Ethical Dilemmas of Rehabilitation Counselors: Results of an International Qualitative Study

This study reports the results of an international qualitative study conducted to inform the process of revising the Commission on Rehabilitation Counselor Certification's Code of Professional Ethicsfor Rehabilitation Counselors. The online survey gathered information regarding ethical dilemmas from a sample of certified rehabilitation counselors and Canadian certified rehabilitation counselors. In sum, 240 participants responded to an open-ended survey regarding current or recent ethical dilemmas and anticipated ethical dilemmas in the near future. Because of the functional purpose of the data set, a qualitative content analysis was performed with open coding guided by the structure of the standards outlined in the 2001 Code. Implications for Code revision and ethics governance are discussed.

Purpose:To identify meaningful educative experiences that contributed to the development of rehabilitation counselors’ abilities to provide effective rehabilitation counseling services to lesbian, gay, bisexual, transgender, and queer (LGBTQ) persons living with chronic illness/disabilities (CID).Method:This was a secondary analysis of a larger qualitative data set that examined the educational experiences and practices of 12 certified rehabilitation counselors working with LGBTQ persons.Results:Using a qualitative content analysis methodology, 5 categories were identified: (a) formal didactic education, (b) affirmative supervision and consultation during practica/internship, (c) extracurricular-related training, (d) identification, and (e) family of origin.Conclusion:To help facilitate future counselors’ self-perceived abilities to provide effective services to LGBTQ persons living with CID, rehabilitation counselor educators should place more emphasis on LGBTQ-related issues in their curriculum and provide practical (experiential) learning opportunities for their students.

The current COVID-19 pandemic has affected the entire globe, including the world of high-performance sports. Accordingly, it has been widely assumed that the thereby caused postponement of the Tokyo 2020 Olympic Games could have negative psychological impacts for aspirants, since they were halted abruptly in the pursuit of their Olympic endeavors and their daily lives drastically altered. Considering the sudden nature of the pandemic, few researchers, if any, have yet scrutinized the individual experience of Olympic aspirants. This qualitative study examines the subjective perceptions of the Tokyo 2020 Olympic Games postponement among Austrian Olympic athletes and coaches. To this end, 21 Austrian athletes (13 male, 8 female; mean age = 26.67 ± 4.93 years) and six male coaches were recruited through a criterion-based purposive sampling strategy. Five athletes had already qualified for the Olympic Games in Tokyo 2020 and 15 athletes were still in an ongoing qualification process. Data was collected by means of short written statements, elicited via open-format questions on an anonymous online survey platform. In order to infer meaning from the text, a qualitative content analysis with an interpretative focus was conducted inductively, which allowed for deriving alternative explanations of findings. The results support the notion that the Olympic postponement was experienced in myriad ways by affected participants. Three general themes comprised of several meaning units of different levels of abstraction were created from the text data. Many respondents experienced an immediate emotional reaction to the postponement characterized by confusion, disappointment and/or relief. Participants associated multiple consequences with the postponement, such as the prolongation of physical and psychological pressure, a lack of motivation, concerns about future performance, living and their occupational career, but also the opportunity for performance improvement and recovery. Respondents displayed various coping strategies, such as distancing themselves from sports, cognitive reframing, appealing for acceptance, and planning behavior. This study gleans first insights into the idiosyncratic experience of the Olympic Games 2020 postponement among Austrian aspirants. The findings could serve to assist sport psychologists in their applied practice by informing them about athletes’ and coaches’ needs in their Olympic preparation during the ongoing pandemic.

The professional status and future of rehabilitation counseling, as a profession, has been enhanced through the certification of rehabilitation counselors. Since July, 1974, the Commission on Rehabilitation Counselor Certification has certified more than 8,000 individuals in the profession. These professionals are located throughout the United States, its territories, and three foreign countries. Beginning in July, 1975, extensive demographic information was collected on each applicant resulting in a composite profile on slightly over 50% of the Certified Rehabilitation Counselors (CRC's) certified during the “grandpersoning” period.

Background. Individuals with traumatic brain injury (TBI) face many challenges when attempting to return to work (RTW). Vocational evaluation (VE) is a systematic process that involves assessment and appraisal of an individual’s current work-related characteristics and abilities.Objective. The aims of this study are to (1) examine demographic and employment characteristics of vocational rehabilitation providers (VRPs), (2) identify the specific evaluation methods that are used in the VE of individuals with TBI, and (3) examine the differences in assessment method practices based upon evaluator assessment preferences.Methods. This exploratory case study used a forty-six-item online survey which was distributed to VRPs.Results. One hundred and nine VRPs accessed the survey. Of these, 74 completed the survey. A majority of respondents were female (79.7%), Caucasian (71.6%), and holding a master’s degree (74.3%), and more than half (56.8%) were employed as state vocational rehabilitation counselors (VRCs). In addition, over two-thirds (67.6%) were certified rehabilitation counselors (CRCs). Respondents reported using several specific tools and assessments during the VE process.Conclusions. Study findings reveal differences in use of and rationales for specific assessments amongst VRPs. Understanding VRP assessment practices and use of an evidence-based framework for VE following TBI may inform and improve VE practice.

This article explores ethical dilemmas associated with rehabilitation counselor service provision to individuals with AIDS/HIV, emphasizing the conflict between the duty to protect confidentiality and the duty to warn and the ethical and legal considerations pertaining to this conflict. A discussion on the limits of confidentiality, the application of the six ethical principles, the specific guidelines provided in rehabilitation counselors' code of ethics, and the legal actions that address the issue of duty to warn are provided.

Purpose:This study surveyed current and projected ethical dilemmas of rehabilitation counselors.Method:As a mixed-methods approach, the study used both quantitative and qualitative analyses.Results:Of the 211 participants who completed the survey, 116 (55.0%) reported an ethical dilemma. Based on the descriptions, common themes involved roles and relationships with clients, professional responsibility and competence, and confidentiality and privacy. In addition, projected dilemmas involved social media, health care legislation, insurance concerns, and professional competence.Conclusions:Implications address areas for ongoing discussion, including considerations for future revisions to the Commission on Rehabilitation Counselor Certification Code of Ethics.

Purpose:To identify the current training needs of state-federal rehabilitation counselors and determine if the self-perceived training needs differ for participants who are a certified rehabilitation counselor (CRC) to those counselors with out the CRC credential.Method:A mixed-methods internet-based survey design was utilized and included descriptive, qualitative, and ex post facto approaches on a sample of rehabilitation counselors (N= 341) via the Knowledge Validation Inventory-Revised (KVI-R).Results:The participants reported high or moderate self-perceived training needs on 9 of the 10 content areas on the KVI-R; however, no significant differences between certified and non-certified rehabilitation counselors were found. Highest degree earned and numbers of years in practice were significant predictors of training needs.Conclusion:The results from this study indicate a self-reported need for additional training of state-federal rehabilitation counselors in many of the CORE knowledge domains considered essential for rehabilitation counseling. The findings also indicate that as level of education and experience increased among this sample, the need for training decreased.

The purpose of this study was to explore certified rehabilitation counselors’ (CRCs’) importance of and preparedness in the labor market survey (LMS) competency through data collected by theKnowledge Validation Inventory-Revised(KVI-R) instrument used by the Commission on Rehabilitation Counselor Certification’s (CRCC) 2011 role and function study (CCRC, 2011b; Leahy, Chan, Sung, & Kim, 2011). LMS is a primary data collection method for gathering occupational and labor market information. Thus, this competency was examined because of its importance in the areas of rehabilitation counseling where this data is usually applied—transferable skills analysis, vocational decision making, job development and placement, and forensics. In this ex post facto secondary analysis, the single largest group of CRCs found LMS to beverytoextremely importantbut also reported onlymoderate preparednessto perform the function. Significant differences were found by geographical location, particularly between the Mid and South Atlantic, and in experience. Discussion, conclusions, implications, and areas for further research are addressed.

Many adolescents reach full cognitive development by age 15. Age 18, however, has been designated the age when adolescents may give consent to medical treatment. When the reasoning ability of the adolescent has reached maturation, but the law does not afford the adolescent the ability to utilize his or her reasoning ability, conflicts may arise. Parents may wish one set of treatments for their child and the child may wish for a different type of treatment. When the adolescent is the client of a rehabilitation counselor or healthcare professional, ethical dilemmas may also arise. Supporting the adolescent will support his or her autonomy, however, it may concurrently stifle beneficence or nonmaleficence. This paper addresses issues relevant when dealing with adolescents and informed medical consent. These issues include legal, ethical, and familial implications. Guidelines to assist rehabilitation counselors are also offered.
