Trauma in the anterior teeth is a relatively common occurrence during childhood, having as main consequence teeth with incomplete root development and open apices. Patient, male, leucoderma, 10 years old, attended the Dental Clinic of the Southern University of the State of Santa Catarina, reporting as main complaint fracture in the crown of element 11. After clinical and radiographic examination, it was possible to identify the incomplete apexogenesis of this tooth and the presence of necrotic pulp. Aiming to stimulate neoformation of mineralized tissue at the dental apex, the calcium hydroxide based dressing was used. He remained for eight months, always with radiographic control. Apexification was observed radiographically and proved through the use of an instrument introduced inside the canal. The use of calcium hydroxide as intracanal medication for eight months stimulated mineralized tissue neoformation in the apical region of element 11 and allowed endodontic treatment.

ABSTRACT Aim To evaluate the interference of the intracanal medication Calen® (SSWhite, São Paulo, Brazil) on the filling of simulated lateral canals. Materials and methods Twenty human anterior teeth were used. Before the endodontic filling procedures the access of cavity was made, and after this root canals were made in all the teeth to simulate the presence of lateral canals. After preparation, the teeth were randomly divided into two groups (n=10). In group I, the root canal system was filled directly after chemicalmechanical preparation; in group II, endodontic treatment was performed in multiple sessions, and after preparation the calcium hydroxide-based intracanal medication Calen® was inserted. After the period of 7 days, the root canals were vigorously irrigated and then they were filled. Next, the teeth were radiographed to verify the quality of the filling. Results The results demonstrated that the teeth treated in a single session, without calcium hydroxide medication, presented 47 canals out of 60 with radiographic evidence of filling, whereas the teeth in which intracanal medication was used, only 07 presented a radiographic image compatible with filling (p < 0.05). Conclusion The use of the calcium hydroxide-based medication Calen made it difficult to obtain a hermetic filling of the root canal system. Clinical significance The clinical significance of this work basing on the fact that once the dentist knowing that property obliteration of calcium hydroxide can be taken care when they are used in the presence of lateral canals. ABSTRACT Management of non vital teeth with open apices isa challenge to the dental practitioners. In this clinical scenario, it is difficult to maintain the obturating material confine within the root canal without encroaching into periapical area. These kinds of cases cannot be managed by conventional endodontic treatment, and treatment of such cases with calcium hydroxide may take longer time for apical closure. But with this new material called BIODENTINE (Septodont) same treatment can be done in single visit with predictable result. Hence this case report present the use ofbiodentine to form an apical plug in open apex followed by complete root canal obturation using thermoplasticized guttapercha.

SUMMARY Tooth wear is a multifactorial condition of growing concern. In clinical practice, it is often a challenge for prevention and treatment since many etiological factors may be involved. This case report describes an esthetic rehabilitation of a young patient presenting tooth wear due to erosion. The etiological factor of this case was the patient sucking on lemons, an acidic fruit. The main complaint of the patient was the appearance of short maxillary incisors. The treatment involved orthodontic and restorative dentistry techniques. First of all, an orthodontic device was used to increase the vertical dimension of occlusion and create an adequate space for the direct restorations. The posterior teeth were restored with two direct composite resin techniques; the anterior teeth were then restored using a balanced occlusion. The seven-year follow-up of the case is presented. Replicas of the restorations were made and visualized under scanning electron microscopy up to the 12-month evaluation. In the clinical follow-up at seven years, maintenance of the results and restorations can be clearly seen.

Besides the risk of filling material extrusion throughout the apex, a satisfactory apical seal can be difficult to achieve in canals with open apices or iatrogenic enlargements of the apical constriction. These situations pose a challenge to root canal filling. This paper describes the root canal filling of a maxillary right canine with an overinstrumented apex, complete loss of the apical stop, extensive canal transportation and apical periodontitis. A 5 mm calcium hydroxide apical plug was placed before root canal filling. The plug was made by soaking paper points with saline, dipping the points in calcium hydroxide powder and then applying it to the apex several times, until a consistent apical plug was obtained. The canal was then irrigated with saline in order to remove any residual calcium hydroxide from the root canal walls, dried with paper points and obturated with an inverted #80 gutta-percha cone and zinc oxide-eugenol based sealer by the lateral condensation technique. An 8-year radiographic follow-up showed formation of mineralized tissue sealing the apical foramen, apical remodeling and no signs of apical periodontitis.

Trauma of anterior teeth is quite a common occurrence in both children and adults. Various degrees of trauma leading to fracture may affect teeth in different ways depending on the age of the patient and extent of fracture and other factors that will be discussed. Guidelines have been given as to how each of these situations should be treated. In the past, often more aggressive restorations were performed to restore fractured teeth. However improved and more efficient adhesion may affect the type of treatment we decide to carry out, leading to more conservative therapies through an increased preservation of tooth structures.

ABSTRACT Objective: The present in vitro study evaluated, by means of the photoelastic technique, the effects generated by the Connecticut Intrusion Arch (CIA), with a 90o bend on the distal surface of molar tubes and using the 4 x 2 appliance on the anterior and posterior regions of the upper dental arch. Methods: Five models were manufactured, in which two different clinical situations were correlated: 1) use of intrusion arch not cinched back and transpalatal bar for anchorage (Group 1); 2) use of intrusion arch cinched back and transpalatal bar for anchorage (Group 2). Stress generated in the apical and middle regions of tooth roots of maxillary anterior teeth and maxillary first molars was evaluated. Results: Taking a reference value of 1.0 MPa = 100%, qualitative descriptive analysis was performed, which showed uniformity between stress values in the apical region of anterior teeth of both groups (G1 and G2). In the posterior region, for models with the arch cinched back (G2), stress remained within 100%. As for G1 models (with the arch not cinched back), variations in the mesial surface of first molars were observed, with an increase of 20% in the generated stress. The apical region did not undergo any changes, while in the distal region of molars there was a decrease of 20% in stress. Conclusion: Laboratory results revealed differences in stress between Groups 1 and 2 in the molar region, thereby indicating that there was a tendency towards mesial root tipping of first molars when the distal end of the CIA was not cinched back.

Mesiodens is a midline supernumerary tooth commonly seen in the maxillary arch and the talon cusp is a rare dental developmental anomaly seen on the lingual surface of anterior teeth. This paper presents a rare clinical case of development of talon cusp in a mesiodens with multiple lobes, which interfered with both occlusion and appearance of an 11-year-old patient. During clinical interview, the patient reported difficulty on mastication. Clinical and radiographic examination revealed that a supernumerary tooth with completely formed root was causing an occlusal interference. The supernumerary tooth was diagnosed as multi-lobed mesiodens associated with a palatal talon cusp. The treatment plan consisted in the extraction of the supernumerary tooth followed by orthodontic treatment for diastema closure and tooth alignment.

Nasopalatine cysts (NPCs) present as maxillary midline radiolucencies and are the most common radiolucency of non-odontogenic origin. The proximity of these radiolucencies to the apical region of the maxillary anterior teeth may complicate diagnosis. We are presenting two cases of maxillary midline radiolucencies with a similar presentation but different diagnoses. Reaching a definitive diagnosis may be complicated, even when a detailed clinical examination has been carried out. The use of CBCT has been a valuable addition to the diagnostic armamentarium which can help clinicians reach more accurate diagnoses in such cases. A definitive diagnosis may be established following histopathological analysis, which is indicated in symptomatic cases. CPD/Clinical Relevance: This article highlights the importance of careful assessment of apical radiolucencies associated with the maxillary midline, leading to the correct diagnoses and appropriate treatment planning.

Purpose: This study was undertaken to compare the clinical and radiographic effectiveness of Mineral Tri-oxide Aggregate (MTA) and Calcium Hydroxide in apexification of traumatized young permanent incisors. Methods: Thirty permanent incisors with necrotic pulps and open apices were evenly divided into two groups –Group I (MTA group) & Group II (Calcium Hydroxide group) and treated by apexification. The time taken for apical barrier formation was analyzed. In MTA group, obturation using gutta-percha points was done after 24 hours, whereas in Calcium Hydroxide group obturation was carried out after radiographic confirmation of an apical barrier. Follow up evaluation (clinical and radiographic) was carried out at- 3, 6, 9 and 12 months. Results: The mean time taken for barrier in Group I was 4.50 ± 1.56 months whereas for Group II was 7.93 ± 2.53 months (p value- 0.0002). Radiographic evidence of mean time taken for completion of lamina dura in Group I was 4.07 ± 1.49 months whereas the time period for Group II was 6.43 ± 2.59 months (p value- 0.0067). Conclusion: MTA demonstrated good success and an effective option for apexification with the advantage of reduced treatment time, good sealing ability, biocompatible and provides barrier for immediate obturation.
