Abstract Objective: To analyze training in integrated health care for the elderly from the perspective of nursing students. Method: A descriptive and exploratory study with a qualitative approach was carried out using the action research method with the participation of 24 nursing students from a university center in Piauí, Brazil. Data were obtained through semi-structured interviews and submitted to content analysis. Results: Two thematic categories emerged: Perspectives on aging, violence and sexuality and A holistic approach to the elderly: integrating training and care. Conclusion: The perspective of the students on integrated care for the elderly is fragmented, stemming from the limitations of the training process for such care.

ABSTRACTObjectives: to identify the main needs of healthcare for the elderly with dementia by home visits and discuss the importance of household visits by scholars, aiming to promote the health of their clients. Method: this is a descriptive study of a kind experience report with a qualitative approach, in which seven were carried out home visits to elderly dementia participating in the Program of Geriatrics and Gerontology in Niterói - RJ, Brazil, September 1 November 24, 2009. The research received approval under No. 062/09 of the Ethics Committee of University Hospital Antonio Pedro - HUAP. Results: the visits, an evaluation of the home as their physical structure and health conditions besides providing guidance to caregivers and family members about changes in the environment and care that meet the specific needs of each client. Conclusion: the activity was very important for the students, allowing the understanding of aspects related to caring for the elderly with dementia at home by means of the holistic approach, taking into account the promotion, prevention and recovery in health. Método: trata-se de um estudo descritivo, do tipo relato de experiência com abordagem qualitativa, no qual foram realizadas sete visitas domiciliares aos idosos demenciados que participam do Programa de Geriatria e Gerontologia no município de Niterói - RJ, no período de 1 de setembro a 24 de novembro de 2009. A pesquisa recebeu a aprovação sob nº 062/09 do Comitê de Ética do Hospital Universitário Antonio Pedro – HUAP. Resultados: as visitas permitiram uma avaliação do domicílio quanto a sua estrutura física e condições sanitárias além de possibilitar a orientação aos cuidadores e/ou familiares sobre as mudanças no ambiente e os cuidados que atendem as necessidades específicas de cada cliente. Conclusão: a atividade foi de suma importância para os acadêmicos, possibilitando a compreensão dos aspectos relacionados ao cuidar do idoso com demência no domicilio por meio da abordagem holística, levando em consideração a promoção, prevenção e recuperação em saúde. Método: estudio descriptivo de un informe experiencia única con un enfoque cualitativo, de los cuales siete se llevaron a cabo visitas domiciliarias a demencia senil que participan en el Programa de Geriatria y Gerontología en Niterói - RJ, Brasil, 1 de septiembre 24 de noviembre 2009. La investigación recibió la aprobación en virtud N º 062/09 de la Comisión de Ética del HUAP. Resultados: las visitas han permitido la evaluación de la vivienda como su estructura física y las condiciones de salud además de proporcionar orientación a los cuidadores y familiares acerca de los cambios en el medio ambiente y la atención que satisfagan las necesidades específicas de cada cliente. Conclusión: la actividad fue muy importante para los estudiantes, permitiendo la comprensión de los aspectos relacionados con el cuidado de los ancianos con demencia en el hogar por medio de un enfoque holístico, teniendo en cuenta la promoción, prevención y recuperación de la salud. Abstract Objective: to analyze the perception of caregivers of elderly persons regarding their own oral health care and that of the elderly individuals. Method: a descriptive-exploratory study with a qualitative approach was carried out. Semi-structured interviews with 13 caregivers were conducted, recorded and transcribed, and the Content Analysis technique was applied. Results: All respondents were informal caregivers, most of whom were female, aged over 50, married, had some degree of kinship with those receiving care, an incomplete elementary school education and no other occupation or job. After analyzing the interviews, the data were grouped into the following categories: a) home visits and health professionals; b) caregiver experience and care of the elderly; and c) the caregiver and their self-care. Conclusion: knowledge of caregivers’ perception of their own oral health and that of dependent/semi-dependent elderly persons assists in planning, promotion, prevention and health recovery. The work of a multi-professional team is therefore essential in the seeking out and instruction of these individuals.

ABSTRACT INTRODUCTION High-risk prescribing in general practice is common and places patients at increased risk of adverse events. AIM The Safer Prescribing and Care for the Elderly (SPACE) intervention, comprising audit and feedback plus practice mail-out to patients with high-risk prescribing, was designed to promote medicines review and support safer prescribing. This study aims to test the SPACE intervention feasibility in general practice. METHODS This feasibility study involved an Auckland Primary Health Organisation (PHO), a clinical advisory pharmacist, two purposively sampled urban general practices, and seven GPs. The acceptability and utility of the SPACE intervention were assessed by semi- structured interviews involving study participants, including 11 patients with high-risk prescribing. Interviews were audio-recorded, transcribed verbatim and analysed using a general inductive approach to identify emergent themes. RESULTS The pharmacist said the SPACE intervention facilitated communication with GPs, and provided a platform for their clinical advisory role at no extra cost to the PHO. GPs said the feedback session with the pharmacist was educational but added to time pressures. GPs selected 29 patients for the mail-out. Some GPs were concerned the mail-out might upset patients, but patients said they felt cared for. Some patients intended to take the letter to their next appointment and discuss their medicines with their GP; others said there were already many things to discuss and not enough time. Some patients were confused by the medicines information brochure. DISCUSSION The SPACE intervention is feasible in general practice. The medicines information brochure needs simplification. Further research is needed to test the effect of SPACE on high-risk prescribing.

This study examines the pedagogical training process of medical professors at a Brazilian university, the meanings attributed to it, and the positive and negative aspects identified in it. This is a descriptive-exploratory study, using a qualitative approach with a questionnaire utilizing open-ended and closed questions and a semi-structured interview. The majority of queried individuals had no formal teacher training and learned to be teachers through a process of socialization that was in part intuitive or by modeling those considered to be good teachers; they received pedagogical training mainly in post-graduate courses. Positives aspects of this training were the possibility of refresher courses in pedagogical methods and increased knowledge in their educational area. Negative factors were a lack of practical activities and a dichotomy between theoretical content and practical teaching. The skills acquired through professional experience formed the basis for teaching competence and pointed to the need for continuing education projects at the institutional level, including these skills themselves as a source of professional knowledge.

Telehealth and telecare have been heralded as major mechanisms by which frail elderly people can continue to live at home but numerous pilot studies have not led to the adoption of these technologies as mainstream contributors to the health and social care of people in the community. This paper reviews why dissemination has proved difficult and concludes that one problem is that these technologies require considerable organisational changes if they are to be effective: successful implementation is not just a technical design issue but is a sociotechnical design challenge. The paper reviews the plans of 25 health communities in England to introduce integrated health and social care for the elderly. It concludes that these plans when implemented will produce organisational environments conducive to the mainstream deployment of telehealth and telecare. However, the plans focus on different kinds of integrated care and each makes different demands on telehealth and telecare. Progress on getting mainstream benefits from telehealth and telecare will therefore depend on building a number of different sociotechnical systems geared to different forms of integrated care and incorporating different forms of telehealth and telecare.

PurposeThe paper examines the different trajectories of externalisation and the development of different kinds of welfare mix in three different sub-sectors of socio-educational services: long-term care for the elderly, early childhood services and kindergartens. By integrating the industrial relations and comparative public administration literatures, it analyses the different rationales underpinning contracting-out decisions of Italian local governments.Design/methodology/approachThe paper adopts a multi-method, multi-level approach: quantitative data on the provision of socio-educational services and the nature of the providers are combined with the analysis of 12 case studies of municipalities through 80 semi-structured interviews and documentary analysis.FindingsThe paper argues that differentials in labour regulation across the public/private divide and the consequent possibility to access labour markets characterised by cheaper labour and higher organisational flexibility are a key explanation in local governments' decisions to outsource. Despite labour market factors playing a prominent role, their relevance is significantly tempered by political and social factors and particularly by the strong opposition of citizens, personnel and trade unions to pure market solutions in the provision of such services. However, the centrality of these factors depends on the nature of the services: political sensibility against privatisation proved to be stronger in kindergartens, while services for the elderly were more frequently and less contentiously privatised.Originality/valueThe main contribution is the integration of the two research traditions to analyse patterns of outsourcing in the socio-educational services in Italy, showing that neither of them is able, alone, to explain the different private/public mix characterising different social and educational services.

ABSTRACT Objective: To describe the contribution of an awareness-raising workshop for nursing students on Alzheimer’s disease in the context of the elderly/family. Methods: Strategic action-research developed with 15 university students from the sixth to the eighth semester of Nursing at a university institution in the Rio Grande do Sul. The data collected through semi-structured interviews, before and after an awareness-raising workshop on Alzheimer’s disease in elderly/family, were subjected to thematic content analysis. Results: Six categories were identified that concern (Mis) perception of Alzheimer’s disease in the context of elderly/ family, from the characterization; risk factors; diagnosis; treatment of Alzheimer’s disease; from care to family members/caregivers; and care strategies for the elderly with Alzheimer’s disease. Conclusion: The awareness-raising workshop enabled the students to broaden and deepen the knowledge on the theme of Alzheimer’s disease in the elderly.
