Abstract This article aims to determine the pattern of alcohol use in the elderly and its associations with sociodemographic characteristics in an elderly sample of patients from the city of Porto Alegre, Rio Grande do Sul, Brazil. A cross-sectional study was conducted involving 557 seniors, aged 60 years or more, through application of the Mini International Neuropsychiatric Interview and a global assessment questionnaire for the elderly. The majority of the 557 senior citizens did not complete elementary school (58.3%), were white (65.1%), married (37.6%), had no caregiver (62.2%), were catholic (65.5%) and practicing their religion (68.6%), were retired (67.7%), and had a personal income of up to one minimum salary (56.1%). The study revealed 67 (12%) elderly people with a history of alcoholism, of which 17 (3.1%) had a diagnosis of current alcoholism, 50 (9%) had a history of alcohol dependence in the past and 16 (2.9%) had a current alcohol abuse problem. Men had a prevalence ratio of 11.6 times for a history of alcoholism in comparison to women. The results confirm that alcoholism is frequent in the population of Brazilian elderly, drawing attention to some socio-demographic characteristics that can make a difference in the early diagnosis of alcoholism.

ABSTRACTObjective: to draw the profile of the hypertensive elderly registered/accompanied in the field of basic health attention. Method: cross sectional study of descriptive characteristic, in which users who were 60 years old or more and who were in the Family Health Strategy program between 2000 and 2009 were analyzed through the HIPERDIA system of João Pessoa-PB city. Data referring to number of hypertensive patients registered in that period and association of hypertension with smoking, sedentary habits and overweigh, classified by sex and age group,  were used. Results: files of 7,156 hypertensive elderly registered/accompanied by HIPERDIA by sex and age group were found, with prevalence of arterial hypertension of 8 % in the period of study. Among these users, 68,4% are female and the age group from 60 to 64 presented the greatest number of hypertensive when both sexes are added (23.6%).  In relation to the presence of associated factors, 49% presented overweigh, 51% had sedentary habits and 15.9% smoked. Conclusion: the hypertensive elderly of João Pessoa showed characteristics which are similar to the other regions of the country. It is necessary to promote and implement public policies towards this population with emphasis on prevention and valorization of the services of Basic Attention. Resultados: fueron encontrados registros de 7156 ancianos hipertensos registrados/acompañados por el HIPERDIA por sexo y faja etaria, con prevalencia de hipertensión arterial de 8% en el periodo estudiado. De estos usuarios el 68,4% son del sexo femenino y los de la faja etaria entre 60 y 64 años presentaron un número mayor de hipertensos de ambos sexos (23,6%). En relación a la presencia de factores asociados el 49% presentaron obesidad, el 51% eran sedentarios y el 15,9% fumadores. Conclusión: los hipertensos mostraron características semejantes a las demás regiones del país. Se hace necesario incentivar e implementar políticas públicas orientadas a dicha población, enfatizando la prevención y dando más valor a los servicios de Atención Básica. Abstract Objective: to evaluate the expression of coercion, confidence and satisfaction with the use of telecare. Method: a cross-sectional study was carried out of prevalent cases with elderly residents in the city of Porto Alegre, Rio Grande do Sul, Brazil, who had already used telecare services. The sociodemographic variables used were: gender, age, schooling and marital status. In order to evaluate the perception of coercion, the Perceived Coercion Scale was applied in relation to the use of telecare and those responsible for deciding to use the technology. Confidence in the use of telecare technology and the satisfaction associated with its use were evaluated. Quantitative data analysis was performed using descriptive and inferential statistical measures. Means and standard deviations were used. Differences were assessed by analysis of variance, with a significance level of 5% (p<0.05). Results: the sample consisted of 25 elderly people who used telecare. When evaluating the distribution of the answers obtained, it was found that 11 (44%) of those interviewed did not perceive any coercion associated with their decision. The others had varying perceptions, with an average of 23%. Respondents expressed high confidence and satisfaction with telecare. Conclusion: the decision-making process associated with the use of telecare had a low perception of coercion, combined with high confidence and satisfaction with the use of the technology.

ABSTRACT Objective: To identify the knowledge and attitudes of doctors and nurses in tracking, diagnosing, treating, and monitoring dementias and their educational needs in this area. Method: Cross-sectional study, carried out among 195 doctors and 274 nurses working in teams of the Family Health Strategy (FHS) in the city of São Paulo. The instrument used was Health Care for Dementia: the Primary Care perspective. The data were submitted for descriptive statistics. Results: Among physicians, 64.1% reported routinely diagnosing dementia, but only 23.1% in the mild phase; 89.2% mentioned difficulties in identifying cases of the disease; 94.9%, difficulties in the treatment and monitoring of patients, including the needs to support the caregiver (28.2%); 84.6% of doctors and 79.2% of nurses reported difficulties in monitoring severe cases of the disease. Conclusion: Gaps in knowledge were identified regarding the tracking and diagnosis of dementia, patient monitoring, diagnostic information, and support for the caregiver.

The objective of the present study was to evaluate the quality of life in hypertensive elderly people assisted in the Family Health Strategy. This is a descriptive, exploratory, cross-sectional study, with quantitative approach. The participants were 544 hypertensive elderly patients. The instruments used for data collection were the sociodemographic form and the WHOQOL-BREF. Regarding the socioeconomic and demographic characteristics of the elderly participants of this study, there was prevalence of elderly women, aged 60-69 years, self-reported white, catholic religion, illiterates, coming from municipalities in the countryside of the state, married/stable union, retired, income of one minimum wage, with children, non-smokers, non-alcohol consumers. Regarding the quality of life, the highest score was obtained in the Social Relationships Domain, followed by the Psychological Domain, Physical Domain and lower score in the Environmental Domain. The results show that the elderly participants presented a good quality of life and, although they have high blood pressure levels, they seek to live as best as possible.

ABSTRACT Objective: to identify the prevalence and factors associated with falls in the elderly population living in rural areas. Method: this is a cross-sectional study, carried out in 2014, with 820 older adults recorded in the Family Health Strategy (Estratégia Saúde da Família). The association between the reporting of falls in 12 months and their associated factors was verified by the Chi-square and Fischer’s exact tests and by multivariate logistic regression analysis. Results: the majority of the sample was female (56.1%), white (90.2%) and aged 60-69 years (54.9%). The prevalence of falls was 27.9%, and being female, hypertensive and diabetic was associated to falls. Conclusion: it is the responsibility of health professionals to have a closer look at the elderly who have these chronic diseases, especially within the scope of the Family Health Strategy, which works longitudinally with these patients, in addition to improving nursing care aimed at this population.

Abstract Objective: To describe the prescribing, dispensing, use, adhesion, and storage of medicines to and by the elderly. Method: A descriptive cross-sectional study was performed in Estratégia Saúde da Família (Family Health Strategy) health centers (ESF), in Marília in the state of São Paulo, Brazil, based on the records of and interviews with 114 individuals seven to ten days after a medical consultation. A descriptive analysis was carried out. Results: The mean number of prescribed drugs was 4.98 per elderly patient. Of the total number of prescribed drugs, 81.5% were supplied by public services, with the nutrient (50%); antilipemic (62.1%); analgesic (30.7%); dermo-protector (66.6%); herbal (40%) and parasite and antifungal (37.5%) classes dispensed the least. A total of 83.8% of the prescribed drugs were used, while the drugs dispensed at the lowest rates were not used by the elderly, except for analgesics. A total of 40.3% of the respondents exhibited low adherence. Most stored their medicines in a suitable place. Conclusion: The prescribing, dispensing, use and storage of medications to and by the elderly can be considered effective, but adherence remains low, requiring new strategies and interventions.

Abstract Background Alcohol use disorder increase the risk of physical harm, mental or social consequences for patients and others in the community. Studies on alcohol use disorder and associated factors among medical and surgical outpatients in Ethiopia are limited. Therefore, this study is meant to provide essential data on alcohol use disorder and associated factors among alcohol user medical and surgical outpatients to intervene in the future. Methods An institution-based cross-sectional study was conducted by using the systematic random sampling technique. Alcohol use disorders were assessed using the World Health Organization’s 10-item Alcohol Use Disorder Identification Test (AUDIT) questionnaire. Bivariate and multivariate logistic regression analyses were performed, a P-value less than 0.05 were considered statistically significant in the multivariate analysis and the strength of association was measured at a 95% confidence interval. Results The prevalence of alcohol use disorder was 34.5% with a 95% CI (29.20, 39.80) among study participants. In the multivariate logistic regression analysis, male sex (AOR = 3.33, 95%CI: 1.40, 7.93), history of mental illness (AOR = 2.68, 95%CI: 1.12, 6.38), drinking for relaxation (AOR = 1.88, 95%CI: 1.02, 3.48) and history of lifetime tobacco use (AOR = 5.64, 95%CI: 1.95, 16.29) were factors significantly associated with alcohol use disorder. Conclusion The prevalence of alcohol use disorders among medical and surgical outpatients was found to be high. Male sex, history of mental illness, alcohol use for relaxation and lifetime cigarette smoking need more attention during the assessment of patients in the medical and surgical outpatient departments.

AIMS: This research aims to determine the epidemiology and the spatial distribution of intestinal parasitosis in the city of Teresina.METHODS: A cross-sectional study was carried out based on the data of parasitological fecal exams performed in the Laboratory Raul Bacelar between January, 2014 and July, 2017. In addition to the prevalence of intestinal parasitosis and polyparasitism, we verified the association of these diseases with gender, zone and period of the year by means of the chi-squared test, whereas the relation with age was analyzed by the Mann-Kendall tests and multiple comparisons of age classes. The spatial distribution was performed using the QGIS georeferencing software.RESULTS: The prevalence of enteroparasitosis in Teresina is 17,8% with Ascaris lumbricoides being the most common species, due to the precarious sanitary conditions of the city. The prevalence of individuals with polyparasitism is 3,13%, in which an association between the species Entamoeba coli and Entamoeba histolytica/dispar was found. There was no relation between intestinal parasitosis with gender, but we verified that individuals in rural areas are more susceptible to these diseases. The species Ascaris lumbricoides and Entamoeba histolytica/dispar occur more frequently in the first and second semester, respectively. We observed that there is an apparent tendency to increase cases of E. histolytica/dispar and reduction of cases of Giardia sp. according to aging. Mapping intestinal parasitosis showed us that there is a prevalence between one and 20% in most of Teresina's neighborhoods, and Ascariasis embodies at least 40% of cases of enteroparasitosis in these neighborhoods.CONCLUSIONS: Investments in basic sanitation and new epidemiological investigations must be carried out to control intestinal parasitosis in Teresina, emphasizing that children and the elderly should be considered priority groups in these programs.

ObjectivesTo estimate the prevalence of healthcare use and associated factors in the Manaus metropolitan region and to describe the reasons for lack of access.DesignCross-sectional population-based study.SettingA survey conducted between May and August of 2015 in eight cities from Manaus metropolitan region, Amazonas, Brazil.Participants4001 adults ≥18 years of age.Primary outcomes measuresPhysician visits, dentist visits and hospitalisations in the last 12 months were the primary outcomes. Associated factors were investigated through the calculation of prevalence ratio (PR) obtained by hierarchical Poisson regression modelling.Results4001 adults were included in the study, 53% of whom were women. The self-reported prevalence of medical visits was 77% (95% CI 75% to 77%); dentist visits, 36% (95% CI 34% to 37%) and hospital admission, 7% (95% CI 6% to 7%). Physician visits were higher in women PR=1.18 (95% CI 1.14 to 1.23), the elderly PR=1.18 (95% CI 1.10 to 1.26) and people with health insurance PR=1.14 (95% CI 1.10 to 1.19). Dentist visits declined with older age PR=0.38 (95% CI 0.30 to 0.49), lower education level PR=0.62 (95% CI 0.51 to 0.74) and lower economic class PR=0.65 (95% CI 0.57 to 0.75). Hospitalisations were found to be twice as frequent for women than for men and three times as frequent among those who reported very poor health status. Among the individuals who did not receive medical attention in the previous 2 weeks, 58% reported lack of facilities or appointment unavailable and 14% reported lack of doctors.ConclusionWhile more than half visited the doctor in the last year, a lower proportion of people with socioeconomic inequities visited the dentist. Organisational and service policies are needed to increase equity in health services in the region.
