Introduction. Chronic combined periodontal and periapical lesions have a negative impact on the human organism, since they are a source of intoxication and sensitization causing progression of infection, development of focal sequelae and secondary immunodeficiency. Problem of successful treatment of apical and marginal periodontitis is associated with a significant prevalence of these diseases, a complexity of medical procedures, a substantial proportion of failures and complications, and with a lack of long-term stability of gained results.Aim. The aim of this work was to investigate clinical effectiveness of drug formulation with thiotriazoline and chloramphenicol in the integrated treatment of combined apical and marginal periodontitis.Meterial and methods. The condition of oral cavity of the 65 patients with combined lesions of periodontal and endodontic tissues before and after treatment was studied. Outcomes measured were X-ray examination, probing depth, OHI-S, PMA, PI, SBI indices. The complex treatment has been worked out in the patients of main group and the ointment with thiotriazoline and chloramphenicol was introduced in the scheme of periodontal treatment Results and conclusions. Results of applied treatment indicated to the acceleration of healing process, reduction of exudation period and decrease of exacerbations frequency in the patients of main group. Clinical experience also demonstrated positive dynamic in changes of periodontal indices after the conducted treatment.

The objective: to study and systematize the main clinical variants of CIN 3 with the spread of atypical epithelium to the vaults and walls of the vagina; to develop complex approaches to the treatment of patients with CIN 3 with the spread of atypical epithelium to the vaults and walls of the vagina with neoadjuvant therapy and surgical treatment; to study the therapeutic effectiveness of the use of a2b-interferon in the form of vaginal suppositories and Tyloron in the complex treatment of patients with CIN 3 with the spread of abnormal epithelium to the vaults and walls of the vagina during the first stage of complex drug treatment. Patients and methods. A survey of 62 patients with histologically verified CIN 3 with the spread of atypical epithelium to the vaults and walls of the vagina was carried out. At the stage of neoadjuvant etiotropic therapy, the patients were randomized into two groups. 31 patients were included in the main group (A), 31 patients were included in the control group (B). In group A, patients with neoadjuvant were given б2b-interferon at 500 000 IU in the form of vaginal suppositories twice a day for 14 days and a Tyloron 1 tablet 125 mg once a day in a day No.10. In group B, the standard therapy is intended for patients – an a2b-interferon of 500 000 IU in the form of vaginal suppositories twice a day for 14 days. The surgical stage of treatment was carried out in accordance with the clinical and histological diagnosis and the variant of the process spread to the walls of the vagina. Diathermoconization of the cervix and combined vaginal trachelectomy type A with resection of the upper third of the vagina were performed. Results. Three clinical variants of CIN 3 with spreading to the vaults and walls of the vagina were established. The first clinical variant – CIN 3 is localized to ectocervix, CIN 1–2 (IHC p16 negative) is localized on the vaults and walls of the vagina. The second clinical variant – CIN 3 is localized on ectocervix and extends to the vault and walls of the vagina. The third clinical variant – CIN 3 is localized on ectocervix and multicentric dissemination of CIN 3 – on vaults and walls of the vagina. The choice of an integrated treatment program with a surgical component depends on the clinical option. Conclusions. 1. Three clinical variants of CIN 3 with spreading to the vault and walls of the vagina have been established. Half the patients had the first clinical variant. 2. The main colposcopic signs of CIN 3 with spreading to the vaults and walls of the vagina: dense acetic-white epithelium, coarse mosaic, a sign of the internal border. 3. In 3 weeks after the course of treatment with neoadjuvant therapy in combination of Тyloron with a2b-interferon in the form of vaginal suppositories, it is possible to achieve from 85 to 100.0% positive dynamics, whereas in the traditional method of treatment, from 41 to 75%, which is statistically significant less (p<0.01). 4. The study showed that there is a relatively strong statistically significant association of neoadjuvant therapy using a combination of Tyloron with interferon-a2b suppositories in the complex treatment of CIN3 with spreading to the vault and vaginal walls compared to conventional therapy (c21=10.64; j=0.41; p<0.01). After three weeks, the positive dynamics in the main group (A) significantly increased (RR=1.6; 95% CI: 1.2–2.2; p<0.01). Key words: CIN 3, vaginal vault, vagina, trachelectomy, treatment.

Relevance. The relevance of the study is dictated by the high prevalence of periodontal disease among young people and by the need to search for new approaches to periodontitis treatment in order to increase the effectiveness of treatment at early stages of pathological process. Objective. To evaluate the effectiveness of complex treatment of chronic generalized periodontitis (CGP) of mild severity using a removable splinting construction made according to the author's technique. Materials and methods. At the orthopedic stage of the complex treatment of mild CGP, splinting constructions were used for 47 young patients: in the main group (n = 16) the authors treatment and preventive splint, in the first comparison group (n = 15) a removable metal splint, in the second comparison group (n = 16) a fixed adhesive fiber splint. Periodontal hemodynamics was assessed using ultrasound Doppler analysis 1, 6 and 12 months after the therapeutic measures and tooth immobilization. To identify the possible negative impact of the treatment on the quality of life (QOL) in patients with splint constructions, we used the OHIP-14-Ru questionnaire; the follow-up period was 1.3, 6, and 12 months. Results. Dental immobilization contributed to the improvement of blood flow in the periodontal tissues in the short and long term follow-up, so after 12 months: Vam in the main group increased by 39,8 % and amounted to 0,397 0,004 cm / s, in patients of the comparison groups Vam was 0,329 0,006 cm / s and 0,335 0,003 cm / s, respectively, that is 15,4 % and 18,4 % higher compared with the figures before treatment. The quality of life index for patients using splinting constructions made according to the author's technique, by the OHIP-14-Ru questionnaire was assessed as good 1 year after (3.46 0.59). Conclusions. The use of the proposed original orthopedic design in a complex plan for mild periodontitis treatment helps to improve regional blood circulation and stabilize the pathological process.

Introduction. Glaucoma is one of the most significant eye diseases. It is often diagnosed, not always amenable to therapy, and can lead to a complete loss of visual functions. In recent years, the method of osteopathic correction has become widespread as one of the effective methods of treatment and rehabilitation of patients with pathologies of various body systems. In the pathogenesis of glaucoma, it is customary to distinguish a dystrophic concept, which considers primary open-angle glaucoma as a result of dystrophic changes in the connective tissue, as well as in the endothelial lining of the trabeculae and Schlemm′s canal, especially destructive changes in mitochondria and the alteration of their functional activity. A vascular concept is also distinguished. According to this concept, the central link in the pathogenesis of glaucoma is circulatory disorder in the ciliary vessels, ocular artery, and major vessels of the head and neck, it can be assumed that osteopathic correction in the treatment of patients with open-angle glaucoma will be pathogenetically substantiated and will have a positive effect on intraocular pressure and trophicity of the optic nerve. The goal of research — to study the influence of in osteopathic correction on the nature of unoperated glaucoma (stage IIA) and to substantiate the possibility of using osteopathic correction in the complex treatment of patients with this pathology.Materials and methods. A prospective controlled randomized study was conducted at 52 city polyclinics, branch 3, Moscow, from January 2018 to January 2019. 40 patients (70 eyes) aged 50 to 75 years with primary open-angle glaucoma IIA stage were examined. At this stage of the disease, patients most often seek medical care and the issue of conservative management is primarily considered. All patients were divided into two groups of 20 people: the main group and the control group. The treatment in the main group included hypotensive drug therapy and osteopathic correction. Patients of the control group received only drug therapy. All patients underwent ophthalmic (visometry, tonometry, perimetry) and osteopathic examination twice: before the treatment and after 3 months.Results. For patients with primary open-angle IIA non-operated glaucoma, regional (most often regions of the head, neck, dura mater) and local (abdominal diaphragm, iliac bones, hip and knee joints) somatic dysfunctions were the most typical. In the main group a statistically significant decrease in the frequency and severity of dysfunctions at all levels was stated. Also, in patients receiving osteopathic correction, a significant decrease in the level of intraocular pressure and perimetric indices was noted. In patients of the control group, no reliable changes in these indicators were obtained.Conclusion. The results obtained indicate that osteopathic correction is clinically effective in the complex treatment of patients with primary open-angle II A glaucoma.

Introduction. Degenerative-dystrophic changes in the spine are the most common chronic human diseases. Pronounced clinical manifestations of vertebrogenic diseases are observed during active labor activity and represent one of the most frequent causes of temporary disability. It is also known that any pain syndromes are accompanied by the development of psychovegetative disorders, which reduces the effectiveness of treatment. In recent years, there has been a reasonable increase in the interest of the medical community to the non-drug treatment methods. It is caused by the polypharmacotherapy side effects, an increased numbers of allergic reactions, problems with polypragmasia, and a low level of compliance. At the same time, the question of the possibility of various non-drug treatment methods combining is quite acute.The goal of research is to substantiate the clinical effectiveness of the combined application of osteopathic correction and reflexotherapy in the complex treatment of patients with dorsopathy at the cervical-thoracic level.Materials and methods. A prospective controlled randomized study was conducted on the basis of the medical clinics of LLC «Family Health» (Kazan) and ANO «Center for SEB assistance» (Kazan) from April 2018 to May 2019. The study involved 52 people with dorsopathy at the cervical-thoracic level, aged from 25 to 45 years. In accordance with the exclusion criteria, 7 people dropped out of the study. The remaining patients (45 people), depending on the used treatment method, were divided by a simple randomization method using a random number generator into three groups of 15 people. The first group (main group) received osteopathic correction and reflexotherapyon the background of standard drug therapy; the second and the third groups (control groups) received only osteopathic correction orreflexotherapy, respectively, on the background of standard drug therapy. The osteopathic examination was performed for all patients regardless of the group, before and after the course of therapy, with the formation of an osteopathic conclusion.Also there was the estimation of the degree of pain syndrome severity,the asthenia, and the degree of accumulated emotional and energy charge that does not get out in a person.Results. It was found in the course of the study that the inclusion of osteopathic correction and reflexotherapy in the complex therapy of patients with dorsopathy at the cervical-thoracic level is clinically more effective than the isolated use of these methods. Such complex therapy allows to achieve a more significant reduction in the severity of the pain syndrome by VAS (p<0,05), relief of internal emotional tension (p<0,05), and increase the effectiveness of correction of dominant somatic dysfunction. Based on the obtained data, it could be assumed that reflexotherapy potentiates the effects of osteopathic correction.Conclusion. Based on the study, it can be concluded that the combination of osteopathic correction and reflexotherapy in the treatment of patients with dorsopathy at the cervical-thoracic level increases the clinical effectiveness of the standard drug therapy. At the same time, it is worth noting the potentiating effect of the combined use of two non-medicinal methods. The question of combining of various non-drug treatment methods is quite acute today, so this study will be continued.

The effectiveness of the immunomodulator Polymuramil (PM) was determined in the treatment of 40 patients with severe chronic generalized periodontitis based on the dynamics of the values of periodontal indices and the content of periodontal pathogens in the periodontal pockets. Patients of the main group (20 people) were treated with professional hygiene (PGO) in combination with daily intramuscular PM for 5 days at a dose of 200 mcg. Patients of the control group (20 people) were performed only PGO. The effectiveness of treatment was evaluated based on the dynamics of the clinical indices of Muehlemann and PMA and the presence or absence of major periodontal pathogens in the periodontal pockets. Results: in patients of the main group, the phenomena of periodontal inflammation and the main periodontal pathogens were eliminated in a shorter time and to a greater extent in comparison with patients of the control group

Background:Objectives:Our aim was to study the effect of biofeedback (BFB) training on the locus of control in patients suffering from rheumatoid arthritis (RA), and to justify the use of this method in the complex treatment of the disease.Methods:40 RA patients hospitalized in the rheumatology department were examined. The average age of patients was 48. 6 years ± 7.73 years (from 30 to 70 years), women accounted for the majority = 26 (86%), the average duration of the disease was 12 years ± 3.44 years. We use J. Rotter’s Locus of Control Scale in E.F.Bazhin adaptation. RA patients were divided into two groups: the main (20 patients) and control (20 patients). Patients of the main group received complex therapy with 12 sessions of BFB training, mainly based on the parameters of the brain’s electrical activity — EEG relaxation using the Reakor™ psychophysiological rehabilitation complex manufactured by Medicom MTD (Taganrog).Results:We revealed externality in RA patients in the general field (3.03 ± 0.3), as well as in the field of relation to the disease (3.86 ± 0.23) and in the field of production relations (3.43 ± 0, 25). After BFB trainings, an increase in internality was observed on the scales of the general sphere (p <0.05) and attitude to the disease (p <0.01) in patients of the main group. In the group of patients receiving conventional treatment, the dynamics of the results was unreliable.Conclusion:It should be noted that the locus of control (or subjective control) is a quality that characterizes a person’s tendency to attribute responsibility for the results of his activity to external forces, or to his own abilities and efforts. Externality is manifested when people prefer to shift responsibility for important events of their life to external circumstances, and external forces (bosses, colleagues, etc.). In the field of attitude to the disease, externality is manifested when patient behaves passively, and believes that he cannot influence the course of the disease in any way, shifting all responsibility for the treatment results to medical staff, which can lead to non-compliance with the treatment regimen and an increase in the level of anxiety and depression, decreased self-esteem. The onset of the disease and its associated social consequences (disability, loss of social roles, etc.) can cause a negative mental state of learned helplessness. Learned helplessness is defined as a condition that occurs as a result of uncontrolled, mainly negative events, which manifests itself in violations of emotional, motivational and cognitive processes. In other words, RA patient suffering from this condition expects treatment failures and reduces control over compliance with the treatment regimen. BFB therapy can be used in order to correct and prevent the state of learned helplessness by increasing the level of internality.It is assumed that increasing internality in the BFB process is associated with teaching the patient the skills of self-regulation of physiological processes. The mechanisms of BFB therapeutic effect are not only changes in physiological parameters (improvement of cerebral and peripheral blood flow, muscle relaxation, and improvement of sleep) but also in a shift in the locus of control from external to internal, which can increase compliance, reduce neurotic complaints, mobilize volitional potential and improve patient self-esteem.As a result of BFB course, an increase in the internality was noted in patients on the scales of the general sphere and the sphere of attitude to the disease. It is advisable to use the BFB to increase the compliance and effectiveness of complex treatment of RA patients.Disclosure of Interests:None declared

The Common Krait (Bungarus caeruleus) shares a distribution range with many other ‘phenotypically-similar’ kraits across the Indian subcontinent. Despite several reports of fatal envenomings by other Bungarus species, commercial Indian antivenoms are only manufactured against B. caeruleus. It is, therefore, imperative to understand the distribution of genetically distinct lineages of kraits, the compositional differences in their venoms, and the consequent impact of venom variation on the (pre)clinical effectiveness of antivenom therapy. To address this knowledge gap, we conducted phylogenetic and comparative venomics investigations of kraits in Southern and Western India. Phylogenetic reconstructions using mitochondrial markers revealed a new species of krait, Romulus’ krait (Bungarus romulusi sp. nov.), in Southern India. Additionally, we found that kraits with 17 mid-body dorsal scale rows in Western India do not represent a subspecies of the Sind Krait (B. sindanus walli) as previously believed, but are genetically very similar to B. sindanus in Pakistan. Furthermore, venom proteomics and comparative transcriptomics revealed completely contrasting venom profiles. While the venom gland transcriptomes of all three species were highly similar, venom proteomes and toxicity profiles differed significantly, suggesting the prominent role of post-genomic regulatory mechanisms in shaping the venoms of these cryptic kraits. In vitro venom recognition and in vivo neutralisation experiments revealed a strong negative impact of venom variability on the preclinical performance of commercial antivenoms. While the venom of B. caeruleus was neutralised as per the manufacturer’s claim, performance against the venoms of B. sindanus and B. romulusi was poor, highlighting the need for regionally-effective antivenoms in India.

The aim of this study was to evaluate the effect of the chemoradiation therapy on the incidence of postoperative complications and mortality rate in patients with locally advanced tumors of the thoracic esophagus.The study included men and women over 18 with histologically verified squamous cell carcinoma of the thoracic esophagus with TNM stage cT3-4aN0-3M0. Patients were divided into two groups: patients in a main group (n=26) received a complex treatment, patients in a second group (n=30) undergoing only surgical treatment.The first stage of treatment was performed for all 26 patients and included 2 courses of preoperative chemotherapy, followed by a chemoradiation therapy with a 44–46 Gy with weekly injections of chemotherapy. Surgical procedure (R0) was performed for 24 patients (92.3 %) in the main group and for 26 (86.6 %) in a second group.Therapeutic complications were developed in 17 (65 %) patients in a first group, while in a second group this rate was in 18 (60 %) patients from 30. The incidence of surgical complications was slightly higher in a group of patients received a complex treatment –7 (26.92 %) patients. In the surgical group such complications was  noted in 6 (20 %) patients. The only death in an early postoperative period was in 1 (3.84 %) patient from the main group. The cause was sepsis and the development of multiple organic failure. There were no lethal outcomes in a surgical group.Estimating the incidence of complications, it should be noted that the use of chemoradiation therapy in the neoadjuvant regimen does not significantly affect the course of the postoperative period. The mortality rate was slightly higher in in the group of patients received complex treatment. At the same time, improvement of the ostoperative period and a reduction of mortality rate in group ofpatients undergoing complex treatment is possible due to  optimization of preoperative preparation, postoperative management and competent selection of patients.

The clinical effectiveness o f complex enzymotherapy with preparation Vobenzim in treatment o f gestosis was studied in comparison with traditional therapy. The author analyzes the course оf gestation and delivery, and the state оf neonates in women with nephropathy o f I degree, who got common treatment (28 patients) and complex treatment including Vobenzim (17patients). Positive clinical effect o f complex enzymotherapy in case o f nephropathy is displayed in decreasing of edematic syndrome, eliminating or decreasing o f proteinuria, lowering o f blood pressure and impairment o f fetalhypoxia in labors. It goes along with the reduction o f immune complexes fixed in placenta.
