Jember Regency is one of the agro-industrial areas in Indonesia with a variety of activities that can produce noise including milling industry. Exposure to noise that is too strong or too long will damage the auditory nerve. The type of material being grounded determines the amount of noise produced so that the potential for hearing nerve damage also varies. This study aims to determine the level of noise produced by the milling industry and analyze the potential for hearing nerve damage in milling workers in Jember Regency. This type of research is observational analytic with cross-sectional approach. This research was conducted by measuring noise in several grinding locations in Jember that were randomly determined. Types of careful milling is grinding meat, coffee, rice, sticky rice and coconut. In addition to measuring the noise level, a short interview was also conducted with the mill workers regarding how long they worked at the mill every day. The measurement results are then analyzed descriptively and compared with standards set by the government and the Occupational Safety and Health Administration (OSHA). In this study it was found that the grinding of meat raw materials produced an average noise level of 88.5 db, coffee of 88.5 db, rice of 88.9 db, sticky rice of 87.3 db and coconut of 80.7 db. The duration of exposure to noise in milling meat, coffee, rice and sticky rice ranges from 9-12 hours a day. While the duration of noise exposure to coconut milling workers ranged from 6-9 hours. Therefore it can be concluded that milling meat, coffee, rice and sticky rice has the potential to cause hearing loss for workers. Keywords: agroindustry, hearing loss, noisy, milling

This chapter discusses the roles of government in promoting occupational and environmental health, with a focus on the U.S. federal government. Governmental interventions, as described here, can range from non-regulatory interventions, such as dissemination of information or generation and communication of information, to establishing regulatory requirements through the promulgation and enforcement of standards and regulations. The chapter describes the U.S. laws and roles of the administrative agencies responsible for occupational and environmental health, including the Occupational Safety and Health Administration, the Mine Safety and Health Administration, and the Environmental Protection Agency. Noting the budgetary and political constraints on these federal agencies, the chapter goes on to discuss briefly the role of the public and the states. The government also plays a role when preventive efforts fail, and the chapter provides a brief summary of programs designed to provide compensation to injured workers.

Noise-induced hearing loss (NIHL) is the second most prevalent illness in the mining industry. According to a study conducted by the National Institute for Occupational Safety and Health (NIOSH), in which over 42,000 audiograms from metal/nonmetal miners were analyzed, approximately 70% of miners have hearing impairment as compared to 9% of non-occupationally noise-exposed workers. One of the machines used extensively in metal/nonmetal mines responsible for high noise exposure levels of its operators is the jumbo drill, used to drill holes at the mines for blasting purposes. In this context, NIOSH is conducting research to develop engineering noise controls for jumbo drills that would reduce the prevalence of hearing loss among operators of this equipment. The first step of the noise control development process consists of identifying and ranking dominant noise sources present during operation of the jumbo drill. To this end, a noise study was conducted at NIOSH’s laboratories in which a microphone phased array system was used to identify dominant noise sources, and the transfer path analysis method was used to rank these sources based on their contribution to the operator location. Results showed that the drill string and the drilling mechanism — known as the drifter — are the dominant sound-radiating components in the operation of the jumbo drill.

In 2014, the Institute for Noise Control Engineering (INCE) Foundation, the Noise Control Foundation and the National Institute for Occupational Safety and Health organized a meeting of industry, government, and academic experts to discuss "Reducing Noise Exposures in the Manufacturing: Best Practices, Innovative Techniques, and the Workplace of the Future." This presentation will review the content of the recommendations for hearing loss prevention programs, successful implementations for noise control engineering, and new techniques to predict noise exposures in the workplace. Efforts to develop Buy Quiet programs and to promote the Safe-in-Sound Excellence in Hearing Loss Prevention and Innovation will be reviewed.

Abstract: Hearing loss due to noise (Noise Induced Hearing Loss - NIHL) is hearing impairment, either part or all, loss, irreversible, occur in one or both ears, can be mild, moderate or severe, occurs because of exposure to the constant noise from the environment. The musician is one of the communities that are susceptible to noise induced hearing loss. Musicians had considerable exposure to noise. Most musicians have a hearing loss in varying degrees. The research objective is to determine the profile of hearing loss in the musicians in Manado, researching threshold musical sound that can cause hearing loss, and looking for a relationship with duration of music exposure to hearing loss. Research is explanatory research, analytical research methods and cross-sectional approach. Research conducted on hearing music group in the City of Manado. The study involved 30 people with the details of 15 respondents guitar players and 15 drummers. The result is: The degree of noise in the gym most of the 110-130 dB, experienced by 46.7% of respondents, mostly studied band of men which is about 90%. Hearing loss is the most followed by drummer and then gitarist, 46.7% and 36.6%, respectively; based upon long exposure to music,> 5 years 46.6%, based on length of exercise per day, more in the 3-4 hours it is 40%. Based on the result of the study, there is a relationship between noise exposure and hearing loss in musicians in the city of Manado. The noise level, exposure time per day, duration of exposure and vulnerability of respondents, closely related to hearing loss disorder the musicians in this study. Pemusik adalah salah satu komunitas yang rentan terhadap gangguan pendengaran akibat bising. Pemusik mengalami paparan suara yang begitu besar. Kebanyakan pemusik mengalami gangguan pendengaran dalam berbagai derajat. Tujuan penelitian adalah untuk mengetahui profil gangguan pendengaran pemusik di kota manado, meneliti ambang batas suara musik yang dapat menyebabkan gangguan pendengaran serta mencari hubungan lama paparan musik dengan gangguan pendengaran. Penelitian bersifat Explanatory Research, dengan metode penelitian analitik dan pendekatan cross sectional. Penelitian gangguan pendengaran dilakukan pada kelompok musik di Kota Manado. Penelitian ini melibatkan 30 orang responden dengan rincian 15 pemain gitar dan 15 pemain drum. Hasil yang diperoleh adalah: Derajat kebisingan pada tempat latihan paling banyak antara 110-130 dB, dialami oleh 46,7 % responden, personil band yang diteliti kebanyakan laki-laki yaitu sekitar 90 %. Gangguan pendengaran terbanyak adalah pemain drum diikuti oleh gitaris 46,7 % and 36,6 %. Berdasarkan lama paparan musik, > 5 tahun 46,6 %; berdasarkan lama latihan per hari, lebih banyak pada 3-4 jam yaitu 40 %. Berdasarkan hasil penelitian kesimpulannya, terdapat hubungan antara paparan kebisingan dan gangguan pendengaran pada pemusik di Kota Manado. Tingkat kebisingan, waktu paparan perhari, lama terpapar serta kerentanan responden, berhubungan erat dengan pendengaran pada pemusik pada penelitian ini. Physical damage from acoustic energy is not a new phenomenon. The first recorded event is in Biblical history when Joshua lead seven priests around Jericho seven times blowing trumpets and the people shouted and the walls fell. Ancient mortar and gun battery crews put cotton in their ears to lessen the sound, and this was a type of noise control. Since World War II much concern has been exhibited about hearing loss due to high noise levels in work environments, In order to limit the hearing loss of the average person, the Occupational Safety and Health Act of 1970 specifies the noise exposure of an employee for an eight-hour day shall not exceed 90 dBA. Noise-induced sensorineural hearing loss has been associated with Industry for many years. One conservative estimate suggests that 10 million Americans may have industry-related, noise-induced hearing loss. Acoustic trauma from any source, whether associated with work or recreations, is detrimental to hearing. The Occupational Safety and Health Administration has set industrial standards for noise levels, with current standards limiting noise exposure to 95 dBA for 2 hours daily. To date, however, there are no recreational standards. Many portable headphone cassette radios produce peak outputs of more than 100 dBA. Temporary threshold shifts could result from listening levels near the maximum output. Permanent sensorineural loss may result with repeated exposure. A pilot study was conducted in which 16 volunteers listened to headphone sets for 3 hours at their usual maximum level. Six volunteers showed transient shifts of 10 dB, and one volunteer showed a transient shift of approximately 30 dB. These shifts returned to normal within 24 hours. As expected, transient shifts frequently occur with recreational use. Therefore, recreational warnings and standards should be established.

Noise exposure can leads to health effect such as Noise Induced Hearing Loss (NIHL) and tinnitus. Besides workplace, noise can be obtained from using earphones. The aim of this study was to analyze the effect of using hearing protection and earphone on NIHL and tinnitus in workers at PT DPS machine workshop. This was a cross sectional with observational analytic study design. This study was conducted in 37 workers at PT DPS machine workshop exposed to noise by using simple random sampling. Results showed that noise level in workplace was 88,57 dBA (8 hours TWA) which exceed TLV 85 dBA. Prevalence of NIHL and tinnitus in workplace was respectively 21.6% and 54%. NIHL affected by using hearing protection and earphone (MLR, p < 0.01; p < 0.05 respectively) and tinnitus affected by using earphone (MLR, p < 0.01) after controlled by working period. It can be concluded that prevalence of NIHL and tinitus are relatively high, NIHL affected by using hearing protection and earphone while tinnitus affected by using earphone. It is recommended for workers to use hearing protection in noise workplace and to reduce using earphone while PT DPS must provide hearing protection and do briefi ng to increase workers’ awareness about using hearing protection.
