The review summarizes data on contemporary approaches to diagnosis, prevention and treatment of severe acute parenchymal respiratory failure of various origins including in acute respiratory distress syndrome (ARDS) caused by bacterial and viral pneumonia. It is based on the data of modern properly organized studies, an analysis of international clinical guidelines with a high degree of evidence, as well as the results of many years of own experimental research and clinical observations of treatment of patients with ARDS of various origin, including those with viral pneumonia in 2009–2016–2020. The article states scientifically-based procedures for prevention, differential diagnosis and personalized treatment of severe acute respiratory failure with the use of innovative medical technologies and a wide range of respiratory treatments. The authors did their best to adapt specific proposals for everyday clinical practice.

Abstract Introduction: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is the third coronavirus that make a global concern in 21th century. Pregnant women are particularly at higher risk of developing severe viral pneumonia, possibly because of a partial immune suppression during their pregnancy. Under such critical and rapidly evolving circumstances, these poor findings might be helpful for the treatment of infected pregnant women with 2019-nCoV.Case presentation: In this study, we have reported a pregnant woman at 25 gestational weeks with COVID-19 who has developed severe complications, including hypoxemia, acute respiratory distress syndrome (ARDS), pulmonary fibrosis, and bilateral pleural effusion. In the end, she died one month after admission to the hospital.Conclusion: Pregnant population are especially more at risk of viral pneumonia caused by coronaviruses, further research on the prevention and treatment of new coronavirus is necessary.

Introduction. Respiratory distress syndrome (RDS) is characterized by immaturity of lung tissue, surfactant deficiency and is a common cause of mortality in premature infants. X-ray is the main method for determining the causes and severity of respiratory failure in newborns.Purpose. Systematization of the results of X-ray examination of the lungs of newborns with varying degrees of prematurity, compared with autopsy data.Materials and methods. The analysis of X-ray data and sectional material of 32 premature infants with low and extremely low body weight who died with clinical manifestations of RDS was performed.Research results. The article provides a comparative analysis of various types of radiological changes in the lungs (reticulo-nodular pulmonary pattern (n=10), cellular deformity of the pulmonary pattern (n=5), «air bronchogram» (n=20), «air leakage» syndrome (n=6), focal-confluent shadows/infiltrative-like foci of darkening (n=9)) and autopsy results of premature newborns.Conclusion. The greatest number of coincidences of radiological and histological data took place in BPD (80%), the smallest — in pulmonary hemorrhages (20%). The coincidence of conclusions for pneumonia and GM disease is 58–56%, respectively. Difficulty in the differential diagnosis of the X-ray picture of the lungs in low birth-weight infants lies in the frequent combination of pathological conditions. Respiratory failure with a wide range of pathological changes in the lungs developed in 15 (53,6%) newborns in the absence of criteria for surfactant insufficiency against the background of respiratory support.

The absence of effective drugs for COVID-19 prevention and treatment requires the search for new candidates among approved medicines. Fundamental studies and clinical observations allow us to approach an understanding of the mechanisms of damage and protection from exposure to SARS-CoV-2, to identify possible points of application for pharmacological interventions. In this review we presented studies on the anti-inflammatory, antioxidant, and immunotropic properties of melatonin. We have attempted to present scientifically proven mechanisms of action for the potential therapeutic use of melatonin during SARS-CoV-2 infection. A wide range of pharmacological properties allows its inclusion as an effective addition to the methods of prevention and treatment of COVID-19.

Purpose The aim of this study was to develop a tool to gain insight into the daily experiences of new hearing aid users and to shed light on aspects of aided performance that may not be unveiled through standard questionnaires. Method The tool is developed based on clinical observations, patient experiences, expert involvement, and existing validated hearing rehabilitation questionnaires. Results An online tool for collecting data related to hearing aid use was developed. The tool is based on 453 prefabricated sentences representing experiences within 13 categories related to hearing aid use. Conclusions The tool has the potential to reflect a wide range of individual experiences with hearing aid use, including auditory and nonauditory aspects. These experiences may hold important knowledge for both the patient and the professional in the hearing rehabilitation process.

The calculation of the entropy of flexible molecules can be challenging, since the number of possible conformers grows exponentially with molecule size and many low-energy conformers may be thermally accessible. Different methods have been proposed to approximate the contribution of conformational entropy to the molecular standard entropy, including performing thermochemistry calculations with all possible stable conformations, and developing empirical corrections from experimental data. We have performed conformer sampling on over 120,000 small molecules generating some 12 million conformers, to develop models to predict conformational entropy across a wide range of molecules. Using insight into the nature of conformational disorder, our cross-validated physically-motivated statistical model can outperform common machine learning and deep learning methods, with a mean absolute error ≈4.8 J/mol•K, or under 0.4 kcal/mol at 300 K. Beyond predicting molecular entropies and free energies, the model implies a high degree of correlation between torsions in most molecules, often as- sumed to be independent. While individual dihedral rotations may have low energetic barriers, the shape and chemical functionality of most molecules necessarily correlate their torsional degrees of freedom, and hence restrict the number of low-energy conformations immensely. Our simple models capture these correlations, and advance our understanding of small molecule conformational entropy.

: The prevention and treatment of various infections caused by microbes through antibiotics are becoming less effective due to antimicrobial resistance. Researches are focused on antimicrobial nanomaterials to inhibit bacterial growth and destroy the cells, to replace conventional antibiotics. Recently, carbon dots (C-Dots) become attractive candidates for a wide range of applications, including the detection and treatment of pathogens. In addition to low toxicity, ease of synthesis and functionalization, and high biocompatibility, C-Dots show excellent optical properties such as multi-emission, high brightness, and photostability. C-Dots have shown great potential in various fields, such as biosensing, nanomedicine, photo-catalysis, and bioimaging. This review focuses on the origin and synthesis of various C-Dots with special emphasis on bacterial detection, the antibacterial effect of CDots, and their mechanism.

Cerebellotrigeminal dermal (CTD) dysplasia is a rare neurocutaneous disorder characterized by a triad of symptoms: bilateral parieto-occipital alopecia, facial anesthesia in the trigeminal area, and rhombencephalosynapsis (RES), confirmed by cranial magnetic resonance imaging. CTD dysplasia is also known as Gómez-López-Hernández syndrome. So far, only 35 cases have been described with varying symptomatology. The etiology remains unknown. Either spontaneous dominant mutations or de novo chromosomal rearrangements have been proposed as possible explanations. In addition to its clinical triad of RES, parietal alopecia, and trigeminal anesthesia, CTD dysplasia is associated with a wide range of phenotypic and neurodevelopmental abnormalities.Treatment is symptomatic and includes physical rehabilitation, special education, dental care, and ocular protection against self-induced corneal trauma that causes ulcers and, later, corneal opacification. The prognosis is correlated to the mental development, motor handicap, corneal–facial anesthesia, and visual problems. Follow-up on a large number of patients with CTD dysplasia has never been reported and experience is limited to few cases to date. High degree of suspicion in a child presenting with characteristic alopecia and RES has a great importance in diagnosis of this syndrome.
