Histopathological profile of ovarian tumors in a tertiary care center- a descriptive study

Background: Ovarian neoplasms are a heterogeneous group of tumors with varied clinical, morphological and histological features. Ovarian cancer accounts for about 3% of all cancer in females and is the 5th most common cause of death due to cancer because most ovarian tumors spread beyond ovary by the time of diagnosis. The objective of the study was to document the histological pattern and prevalence of ovarian tumors in specimens received at department of pathology government medical college Alappuzha.Methods: This was a prospective study of 18 months duration which comprised of 245 cases of ovariectomy and ovariotomy specimens received in the department of pathology, govt. T.D medical college Alappuzha, Kerala. After detailed and thorough gross examination of the specimens, bits from representative areas were routinely processed and stained with H and E. Tumors were classified as per WHO classification. Appropriate immunohistochemical studies were performed wherever required.Results: Out of 245 cases studied, majority were benign tumors (78.36%), followed by malignant tumors (15. 11%). Borderline tumors comprised (6.53%) of the total cases. Age groups studied ranged from 11-70 years. Epithelial tumors were the most common (76.32%) followed by germ cell tumors (17.55%), sex cord stromal tumors (5.03%) and carcinoma arising in germ cell tumors (0.81%). Serous cystadenoma was found to be the commonest benign tumor and serous cystadenocarcinoma was the commonest malignant ovarian neoplasm.Conclusions: Surface epithelial tumors were the most common ovarian tumors. The maximum number of tumors were noted in the age group 21-40 years. Malignant tumors were common above 40 years.

Background: Ovarian neoplasms are a distinct entity in women health care and are increasingly contributing to morbidity and mortality among women. The burden is not only related to the increasing incidence but also to the varied pathological features depending on the tissue of origin and pathogenesis. The study was carried out to find the prevalence and determine the clinical presentation and histo-pathological distribution of ovarian neoplasms. Management options were also noted.Methods: It is a retrospective study carried out at Goa Medical College between January 2013 to December 2015. All patients diagnosed and treated for ovarian neoplasm were included in the study. Data was tabulated using Microsoft Excel and descriptive statistical analysis was carried out using SPSS version 23.Results: A total of 3111 patients were admitted in gynecology at Goa Medical College during the specified time period. Of these 358 cases were diagnosed with ovarian neoplasm. On histopathology 196 were benign tumors and 162 were reported to be malignant. Commonest presenting symptom was abdominal distention seen in 51.1% of the patients, pain in 44.4%, followed by dyspepsia in 26.85%. Epithelial tumors were most common (Benign - 39.3%, Malignant - 41%) followed by sex cord stromal tumors and germ cell tumors in 7.26% of cases.Conclusions: Surface epithelial tumors were most common neoplasm. An alarming high no. of malignant tumors (45.25%) was found in present study. 44.4% tumors presented in 41-50 years age group. Presenting complaints were vague and nonspecific leading to delay in diagnosis. Histological type correlates with prognosis; therefore, preponderance of histological type will guide treatment options and patient education with respect to epidemiology.

Background: Ovarian tumors account for 30% of all cancers of female genital tract which represents the sixth most common cancer and fourth leading cause of death in women. The present study was done with an objective to determine the frequency and distribution of various ovarian tumors and to study their clinical and histopathological presentations.Methods: It is a retrospective observational study of patients with ovarian tumors subjected to surgery in the department of obstetrics and gynecology, SDM College of Medical Sciences and Hospital from January 2005 to December 2015.Results: There were 642 cases comprising of 77.7% of neoplastic lesions and 22.3% of non-neoplastic lesions. Majority of the ovarian tumors (83%) were seen in the age group of 20 to 50 years. Mean age for ovarian tumors was 38 years.  Among the neoplastic lesions 84% were benign, 14.2% were malignant and 1.8% were borderline tumors. Surface epithelial tumors were the commonest tumors (60.9%) followed by germ cell tumors (12.8%).  Serous cystadenoma was the most common benign tumors (47.1%) followed by mucinous cystadenoma (18.4%). Among the ovarian malignant tumors; serous cystadenocarcinoma was the most common (4.5%) followed by mucinous cystadenocarcinoma (2.6%). Chocolate cysts were the most common among the non-neoplastic lesions (36.4%).Conclusions: Ovarian neoplastic lesions were more common than non-neoplastic lesions. Benign ovarian tumors were common in reproductive age group.  The mean age of occurrence for ovarian tumor was 38 years. The most common neoplasm was surface epithelial tumors, of which serous tumors was the commonest. Chocolate cysts were the most frequently encountered histopathological finding among the non-neoplastic lesions.

Background: Ovarian tumors are histogenetically varied and complex tumors spanning all age groups. They account for 30% of all gynaecologic cancers. Malignant tumors carry a high mortality due to its late detection and ineffective screening programs. Current study aims at finding association between age at presentation and size of tumor with behavior of tumors and also association between category of tumor and age at presentation. Methods: This Hospital record based cross-sectional (historical) study was carried out on 158 cases of ovarian tumors received in Department of Pathology, College of Medical Sciences and Teaching Hospital during a time period of five and a half years from January 2012 to June 2017. Results: Age of the patients ranged from 12 to 88 years with maximum cases 96 (60.8%) in 20 – 40 years age group. The mean ±SD of age was 36.6±14.4 years. Majority of the cases 137 (86.7%) were benign. Sixteen (10.1%) cases were malignant and 5 (3.2%) cases were borderline. Majority of both benign and malignant cases were seen in 20 – 40 years age group. Size of the tumors ranged from 2 – 30 cm with mean±SD of 9.9±5.0 cm and maximum 93 (58.9%) cases in the size range of 5 – 10 cm. Majority of both benign and malignant tumors were in the size range of 5 – 10 cm. There was no association of age at presentation and size of tumor with behavior of tumor (p > 0.05). Sevety five (47.5%) cases were germ cell tumors, 72 (45.6%) cases were surface epithelial-stromal tumors, 9 (5.7%) cases were sex cord-stromal tumors, 1 (0.6%) was bilateral with surface epithelial tumor in right ovary and germ cell tumor in left ovary and 1 (0.6%) case was soft tissue tumor not specific to ovary. Majority of cases of germ cell tumors were seen below 40 years age whereas significant proportion of surface epithelial-stromal tumors was seen after 40 years with significant association between category and age at presentation (p < 0.05). Most common histopathological diagnosis overall was dermoid cyst in 43.7% cases. Conclusions: Benign ovarian tumors were more common than  malignant ones. Malignancy was seen in all age groups and in any size tumor. Surface epithelial-stromal   tumors and germ cell tumors were roughly equal in frequency with slight predominance of latter. Germ cell tumors were common in younger whereas surface epithelial-stromal tumors were more common in older   individuals. Histopathological examination in any ovarian tumor at any age with any size is mandatory.

Background: Ovarian tumors are one of the ubiquitous and common forms of neoplasms in women. The aim of the study was to understand the pattern of benign and malignant ovarian neoplasms and their distribution in different age groups in rural population of India.Methods: A retrospective study conducted in the Department of Pathology in close collaboration with Department of Obstetrics and Gynecology, Dr. Rajendra Prasad Government Medical College, Kangra at Tanda (HP), India. All the patients irrespective of age group who were operated for ovarian neoplasms (benign or malignant) were included in this retrospective analysis over duration of four years (2013 to 2016). “WHO classification system” was used, for classification of all these ovarian tumors. The incidence of these tumors with respect to age group was also studied.Results: During the study period (2013-2016), there were a total of 242 surgeries for ovarian neoplasms. Of these, majority of the tumours were benign 184 (76%), but an alarming number of women had malignant ovarian tumours 51 (21%), remaining 7 (3%) cases were borderline. Age wise distribution was 7% (16/242) in less than 20 years age, 19% (46/242) in 20-30 years age, 29% (69/242) in 30-40 years age group, 24% (59/242) in 40-50 years and remaining 21% (52/242) in more than 50 years age group. Pre-dominantly benign tumors were surface epithelial tumors (serous/ mucinous cystadenoma), germ cell tumors (mature cystic teratoma) and endometrioma. Major malignant tumors were surface epithelial tumors (serous/mucinous cystadeno-carcinoma), and germ cell tumors (dysgerminoma, immature teratoma).Conclusions: In this sub-Himalayan belt of rural India, the incidence of benign ovarian tumors was 76%. Borderline ovarian tumors were seen in 3% cases and the remaining 21% cases were malignant ones. Even though benign tumors were the commonest for each age group, however as the age of women increased the proportion of malignant tumors in them increased. Surface epithelial tumors are the most common class of tumors in both benign and malignant tumors. Serous cystadenoma is the most common ovarian tumor overall as well as most common benign tumor whereas serous cystadeno-carcinoma is most common malignancy. Stromal ovarian tumor (one case) is a rarity. Only one woman had bilateral ovarian tumor.

Background: Ovarian tumors are a heterogeneous group of neoplasms with variable clinical, morphological, and histological features. Ovarian cancer is the leading cause of death in females. Aims and Objectives: (1) To study and characterize the ovarian tumors based on gross and histopathological features. (2) To study prevalence and age distribution of various ovarian tumors. (3) To study the clinical features in patients with ovarian tumors. (4)To compare the frequency of benign and malignant neoplasms of the ovary with other studies. Materials and Methods: This is a prospective study conducted in Upgraded Department of Pathology, Modern Government maternity hospital, and Osmania General Hospital, Hyderabad, Telangana from March 2018 to February 2021. A total of 200 ovarian tumors were studied. Results: Out of 200 ovarian tumors, 132 were benign, seven were borderline and 61 were malignant. The surface epithelial tumors were the most common tumors accounting for 159cases (79.5%), germ cell tumors were seen in 27 cases (13.5%), sex-cord stromal tumors formed 10 cases (5%), and metastasis in 4 cases (2%). Conclusion: Ovary is a common site of tumors in the female genital tract and usually presents with a variety of clinocomorphological and histological features. Benign are the most common, of these surface epithelial tumors are the commonest, affects mainly reproductive age group.

Background: International treatment guidelines dedicated to children and adolescents are not yet established, hence need for such studies like ours is required to throw light on demographic variations of ovarian tumors and developing specific guidelines for their management.Methods: It was a descriptive longitudinal study undertaken in the Department of Obstetrics and Gynaecology from February 2018 to July 2019. According to prevalence in our setup, 50 cases of adolescent girls and children were taken for study and subjected to thorough clinical assessment and investigations. After proper diagnosis, surgery was performed in all cases and depending upon histopathology and cytology, adjuvant chemotherapy was given in selected cases. Patients were followed up till 6 months after discharge.Results: The most common symptom was found to be abdominal pain (42%), most common sign abdominal mass (38%). Out of 50 cases, 12% were found to be malignant, all of germ cell origin. Epithelial tumors were found to be the most common ovarian tumors (52%) up to 20 years of age, out of which benign tumors had 48% epithelial origin. However, up to 10 years of age, germ cell tumors were found to be the most common (75%). Only 1 malignant case expired, belonging to mixed germ cell variety of advanced stage.Conclusions: Ovarian neoplasm is highly challenging, especially in adolescents and children due to the need for fertility preservation. Early detection, optimal therapy and intense follow up with psychological support to patients and families are the mainstay of treatment.

Background: Globally ovarian tumor is one of the leading causes of cancer death among women. Ovarian tumor has varied histiogenesis, clinical behavior and malignant potential. The aims and objectives of the present study is to study the histopathological pattern and age distribution of the ovarian tumors.Methods: A total of 84 cases were studied from January 2016 to December 2016. They were reviewed and analyzed for age, histopathological findings and clinical presentations. Classification was done according to WHO histologic classification of ovarian tumors.Results: Of the 84 cases, 75% were benign, 3.6% cases were borderline and 21.4% cases were malignant. Among the histological subtypes surface epithelial tumors comprised of 66.7% followed by germ cell tumors (23.9%). Serous cystadenoma (36.9%) was the most common benign tumor followed by mature cystic teratoma (17.9%). Serous cyst adenocarcinoma (10.7%) was the most common malignant tumour. Tumors were seen over age range of 10-78 years and maximum number of cases were in the 4th to 6th decade. Younger age group primarily presented with benign tumors whereas malignant tumors were common in elderly age group.Conclusions: Surface epithelial tumors were the commonest ovarian tumor. Maximum numbers of ovarian tumors were in the age range 40-59years and malignant tumors were common in >40 years of age.

Background: Cancer antigen 19-9 (CA 19-9) is a tumor-associated mucin glycoprotein antigen that may be elevated in healthy individuals as well as in patients with benign and malignant tumors. It is useful in the management of pancreatic and other gastrointestinal tumors. CA 19-9 is also elevated in benign and malignant ovarian tumors. Aim: To study the pattern of serum CA19-9 in complex ovarian tumors. Methods: The study design was descriptive, based on data collected from medical records. Patients with a complex ovarian mass, who were investigated with CA 19-9 and had undergone surgery, wereincluded in the study. The study duration was 2 years from January 2014 to December 2015. A total of 273 patients (119 - benign and 154 malignant) with complex ovarian mass and elevated CA 19-9 underwent surgery during the study period. Results: CA 19-9 was elevated in 55 patients (20%). Of these, 23 patients had benign tumors while 32 had malignant tumors.Among patients with benign tumors, 21 had dermoid, 23 had mucinous tumors and 75 had other types of tumors. CA 19-9 was elevated in 10 (47.6%) of the dermoids, 7 (30.4%) of the mucinous tumors and 6 (8%) of the other benign tumors. Among patients with malignant tumors, 138 were epithelial and 16 were non epithelial tumors. Of the epithelial tumors, 31 were mucinous and 107 were non mucinous types. Overall, 29 (21%) had elevated CA 19-9. Of the epithelial tumors, 22.6% of the mucinous type and 20.6% of the non mucinous type had elevated CA 19-9. Among the non-epithelial tumors, 3 (18.8%) had elevated CA19-9. Conclusion: CA 19-9 is elevated in several conditions but most likely to be raised in dermoid cysts and mucinous tumours. CA19-9 levels need to be interpreted along with clinical and radiological findings.

Background: Ovarian cancer accounts for 3% of all cancers in females. About 80% of these are benign, and they occur mostly in young women between 20 and 45 years. Borderline tumors occur at slightly older ages while incidence of malignant tumors increases with age, occurring predominantly in perimenopausal and postmenopausal women. About 190,000 new cases and 114,000 deaths from ovarian cancer are estimated to occur annually worldwide. The aim of the study was to fi nd the incidence of surface ovarian tumor in a tertiary referral centre. Materials and methods: This was a retrospective study carried out in the department of pathology, Manipal Teaching Hospital from January 2001 to December 2012. Specimens were received from the same and other hospitals. Records were retrieved from the departmental data bank and were analyzed. Results: : A total of 310 cases of ovarian tumors have been reported in the same period. Among them, 180 cases were of surface epithelial origin and out of which 24 cases had bilateral tumors. Benign tumors comprised of 148 cases, 6 were borderline and 44 were malignant. Among these, the commonest was serous cystadenoma (98 cases) and the least common was malignant Brenner (2 cases). Combined or mixed tumor was seen in 9 cases. Conclusion: : In our study surface epithelial tumors comprised 58% of all ovarian tumors. In both benign and malignant cases, serous tumor was the commonest followed by mucinous tumors. Abstract BACKGROUND: Ovarian tumors are one of the leading cancers in females with variable pathological types. This study describes the distribution, clinical and pathological details of various histopathological types of ovarian tumors in a tertiary care hospital in North India. MATERIALS AND METHODS: A retrospective data of 3 years were collected for ovarian tumors submitted to the pathology department of a tertiary care hospital. Data were classified according to the latest World Health Organization (WHO) Classification into epithelial tumors, germ cell tumors, sex cord–stromal tumors, and others. RESULTS: A total of 212 cases of ovarian tumors were studied, 186 were unilateral and 26 were bilateral. Resection specimen, part of specimen, and block review formed 80.2%, 15.1%, 4.7%, respectively. Epithelial tumors formed the majority in 71.7% of cases followed by germ cell tumors (22.2%), sex cord–stromal tumors (3.8%) and others (2.3%). Maximum number of cases in the respective groups occurred in the age groups 31–40, 21–30, 51–60, and 41–50 years, respectively. Overall, benign tumors were 63.7%, malignant tumors were 31.1%, and borderline were 5.2%. The most common histopathological type of benign and malignant tumor was benign serous cystadenoma (18.8%) and serous carcinoma (9.9%), respectively. CONCLUSION: In the present study, ovarian tumors were classified according to the WHO classification, epithelial and germ cell tumors were the major types of ovarian tumors. Benign epithelial tumor formed the majority with 46.2% cases. Serous cystadenoma and mature cystic teratoma were the predominant type of epithelial and germ cell tumors, respectively.
