Purpose: to study the changes in quantitative values and functional activity of immunocompetent cells on application of glucoseminylmuramildipeptide in children with allergen-induced phenotype of bronchial asthma.We have performed an integrated assessment of parameters of innate and acquired immunity in 60 children at the age of 3-11 years old with allergen-induced bronchial asthma (BA) with mild clinical course of disease, and in 30 healthy children of the same age. BA phenotypes were verified in accordance with PRACTALL international consensus report (2008). Study exclusion criteria were: severe course of bronchial asthma and application of immunocorrecting therapy during preceding six months. We conducted a prospective parallel open study of the effect of glucoseminylmuramildipeptide on the parameters of immunocompetent cells during three months with division into two control groups by random sampling technique on the basis of therapy being performed. To analyze the venous blood cells, we used flow cytofluorometer COULTER EPICS XL by Beckman Coulter Inc. Cytokine levels were determined using immunoenzyme method with reagents by R&D Diagnostics Inc (USA) and IgE – with reagents by Alkor Bio Company (St. Petersburg), production of cytokines – using reagents by Vektor-Best (Novosibirsk). Statistical processing of data was performed using Statistica 10 program with significance level p < 0.05, assessment of correlations by Spearman correlation analysis, multidimensional correlation analysis with V.P. Terentyev’s method of correlation pleiades (1959) and testing for normal distribution of characteristic values (Shapiro–Wilk). The scope of our study permitted to evaluate its findings with accuracy 95-99%.The changes of the adaptive response system in children with allergen-induced phenotype of BA were characterized by the intensified proliferation, suppression of negative regulation processes, activation of synthesis of Th2-profile cytokines, and intensified synthesis of IgE.The identified impairments of availability, functional activity of immunocompetent cells and cytokine production were preserved in application of inhaled corticosteroids therapy, with further decreasing of IFNγ synthesis.Application of glucoseminylmuramildipeptide in children with BA provided for reduction of spontaneous and mitogen-induced production of IL-4 and correction of deviated structural and functional characteristics of immunocompetent cells.Incorporation of glucoseminylmuramildipeptide into therapeutic regimens for allergen-induced phenotype of BA in children promoted normalization of parameters of the cellular component of immune system, amelioration of Th1/Th2-imbalance, increase of Th1 activity, and adequate spontaneous and induced production of IFNγ by peripheral blood cells.

Background. Peculiarities of the disease and effectiveness of therapy depend on changes in availability and functions of effector immunocompetent cells and cytokine profile. Purpose. To study the peculiarities of immune pathogenetic mechanisms of virus-induced and allergen-induced bronchial asthma phenotypes in children in order to justify the use of immune therapy and to analyze it’s effectiveness. Materials and methods. We have performed an integrated assessment of immunological parameters in 120 children at the age of 3-11 y.o. with virus-induced and allergen-induced phenotypes of bronchial asthma (BA) and in 30 healthy children. Withdrawal criteria: severe BA, immune therapy during preceding 6 months. We used flow flow cytometer COULTER EPICS XL (Beckman Coulter Inc.) for identification of venous blood cells, CYTOKINE-STIMUL-BEST sets (Vektor-Best JSC, Novosibirsk) for levels of spontaneous and mitogen-induced production of cytokines identification. The immunological effectiveness of treatment was analyzed in the open parallel prospective study using random sampling technique in subgroups on the basis of treatment regimen. Hemogram Reference intervals are established since a healthy population is critical to accurately interpret laboratory tests, which include Hemoglobin estimation, Red blood cells count and indices, White blood cells count and differential in addition to Platelets count. This study aims to establish the reference interval of the complete hemogram amongst healthy Sudanese children in Elobied city, Sudan. A descriptive cross-sectional study  included 354 healthy children, aged between 3 to 17 years, who were categorized into three groups according to age. A questionnaire was fulfilled, EDTA anti-coagulated venous blood sample was collected from each child then the complete hemogram was performed automated hematological analyzer (Sysmex Xp 300), Finally, Data was analyzed by a software program (SPSS version 21). The hematological reference intervals for healthy children [Hb g/dl, HCT%, RBCs count x106μL, MCV/fL ,MCH /pg., MCHC g/dl ,WBCs count x10³/μL, Neutrophil count%, Eosinophil count %, Basophile count %, lymphocyte count ,Monocytes count%, RDW CV and PLTs count×10³/μL] are [(12.4±1.2),(37±4),(5.0 ±0.4), (82±5.0), (26±3.0), (32±3.0) ,(7.0±2.0) (47±10), (1±0.1) ,(0±0),(45± 10),(7 ± 4), (13±1.8) &(227±91)] respectively. The hematological RI for healthy children in Elobied was established in this study to be representative of this population, there was a significant gender-based difference in all the evaluated hematological parameters, they were found to be higher in males than in females except for basophil (%). Finally, the results of this study would shed a light on the importance of establishing RI for the children population in Elobied.

Расширение спектра изучаемых клонов Тхелперов определило более сложные иммунные механизмы реализации аллергического воспаления. Цель. Характеристика показателей и взаимосвязей цитокинового профиля сыворотки и субпопуляционного состава Тлимфоцитов периферической крови у детей с бронхиальной астмой и аллергическим ринитом. Материалы и методы. Проведено комплексное обследование 150 детей в возрасте 311 лет с верифицированным диагнозом бронхиальной астмы, аллергического ринита и 30 здоровых сверстников. Иммунологические параметры крови оценивали методом проточной цитометрии, концентрации интерлейкинов и IgE в сыворотке крови определяли методом твердофазного иммуноферментного анализа. При статистической обработке использовали программы Statistica 10 с критическим уровнем значимости р0,05. Результаты. У детей с аллергическими заболеваниями в сыворотке крови определены высокие уровни содержания интерлейкинов4, 8, 13, 17А, сопоставимый с показателями группы контроля уровень IL17F и низкое содержание IFNy. При бронхиальной астме и аллергическом рините у детей выявлено увеличение количества CD3CD8CD45RO, CD3CD8CD45RACD45RO Тлимфоцитов и CD3CD4 Тхелперов и повышение количество Th17 при снижении CD3CD4CD45RO клеток памяти. В группе здоровых детей популяция Th17 составляла 9,491,6, у детей с аллергическими заболеваниями количество данных клеток было значимо выше 14,50,77 (р0,001). Анализ сывороточного содержания цитокинов у детей с изолированным течением БА и в сочетании с аллергическим ринитом выявил разнонаправленные корреляции, отличающиеся по силе и направленности от таковых в группе здоровых детей. Заключение. У детей при изолированном течении бронхиальной астмы и в сочетании с аллергическим ринитом выявлены: сопоставимое с показателями здоровых детей количество CD3CD4 Тклеток, дисбаланс в субпопуляционном составе Тхелперов за счет преобладания Th2 и Th17, активация синтеза IL17A, IL4, IL8, IL13, низкий уровень сывороточного IFNy, изменения силы и направленности взаимосвязей цитокинового профиля и спектра субпопуляций Тлимфоцитов.Expansion of the range of examined Thelper clones has determined more complex immune mechanisms for the implementation of allergic inflammation. Objective. To characterize the parameters and relationships between the serum cytokine profile and Tlymphocyte subpopulation in peripheral blood of children with bronchial asthma and allergic rhinitis. Materials and methods. 150 children aged between 311 years old with bronchial asthma, and allergic rhinitis and 30 healthy volunteers were examined. Immunological parameters were assessed by flow cytometry, the concentration of serum interleukins and IgE were determined by means of enzymelinked immunosorbent assay. Statistical analysis was performed with Statistica 10 program with a critical level of significance p0.05. Results. High levels of interleukins 4, 8, 13, 17A were determined, IL7F level was not significantly different from that in control group and low level of IFNy was found in the serum of children with allergic diseases. The number of CD3CD8CD45RO, CD3CD8CD45RACD45RO Tlymphocytes, CD3CD4 Thelper cells and Th17 were increased and at the same time CD3CD4CD45RO memory cells were decreased In bronchial asthma and allergic rhinitis children. Number of Th17 cells in healthy children was 9.491.6, in allergic children it was significantly higher 14.50.77 (p0.001). Analyses of serum cytokine count in children with isolated BA and in association with allergic rhinitis revealed multidirectional correlations differing in strength and direction from those in the group of healthy children. Conclusion. In children with isolated bronchial asthma and associated with allergic rhinitis the following parameters were found: CD3CD4 Tcells count was comparable to that in healthy children, the imbalance of Thelper subpopulation: prevalence of Th2 and Th17, activation of IL17A, IL4, IL8, IL13 synthesis and low level of serum IFNy.

The aim: To study the clinical and spirographic features persistence of the bronchial asthma in schoolchildren against the background of the alternative daily doses of inhaled corticosteroids to increase the effectiveness of anti-inflammatory therapy for this disease. Materials and methods: A complete comprehensive clinical-paraclinical examination of 65 schoolchildren with persistent asthma was conducted. According to the average daily dose of inhaled corticosteroids (ICS) the patients were divided into two clinical groups. The first (I) group consisted of 46 children who received ICS in the regimen of low-to-medium equipotent doses (253.95±9.98 μg per day), and the second (II) comparison group was formed of 19 patients who controlled the pBA using high doses of ICS (494.74±5.56 μg per day). Results: The patients of the І clinical group compared to patients of the ІІ group have a higher risk of the mild bronchial obstructive syndrome during asthma attacks. In assessing the level of control of persistent bronchial asthma using the CIA-scale, it was found that in II group cases of the controlled course of the disease were observed almost two times less than in children of the I group of comparison. In conducting spirography in children of comparison groups, it was shown that the ratio of indices of bronchospasm (FEV1/ FVC) was worse in patients receiving high doses of ICS. Conclusions: So, сharacteristic clinical feature of asthma controlled by high doses of ICS is more severe nature of bronchial obstructive syndrome during the period of exacerbation (OR=1.9-3.0). In the management of persistent bronchial asthma, the Gensler index which has high specificity (94.4%) and accuracy (92.2%) should be used for disease control verification.

Goals. To reveal the clinical efficacy and safety of Asmanex in patients with moderate and severe bronchial asthma (BA), treated with inhaled and/or systemic glucocorticosteroids (GCS), depending on the severity and level of control. Methods. 40 patients (age 18 to 65 years) were treated with Asmanex with equivalent to prior therapy with inhaled corticosteroids (ICS) doses or in combination with systemic corticosteroids prior therapy (without increasing the dose of systemic corticosteroids) for 3 months. Efficacy was assessed before and after the treatment on a background of Asmanex treatment by the need of additional using of agonists of β 2-adrenoreceptors, severity of daytime and nighttime asthma symptoms, dynamics of spirometry parameters. The efficacy of Asmanex was compared with the original baseline therapy of other inhaled corticosteroids at equivalent doses using the new delivery system.

The current article analyzes the parental defensive styles, codes and types of upbringing in families with children diagnosed with asthma in comparison with families of healthy children. The study involved 80 families. Of these, 41 families with children with asthma (41 mothers and 37 fathers) and 39 families with children without chronic diseases (39 mothers and 38 fathers). It was revealed that the fathers of children with asthma differ in educational insecurity, and mothers have persistent educational stereotypes that interfere with effective interaction with the child. In mothers of children with asthma, non-adaptive protective styles predominate, and fathers distort, whereas in the parents of healthy children adaptive protective styles are expressed. With the cluster analysis, three groups of families with children with asthma are distinguished, differing in the level of effectiveness of family functioning. The conjugation of violations of family functioning and the severity of the asthma in a child was determined. Based on the data obtained, differentiated directions of psycho-correction for families with children with asthma are described, depending on the severity of the child's disease and the level of effectiveness of family functioning. A trial program of psychological correction was conducted, according to the described directions. The analysis of the results of the second study showed the effectiveness of the proposed program of psychological correction for families with children suffering from bronchial asthma.

Introduction. A literature review on the use of plethysmographic methods for studying the features of venous blood flow in lower extremities under physiological conditions and for global assessment of venous hemodynamics in chronic venous diseases urges to conduct further studies.The aim of the study was to carrying off a detailed statistical analysis of the value of the refilling time (RT) obtained during a photoplethysmographic study (PPG) to evaluate the degree of effectiveness of the calf muscle-venous pump in normal and in patients of different clinical classes according to CEAR.Materials and methods. For the period from 2002 to 2012, 116 healthy individuals with no symptoms of chronic venous diseases and 530 patients with CVD were examined with the use of PPG, with their distribution in accordance with the CEAR international classification as follows: C0-C1 – 140 people (26.42%), C2-C3 – 218 people (41.13%) and 172 persons (32.45%) had trophic disorders, that is, they belonged to the C4-C6 clinical classes. The analysis of the statistical significance of the obtained mathematical characteristics in groups by conducting a t-test for two independent samples calculated by the program and the student’s criterion Z-statistics, to conduct a correlation analysis.Results and discussion. Statistical comparative analysis of the RT value as a result of the t-test at the significance level p < 0,05 showed statistically significant differences between С0-С1, С2-С3 and С4-С6 groups with test with (or no) compression cuff. A stable correlation of average strength was observed between the groups in all cases. The differences were not statistically significant between the Norm and C0-C1 groups. The testing hypotheses for adequacy using linear regression methods allowed us to conclude that conducting a photoplethysmographic examination of patients with CVD of different clinical classes as part of the venous reflux test can provide objective information about the effectiveness of the calf muscle - venous pump. A comparative analysis in groups using the criteria RT > 20 sec and 10 < RT < 20 sec showed that there was no correlation between many groups.Conclusion. PPG is not a universal tool for diagnosing disorders of venous hemodynamics in the lower extremities. The value of RT > 20 seconds can be used as a criterion for evaluating the effectiveness of the calf muscle-venous pump, since more than 70% of cases occur in norm and in patients with C0-C1 clinical classes.
