faso.MethodThis multicenter, descriptive cross-sectional study involved 1321 sexually active women in five cities in three regions of Burkina Faso: Central, Central-Eastern and Hauts-Bassins regions. After collection of endocervical specimens, pre-cervical lesions were screened by visual inspection with acetic acid and lugol (VIA / VILI). HR-HPV genotypes were characterized by multiplex real-time PCR after extraction of viral DNA.ResultsThe mean age of women was 31.98 ± 10.09 years. The HR-HPV infection in the three regions ranged from 26.16% to 43.26% with 35.42% as overall prevalence in women. The most common HR-HPV genotypes in descending order were: HPV 56, 52, 66, 59, 39, 51, 18, 35. The prevalence of bivalent vaccine genotypes (HPV16 / 18) was 7.83% against 63.78% of genotypes not covered by HPV vaccine; 36.32% (170/468) of women had multiple concomitant HR-HPV infections.Conclusionthis study showed significant regional variation and high prevalence of HR-HPV infection in women. The predominant genotypes differ from those covered by available vaccines in Burkina Faso. These results will help guide our health policies towards better prevention of cervical cancer. The diversity of oncogenic genotypes is sparking a large-scale study in the West African sub-region, particularly in cases of cancer and the introduction of the nonavalent vaccine which includes HPV 52 found among the predominant genotypes in this study.

Abstract Background Human papillomavirus (HPV) infection is a public health problem in Africa because of its frequency and its various consequences in both men and women. In Burkina Faso, there is little data on the carriage of HPV infection in male subjects. This study aimed to determine the carriage of HPV infection in male subjects in Ouagadougou, Burkina Faso.Methods This was a cross-sectional study conducted from December 2015 to September 2016. During this study period, a total of 124 male subjects who gave informed consent were recruited as part of a fertility assessment. Semen samples were analyzed by Real- Time PCR using Real TM Quant High Risk Screen HPV kit to detect 14 high-risk HPV genotypes. The Chi square test was used for comparisons.Results Of the 124 male subjects, 22 were positive for at least one high-risk HPV, with a prevalence of 17.7%. HPV 56 was the most represented genotype with a frequency of 20%, followed by HPV 39 (11%) and HPV 68 (11%), HPV 16 (8%). The genotypes HPV 18, HPV33, HPV35, HPV51 and HPV52 also had the same frequency of 6% each and the HPV 31, 45, 59 and 66 had the same frequency of 3% each. Of the 22 positive cases, 10 were multiple infections. Spermocytogram of patients showed at least one abnormality in 87.9% of cases. Necrozoospermia and oligozoospermia were the most abnormalities observed with 42% and 40% respectively followed by asthenospermia (18%); teratozoospermia (16%) and azoospermia (14%).Conclusion This study showed that like women, men are infected by HPV with a relatively high prevalence. In addition, the predominant genotypes in the male subjects of this study are not those targeted by the HPV vaccines available in our countries. Hence the need to reinforce the strategies for controlling HPV infection and HPV prophylactic vaccination for young boys.

Abstract Background: The epidemiology on the human papillomavirus (HPV) among females in Southern China is not well-established. Baseline data on the prevalence of HPV infection in China prior to mass prophylactic HPV vaccination would be useful. Thus, this study aimed to determine the type-specific HPV prevalence and distribution among females from Southern China prior to mass HPV vaccination. Methods: A retrospective cross-sectional study employing 214,715 women attending ChenZhou NO.1 People’s Hospital for cervical screening during 2012-2018 was conducted prior to widespread HPV vaccination. HPV genotype was detected using nucleic acid molecular diversion hybridization tests. The overall prevalence, age-specific prevalence, type distribution, and annual trend were analyzed. Results: The overall HPV prevalence was 18.71% (95% confidence interval [CI], 18.55%-18.88%) among Southern China females. During 2012–2018, the prevalence of HPV infection showed a downward tendency, from 21.63% (95% CI, 21.07%-22.20%) in 2012 to 18.75% (95% CI, 18.35%-19.16%) in 2018. Age-specific HPV distribution displayed a peak at young women aged less than 21 years (33.11%, 95% CI, 31.13%-35.15%), 20.07% (95% CI, 19.70%-20.44%) among women aged 21-30 years, 17.29% (95% CI, 17.01%-17.57%) among women aged 31-40 years, 17.23% (95% CI, 16.95%-17.51%) among women aged 41-50 years, 21.65% (95% CI, 21.11%-22.20%) among women aged 51-60 years, and 25.95% (95% CI, 24.86%-27.07%) among women aged over 60 years. Of the 21 subtypes identified, the most three prevalent high-risk HPV (HR-HPV) genotypes were HPV52 (5.12%; 95% CI, 21.11%-22.20%), -16 (2.96%; 95% CI, 2.89%-3.03%), and -58 (2.51%; 95% CI, 2.44%-2.58%); the predominant low-risk HPV (LR-HPV) genotypes were HPV81 (1.86%; 95%CI, 1.80%-1.92%) and -6 (0.69%; 95% CI, 0.66%-0.73%) respectively. Incidence of HR-HPV only, LR-HPV only and mixed LR- and HR-HPV were 15.17%, 2.07% and 1.47% respectively. Besides, single HPV infection accounted for 77.30% of all positive cases in this study.Conclusions: This study highlights 1) a high prevalence of HPV infection among females with a decreasing tendency towards 2012-2018, especially for young women under the age of 21 prior to mass HPV vaccination; 2) HPV52, -16 and -58 were the predominant HPV genotypes, suggesting potential use of HPV vaccine covering these HPV genotypes in Southern China.

There are few studies on the circulation of Human Papilloma Virus (HPV) in indigenous Ecuadorian women. The aim of the study is to determine the circulation of HPV and identify cytological alterations in cervical samples of indigenous Ecuadorian women and to define the behavior of some socio-demographic and clinical-epidemiological variables. An analytical cross-sectional study was done between July 2017 and September 2018 to determine the presence of cytological alterations and HPV infection in 100 indigenous women between 15 and 55 years of age, residing in Cañar, Ecuador. The association between socio-demographic and clinical-epidemiological variables with viral infection was investigated. Was obtained a 98% of negative cytology for malignancy and 2% of cervical lesions. Cases with high-grade lesions were not diagnosed. In general, a 34% (34/100) tested was positive for HPV, predominating oncogenic genotypes. HPV 31 was the most frequent in 41.2% (14/34) of cases followed by HPV 16 in 20.6% (7/34). Women between 20 and 30 years of age were five times more likely to be infected with HPV (44.1%, 15/34). The frequency of infection was significantly higher in single women and in those who reported having 2 to 3 births. Infection with HPV 16 was associated with the use of hormonal contraceptives, in 57.1% (4/7) of the cases; p = 0.005, RP = 12.44 IC95% (2.40-64.62). The high prevalence of oncogenic HPV infection indicates the need to incorporate this indigenous population into early detection programs for cervical cancer.

Abstract Background The COVID-19 pandemic is an unprecedented international health crisis with different approaches to the response at country levels. The objective of this study was to analyze the evolution of this pandemic in Guinea, Mali, Senegal, and Burkina Faso over the first six weeks. We hypothesize that there exist significant differences in the incidence of COVID-19 between countries. Methods A cross-sectional study was conducted as part of a collaborative project. Data collection focused on the epidemiological surveillance indicators available in the countries’ COVID-19 daily situation reports. Data were entered into a standardized Microsoft Excel spreadsheet which was exported to the STATA 15 software for analysis. Results COVID-19 had a different dynamic in the four countries over the first six weeks. Its incidence in Burkina Faso and Senegal was dropping, while it was increasing slowly in Mali; only in Guinea, it increased rapidly. The analysis of variance revealed that the differences observed in the weekly COVID-19 incidence in the four countries were statistically significant (p ˂ 0.01). Different screening approaches have been used by the four countries. Guinea (n = 4,539) performed more tests compared to Senegal (n = 2,961), Burkina Faso (n = 2,455) and Mali (n = 2,397). The positivity rates were significantly higher in Mali (25.5%) and Burkina Faso (23.7%) than in Guinea (19.0%) and Senegal (9.5%) (p ˂ 0.0001). Hospitalization rates by country ranged from 32.0–79.6%, with Guinea’s hospitalization rate (79.6%) being significantly higher as compared to that of the three other countries (p ˂ 0.0001). The cure rates were significantly higher in Burkina Faso (61.4%) and Senegal (61.1%) than in Mali (37.3%) and Guinea (19.7%) (p ˂ 0.0001). The case fatality rate was significantly higher in Burkina Faso (6.5%) than in Mali (5.2%) (p ˂ 0.001), Guinea (0.7%), and Senegal (0.7%) (p ˂ 0.0001). Conclusion This study highlighted insights from COVID-19 surveillance data, as a basis for improving response strategies in the four countries. Large-scale screening seems relevant to not only facilitate controlling the disease spread but also reducing case fatality rates by early case detection, prior to the occurrence of complications.

Background.HIV-infected adolescents may be at higher risk for high-grade cervical lesions than HIV-uninfected adolescents. The purpose of this study was to compare the prevalence of high-risk HPV (HR-HPV) infections and Pap smear abnormalities between these two groups.Methods.In this cross-sectional study, we compared the HPV DNA and Pap smear results between 35 HIV-infected and 50 HIV-uninfected adolescents in order to determine the prevalence of HR-HPV genotypes and cervical cytological abnormalities. Comparisons were made using Pearsonχ2and independent-samplest-tests analyses, and associations between demographic and behavioral characteristics and HPV infections were examined.Results.HIV-infected participants were more likely to be infected with any HPV (88.6% versus 48.0%;P<0.001) and with at least one HR-HPV (60.0% versus 24.0%;P=0.001), and to have multiple concurrent HPV infections (68.6% versus 22.0%;P<0.001). HPV 16 and 18 were relatively underrepresented among HR-HPV infections. Abnormal Pap test results were more common among HIV-infected participants (28.8% versus 12.0%;P=0.054). A history of smoking was associated with HR-HPV infection.Conclusions.HIV-infected adolescents have an increased risk of infection with HR-HPV and of Pap test abnormalities. The majority of HR-HPV infections among our participants would not be prevented by the currently available vaccinations against HPV.

Abstract Background The natural history of human papillomavirus (HPV) is influenced by vaginal microenvironment disorders, such as bacterial vaginosis (BV). The objective of this study was to assess the epidemiology of HPV combined with BV prevalence among Chinese women aged 20–35 years. Methods A total of 2000 sexually active women aged 20–35 years voluntarily enrolled in this study and underwent a ThinPrep cytologic test and PCR-reverse dot blot human papillomavirus genotyping (PCR-RDB HPV test). BV was diagnosed if clue cells were observed (20% more than epithelial cells). Results The overall HPV infection rate in this population was 16.2% (324/2000). Compared with HPV-negative individuals, BV prevalence was higher in the High-risk human papillomavirus (HR-HPV) (5.9% vs. 3.1%, P < 0.001). BV and HPV-51, -52 infection were more commonly associated with each other. In patients with cervical lesions (≥ CIN 1), the BV prevalence rate was higher than in patients with negative for intraepithelial lesion or malignancy (NILM) (11.9% vs. 3.8%, P = 0.002). Conclusion BV was found to be related to HPV-51, -52 infections and cervical lesions. To better manage HPV infected population, more attention should be paid to the prevention and proper treatment of BV.

Abstract PURPOSE: Dynamic thiol disulfide homeostasis (TDH) is critical in cervical carcinogenesis at HPV infection as a sign of antioxidant consumption native and total thiol levels decrease in progress to cervical intraepithelial lesions. TDH is the main actor in signaling pathways, apoptosis, antioxidant and detoxification reactions. In this study, we aimed to evaluate the effect of TDH intraepithelial progression of cervical precancerous lesions on HPV positive women.METHODS: This was a prospective cross-sectional study. Subjects were selected from newly diagnosed high risk HPV DNA-positive patients. TDH results were calculated as the levels of disulfide, native and total thiol, the ratios of disulfide/total thiol (SS/SH+SS), disulfide/native thiol (SS/SH) and native thiol/total thiol (SH/SH+SS).RESULTS: A total of 146 women were included in the study. Study groups were as group one; control included 66 participants, group two; HPV DNA-positive women without preinvasive cervical lesion included 30 participants and group three; HPV DNA-positive women with preinvasive cervical lesion included 50 participants. Native and total thiol levels were elevated on HPV-positive women without preinvasive cervical lesions. There were no significant differences between groups related to the ratios of SS/SH, SS/ Total SH, SH/ Total SH levels. CONCLUSIONS: HPV infection related to oxidative stress has effects on oxidant/antioxidant balance and could be demonstrated in systemic circulation by TDH parameters. Consumption of thiol substances play role in the cervical neoplastic process, replacement with antioxidants would be a treatment option for HPV infections.

Background: Uterine cervical malignancy is one of the commonly detected malignancies related to the human papillomavirus (HPV) and is increasing incidentally in developing countries. Therefore, the use of an efficient diagnostic method is required as an effectual step for cervical cancer prevention and treatment. The purpose of the study was to diagnose various types of HPV in the cervical cytology specimens in the South-East of Iran. Methods: This cross-sectional study was performed on 1079 cervical fluid cytology specimens referred for two years, between 2018-2020. Polymerase chain reaction (PCR) and hybridization (INNO-LiPA HPV Genotyping EXTRA II assay) were used to determine HPV DNA and their genotypes, respectively. Results: HPV was positive in 37.7% (407 of 1079) patients with a mean age of 34.62 ± 8.82. Among positive cases, 252 (62%) had only one HPV genotype and 155 (38.05%) had multiplex HPV genotypes, which included 94 (60.7%), 38 (24.6%), 18 (11.6%) and 5 (3.2%) cases with two, three, four and five or more genotypes, respectively. The samples with multiple strains revealed 31 HPV genotypes with the four most prevalent being HPV6 (14.7%), HPV16 (10.9%), HPV53 (9.6%) and HPV51 (5.9%). Conclusion: HPV infection is the main health challenge for women that requires improved health service programs and appropriate epidemic vaccination.
