Abstract Background: Chronic kidney disease (CKD) is a global public health concern and diabetes is one of the main risk factors for its occurrence and progression. The aim of this research is to determine the prevalence of chronic kidney disease in a cross-sectional population of patients with type 2 diabetes in primary health centers in North West Bank.Methods: Patient data including patient characteristics, creatinine level, blood pressure, HbA1c, and hypertension and period of diabetes were obtained from primary health care centers. The eGFR has been determined using the CKD-EPI equation. CKD was staged according to the 2012 Kidney Disease Improving Global Outcomes Framework (KDIGO) guideline. Both univariable and multivariable statistical analysis was conducted using SPSS. Results: The prevalence of chronic kidney disease among diabetic adults in North West Bank was found to be 23.6% (95% CI: 19.4%-28.1%) divided as follows: 19.7% had stage 3 CKD, 2.6% had stage 4 CKD and 1.3% had stage 5 CKD. In multivariable logistic regression, CKD was significantly associated with Age ≥60 years [adjusted OR: 3.2, 95% CI: 1.8-5.9], hypertension [adjusted OR: 5.7, 95% CI: 2.2-15.2], and smoking [adjusted OR: 2.3, 95% CI: 1.3-4.2].Conclusions: CKD is very prevalent among diabetic adults in Palestine. Co-morbid hypertension, smoking and older age has been shown to increase the risk of developing CKD. Extensive screening for diabetic patients to diagnose CKD at an early stage and to follow more aggressive treatment methods for diabetes as well as other important risk factors, especially hypertension and smoking, is recommended.

In multivariate logistic regression, CKD was significantly associated with hypertension [adjusted OR= 4.43, P value=0.005], smoking [adjusted OR= 2.1, P value=0.01], and age [adjusted OR= 1.1, P value <0.001] Conclusions CKD is highly prevalent among Palestinian diabetic adults. Co-morbid hypertension, smoking and older age were found to increase the probability of developing this condition. Intensive screening for diabetic patients to detect CKD at earlier stages and implementation of more aggressive treatment modalities for diabetes as well as other important risk factors, especially hypertension and smoking are recommended.

Objective. Sclerostin is a Wnt inhibitor produced specifically by osteocytes. However, it is not currently clear whether renal dysfunction has an effect on circulating sclerostin level in patients with type 2 diabetes. The aim of the study was to evaluate this relationship. Design and Patients. We conducted a cross-sectional observational study of 302 type 2 diabetic patients with or without chronic kidney disease. Serum sclerostin level was analyzed by ELISA, and renal function was assessed by estimated glomerular filtration rate (eGFR) using chronic kidney disease epidemiology collaboration (CKD-EPI) equation. Results. There was a strong correlation between sclerostin level with renal function presented as serum creatinine (r=0.745, P<0.001) and eGFR (r=-0.590, P<0.001). Serum sclerostin level was significantly higher in patients with CKD-G3 stage than those with CKD-G1/2 stages after adjusting for age, sex, and BMI (P=0.011). Patients with CKD-G4/5 stages had dramatically increased level of circulating sclerostin. Multiple regression analyses found that age, sex, and eGFR were independent determining factors for circulating sclerostin level. Conclusion. Our data showed that serum sclerostin levels start to increase in diabetic patients with CKD-G3 stage. Further studies are needed to establish the potential role of elevated sclerostin in diabetic patients with CKD.

Aim. To understand glucose lowering therapeutic strategies of French general practitioners (GPs) in the management of type 2 diabetes mellitus (T2DM) patients with chronic kidney disease (CKD).Methods. A multicenter cross-sectional study was conducted from March to June 2011 among a sample of French GPs who contribute to the IMS Lifelink Disease Analyzer database. Eligible patients were those with T2DM and moderate-to-severe CKD who visited their GPs at least once during the study period. Data were collected through electronic medical records and an additional questionnaire.Results. 116 GPs included 297 patients: 86 with stage 3a (Group 1, GFR = 45–60 mL/min/1.73 m2) and 211 with stages 3b, 4, or 5 (Group 2, GFR < 45 mL/min/1.73 m2). Patients’ mean age was approximately 75 years. Insulin was used in 19% of patients, and was predominant in those with severe CKD. More than two-thirds of patients were treated with glucose lowering agents which were either contraindicated or not recommended for CKD. Conclusion Physicians most commonly considered the severity of diabetes and not CKD in their therapeutic decision making, exposing patients to potential iatrogenic risks. The recent patient oriented approach and individualization of glycemic objectives according to patient profile rather than standard HbA1c would improve this situation.

BACKGROUND: The prevalence of chronic kidney disease, particularly in diabetic patients, is increasing rapidly throughout the world. Nowadays, many individuals in developing nations are suffering from diabetes which is one of the primary risk factors of chronic kidney disease.METHODS: Institution based cross-sectional study was conducted at the University of Gondar Hospital from February to April 2016. A total of 229 study participants were selected using systematic random sampling technique. Urine sample was collected for albumin determination by dipstick. The Simplified Modification of Diet in Renal Disease study equation was used to estimate glomerular filtration rate. Binary logistic regression model was used to identify risk factors.RESULTS: Of the total 229 study participants, 50.2% were females and the mean age was 47±15.7 years. Among study participants, the prevalence of chronic kidney disease (CKD) was found to be 21.8% (95% CI: 16% - 27%). Of all study participants, 9(3.9%) had renal impairment (eGFR < 60 ml/min/ 1.73 m2) and 46 (20.1%) had albuminuria. Older age (AOR: 5.239, 95% CI: 2.255-12.175), systolic blood pressure ≥140mmHg (AOR: 3.633, 95% CI: 1.597-8.265), type 2 diabetes mellitus (AOR: 3.751, 95% CI: 1.507-9.336) and longer duration of diabetes (AOR: 3.380, 95% CI: 1.393-8.197) were independent risk factors of CKD.CONCLUSIONS: The study identified high prevalence (21.8%) of CKD among diabetic adults. CKD was significantly associated with older age, systolic blood pressure, type 2 DM and longer duration of DM. Thus, DM patients should be diagnosed for chronic kidney disease and then managed accordingly.

Background. Type 2 diabetes is a worldwide disaster including in Uganda, specifically in Kanungu District which had a rise in diabetic patients in 2018/2019 as compared to the 2017/2018 financial year. This research was determined to access the prevalence and risk factors associated with type 2 diabetes on elderly patients aged 45-80 years attending Kanungu Health Centre IV, Kanungu District. Methods. A cross-sectional study was conducted among patients aged 45-80 years attending Kanungu Health Centre IV from June to August 2019. The prevalence of type 2 diabetes was determined by the blood sugar of patients. Questionnaires were used to collect data for factors associated with type 2 diabetes. Data were statistically analyzed using the statistical package for social sciences (SPSS) version 25 (SPSS Inc., USA) at P<0.05. Results. The overall prevalence of type 2 diabetes was 18.7% among the tested patients. 22.8% of diabetic patients were females as 7.8% were males. The age group most affected by diabetes was 61-65 years. Alcoholism, smoking, body mass index (BMI), and family history were found to be significantly associated with type 2 diabetes at P value < 0.05. Conclusion. There was a high prevalence of type 2 diabetes observed in this study compared to studies done in previous years which raise a public health concern. This study also found that females and patients aged 61-65 years were most affected by diabetes. Lastly, the presence of family history for diabetes, overweight, and being obese increases the chances of acquiring type 2 diabetes.

INTRODUCTION: Having proper knowledge, attitude, and practices (KAP) towards chronic kidney disease (CKD) among type-2 diabetes mellitus (T2DM) patients is very important to reduce the disease burden. However, the information about KAP towards CKD among Bangladeshi T2DM patients is unknown, based on the available literatures till the date. OBJECTIVES: We aimed to assess the KAP towards CKD among T2DM patients attended a selected hospital in Dhaka city. METHODS AND MATERIALS: In this cross-sectional study, we conveniently selected Aalok hospital and also its 224 T2DM patients who attended the outdoor. We excluded the already developed CKD patients and aged over 70 years. We adopted a questionnaire from a validated instrument consisted of a 10 itemed knowledge, 8 itemed attitude, and 7 itemed practice domains towards CKD (total 25 items). Data were collected by face-to-face interviews. Both descriptive and comparative (Independent Sample t-tests and One-way ANOVA tests) statistics were used for data analysis where appropriates. RESULTS: Women respondents were higher (69.6%). The mean age of the respondents was 49.8±11.3 years. Most of them had average level of knowledge (69.6%) and attitude (60.7%). Majority also reported a good hypothetical practices in general if they would find out to have CKD. Knowledge was significantly associated (p<0.05) with sex, level of education, occupation, and income, whereas attitude was with sex, occupation, and income. CONCLUSION: T2DM patients of our study reported mostly average level of knowledge and attitude, and a good hypothetical practices in general towards CKD. Proper health awareness programs are needed to increase the KAP towards CKD among T2DM patients in Bangladesh.
