Objective.To validate MR-criteria used for diagnosis of trigeminal neuralgia (TN).Material and methods.Study population consisted of 133 patients: with trigeminal neuralgia (86, surgically (56) and clinically (30) verified; 55 ± 11 years) and control group (47; 51 ± 16 years). The data were obtained using the FIESTA sequence (slice thickness 0.5 mm) on 3T and 1.5T (Discovery 750w and Optima 450w, General Electric).Results.The diameters of the adjacent vessels on the symptomatic side were significantly bigger than the diameters on the asymptomatic side in patients with neuralgia (medians 1.2 and 1 mm, p<0.001). The shortest distances between nerves and vessels on the symptomatic side were significantly smaller than the distances on the asymptomatic side (medians 0 and 1.6 mm, p<0.001). The distances between points of closest neurovascular contact and pons on the symptomatic side were significantly smaller in comparison with the controls (medians 2.7 and 5.3 mm, p<0.001); the distances between the symptomatic and asymptomatic side were not statistically different (medians 2.7 and 3.9 mm, p=0.09) in this parameter. The lengths of closest neurovascular contact was bigger in patient with TN (medians 3 and 0 mm, p<0.05), as well as in comparison with the asymptomatic side (medians 3 and 0 mm, p<0.0001).Conclusion.Noninvasive MR-measurements of the posterior cranial fossa vessels show the differences between the symptomatic and asymptomatic side in the patients with TN as well as healthy individuals. This result may be used as a predictor of TN.

Objectives: Hemifacial spasm is characterised by involuntary, rhythmic, tonic or clonic contractions originating from the facial nerve and occurring intermittently in unilateral facial muscles. The most commonly reported cause of hemifacial spasm is the compression of the facial nerve at its root exit/ entry zone by an ectatic or aberrant blood vessel, leading to local demyelination. In this study, we compared magnetic resonance angiography (MRA) findings from patients with hemifacial spasm and controls to determine whether vertebral artery dominance is part of the aetiology of this condition. Methods: This retrospective case-control study was conducted with data from 40 patients with hemifacial spasm who received botulinum toxin injections and for whom MRA findings were accessible, and 42 patients of similar age and sex who presented with symptoms of nonspecific headache in the outpatient neurology clinic. Magnetic resonance imaging and unenhanced time-of-flight MRA examinations from the study and control groups were evaluated. Results: A dominant vertebral artery was present in 52.2% of patients in the hemifacial spasm group and 33.3% of those in the control group, but this difference was not significant (p = 0.079). Conclusions: The lack of significant results supporting our hypothesis may be attributable to the small study population. Further studies with larger groups are warranted.

Infant Mortality in Bengkulu Province is still high, this can be prevented through breastfeeding as a staple food in infants 0-6 months without other complementary foods because breast milk boosts immune status for infants and thus can reduce mobility and mortality rates but exclusive ASI coverage still low, this is influenced by pralactal feeding in infants 0-3 days of birth. Research Objective to increase knowledge and attitude of cadre through training as promotion effort of prevention of pralactal food in 0-3 days old baby. Quasi pre and post test experiments with comparison groups. The study population were all cadres in Talang Tinggi Public Health Center, which were 50 people in Talang Tinggi Public Health Center, Seluma District with experimental group samples were cadres who were given training by using module and control group were cadres who were trained by using lecture and question and answer method. There is an average difference of attitude and knowledge before and after intervention in the cadres given training on prevention of pralactal food in infants 0-3 days. There is an effect of training on prevention efforts of pralactal feeding using modules with cadre attitude. Puskesmas should increase the promotion of prevention of pralactal food to change cadre attitude to support program exclusive breastfeeding.

Introduction: To study the sonographic appearance of plantar fascia in clinically suspected cases of plantar fasciitis (PFs) using both quantitative and qualitative parameters, and to establish the correlation between plantar fasciitis, body mass index (BMI) and heel pad thickness (HPT). Methods: In this case controlled analytical study, we sonographically evaluated 100 patients with clinical plantar fasciitis (unilateral: 90, bilateral: 10 with mean age 46.9yrs) and control group of 60 (120 heels) healthy volunteers with mean age 45.3yrs. Plantar Fascia Thickness (PFT), HPT, hypoechogenity, biconvexity, perifascial fluid, intrafascial calcification and subcalcaneal spurs were evaluated sonographically. PFT was measured 5mm distal to the calcaneal insertion of plantar aponeurosis. The unloaded HPT was measured from the skin surface to the nearest calcaneal tuberosity. Results: Mean Plantar Fascia Thickness (PFT) and Heel Pad Thickness (HPT) are greater on the symptomatic side for patients with unilateral and bilateral PFs than on the asymptomatic side for patients with unilateral PFs, and also control subjects (PFT values are 4.41 ± 0.59, 4.63 ± 0.55, 2.83 ± 0.36, 2.62± 0.37 mm, and HPT values 17.64 ± 1.07, 17.28 ± 1.10, 16.91 ± 1.06, 16.73 ± 1.13 mm, respectively) (p <0.0001). Mean BMI values of the case and control groups are 26.14 ± 1.9 and 24.42 ± 0.89 Kg/m2, respectively (p <0.05). We found hypoechogenicity of plantar fascia in 80 cases (72.7%), calcaneal spur in 69 cases (62.7%), biconvexity in 11 cases (10%) and perifascial fluid in 16 cases (14.5%) within the plantar fasciitis group (110 symptomatic heels). Conclusion: Increased plantar fascia thickness, increased heel pad thickness and hypoechogenicity of plantar fascia are consistent sonographic findings in plantar fasciitis. Its occurrence has significant relation to high BMI.

AbstractAim:This study aimed to determine whether or not the middle cranial fossa dural plate is located lower (i.e. more caudally) in patients with chronic otitis media, relative to adjacent structures.Methods:The authors retrospectively investigated computed tomography temporal bone scans of 267 ears of 206 patients who had undergone surgery with a diagnosis of chronic otitis media, together with scans of 222 ears of 111 patients without chronic otitis media. The depth of the middle cranial fossa dural plates was recorded.Results:The mean depth of the middle cranial fossa dural plate was 4.59 mm in the study group and 2.71 mm in the control group (p < 0.001). The middle cranial fossa dural plate was located lower in the right ear in both the study and control groups.Conclusion:The middle cranial fossa dural plate was located lower in patients with chronic otitis media, and in the right ears of both patients and controls. Surgeons should take this low location into consideration, and take extra care, during relevant surgery on patients with chronic otitis media.

ObjectivePheochromocytomas (PHs) and sympathetic paragangliomas (PGs) are tumors that produce catecholamines, predisposing patients to cardiovascular disease and gastrointestinal effects such as constipation. Objectives: i) determine the prevalence of constipation, its risk factors, and its impact on survival; ii) identify whether a systematic combination of fiber, water, and laxatives was effective for treatment of constipation.Design and methodsWe retrospectively studied 396 patients with PH/PG diagnosed in 2005–2014. The study population was patients with constipation as a presenting symptom; the control group was patients without constipation as a presenting symptom. The MD Anderson Symptom Inventory was used to assess constipation and quality of life.ResultsTwenty-three patients (6%) had constipation. Constipation was associated with headaches, palpitations, diaphoresis, weight loss, and excessive noradrenaline production (P<0.0001). Eighteen of these patients had non-metastatic primary tumors larger than 5 cm and/or extensive metastases. No statistically significant differences in age, sex, and genotype were noted between the study and control groups. In patients without metastases, resection of the primary tumor led to symptom disappearance. A systematic combination of fiber, water, and laxatives was associated with symptom improvement. Two patients who presented unmanaged constipation died because of sepsis from toxic megacolon.ConclusionsConstipation is a rare and potentially lethal complication in patients with PH/PGs. Severe constipation can be prevented by recognizing and treating mild symptoms.

OBJECTIVE: To analyse smoking behaviour in patients suffering from non-small cell lung cancer (NSCLC), chronic obstructive pulmonary disease (COPD) and pulmonary TB (PTB).METHODS: The study population comprised 421 inpatients newly diagnosed with NSCLC, COPD and PTB at the First Affiliated Hospital of Chongqing Medical University, Chongqing, China, and 362 healthy individuals (controls) recruited from September 2016 to March 2017. All participants were current smokers. After enrolment, the subjects were invited to participate in face-to-face interviews to complete the self-designed questionnaire. A follow-up survey was performed 2 years later.RESULTS: Two years after being diagnosed, patients with NSCLC, COPD and PTB had smoking cessation rates of respectively 76.8%, 62.8% and 63.7%. The cessation rate was only 10.2% in the control group (P < 0.01). The difficulty of smoking cessation after diagnosis in patients with the three diseases was significantly decreased (P < 0.01).CONCLUSION: The smoking cessation rate among participants suffering from NSCLC, COPD and PTB increased after diagnosis compared with that before diagnosis, and was significantly higher than the control group of healthy individuals.

Treating patients with systemic lupus erythematosus (SLE) with steroids and immunosuppressive drugs may interfere in the presence of potentially opportunistic microorganisms in the oral cavity. The aim of this study was to evaluate the presence of Candida spp., Staphylococcus spp., Enterobacteria and Pseudomonas spp. in the oral cavity of SLE patients, compared with healthy controls. A group of 40 patients who had received therapy for at least 60 days was selected (19–53 years). For the control group, 40 healthy individuals matched for age, gender and use of partial prosthesis were selected. Oral rinse samples were collected and plated on specific culture media. After incubation, the number of colony forming units (CFU) was obtained and the isolates were identified at species level. Microbial counts were compared between SLE and control by analysis of variance (ANOVA) and Mann–Whitney ( p < 0.05 significant). Microorganism counts in patients with and without immunosuppressive drugs, as well with active and inactive disease (according to SLEDAI score) were also compared. No significant differences in CFU/mL between SLE and control patients were observed (yeasts, p = 0.55; Staphylococci, p = 0.24; Enterobacteria/ Pseudomonas spp., p = 0.26). No differences in microbial counts were observed regarding clinical parameters tested. The most frequent species isolated in the SLE group were Candida albicans, Staphylococcus epidermidis and Klebsiella oxytoca. In conclusion, no differences in frequency and microorganism levels were found between SLE patients and healthy individuals.

The authors report the case of an infant with a giant glioependymal cyst. Although it has been suggested that these cysts originate from the tela choroidea, their origin remains controversial. This 35-month-old girl with truncal ataxia was referred to the authors' hospital. Magnetic resonance imaging revealed a giant cystic mass extending from the anterior to the posterior cranial fossa. Hydrocephalus was caused by obstruction of the sylvian aqueduct. Endoscopic fenestration of the cyst wall was performed. Histochemical and immunohistochemical staining identified the lesion as a glioependymal cyst. Magnetic resonance imaging performed 8 months later suggested that the cyst originated from the tela choroidea. At 5-year follow-up, there was no tumor recurrence and she had fully recovered. The origin of glioependymal cysts is discussed, and the authors suggest that their origin is the tela choroidea.
