CHARACTERISTICS OF PATIENTS WITH BRAIN INJURY DUE TO LAND TRAFFIC ACCIDENTS TREATED AT NINH BINH PROVINCIAL GENERAL HOSPITAL IN 2018

This was an epidemiological study through a retrospective review of 215 medical records of patients with brain injury due to land traffic accidents treated at Ninh Binh Provincial General Hospital. Theresults showed that the majority of research subjects were male, accounting for 80.5%; The age group with higher prevalence of accidents was under 30 years old, accounting for 31.2%; The agegroup of 50-59 years old was at lower prevalence of accidents accounting for 15.8%. The time of the higher chance of occurring accident was between 4p.m and 11p.m daily. The majorityof traffic accidents occurred when riding motorbikes and electric bicycles (82.3%). About 75.0% of the victims received first aid with a gauze bandage; 5.4% of them had broken bones fixed. Amongthe brain injuries, 34.4% of the victims suffered from frontal trauma; 29.3% had unilateral cerebral hemisphere injury, 24.2% had temporal region injury, 15.8% had occipital region injury, and 8.4% ofvictims had parietal injury. More than half of the study subjects (53.5%) were hospitalized for 7-10 days. The rate of patients who have been discharged from the hospital with improved symptoms was81.4%; and 14.9% of patients were transferred to higher level hospitals.

This is an epidemiological study through a retrospective review of 215 medical records of patients with brain injury due to land traffic accidents treated at Ninh Binh Provincial General Hospital. Research results show that among the 5 factors of the scale measuring quality of life in Vietnam, the factor of anxiety and sadness was the least recorded by patients. Up to 96.5% of subjects did not feel the presence of this factor.Up to 42.7% of the subjects felt a little element of pain and discomfort. Married people living with family had higher quality of life scores than single or separated/divorced people. The older the age group, the lower the mean score of quality of life (p<0.05). People with higher incomes had higher quality of life scores than those with lower incomes (p<0.05).

Background: Trauma is one of the leading causes of death among people under 40 years of age and approximately 10 percent of the cases have been maxillofacial trauma. There was limited number of studies on maxillofacial fractures in Indonesia. Thus, this research attempted to investigate the characteristics and patterns of maxillofacial fractures at Mangusada General Hospital in Badung-Bali.Methods: This research was a cross-sectional descriptive study which conducted at Mangusada General Hospital in the period of 1 January 2016 - 31 December 2017. The 127 samples selected using non-probability sampling. The inclusive criteria involved all maxillofacial trauma cases and the exclusive criteria focused on maxillofacial fractures that received intervention or with incomplete medical records. Each data was collected from the medical records and then analysed descriptively.Results: From 127 samples, male dominated the sample on the gender-based criteria (70.1%) and the highest frequency of all age groups is 21-30 years old on the age group based criteria (23.6%). Maxillary fractures are the most occurring maxillofacial cases, which took up 33.6%. The main cause of the cases is traffic accidents (89.0%).Conclusions: Maxillary fractures are the highest maxillofacial cases at Mangusada General Hospital on 1 January 2016 - 31 December 2016 period of time. The productive male age groups are the most affected groups due to traffic accidents.

Objective: This study aimed to analyze the presence of lesions in the skull and face and the associated factors in pedestrian victims of traffic accidents. Methods: A cross-sectional, descriptive-analytical study carried out through the analysis of medical records of pedestrian victims of traffic accidents in an emergency service in the city of Campina Grande, Brazil, during the year of 2016. Information was collected regarding gender, age group, day of the week, time of the accident, type of vehicle involved, presence of trauma to the skull and face, and outcomes. The Chi-square and Fisher's Exact tests were used, with a significance level of 5%. Results: A total of 1,884 medical records were evaluated, out of which 7.1% (n = 133) involved pedestrians. Men were the most frequent victims (68.4%), and victims of age 60 years old or over (30.5%) predominated. Almost one-third of the cases were recorded during the weekends (30.5%), and the most prevalent time was at night (52.7%). Regarding the type of vehicle involved, motorcycles predominated (47.4%). Head trauma was present in 37.6% of victims, while facial injuries corresponded to 8.2%. In 12% of cases, the victims died. The variables of gender, age group, occurrence on weekends, and trauma to the face showed a statistically significant association with the occurrence of traffic accidents (Chi-square test; p<0.05). Conclusion: Among pedestrian victims of traffic accidents, there is a predominance of men aged 65 years or over. Accidents are frequent at night, and motorcycles are the main vehicles involved. The presence of trauma to the skull and face regions is high.

Abstract Background: Since 2006, efforts have been made to increase the identification of traumatic brain injuries (TBIs) in post-9/11 military personnel. The BAT-L is the first validated instrument to diagnose TBIs throughout the lifespan in post-9/11 Veterans. The objective is to investigate the correspondence of the Boston Assessment of TBI-Lifetime (BAT-L) diagnostic prevalence and injury severity of traumatic brain injury with in-theater medical records from Department of Defense (DoD). Methods: A convenience sample of 153 Veterans deployed in 2011 enrolled in the TRACTS longitudinal cohort study was examined. Retrospective review of DoD online medical records to determine diagnostic prevalence and injury severity for all head injury cases during deployment were compared with diagnostic prevalence and injury severity from the BAT-L clinical interview using Chi-square analyses.Results: There was moderate correspondence for TBI diagnosis between the BAT-L and DoD records (κ = 0.42). Sensitivity was 72.7% and specificity was 82.8%. Comparison of injury severity also had moderate correspondence (κ = 0.41). Missing TBI diagnostic data from DoD records was frequent; 43% percent of TBIs reported on the BAT-L did not have any documentation of mTBI assessment or diagnosis in DoD records while 83% did not have in-theater documentation.Conclusions: Diagnosis of TBI via the BAT-L retrospective interview was both sensitive and specific when compared to DoD medical records. However, diagnostic correspondence was only moderate. This lack of diagnostic agreement was related to multiple factors including lack of documentation of injury, differences in assessment tools and goals, and other combat-related motivational factors associated with failure to report injuries while deployed. Several policies were implemented to address underreporting and under-documentation of TBI, yet challenges remain. Findings suggest changes at both individual-level (e.g. service members) and system-level (e.g. DoD/military branches) are needed to adequately diagnose and document all TBI during deployment.

Abstract: Non-infectious skin disease is a skin disease that is not caused by pathogenic bacteria or by transmission. Many non-infectious skin diseases showed different manifestations in infants and children. This study aimed to obtain the patterns of non-infectious skin diseases in children at Dermatovenorology Department Prof. Dr. R. D. Kandou General Hospital period 2009-2011. This was a retrospective study. Data of the annual cases, gender, and ages were obtained from the medical records. From a total of 595 patients in 2009-2011 there were 323 (54.29%) children with non-infectious skin diseases, females (167 patients, 51.70%) were more frequent than males, and most commonly in the age group of 5 to 14 years (59.13%). Conclusion: Non-infectious skin diseases were still common in Dermatovenorology Department Prof. Dr. R. D. Kandou Manado periode 2009-2011.Metode Penelitian dilakukan secara retrospektif pada pasien anak dengan penyakit kulit non-infeksi yang datang ke Poliklinik Kulit dan Kelamin pada tahun 2009-2011. Data diperoleh dari rekam medik mengenai jumlah kasus pertahun, jenis kelamin, dan umur. Hasil penelitian memperlihatkan dari sejumlah 595 kunjungan pada tahun 2009-2011 terdapat 323 (54,29%) pasien anak dengan penyakit kulit non-infeksi, distribusi jenis kelamin memperlihatkan perempuan yang terbanyak 167 (51,70%), dan ter banyak pada kelompok umur 5-14 tahun (59,13%). DBD is a disease caused by the dengue virus which is transmitted through mosquito bites and reduces the platelet count in the sufferer. This study aims to determine the medical records of dengue hemorrhagic fever at Lubuk Pakam General Hospital. This type of research is descriptive. Data collection methods were medical records for all DHF patients from 2005 to 2009 at Lubuk Pakam General Hospital. Then the medical record data are grouped by age, sex and occupation. Based on the results of the study, it was found that the number of DHF sufferers from 2005 to 2009 increased in the 21-56 year age group with all male sex and work as entrepreneurs. This is due to a narrower environment, increasing population, and inadequate living quarters with no good air ventilation, so that the spread of dengue is increasing.

Daugiausia asmenų iki 40 metų miršta dėl išorinių priežasčių, tarp jų ir įvairių traumų. Mirtys dėl galvos smegenų traumų sudaro apie 30% visų trauminių mirčių. Ši studija buvo suplanuota dėl to, kad epidemiologinė galvos smegenų traumos situacija mūsų krašte yra neaiški. Ligoniai ir metodai Retrospektyvusis tyrimas atliktas Vilniaus greitosios pagalbos universitetinėje ligoninėje. Buvo ištirta 622 dėl galvos smegenų traumos hospitalizuotų pacientų. Duomenys buvo renkami į asmeninį kompiuterį ir apdoroti naudojant "MS office Excel 2003" ir "SPSS 10" programas. Rezultatai Vyrai galvos smegenų traumą patiria vidutiniškai tris kartus dažniau negu moterys. 20–59 metų žmonės sudarė 72,5%. Dažniausios galvos smegenų traumos priežastys: kritimai (40,7%), eismo nelaimės (20,5%) ir smurtiniai sužalojimai (19%). Lengvų galvos smegenų traumų (pagal Glasgow komų skalę (GKS) 13–15 balų) pasitaikė 67,8% atvejų, vidutinio sunkumo (GKS 9–12 balų) buvo 15,2%, o sunkių galvos smegenų traumų (GKS < 8 balai) – 17%. Remiantis radiologiniais duomenimis dažniausiai buvo diagnozuojama subduralinė kraujosruva (29,1%), kiek rečiau – trauminė subarachnoidinė kraujosruva (19,5%). Net 86,4% baigtis buvo gera (pagal Glasgow baigčių skalę 4–5 balai), 6,4% pacientų, patyrusių galvos smegenų traumą, neišgyveno. Išvados Galvos smegenų traumą reikšmingai dažniau patiria 20–59 metų vyrai. Dažniausia traumos priežastis – kritimai. Vilniaus ligoninėse dažniausiai gydomi pacientai, patyrę lengvą galvos smegenų traumą (GKS 15–13 balų). Net trys ketvirtadaliai baigčių yra labai geros. Pusė žmonių, patyrusių sunkią galvos traumą (GKS 3 balai), neišgyvena. Pasitvirtino ankstyvieji galvos smegenų traumos prognoziniai veiksniai: atvykimo GKS, amžius, vyzdžių skersmuo ir jų reakcija į šviesą, pakitimai galvos kompiuterinėje tomogramoje. Most individuals younger than 40 years die due to external causes including different traumas. Deaths caused by traumatic brain injury comprise about 30% of all traumatic deaths. This study has been planned because of the unclear epidemiology of traumatic brain injuries in our country. Patients and methods A retrospective study was carried out at Vilnius University Emergency Hospital. 622 hospitalized traumatic brain injury patients were enrolled in the study. The data were stored in a personal computer and analyzed using Microsoft Excel 2003 and SPSS 10 statistical package. Results The male gender was dominant among the brain-injured. 72.5% of these patients were 20–59 years of age. Most frequent causes of traumatic brain injuries were falls (40.7%), traffic accidents (20.5%) and assaults (19%). Mild traumatic brain injuries (Glasgow Coma Scale (GCS) 13–15) were proved in 67.8%, moderate (GCS 9–12) in 15.2% and severe (GCS < 8) – in 17.0% of cases. Radiological evaluation revealed subdural hemorrhage in 29.1% and traumatic subarachnoid hemorrhage in 19.5% of victims. Good outcomes (according to GOS 4–5) were reGBStered in 86.4% patients, 6.4% patients did not survive. Conclusions Traumatic brain injuries in males were three times more frequent than in females. The age group of 20–59 years was prevalent. The most frequent cause of trauma was fall. Mild traumatic brain injuries (GCS 15–13) prevailed in Vilnius among the in-hospital patients. Three quarters of the outcomes were very good. Half of the patients brought in with three-point coma did not survive. The following early prognostic factors of traumatic brain injury were approved: GCS score, age, pupils diameter and light reflex, CT features. It was established that hospitalization of mild traumatic brain injuries in Lithuania was more frequent than in neurosurgical departments of other countries. ABSTRACTObjectives: to describe the most frequent causes for realization of thoracotomies and the epidemiological profile of individuals submitted of its. Method: this is about a retrospective study from quantitative approach conducted by medical records of a large general hospital of region Center-Western of Paraná (PR) were analysed in from June 2005 to july 2006. The study has been approved by the Research Ethics Committee of the Universidade Estadual do Centro-Oeste in Paraná (BR), in accordance to registration number 019/2006. Results: it were studied 50 cases, 56% were men; the age-group where the most number of cases were concentred was between 40 and 49 years old (22%), the average age was 42,5 years. Most of the case studied (50%) was closed thoracotomy drainage; 42% was thoracotomized for exhibit pleural empyema and 22% for exhibit pleural effusion. Conclusion: males aged up to 49 years old predominated among the thoracotomized patients in the study sample. O trabalho foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Estadual do Centro-Oeste, sob registro n.019/2006. Resultados: foram estudados 50 casos, destes, 56% constituem-se de homens; a faixa etária onde se concentraram o maior número de casos foi entre 40 e 49 anos (22%), com idade média de 42,5 anos. A maioria dos casos analisados (50%) correspondeu a toracotomias com drenagem fechada; 42% foram toracotomizados por apresentarem empiema pleural e 22% por apresentarem derrame pleural. Conclusão: indivíduos do sexo masculino na faixa etária de até 49 anos de idade predominaram entre os pacientes toracotomizados da amostra do estudo, todos procedentes da região estudada. As toracotomias laterais foram as principais incisões torácicas realizadas em indicação ao tratamento de derrames e empiemas pleurais. Resultados: fueron estudiados 50 casos, de estos, 56% se constituyen de hombres; la faja etaria donde se concentraron el mayor número de casos fue entre 40 y 49 años (22%) con edad media de 42,5 años. La mayoría de los casos estudiados (50%) correspondió a toracotomías con drenaje cerrada; 42% fueron toracotomizados por presentaren empiema pleural y 22% por presentaren derrame pleural. Conclusión: hombres en la faja etaria hasta 49 años predominarán entre los toracotomizados en la amostrad del estudio, todos procedentes de la región estudiada. Aims: The goal of this study was to analyze outcomes of the patients underwent surgery for traumatic brain injury (TBI) with critical Glasgow coma scale at Dr. Soetomo General Hospital Surabaya, Indonesia in term of mortality.Methods: All cases of critical TBI patients with hemorrhage was operated (evacuation craniotomy) from January 2015 - December 2016 at Dr. Soetomo General Hospital Surabaya, Indonesia. The data were retrospectively reviewed from the medical records. Information regarding patient age, sex, diagnoses, Glasgow Coma Scale (GCS), surgery type, and the outcome after surgery was collected and analyzed.Results: 36 samples were analyzed from the medical records of Dr. Soetomo hospital. The operation was performedfor the patients with epidural hemorrhage (EDH), subdural hemorrhage (SDH) and intracerebral hemorrhage (ICH). In the postoperative period, 83.78% (30 patients) of patients died and only 16.22% (6 patients) survived. Survived patients were followed until six months according to Glasgow Outcome Scale (GOS).As regard with the six-month evaluation of GOS, only 2 patients came out with good functional outcome (GOS Extended8), 2 patients came out with lower moderate disability (GOS Extended 5), and 2 patients with dead status (GOS Extended 1).Conclusions: In this retrospective study, overall 83.78% (30 patients) patients not survived but 16.22% (6 patients) survived. Two patients had better functional outcome after six months postoperative. The authors conclude that TBI patients with critical GCS should manage with surgery as indicated since good functional results can be obtained in some cases. Many factors affected the good and bad condition of the patient after craniotomy.Keywords: Critical GCS, hemorrhage evacuation craniotomy, GOS Extended
