The use of compulsory observation and treatment by a psychiatrist according to the current legislation is carried out along with the imposition of punishment in relation to persons with mental disorders of a non-psychotic level. In relation to persons who do not need to apply coercive measures of a medical nature in stationary conditions, but who have mental disorders of a psychotic level, the legislator does not consider the possibility of coercive observation and treatment by a psychiatrist in accordance with Chapter 51 of the Code of Criminal Procedure of the Russian Federation, while there are no grounds for terminating the criminal case against these persons. According to the authors, it is necessary to expand the list of grounds for applying coercive measures of a medical nature in the form of coercive observation and treatment by a psychiatrist, and also establish the production procedure determined by Chapter 51 of the Code of Criminal Procedure of the Russian Federation, for persons recognized as insane, but not in need of coercive medical measures in stationary conditions.

Background: the reduction in the premature mortality as a result of non-contagious diseases by way of the improvement measures to be adopted with the purpose of prevention and treatment of the substance abuse and general health care activities were outlined by the United Nations Organization as the primary objectives as far back as 2015 in order to work towards sustainable development (the objective № 3 “Promotion of healthy lifestyle and furtherance of wellbeing of individuals of all ages” in 2030 perspective). There is a high risks for the Russian Federation of transnational drugs expansion developing [1]. Concurrently, in order to build a national dependence prevention system, Decree № 581н as of 06.10.2014 “On the Preventive Medical Screening to be made at educational establishments” was issued by the Ministry of Health of the Russian Federation. Purpose: the research was supposed to focus on Preventive Medical Screening as a measure of prevention of illegal usage of substances and mental disorders among minor students and on the optimization of such preventive medical screening. Materials and methods: the fi ndings are based on the posthoc analysis of the data of the Federal statistical survey form and of the research and information reports (n = 491 280 students). The results: the fi ndings of the preventive medical screening made across the Russian Federation and in Moscow over the years 2016–2018 show the increase of minor students involved into such preventive medical screening, as well as the decrease of those who might use psychoactive drugs. The same fi ndings show that the majority of the illegal drug users were students of secondary vocational schools. The fi ndings can be called a trend across the Russian Federation and in Moscow. Conclusions: non-medical usage of substances among minor students can be a serious threat to the mental health of the youth. There are suggestions for improvement prevention of mental disorders among students at the results of the research. These include methodology of Brief Motivational Consulting at a time of preventive medical screening and the ways of increase effectivity toxicology screening in different educational organizations offering to add in preventive medical screening cotinine (it’s metabolite of nicotine) screening for students of primary school; it’s have to systematically update and expand types of screening tools for substances in preventive medical screening in all educations organizations.

Comparative analysis of the prevalence of mental disorders in the Russian Federation in 1991–2018 has been carried out on the basis of the study of official statistics (information of reporting form № 10 introduced by the Ministry of Health of the Russian Federation since 1991) and calculation of indicators of general incidence of mental disorders. Age characteristics in the dynamics of general incidence of mental disorders have been identified: reduction in prevalence of non-psychotic psychopathology in groups of 0–17, 20–59 years old and growth of number of psychotic forms in groups of 0–17 and 18–19 years old in 2007–2018. The authors of the article conclude that decrease in the incidence of mental disorders, and especially non-psychotic forms of mental disorders, along with positive factors, indicates a decrease in referrals for psychiatric care by the population of the Russian Federation, which in turn is related to socio-economic problems, attitude of society to psychiatry, accessibility of psychiatric care.

The issues of prevention of suicidal incidents in the Armed Forces of the Russian Federation are considered. The analysis of suicide prevalence among military personnel of various categories and their prevailing causes for the period from 2013 to 2017 is indicated. Indicates a change in the percentage ratio of various categories of military personnel who committed suicide, with a steady increase in the proportion of privates and sergeants serving in military service under the contract (from 24,6% in 2011 to 57,4% in 2017). It was established that among the servicemen under the contract family and everyday reasons for suicide prevailed, while conscripts were military professional. A comparison was also made of the long-term (2007-2017) dynamics of suicide rates and the incidence of mental disorders among military personnel, and certain patterns were identified. Thus, in the group of officers and ensigns, a significant correlation was noted between the long-term dynamics of suicide rates with neurotic (r=0,74) and addictive disorders (r=0,86), as well as general mental morbidity (r=0,83), and for conscripts, with personality disorders (r=0,79) and organic mental disorders (r=0,71). A comparison of the long-term dynamics of the overall incidence of mental disorders and the suicide rate among conscripts also showed a positive correlation (r=0,69). Analyzed the system of prevention of suicidal incidents in military personnel, which currently exists and proposed measures for its improvement, including the areas of activity of officials for the prevention of suicidal incidents in the military. It also indicates the need to shift the focus from specific prevention (direct detection and prevention of suicidal actions in military personnel with mental disorders) to non-specific (prevention of general mental distress and suicidal tendencies in mentally healthy people).

The aim of the study was to identify the main trends in the overall incidence of mental disorders in the Russian Federation in 2017–2019. The analysis of indices based on statistical reports, revealed changes in the direction of the dynamics of the overall incidence of mental disorders in the Russian Federation. In 2018–2019, the indicator increased (by 0,1 %) for the first time after a yearslong (2007–2018) rather intense annual decline. The most noticeable increase is noted in the incidence of organic mental disorders, chronic inorganic disorders, childhood psychoses, including autism, as well as in indices in the groups of children and adolescents 0–14, 15–17 years old and individuals of 60 years and older. The authors of the article conclude that the change in the dynamics of overall incidence in the Russian Federation observed in 2017–2019 indicates a rise in the referral to psychiatric institutions in the last year with an increase in the total number of patients receiving specialized care.

To study the epidemic situation of socially significant diseases in women in correctional facilities of the Russian Federation, models for calculating expected total (cumulative) incidence. There is more than 10 times the total incidence of mental disorders and behavioral disorders (35,179.6 per 100,000 women and 2,764.9 per 100,000 population), the frequency of positive HIV test results by the method of immune blotting (24,836.2 and 426.3 per 100,000 women), the incidence of tuberculosis (394.9 and 34.2 per 100,000 women). There is a tendency to increase the incidence of women in correctional facilities by HIV infection, hypertension, diabetes, malignant neoplasms, reduce the incidence of sexually transmitted infections, viral hepatitis B, C.

The paper discusses the author’s draft of Chapter 15 of the Criminal code of the Russian Federation with a brief comment prepared by Professor G.V. Nazarenko and published in the journal in the framework of the correspondence round table «New Criminal Code of Russia: Conceptual Framework and Theoretical Model». The new title of Chapter 15 proposed in the draft is being discussed, which, like its current title, does not indicate that the measures in question are psychiatric in content. The authors themselves distinguish two types of such measures: «mandatory» in the strict sense of the word, applied to subjects with severe mental disorders, excluding sanity or making impossible the appointment or execution of punishment, and «obligatory», applied to convicts with mental disorders within the limits of sanity. In this regard, it is proposed to name Chapter 15 «Mandatory and obligatory psychiatric measures». The paper discusses the rationale and purpose of the application of mandatory psychiatric measures. According to the authors, the criminal law should disclose the content of the «danger criterion» and define it as a real possibility (a high degree of probability) of committing new acts by this person, provided for in the articles of the Special part of the Criminal Code of the Russian Federation. Analyzing the proposed by G.V. Nazarenko definition of the purposes for the use of mandatory psychiatric measures, the authors believe that it is hardly appropriate to declare such a «change» of the mental state of a person with a mental disorder «which eliminates its social danger» as their purpose. Mandatory psychiatric measures should be aimed solely at improving the mental state of the patient, while the elimination of the danger may in some cases be the result not of an improvement, but of the deterioration of his / her condition or the influence of other unfavourable medical factors. It is concluded that, despite the divergence of scientific positions on a number of issues, the author’s draft of Chapter 15 of the Criminal Code of the Russian Federation, prepared by G.V. Nazarenko, is a significant contribution to the theoretical understanding of the current criminal law governing mandatory medical measures, and outlines generally right directions for its improvement.

The purpose of the study was to determine age characteristics in the dynamics of primary incidence of mental disorders in the Russian Federation in 1991–2018. 2 periods of distinct dynamics of indicators related to the primary incidence of mental disorders were distinguished on the basis of analysis of statistical reports: from 1991 to 2004, when the indicators grew rapidly, and from 2005 to 2018, when the indicators decreased. Change in the direction of the indicator dynamics coincided with the beginning of restructuring of the psychiatric service of the Russian Federation in 2005 as part of the health care reform. Age differences in the dynamics of indicators in 2004–2018 with a significant decrease in the primary incidence of the main forms of mental disorders among working-age people (20–59 years) and an increase in the rates of psychotic disorders among children 0–17 years old were noted. The authors of the article came to the conclusion that one of the main reasons for these changes was a decrease in the number of referrals due to problems of stigmatization and insufficient availability of psychiatric care.
