AbstractTechnical quality of endodontic treatment can influence dental prognosis. The aim of the present study was to evaluate radiographically the initial periapical condition and the technical quality of root canal fillings performed by endodontic residents at UEM in 2015 and 2016. After data collection and descriptive statistics, performed by two independent examiners, the Kappa coefficient was used to assess the agreement between them. The chi-square test was used to verify a possible association of sex, tooth and quality of the restoration, with the presence of a periapical lesion. Most referrals for endodontic treatment were molars (50.5%) and female patients (63.2%), with a mean age of 39.3 (± 13.3) years. Most teeth were diagnosed with pulp necrosis (44.1%), and this condition held 74% of the teeth with chronic periapical lesions. Bone rarefaction was seen mainly in molars (20%) and teeth without restorative treatments (43.5%). Chi-square test evidenced that sex (p=0.303), tooth (p=0.349) and quality of restoration (p=0.070) were not associated with the presence of periapical bone rarefaction. After finishing the 233 root canal fillings, the apical limit and condensation were satisfactory in 91% and 100% of cases, respectively. In conclusion, the presence of chronic periapical lesion was not associated with sex, tooth or quality of the restoration. The root canal fillings performed by endodontic residents was satisfactory. Este estudo teve como objetivo avaliar radiograficamente a condição periapical inicial e a qualidade técnica das obturações dos canais radiculares realizadas pelos residentes em Endodontia da UEM nos anos de 2015 e 2016. Após a coleta de dados e estatística descritiva, realizada por duas examinadoras independentes, empregou-se o coeficiente Kappa para avaliar a concordância entre as mesmas. O teste de qui-quadrado foi utilizado para verificar uma possível associação entre as variáveis sexo, tipo dentário e qualidade da restauração, com a presença de lesão. A maioria dos encaminhamentos para atendimento foram molares (50,5%) e pacientes do sexo feminino (63,2%), com idade média de 39,3 (± 13,3) anos. Uma grande parcela dos dentes foi diagnosticada com necrose pulpar (44,1%), sendo que esta condição abrangeu 74% dos dentes com lesão periapical crônica. A rarefação óssea foi visualizada sobretudo em molares (20%) e dentes sem tratamentos restauradores (43,5%). Por meio do teste de qui-quadrado, houve evidências amostrais de que o sexo (p=0,303), o tipo dentário (p=0,349) e a qualidade da restauração (p=0,070) não estiveram associados à presença da rarefação óssea periapical. Após a finalização dos tratamentos e um total de 233 canais obturados, o limite apical e a condensação da obturação se mostraram satisfatórios em 91% e 100% dos casos, respectivamente. Conclui-se que a presença da lesão periapical crônica não esteve associada ao sexo, tipo dentário ou qualidade da restauração. Introduction: The present study sought to assess the effect of apical foramen cleaning in the repair of periapical lesions detected by radiography. Methods: The sample comprised 980 teeth collected from 25 February 1997 to 15 March 2005 which had been subjected to endodontic treatment and exhibited radiographically visible periapical lesions. The sample was then divided into two groups: Group I, 402 root canal treatments in which cleaning of the apical foramen had not been performed, and Group II, composed of the remaining 578 root canal treatments where the procedure had been performed. After one year, the teeth were clinically and radiographically evaluated. Results: In Group I, 360 canals (89.55%) had no evidence of periapical lesions, versus 521 (90.13%) in Group II. Partial presence of these lesions was observed in 23 canals (5.72%) in Group I versus 27 (4.67%) in Group II. No changes in images were observed in 19 (4.72%) canals in Group I and 30 in Group II. Pearson’s chi-square test showed no statistically significant difference between the Groups I and II (p = 0.732). Conclusions: These findings suggest that foramen cleaning is not a determinant of periapical lesion repair.

ABSTRACTObjective: This study aimed to evaluate the relationship between the quality of the coronal restoration and the root canal filling on the periapical status of endodontically treated teeth using CBCT. Materials and Methods: CBCT data were obtained from the records of patients who deny any dental treatment in the 2 years prior to the CBCT examination. CBCT images (90 kVp and 7 mA, exposure time of 23 s, and a voxel size of 0.2 mm, with a field of view of 13 cm x 13 cm) of 1011 endodontically treated teeth were observed. A score was given to the quality of the root filling and the quality of the coronal restoration. Statistical Analysis Used: Data were statistically analyzed to correlate the periapical status with gender, dental group. and quality of endodontic treatment and restoration (Chi-square test with a significance level of P < 0.001). Results: Absence of periapical periodontitis was found in 54.9% of the cases. The periapical outcome was not related to gender or dental group (P > 0.05). A statistically significant factor (Chi-square test, P < 0.0001) resulted when different qualities of sealing were compared. Conclusions: CBCT showed that high-quality root canal treatments followed by an adequate coronal sealing restoration avoid the presence of periapical periodontitis in time.

The aim of this study was to assess the prevalence and risk factors of apical periodontitis in endodontically treated teeth in a selected population of Brazilian adults. A total of 1,372 periapical radiographs of endodontically treated teeth were analyzed based on the quality of root filling, status of coronal restoration and presence of posts associated with apical periodontitis (AP). Data were analyzed statistically using odds ratio, confidence intervals and chi-square test. The prevalence of AP with adequate endodontic treatment was low (16.5%). This percentage dropped to 12.1% in cases with adequate root filling and adequate coronal restoration. Teeth with adequate endodontic treatment and poor coronal restoration had an AP prevalence of 27.9%. AP increased to 71.7% in teeth with poor endodontic treatment associated with poor coronal restoration. When poor endodontic treatment was combined with adequate coronal restoration, AP prevalence was 61.8%. The prevalence of AP was low when associated with high technical quality of root canal treatment. Poor coronal restoration increased the risk of AP even when endodontic treatment was adequate (OR=2.80; 95%CI=1.87-4.22). The presence of intracanal posts had no influence on AP prevalence.

Objective: To investigate the implication of quality of obturation and coronal restoration on periapical tissue in failed endodontically treated teeth. Study Design and Setting: This cross-sectional research was performed in the Operative Department of BUMDC on 187 patients reporting with root canal failure from March 2019 to August 2019. Methodology: Patients of both genders aged between 20-60 years were considered for this study. Single and multirooted teeth indicated for repeated endodontic treatment due to under filled, overfilled obturation, voids in obturation, absence and presence of coronal restoration were included. One operator carried out clinical examination of the teeth and periapical radiograph was taken for each patient by using E-Speed film and evaluated by the same operator using an illuminated viewer box. SPSS 17 for windows software was used for data entering and chi- square test was applied for statistical calculation of the outcomes. Results: Total n=187 endodontic treated failed teeth were evaluated, out of which 52.9% were of females and 47.1% to males. The number of obturations with acceptable length were 81(43.3%), with adequate density were 107 (57.2%) and with consistent taper were 116(62%). Periapical lesion was observed in 118(63.1%) cases. Quality of obturation significantly affects the periapical health. Cross tabulation showed a significant association (p-value <0.000) between inadequate coronal restoration and changes in periapical area. Conclusion: The successful prognosis of the root canal treatment relies on the good quality of obturation and adequate coronal filling.

Objective: The aim of this study was evaluate radiographically prevalence of apical periodontitis (AP) in endodontically treated teeth (ETT) and relate with demographic factors, quality of endodontic treatment and coronal restoration. Material and methods: Data were collected from dental records wich contained full-mouth radiographic series. In ETT, arch, dental group and presence of AP were evaluated. Quality of root canal filling, presence and quality of coronal restoration and type of restorative material were also collected. Data were evaluated using descriptive statistics and Chi-square Test. Results: Of 70 patients included in the final sample, the majority were female (54.3%) and age ranged from 13 to 77 years (47.14 ± 13.18). Of 1,333 teeth evaluated, 73 (5.4%) had endodontic treatment, being the majority maxillary (74%) and anterior teeth (52.1%). Of teeth without endodontic treatment, 320 (25.4%) had AP. Failure rate was high (52.8%) and quality of root canal filling and coronal restoration were considered inadequate in majority cases (58.3% and 47.7%, respectively). Dental group had a statistically significant relationship with presence of AP in ETT, being more frequent in anterior teeth (p = 0.019). Conclusion: It was concluded that prevalence of AP in ETT was high and dental group was the factor that most influenced prevalence of AP in ETT.KeywordsApical periodontitis; Radiography; Root canal filling.

Aim: A retrospective clinical audit was carried out on records of endodontic treatment performed by dental undergraduates. The audit was performed to evaluate the technical quality of root canal fillings performed by dental undergraduates and determine the associated factors. Methods: 140 records of patients who had received root canal treatment by dental undergraduates were evaluated through periapical radiographs by two examiners (κ =0.74). The root canal fillings had their quality evaluated according to extent, condensation and presence of procedural mishap. Possible factors associated with technical quality such as tooth type, canal curvature, student level and quality of record keeping were evaluated. Data were statistically analyzed using chi-square test (p<0.05). Results: Among the 140 root-filled teeth, acceptable extent, condensation and no-mishap were observed in 72.1%, 66.4% and 77.9% cases respectively. Overall, the technical quality of 68 (48.6%) root-filled teeth was considered acceptable. Overall, non-acceptable root canal fillings were significantly more likely to be observed in molars (69.2%), moderately and severely curved canals (71.4%) and junior students (61.5%). There was no association between acceptable root canal fillings and quality of record keeping. Conclusions: The technical quality of root canal fillings was acceptable in 48.6% cases and it was associated with tooth type, degree of canal curvature and student seniority.

Abstract Introduction Endodontically treated teeth with extensive coronary destruction require posts and cores to enable prosthetic restoration. Despite the increasing use of prefabricated posts, cast metal posts and cores are still widely used. The quality of the latter is important for the longevity of restorative treatment, and failure can occur if the fundamental principles are not followed. Objective To radiographically evaluate the prosthetic principles of 1000 cast metal post-and-core restorations performed in single-rooted teeth and their coherence with the principles used for their confection. Material and method Digital periapical radiographs of 1000 endodontically treated, single-rooted teeth with cast metal posts and cores were selected from the collection of a radiology clinic (Centro de Imagem, Aracaju/SE, Brazil). The images were analyzed using a measurement software (ImageJ, USA) in accordance with the fundamental prosthetic principles: length and diameter of the post, ratio between post and bone crest, contiguity of post to the root canal, gap between post and the remaining root canal filling, amount of remaining root canal filling, and absence of periapical lesion. The data were qualitatively analyzed, classified into ideal and not ideal, and submitted to the chi-square test (α=0.05). Result Only 6.7% of the cast metal posts analyzed were satisfactorily fabricated. Conclusion Many prosthetic criteria are neglected during the manufacturing of cast metal post and cores, resulting in inadequate work that may compromise the longevity of restorative treatments.

ABSTRACTObjectives: The purpose of this study was to evaluate the radiographic quality of root canal fillings performed by first and second clinical year dental students at the Faculty of Dentistry in Süleyman Demirel University, Turkey. Methods: The technical quality of 833 root canal fillings was evaluated by three examiners according to the density of the filling and the distance between the end of the filling and the radiographic apex. A root canal with acceptable filling length and homogeneous root filling was defined as being good quality endodontic work (GQEW). A treated tooth was defined as having good quality endodontic work tooth (GQEW-T) when all its canals had a GQEW rating. Results: A Chi-square (χ2) test at the 0.05 significance level was used for the statistical analysis of the data. Of a total of 833 root canals, 662 (79.47%) root fillings were classed as GQEW. Two hundred fifty canals (73.4%) (135 teeth) treated by first clinical year dental students were defined as GQEW-T while 412 canals (204 teeth) (73.9%) treated by second clinical year dental students were defined as GQEW-T (P>.05). Most of the GQEW-T were defined in anterior teeth (90.1%), whereas the fewest were in molar teeth (46.6%) for both first and second clinical year dental students (P<.001). Conclusions: The quality of root canal fillings in anterior teeth performed by undergraduate dental students in Isparta, Turkey was satisfactory. However, to improve the success with molar teeth, education about newer techniques and instruments must be incorporated into the preclinical and clinical curriculum. (Eur J Dent 2011;5:324-330)

Objectives: This study aimed to evaluate the technical quality of root canal treatment performed by fifth-year underperforming students, in the extra sessions of the 2018–2019 academic year. Methods: Periapical radiographs of teeth endodontically treated by underperforming undergraduate students at King Abdulaziz University Faculty of Dentistry, were collected. The quality of obturation was evaluated radiographically in relation to the length of obturation to root apex, homogeneity and density of obturation, root canal tapering, and incidence of mishaps (such as perforation, ledge, missed canal, separated instruments). The data were subjected to descriptive analysis. Results: The periapical radiographs of 70 treated teeth showed acceptable length, filling density and root canal taper in 96 (80%),50 (41.7%) and 66 (55%) root canals, respectively. Only four teeth were subjected to mishaps. Conclusion: Under the circumstances of this study, the quality of endodontic treatment performed by underperforming students at the extra session was low. More studies are needed to address the student underperformance source and hence amend the quality of root canal filling.
