Lean management is a work philosophy of continuous improvement to achieve a smooth work flow by minimizing all kinds of waste including time, cost and manpower. The concept of lean management started as early as 1980’s and was soon adopted by production industry. However, incorporation of this concept in healthcare has been very gradual and hesitant, though lean heath care has immense potential to improve operational performance. A clinical laboratory, which is essential in practice of evidence-based medicine, can be efficiently managed using lean management concept. Only a few publications in English literature have studied the effect of implementation of lean management in a clinical laboratory. There are certain challenges that a health care manager is likely to face in the implementation of lean management in clinical laboratory especially in developing nations. The present review discusses the implementation of lean management in a clinical laboratory with identification of various steps which require lean thinking.

The study aimed to conduct a cost-value analysis of the production process of a newly introduced batch of external doors in the context of value-added creation and to identify redundant processes that do not create added value and for which appropriate corrective actions could contribute to their elimination. The result of applying improvement actions following the lean management concept was the optimization time nationalized analyzed by eliminating, among others, operations related to unnecessary transport and storage of products. In addition, the optimization production process impacted both shortening the process implementation time and reducing the costs of its implementation. Further activities will be related to the use of the presented methodology to analyze the processes implemented in the company in order to increase their efficiency.

The paper presents the experience of implementing selected instruments of the Lean concept at a steel works. The functioning of an enterprises in the currently prevailing conditions, i.e. in a turbulent environment, results in increased customer requirements. They expect, among other things, high quality of steel products delivered on time at a low price. Many organizations decide to implement the lean management concept without proper preparation. The purpose of this paper is to present the limitations occurring during the implementation of the lean management concept in a steel mill.

Abstract Background Systematic reviews (SRs) play a critical role in evidence-based medicine. Objective To determine the publication trends of SRs in clinical laboratory science (CLS). Methods We searched Scopus to identify all reviews published in the top 20 CLS journals during the past 10 years (2008–2017). We determined year of publication, review type (systematic vs narrative), citations, and whether the review was accompanied by a meta-analysis (MA). Results We identified 2934 reviews. Of these, 2833 (96.6%) were narrative reviews, and 98 (3.3%) were SRs. A total of 67 (66.3%) of the SRs were accompanied by a MA. Three journals accounted for 68 of 98 (69.4%) SRs. The percentage of SRs (relative to all reviews) has increased during the past decade (P = .01). SRs were more frequently published in high-impact journals (P <.001). Conclusion The publication rate of SRs in CLS journals has increased during the past decade.

A Dacron wrap aortoplasty has been described as an adjunct to reduction ascending aortoplasty for the repair of Type A aortic dissections. We report a case of an uncomplicated hemiarch repair with wrap aortoplasty of the distal anastomosis which resulted in severe acute hemolysis. Despite only minimal focal graft deformation on imaging, the patient was found to have a flow gradient across the distal anastomosis, which was reduced by > 50% following release of the outer graft wrap. To our knowledge, only 29 additional cases of hemolytic anemia following aortic dissection repair have been described in the English literature. The reported mechanisms included aortic graft stenosis (50%), graft kinking (23%), external compression of the graft (20%), and a folded elephant trunk appendage (7%). The mean onset of hemolysis following aortic dissection repair occurred 32.2 ± 44.4 months after surgery, with only 16.7% of cases occurring within 2 weeks. This review details the clinical, laboratory, and imaging findings suggestive of mechanical hemolysis following aortic surgery.

AbstractThe article presents the issues of product safety management in enterprises managed in accordance with the Lean Management concept. The Lean Management concept is aimed at limiting or even completely excluding all forms of waste by intensifying and stimulating continuous improvement. One of the ways to reduce waste is to minimize waste and production shortages that arise due to failures in meeting the product safety requirements, quality criteria for a specific product. In addition, good practices in product safety in Lean organizations will be shown, which show the care for the safety of the products offered. On the basis of research, the following will be presented in the article: a general approach to product safety and detection of production deficiencies, as well as the autor’s original proposal for a system ensuring the safety of manufactured products.

Primary hyperparathyroidism (PHT) is the third most common endocrine disorder in men and women after diabetes mellitus and thyroid disease and one of the most frequent causes of osteoporosis and fractures among the secondary osteopathy. PHT refers to socially significant problems in connection with involvement in the pathological process of the majority of organs and system In recent years, the conception of epidemiology, clinic and tactics of management of patients with PHT has changed due to significant increase of morbidity at the expense of identification of mild forms of disease. The modern management concept tactics depends on clinical manifestations. Bearing in mind the importance of the problem under consideration, the working group was set up for the development of federal recommendations on the treatment of PHT based on the principles of evidence-based medicine. The experience accumulated by the domestic and international experts was summarized in the federal clinical guidelines on PHT.

A law clinic typically involves staff and students in a range of complex processes that are highly resource-intensive and which have the potential to detract from core value-adding activities. This paper aims to highlight the challenges associated with resourcing a university law clinic, and evaluate the extent to which lean management is able to provide solutions. It is submitted that proactive and deliberate application of lean management philosophies to law clinic process design has the potential to both reduce resource intensity and enhance value. A literature review was conducted in order to identify lean management principles and methodologies that might be applicable. A case study approach was then used to evaluate key resourcing challenges faced by a UK university law clinic and to explore the extent to which lean thinking might help to overcome them. There is very little literature which discusses the application of lean thinking in the higher education sector, and none which considers the university law clinic context specifically. This paper will provide law school leaders with a resource that will enable them to evaluate and design their clinic processes more effectively, improving the wellbeing of clinic staff and enhancing the pedagogical value of clinic work for students. It will also contribute to the emerging body of literature which highlights the benefits of lean thinking within the higher education sector.
