Renal cell carcinoma (RCC) occupies one of the leading places in the world for morbidity among malignant neoplasms of the genitourinary system. The frequency of occurrence of bilateral RCC according to different authors is 2–6% of the total population of patients with RCC. Currently, the only effective method of treatment of bilateral RCC is surgical treatment. Patients with bilateral RCC are at high risk of dev eloping of local recurrence or progression of the disease after organ-preserving surgeries, which is why the surgeon is faced with a choice between a high risk of developing renal failure or relapse and/or progression of the disease, depending on the extent of the surgical intervention. According to the literature, in patients with bilateral RCC there was an increase in the incidence of papillary variant of RCC up to 19% and the presence of multifocal lesion. Surgical treatment of bilateral RCC is the only effective method to achieve satisfactory oncological results at a low incidence of complications. The m ost justified option for the treatment of bilateral RCC is the implementation of bilateral organ-preserving treatment, which allows achieving the optimal functional results. This article presents a clinical case of successful surgical treatment of a patient with bilateral RCC with multiple tumors.

Along with Crohn’s disease, ulcerative colitis occupies one of the leading places among the most dangerous and severe diseases of the gastrointestinal tract. Persons aged 20–40, belonging to the most active and efficient group of the population, are most susceptible to this pathology. All over the world, there is a steady trend towards an increase in the incidence of ulcerative colitis, and the complexity and high cost of treatment to some extent transfer this problem from a number of purely medical to the category of socio-economic issues. The maximum mortality rates are recorded during the first year of the disease due to cases of extremely severe fulminant course of the disease, as well as 10 years after its onset due to the high probability of developing colorectal cancer. The first report on the surgical treatment of ulcerative colitis in Russia belongs to V. A. Oppel, who in 1907, at the meeting of the Society of Russian Physicians, made a report on the topic: «On the issue of surgical treatment of chronic ulcerative colitis». Since then, surgical intervention continues to be one of the most effective methods of treating this disease.

The aim of the work: to demonstrate modern possibilities and ways of providing rehabilitation of patients after radical surgical treatment of non-tumour colon diseases with available non-typical anatomical and functional states. Materials and Methods. The system of surgical rehabilitation measures was developed, which was used in 32 patients with available atypical anatomical and functional states after radical organ-preserving operations in non-tumour colon diseases. They are aimed at implementing sufficiently radical, reconstruction and restoration operations, forecasting and preventing postoperative complications, improving functional results, reducing the incidence and severity of severe forms of diarrhea and postcolectomy syndromes. Results and Discussion. In 2 (6.3 %) patients there were complications due to the implementation of reconstruction and restoration stage of surgical intervention. They led to the necessity of extirpation of the anal canal fistula, removal of ileoendoanal anastomosis, and the formation of a lifelong ileostomy. In 30 (93.7 %) patients, favourable functional results were obtained, and a significant level of surgical and social work rehabilitation was achieved. A significant level of rehabilitation of operated patients was achieved through the implementation of sufficiently radical and developed reconstruction and restoration operations that prevented the relapse of the disease, the emergence of pathological conditions caused by the removal of the large intestine; prognostication and purposeful prevention of severe postoperative complications, stage monitoring of the condition of operated patients. Conclusions. 1. Surgical rehabilitation of patients operated for serious non-tumour colon diseases is a complex of organizational and therapeutic and prophylactic means aimed at returning to a society of a citizen sufficiently adapted to the conditions of modern society. Surgical rehabilitation of patients with atypical anatomical and functional states in the radical surgical treatment of non-tumour colon diseases requires further improvement of surgical treatment methods and lifelong monitoring of the condition of operated patients. 3. Performing sufficiently radical surgical operations, developing new reconstruction and rehabilitation operations for existing non-typical anatomical and functional states contributed to the provision of favourable functional results, a significant level of surgical and social and labour rehabilitation in 93.7% of patients.

Introduction. Despite the improvement of various methods of surgical and ablative methods of treating kidney cancer, the question remains about the functional state of the kidney after surgical treatment. Along with the achievement of oncological results, the preservation of renal function is the main task in the treatment of tumorous diseases of the kidneys. Aim. To evaluate the functional results of organ-preserving operations with a preventive suture with preserved blood flow, as well as in patients who underwent a resection of the kidney with compression of the renal pedicle, using nephroscintigraphy. Material and Methods. The study included 175 patients in stages pT1a-T2bN0M0, which are divided into 2 groups. Group 1 – 150 patients who underwent organ-preserving options with the use of preventive hemostatic sutures, allowing operations to be performed without clamping the kidney leg. 25 patients underwent a resection of the kidney with compression of the renal pedicle. To assess renal function, patients underwent dynamic nephroscintigraphy. In the first group, the tumor was located in the central segment in 64 patients, in the upper segment in 45 patients, in the lower segment in 41 patients. Stage T1a was established in 74 patients, stage T1b in 53 patients, stage T2a in 4 patients. In 19 patients diagnosed with cystic T1a cancers. In the second group in the central segment – in 10 patients, in the upper segment in 7 patients and in 8 patients in the lower. According to the stages, patients were divided as follows – T1a – 16, T1b – 8, T2a – 1. Results. Organ-preserving operations were performed for all patients. In the first group, in no case did the renal pedicle be pinched. The average time of thermal ischemia in the second group was 13 ± 2 min. The perfusion and renal index for the operated kidney in both groups was comparable, and amounted to 54.13 ± 2% and 53.24 ± 4%. However, after surgical treatment, the indicators were significantly different. In the first group, the perfusion index was 46.82 ± 2.39% against 35 ± 1.52% in the second, the renal index in the first group was 46.82 ± 2.39%, against 39 ± 0.88% in the second. Conclusion. Thus, even short-term thermal ischemia leads to significant impairment of renal function. Preventive suture allows you to remove a tumor from any part of the kidney, while maintaining blood flow, and allows you to protect the renal parenchyma from ischemic damage.

The volume of derivatives in American banks grows significantly, high risk instruments are back in place and their use becomes more active, global imbalances increase. All of the above requires thorough approaches when creating mechanisms which can neutralize external shocks for the Russian economy and make it possible to develop in the new post-crisis environment.

The combined pathology of intra-mural haematoma (IMH) and penetrating aortic ulcer (PAU) represents disease progression of the PAU with high risk for further progression to either rupture or pseudo-aneurysm formation, and hence surgical intervention should be offered once diagnosis is made regardless of the presentation. We present a 70-year-old fit lady with chronic type A IMH associated with multiple PAUs, diagnosed incidentally, the patient underwent urgent surgical repair with good outcome.

During the second half of 20th century, a high level of plague incidence in the world was in 1960–1979 and 1990–2009. The significant decrease of infection cases was in 1950–1959, 1980–1989, 2010–2015. It is noticed, that the observed cyclical nature of the alternation of high and low incidence plague’s periods, in many respects related to modern trend of climate fluctuations.

During ENT practice, we have to examine the nose and perform several nasal procedures in our clinics. Otolaryngologists are at a high risk of exposure to the COVID-19 virus. Nasal endoscopy is a major procedure for the diagnosis of the nasal and paranasal sinus diseases and introducing proper health services for our patients. In addition, the world is living the era of the COVID-19 pandemic, for which we have to protect ourselves, educate our medical staff, and work together against the spread of this severely contagious disease within our communities. The main purpose of this study is to review the protocol of nasal endoscopy in the ENT clinic and enhance the safest way to deal with patients during this pandemic.(1)
