Fractures in children are important problem nowadays. Governmental census lists people under 17 years old as the largest contributor to fracture cases in Indonesia. The rapid increase in motor vehicle use, a hallmark of economic growth in developing countries, led to sharp increase in road saturation and consequently, rise in traffic related injuries. Variables involved in pediatric fractures needs to be identified and mapped to provide basis for the creation of better preventive measures to reduce problem before it happens. Thus, the purpose of this research is to create a profile of fracture in patients under the age of 17 years’ old at RSUD Dr Soetomo. The research is descriptive study which is performed by analyzing medical records in RSUD Dr Soetomo against the following: Age, Sex, Type of fracture, Cause of fracture, Time of fracture, Location of fracture, and Duration between admission and treatment. Data is then tabulated and converted into a bar graph for easier analysis. The research found that fractures in children most commonly occur at the 10-14 years’ age group (41.8%), happens more frequently in boys than in girls (69.5%), is dominated by closed fractures over open fractures (75.9%), and is most often caused by traffic accidents (60.9%). Consequently, the location in which fractures are most prevalent is the “street” (63.1%). It is also found that the highest incidence of fractures cases happens within the 12.01-18.00 time-span (19.1%) and is treated within the first 8 hours of admission to the hospital (79.4%).

Objective: This study aimed to analyze the presence of lesions in the skull and face and the associated factors in pedestrian victims of traffic accidents. Methods: A cross-sectional, descriptive-analytical study carried out through the analysis of medical records of pedestrian victims of traffic accidents in an emergency service in the city of Campina Grande, Brazil, during the year of 2016. Information was collected regarding gender, age group, day of the week, time of the accident, type of vehicle involved, presence of trauma to the skull and face, and outcomes. The Chi-square and Fisher's Exact tests were used, with a significance level of 5%. Results: A total of 1,884 medical records were evaluated, out of which 7.1% (n = 133) involved pedestrians. Men were the most frequent victims (68.4%), and victims of age 60 years old or over (30.5%) predominated. Almost one-third of the cases were recorded during the weekends (30.5%), and the most prevalent time was at night (52.7%). Regarding the type of vehicle involved, motorcycles predominated (47.4%). Head trauma was present in 37.6% of victims, while facial injuries corresponded to 8.2%. In 12% of cases, the victims died. The variables of gender, age group, occurrence on weekends, and trauma to the face showed a statistically significant association with the occurrence of traffic accidents (Chi-square test; p<0.05). Conclusion: Among pedestrian victims of traffic accidents, there is a predominance of men aged 65 years or over. Accidents are frequent at night, and motorcycles are the main vehicles involved. The presence of trauma to the skull and face regions is high.

This was an epidemiological study through a retrospective review of 215 medical records of patients with brain injury due to land traffic accidents treated at Ninh Binh Provincial General Hospital. Theresults showed that the majority of research subjects were male, accounting for 80.5%; The age group with higher prevalence of accidents was under 30 years old, accounting for 31.2%; The agegroup of 50-59 years old was at lower prevalence of accidents accounting for 15.8%. The time of the higher chance of occurring accident was between 4p.m and 11p.m daily. The majorityof traffic accidents occurred when riding motorbikes and electric bicycles (82.3%). About 75.0% of the victims received first aid with a gauze bandage; 5.4% of them had broken bones fixed. Amongthe brain injuries, 34.4% of the victims suffered from frontal trauma; 29.3% had unilateral cerebral hemisphere injury, 24.2% had temporal region injury, 15.8% had occipital region injury, and 8.4% ofvictims had parietal injury. More than half of the study subjects (53.5%) were hospitalized for 7-10 days. The rate of patients who have been discharged from the hospital with improved symptoms was81.4%; and 14.9% of patients were transferred to higher level hospitals.

OBJECTIVE: Identifying the frequency and gravity of injuries in patients who suffered accidents in traffic and the analgesic drugs utilized. METHODS: Retrospective study, with a sample of 200 medical records of patients admitted to the emergency services of a reference hospital for trauma care. The gravity of the injuries was characterized by anatomic gravity rates and the analgesic therapy was based on the World Health Organization's Analgesic Ladder. RESULTS: The main findings pointed to injuries in limbs, head, face and outer surface as the most frequent, and, in 85% of the cases, gravity was equal or lower than 3; As for analgesia, it was verified that 46;6% of the patients received dipyrone and paracetamol. Among the opioids, meperidine was used in 10.4% of the cases. CONCLUSION: The gravity of most injuries was equal or lower to 3, indicating injuries of light, moderate and serious gravity, located especially in four body regions; regarding analgesia, dipyrone was shown to be the most commonly-used drug and a low use of opioids was verified.

Introduction. Correct diagnosis is based on visualization and knowledge of fracture patterns characteristic of children. Traditionally, radiography is used to visualize bone damage. In fractures in children due to the high risk of damage to the germinal zones, it is necessary to have clear and reliable information about their condition, which cannot always be obtained by x-ray method. For these purposes and the requirements of modern surgery, CT is used, which with high diagnostic accuracy clarifies the degree of displacement of fragments, present a qualitative characteristic of fractures and reveals associated damage.Purpose: to show the capabilities of computed tomography in diagnosis of fractures of ankle joint.Materials and methods. The results of computed tomography (CT) are presented in 226 children and adolescents aged 3 to 17 years. There were 142 boys (62.8%), 84 girls (37.2%). Scanning was carried out depending on the weight and age of the patient with the minimum indicators of kV and mAS, a slice thickness of 0.75 mm.Results. Average time for seeking medical help was 32 hours from moment of injury. Among causes of injury in the first place was domestic injury (73, 8%), followed by sports injury (22, 3%) and traffic accidents (3.9%). Right-sided ankle fractures were found in 147 (65.1%), left-sided – in 79 (34.9%) children. The most common types of tibial fractures were metaepiphysiolysis of the distal tibia and metaepiphysiolysis of the distal tibia of both tibia, which together accounted for 67.7% (n = 153) of all tibial fractures.Conclusion. Computed tomography should be performed in all cases of intraarticular fractures of the ankle joint. It is especially important for evaluating fractures with damage to germ zones. Scanning must be carried out with a cutting thickness of not more than 1 mm. CT with multi-planar data reformatting is an important factor in determining whether a patient needs surgical treatment. As a result of CT, a final diagnosis was established and a decision was made on treatment tactics.

In this study, a series of instances of children injured by a motor vehicle set in motion by an unsupervised child are reviewed. During a 24-month period, nine such children were identified through a multihospital and coroner's office monitoring system in a single urban county. Injuries ranged from multiple abrasions and contusions to serious leg and head injuries. Three children died. The typical circumstance involved a child releasing the brake or placing the vehicle in gear in a private driveway which resulted in the vehicle striking or rolling over the victim. In four of the nine cases, the child who set the vehicle in motion fell or jumped from the vehicle and then became the injured victim. The extent of these unusual motor vehicle-related injuries is unknown because they are unlikely to be reported in official police statistics. According to the study findings, there is a need to educate the public and health professionals about the risks associated with leaving a child unattended in a motor vehicle and the hazardous environment of the private driveway. Preventive measures would include not leaving a child unattended in a vehicle, locking unattended vehicles to prevent access, and redesigning of private driveways.

Abstract Background The outbreak of coronavirus disease 2019 (COVID-19) has become a large threat to public health in China, with high contagious capacity and varied mortality. This study aimed to investigate the epidemiological and clinical characteristics of older patients with COVID-19 outside Wuhan. Methods A retrospective study was performed, with collecting data from medical records of confirmed COVID-19 patients in Zhejiang province from 17 January to 12 February 2020. Epidemiological, clinical, and treatment data were analyzed between older (≥ 60 years) and younger (&lt; 60 years) patients. Results A total of 788 patients with confirmed COVID-19 were selected; 136 were older patients with corresponding mean age of 68.28 ± 7.31 years. There was a significantly higher frequency of women in older patient group compared with younger patients (57.35% vs 46.47%, P = .021). The presence of coexisting medical conditions was significantly higher in older patients compared with younger patients (55.15% vs 21.93%, P &lt; .001), including the rate of hypertension, diabetes, heart disease, and chronic obstructive pulmonary disease. Significantly higher rates of severe clinical type (older vs younger groups: 16.18% vs 5.98%, P &lt; .001), critical clinical type (8.82% vs 0.77%, P &lt; .001), shortness of breath (12.50% vs 3.07%, P &lt; .001), and temperature of &gt; 39.0°C (13.97% vs 7.21%, P = .010) were observed in older patients compared with younger patients. Finally, higher rates of intensive care unit admission (9.56% vs 1.38%, P &lt; .001) and methylprednisolone application (28.68% vs 9.36%, P &lt; .001) were also identified in older patients compared with younger ones. Conclusions The specific epidemiological and clinical features of older COVID-19 patients included significantly higher female sex, body temperature, comorbidities, and rate of severe and critical type disease.

Abstract Male genital injuries in the form of avulsed laceration of penis and scrotum are less frequent injuries in urological practice. The cases that occur are mostly caused by road traffic accidents, animal attacks, machinery-related accidents and physical/sexual assaults. Here, we report a case of a 28-year-old male with avulsion and traumatic degloving of the penile and scrotal skin with the exposure of the cavernous and spongy penile body, bilateral testes and total amputation of scrotal skin secondary to motor vehicle accident from Nepal. The patient was managed by emergency surgical debridement and reconstruction of the avulsed penile skin and burial of testis in the medial thigh pockets with primary suturing and hemostasis of the amputated scrotal region, which healed with satisfactory esthetic results with normal voiding function and erection of penis.
