Platelet-rich plasma (PRP) is an autogenic source of many growth factors that are a part of the healing process and tissue regeneration, which is the cause of its growing popularity in regenerative medicine. In this paper, we present a definition and characteristics of PRP, as well as ways of extracting it. We also present the issue of thrombocyte activation and the impact of thrombocytes on PRP. We explain how to obtain the optimal density of platelets in PRP, as well as its influence on tissues. Additionally, we describe some of the applications of PRP in experimental and clinical work.

Growth factors (GFs) play a vital role in cell proliferation, migration, differentiation and angiogenesis. Autologous platelet concentrates (APCs) which contain high levels of GFs make them especially suitable for periodontal regeneration and facial rejuvenation. The main generations of APCs presented are platelet-rich plasma (PRP), platelet-rich fibrin (PRF) and concentrated growth factor (CGF) techniques. The purpose of this review is to provide the clinician with an overview of APCs’ evolution over the past decade in order to give reliable and useful information to be used in clinical work. This review summarizes the most interesting and novel articles published between 1997 and 2020. Electronic and manual searches were conducted in the following databases: Pubmed, Scopus, Cochrane Library and Embase. The following keywords were used: growth factors, VEGF, TGF-b1, PRP, PRF, CGF and periodontal regeneration and/or facial rejuvenation. A total of 73 articles were finally included. The review then addresses the uses of the three different techniques in the two disciplines, as well as the advantages and limitations of each technique. Overall, PRP is mainly used in cases of hard and soft tissue procedures, while PRF is used in gingival recession and the treatment of furcation and intrabony defects; CGF is mainly used in bone regeneration.

Introduction: Venous ulcers are wounds that are thought to occur due to improper functioning of venous valves, usually of the lower limbs. Considering that PRP is a source of growth factors, and consequently has mitogenic, angiogenic, and chemotactic properties, it represents an adjunctive treatment for recalcitrant wounds. Moreover, PRP provides the wound with adhesive proteins, such as fibrinogen, which are important in wound healing. PRP contain more amount of platelets, cytokines and growth factors which are dispersed in a very small amount of plasma which can be prepared from a sample of centrifuged autologous blood. Application of PRP has been reported to be effective in both acute as well as chronic non healing venous ulcers. Aim and Objective: To evaluate the efficacy of autologous platelet rich plasma in the management of chronic venous ulcer. Material and Method: A Prospective study conducted on 100 patients of chronic venous ulcers admitted in Sir T. Hospital Bhavnagar from June 2018 to June 2019 after fulfilled our inclusion and exclusion criteria. PRP then injected intalesionally inside and around the periphery of the wound/ulcer. This process was done once/week for 12 weeks. At every week, the area and volume of ulcer was calculated and photographs were taken. Result: All the patients showed healing of the ulcer with reduction in size of ulcer more than 90% was observed in 72 patients, followed by 80–90% reduction in wound size in 18 patients after the 12 weeks follow-up. Overall, significant reduction in size of ulcer was observed in all the treated patients. Conclusion: Autologous platelet rich plasma (PRP) as an autologous method, it is biocompatible, simple, safe, affordable and less expensive procedure in the treatment of chronic venous ulcers. PRP is found to be useful in improving and enhancing the healing process in chronic venous leg ulcers without any side effect. Keywords:  Venous ulcer, Platelet rich plasma

Platelets release several growth factors which stimulate tissue regeneration. Several techniques for platelet concentrates such as platelet rich plasma (PRP), plasma rich in growth factors (PRGF), platelet rich fibrin (PRF) and concentrated growth factors (CGF) have been introduced in dental surgeries for the prevention of hemorrhage and acceleration of tissue regeneration. However, a fabricating growth factors-enriched bone graft matrix which is called “sticky bone” has been demonstrated to provide stabilization in bony defects. In this article, we presented the method of preparing and utilizing CGF and sticky bone and evaluate the effect of CGF mixed with bone graft and CGF barrier membrane in periodontal surgeries.Methodology: We used websites such as PubMed, Scopus, and ISI Web of knowledge to get related articles about this subject. The research process involved specific key words " concentrated growth factor”- “Platelet rich fibrin”- growth factors-enriched bone graft” - “sticky bone” to find more articles which published from 2007 to March 2018.Results: We reviewed 48 articles, 43 articles were excluded. Only five articles have been conducted. Original human studies and case report were included.Conclusion: We concluded that the use of sticky bone and CGF is effective in bone grafting and implant.

The regenerative medicine field has led to the development of various biomaterials. One such development is in the form of Hydrogel Scaffold for Tissue regeneration. This review describes the biomedical advances in Hydrogel Scaffolds as emerging biomaterial for regenerative medicine.  Their unique ability to mimic the extra cellular environment, biocompatibility, flexible method of synthesis, desirable framework for cellular proliferation and survival has made them the material of choice. Hydrogels have demonstrated features which exemplify many of the broad-based manifestations of tissue engineering, providing realized as well as potential commercial value. Direct incorporation of cells and growth factors has led to efficient and more promising results in regenerative medicine. This review gives an overview of the various kinds of hydrogels, fabrication methods with specific features and few of the recent applications of hydrogels in the field of regenerative medicine.

Achilles tendinopathy and patellar tendinopathy are among the most frequent diagnoses in sports medicine. Therapeutic treatment of the disease is difficult, particularly in chronic cases. In literature, several studies suggest the employment of Platelet-Rich Plasma as a therapeutic alternative in tendinopathies. The choice of employing this method is based on the activity of growth factors contained in platelets which activate, amplify, and optimize the healing process. We selected 14 patients affected by Achilles tendinopathy and 7 patients affected by patellar tendinopathy, with a two-year final follow-up. These patients underwent a cycle of three tendinous infiltrations, after clinical and instrumental evaluation carried out by means of specific questionnaires and repeated ultrasound scans. Ultrasound scans of 18 patients showed signs of reduction in insertional irregularities. The result is confirmed by complete functional recovery of the patients, with painful symptomatology disappearing. The patients showed a clear pain reduction, along with an enhanced VISA score after the 24-month follow-up, equal to 84.2 points on a scale of 0 to 100. In conclusion, the present study provides evidence to suggest that PRP infiltration is a valid option to patients with chronic tendinopathy who did not benefit from other treatments.

AbstractA promising approach in regenerative medicine is to modify cell behaviour with growth factors. However, the side of action has to be in spatial control. We present a new strategy in the field of regenerative medicine based on the combination of implant coatings with nanoscaled gene vectors. This enables the local restricted in situ transfection of cells to induce the production of cytokines. Therewith, the migration and differentiation of cells can be controlled to support tissue regeneration.

Abstract Purpose To systematically review the effect of PRP on healing (vascularization, inflammation and ligamentization) and clinical outcomes (pain, knee function and stability) in patients undergoing ACL reconstruction and compare the preparation and application of PRP. Methods Independent systematic searches of online databases (Medline, Embase and Web of Science) were conducted following PRISMA guidelines (final search 10th July 2021). Studies were screened against inclusion criteria and risk of bias assessed using Critical appraisal skills programme (CASP) Randomised controlled trial (RCT) checklist. Independent data extraction preceded narrative analysis. Results 13 RCTs were included. The methods of PRP collection and application were varied. Significant early increases in rate of ligamentization and vascularisation were observed alongside early decreases in inflammation. No significant results were achieved in the later stages of the healing process. Significantly improved pain and knee function was found but no consensus reached. Conclusions PRP influences healing through early vascularisation, culminating in higher rates of ligamentization. Long-term effects were not demonstrated suggesting the influence of PRP is limited. No consensus was reached on the impact of PRP on pain, knee stability and resultant knee function, providing avenues for further research. Subsequent investigations could incorporate multiple doses over time, more frequent observation and comparisons of different forms of PRP. The lack of standardisation of PRP collection and application techniques makes comparison difficult. Due to considerable heterogeneity, (I2 > 50%), a formal meta-analysis was not possible highlighting the need for further high quality RCTs to assess the effectiveness of PRP. The biasing towards young males highlights the need for a more diverse range of participants to make the study more applicable to the general population. Rotator cuff tear is one of the most common shoulder problems encountered by orthopedic surgeons. Due to the slow healing process and high retear rate, rotator cuff tear has distressed millions of people all around the world every year, especially for the elderly and active athletes. This disease significantly impairs patients’ motor ability and reduces their quality of life. Besides conservative treatment, open and arthroscopic surgery contributes a lot to accelerate the healing process of rotator cuff tear. Currently, there are many emerging novel treatment methods to promote rotator cuff repair. A variety of biological stimulus has been utilized in clinical practice. Among them, platelet-rich plasma, growth factors, stem cells, and exosomes are the most popular biologics in laboratory research and clinical trials. This review will focus on the biologics of bioaugmentation methods for rotator cuff repair and tendon healing, including platelet-rich plasma, growth factors, exosomes and stem cells, etc. Relevant studies are summarized in this review and future research perspectives are introduced.
