Background and Objectives: Treatment choice for fungal ball is endoscopic endonasal removal. However, it is not easy to remove fungal elements from the maxilla using only an endonasal approach. To overcome this difficulty, we introduced a cotton pledget technique and evaluated its efficacy through a cadaveric study and clinical research.Materials and Method: A cadaveric study was performed using 10 half heads of seven cadavers. The ease and safety of the cotton pledget technique were compared to those of a previously reported technique. In clinical research, we enrolled 52 patients who underwent surgery with the cotton pledget technique and 36 patients who underwent surgery using the conventional endoscopic approach. Demographic factors, preoperative Lund-Mackay (LM) score, sinonasal outcome test (SNOT) score, surgical morbidity, and incomplete removal rate were analyzed.Results: The cadaveric study showed that the cotton pledget technique was easier (p=0.011) than the conventional technique. In addition, clinical evaluation showed that the cotton pledget group had significantly lower incomplete removal rate than that of the control group (p=0.010).Conclusion: The cotton pledget technique is an easy and safe method that enables fungal ball removal more effectively than the conventional technique without need for inferior meatal antrostomy (IMA) or the Caldwell-Luc (CL) approach.

The use of chemotherapy and irradiation for organ preservation attempts to eliminate the need for extensive surgery in patients with advanced squamous cell carcinoma of the head and neck (SCCHN). We sought to characterize the morbidity of surgery in patients who needed surgery after treatment with induction chemotherapy followed by simultaneous chemotherapy and radiotherapy (chemoradiotherapy). The surgical morbidity within the first 30 postoperative days of 17 patients treated in an organ preservation approach between July 1991 and December 1994 was compared with a control group of patients undergoing similar surgical procedures during the same period. The organ preservation study patients underwent surgical procedures consisting of 18 neck dissections and 5 resections of the primary site. Six patients in the organ preservation study group experienced 8 surgical complications within the first 30 postoperative days, and most complications were minor. There was no significant difference in the duration of surgery or length of hospitalization between study patients and matched controls. Our surgical complication rate (35.3%) was higher but not statistically different from that of the control group, and compared favorably to reports of surgical morbidity (44% to 61%) in the literature on patients treated with chemoradiotherapy. The lower complication rate seen in this study may be a reflection of early surgical intervention as part of our organ preservation study scheme, the preponderance of neck dissections performed, and the limited number of pharyngeal procedures performed.

Objective: The study aimed to determine the efficacy of single dose of glucocorticoid (125 mg of Solumedrol intravenous) in terms of seroma formation after mastectomy in patient with carcinoma of breast. Study Design: Randomized control trial. Place and Duration of Study: Study was conducted in the Department of General Surgery, Liaquat National Hospital Karachi, Pakistan from july 1 to dec 31, 2010. Patients and Methods: Patients were randomly divided in two groups (study and control) each group consisting of 30 patients. Randomization was done by opening of a sealed envelope which had a slip bearing the name of study medication (solumederol or saline as placebo) to be administered. The study group received a single dose of inj 125mg solumedrol IV half an hour prior to surgery by resident scrub in surgery. A similar procedure was applied to the control group and patients in controlled group were administered an equal volume of saline intravenously. After drain removal patients in both groups were observed for a duration of 2 weeks for sermoa formation. Detection of seroma formation was based on clinical grounds by absence of any fluid collection at mastectomy bed as detected by manual palpation. SPSS 10 was used for analysis. Results: Seroma formation was observed in 66.7% (40/60) women 2 weeks post drain removal. Rate of seroma formation was significantly low in study groups than control groups (33.3% vs. 100%; p=0.0001). Conclusion: Single dose of steroid is efficacious in reducing the post mastectomy seroma formation.

Penelitian ini bertujuan untuk mendeskripsikan perbedaan motivasi belajar dan keterampilan menulis secara bersama-sama diantara siswa yang mengikuti teknik mind mapping dengan siswa yang mengikuti teknik pembelajaran menulis konvensional, perbedaan motivasi belajar antara siswa yang mengikuti teknik mind mapping dengan siswa yang mengikuti teknik pembelajaran menulis konvensional, dan perbedaan keterampilan menulis antara siswa yang mengikuti teknik mind mapping dengan siswa yang mengikuti teknik pembelajaran menulis konvensional. Populasi penelitian ini adalah siswa kelas VIII SMP Negeri 4 Abang dengan 2 kelas dipilih sebagai sampel. Pemilihan sampel dilakukan dengan teknik Random Sampling. Penelitian ini merupakan penelitian quasi experiment dengan rancangan Non Equivalent Pretest-Posttest Control Group Design. Pengumpulan data dilakukan menggunakan angket motivasi belajar menulis dan tes keterampilan menulis. Data yang diperoleh dianalisis dengan MANCOVA dengan menggunakan motivasi belajar awal dan keterampilan menulis awal sebagai kovariat. Pengujian hipotesis dilakukan pada taraf signifikansi 5%. Hasil penelitian menunjukkan bahwa: pertama terdapat perbedaan yang signifikan pada motivasi belajar dan keterampilan menulis secara bersama-sama antara siswa yang mengikuti teknik mind mapping dengan siswa yang mengikuti teknik pembelajaran menulis konvensional; kedua terdapat perbedaan yang signifikan pada motivasi belajar antara siswa yang mengikuti teknik mind mapping dengan siswa yang mengikuti teknik pembelajaran menulis konvensional; ketiga terdapat perbedaan yang signifikan pada keterampilan menulis antara siswa yang mengikuti teknik mind mapping dengan siswa yang mengikuti teknik pembelajaran menulis konvensional.Kata Kunci : Teknik Mind Mapping, Motivasi Belajar, Keterampilan Menulis This study aimed at describing the difference in learning motivation and writing skill simultaneously of the students who learned by mind mapping technique and those who learned by conventional technique, the difference in learning motivation of the students who learned by mind mapping technique and those who learned by conventional technique, the difference in writing skill of the students who learned by mind mapping technique and those who learned by conventional technique. The data was analyzed by using MANCOVA in which pre-students’ learning motivation and pre-students’ writing skill used as covariat. The hypothesis was tested at 5% level of significance. The results indicated that (1) there was a significant difference in learning motivation and writing skill simultaneously of the students who learned by mind mapping technique and by conventional technique, (2) there was a significant difference learning motivation in writing of the students who learned by mind mapping technique and by conventional technique, (3) there was a significant difference in writing skill of the students who learned mind mapping technique and by conventional technique.keyword : Mind Mapping Technique, Learning Motivation, Writing Skill

Objectives In the setting of craniotomy, complications after traversing the frontal sinus can lead to mucocele formation and frontal sinusitis. We review the etiology of frontal sinus violation, timeline to mucocele development, intraoperative management of the violated sinus, and treatment of frontal mucoceles. Design Case series in conjunction with a literature review. Participants A total of 35 patients were included in this meta-analysis. Nine of these patients were treated at a tertiary academic medical center between 2005 and 2014. The remaining patients were identified through a literature review for which 2,763 articles were identified, of which 4 articles met inclusion criteria. Main Outcomes Measures Etiology of frontal violation, timeline to mucocele development, and method of management. Results The overall interval from initial frontal sinus violation until mucocele identification was 14.5 years, with a range of 3 months to 36 years. The most common cause of mucocele formation was obstruction of the frontal recess with incomplete removal of the frontal sinus mucosa. The majority of patients were successfully managed with an endoscopic endonasal approach. Conclusions Violation of the frontal sinus during craniotomy can result in mucocele formation as an early or late sequela. Image guidance may help avoid unnecessary frontal sinus violation. Mucoceles may develop decades after the initial frontal sinus violation, and long-term follow-up with imaging is recommended. While the endoscopic endonasal approach is usually the preferred method to treat these lesions, it may be necessary to perform obliteration or cranialization in unique situations.

E-learning is a popular technique that can solve learning problems of student. In this study, the learning package of data analysis by using graphing paper to attain linear relationship between independent and dependent variables was invented and utilized. The learning package, consisting of contents and answer key video made from Power Point was converted to video format. The samples were 58 students of Bachelor of Science program who were selected by multi-stage sampling and divided into experimental and control groups. The experimental group composed of 29 students, learnt by using e-learning via LMS Moodle at RMUTP website. The control group, 29 students, was performed by a conventional technique through multimedia teaching with PowerPoint. The research tools consisted of the lesson of graphical analysis, the questionnaire for quality evaluation of the lesson and the achievement evaluation. The results were found that the quality of research tools was very good and the average value of learning achievement of the experimental group was higher than the control group, no significant statistic at level of 0.05 by comparing with statistical method of t-test (n=29). It indicated that e-learning system can be used as a practical tool for increasing skill of the samples as well as correspondence between them in classroom.

Cryopreservation of testicular tissue is beneficial for valuable animals that die unexpectedly or when elective castration is required. Until recently, knowledge regarding cryopreservation of testicular tissue/sperm in the domestic cat has been limited. The objective of this study was to determine the effects of freezing techniques and cryoprotectants on quality of testicular sperm. In Experiment 1, each testis was cut into 10 equal small pieces (∼2 × 3 × 5 mm) and cryopreserved in freezing medium containing with 5% (v/v) glycerol using conventional (10 min in liquid nitrogen vapors) or controlled-rate freezing techniques. In Experiment 2, testicular tissues were conventionally frozen with different types of 5% (v/v) cryoprotectants [glycerol (Gly), dimethylsulphoxide (DMSO), 1,2-propanediol (PrOH), or ethylene glycol (EG)]. Non-cryopreserved testicular sperm was used as a control. After thawing, testicular sperm were extracted and examined for viability and DNA integrity using non-membrane-permeable DNA staining (Ethidium homodimer-1) and TUNEL assay, respectively. Viability of testicular sperm cryopreserved by controlled-rate cryopreservation (45.9 ± 3.7) was significantly lower than non-frozen control (60.3 ± 0.9) and conventional freezing technique (55.0 ± 2.7). Gly (58.2 ± 2.6) and EG (53.3 ± 2.3) yielded a similar viability compared with non-frozen control (P > 0.05), whereas DMSO and PrOH demonstrated an inferior cryoprotectant for feline testicular sperm (% viability for DMSO and PrOH: 46.3 ± 3.3 and 44.3 ± 2.9, respectively). In both experiments, DNA integrity of frozen–thawed testicular sperm did not significantly differ from the control group. In conclusion, cat testicular tissue can be frozen as small pieces using both conventional technique and controlled rate freezing. However, freezing technique and type of cryoprotectant markedly affect the post-thawed quality of testicular sperm. Further study requires examination of the optimal cooling rate during cryopreservation and also the fertilizability of frozen–thawed testicular sperm.
