Palavras – Chave: Odontologia; Anestésico local; Cirurgia oral. ABSTRACT The use of anesthetic solutions is performed almost daily by dental professionals. However, the local anesthetics available on the market come in the form of solutions with varying compositions and concentrations, both with respect to the anesthetic salt, vasoconstrictor and its preservative. Perhaps, due to the wide variety of associations, several doubts may arise regarding the best choice during daily clinical practice, especially when it presents some type of systemic disease. Thus, the objective of this work is to carry out a literature review of the best indications and possible contraindications for local anesthetic solutions available in the national market, relating to systemic diseases that are commonly seen in the dental clinic. Key words: Dentistry; Local anesthetic; Oral surgery.

The treatment of tendinopathy can be challenging for clinicians and might be associated with serious adverse events. However, evidence shows that tendinopathy can significantly impact the professional careers of the affected athletes. The main clinical features of the condition include decreased performance and strength of the affected tendons, swelling, and localized pain.  Although tendinopathy might seem a non-critical condition, estimates show that it might be career-ending for many professional athletes, and therefore, reducing their quality of life. In the present literature review, we have discussed the epidemiology, etiology, and pathogenesis of tendinopathy among athletes. Different intrinsic and extrinsic factors were found to contribute to the development of the condition and the impact of various systemic diseases. Evidence also shows that the prevalence of the condition is high among athletes. However, these rates were variable across the different studies in the literature. This is probably owing to various factors, like demographics and the process of detection. The pathogenesis of the condition is also very complicated, and the development of pain has been attributed to the process of angiogenesis and associated ingrowth of nerve fibers.

Nowadays, dental implants are the most reliable treatment option for edentulism be it partial or complete. Certain systemic diseases have shown to adversely affect the outcome of dental implants and result in complications. Therefore it is prudent that a clinician should be aware of the precautions to be taken in such patients and the complications that may arise if any such medical condition is present in the patient. This article reviews the care that should be taken in systemic diseases that complicate implant therapy and the risks involved if safeguard is not maintained.

Although pruritis might not be a serious condition with significant healthcare impacts, it is usually associated with an unpleasant sensation that leads to scratching the skin. It has been demonstrated that the severity of the condition is significantly variable and ranges between disabling and mild conditions. Chronic pruritis has been defined as the presence of daily itching for >6 months. In the present literature review, we have discussed the different approaches that have been previously indicated to assess and evaluate chronic pruritis, and the classification of the condition its relation to the different systemic diseases. The classification of chronic pruritis can be done using a clinical or an etiological diagnosis. The clinical diagnosis is usually based a primary skin condition, while the etiological diagnosis is based on the presence of different diseases that may be systematic, neurological, or psychiatric disorders. Accordingly, conducting a thorough examination is essential to establish a proper diagnosis before adequately managing the affected patients. Furthermore, the treatment of the underlying etiology should also be adequately considered for adequate management and enhanced prognosis.

In the first part of this literature review, published in October 2019 in this journal, we summarized the conceptual background of the oral microbiota, and the main methods used in microbiology to characterize oral organisms. We also presented the most studied bacteria species in the oral microbiota. In this second part, we will discuss the evidence regarding the biological plausibility linking the oral microbiota dysbiosis and systemic diseases, as well as the main factors and mechanisms suspected in this association.

The article presents a literature review on the description of paracentral acute middle maculopathy. The historical information about the disease, the mechanism of pathogenesis is given. The authors describe the anatomical structure of the microcirculation of the retina. Cases of association of this condition with retinal vascular pathology and systemic diseases are described. The literature data on the prognosis and possible direction of therapy are presented. Key words: optical coherence tomography (OCT), optical coherence tomography-angiography (OCT-A), retina, isсhemia.

Context Many diseases that cause granulomas or granulomatous inflammation involve the liver. Some of these disease processes are intrinsic hepatic diseases, whereas others are disseminated systemic diseases that involve the liver as well as other organs. Objective To review the evaluation of granulomas in the liver with an emphasis on infectious causes, as well as the use of special stains, serologic studies, and molecular diagnostic techniques. Pertinent noninfectious causes of hepatic granulomas that are in the differential diagnosis are also discussed. Data Sources Literature review and cases acquired during years of practice. Conclusions A wide variety of infectious and noninfectious entities cause hepatic granulomas.

Background: Posterior fossa extradural hematomas are an uncommon condition with more than half of cases appearing in the pediatric population. Although the majority of cases are caused by substantial trauma, it may also be related to systemic diseases and bleeding disorders without the history of head trauma. Case report: We present the case of a 6-year-old female affected by Sensenbrenner syndrome with a non-traumatic subacute posterior fossa epidural hematoma. The patient was surgically treated and medically discharged without further neurological alteration. This is the first report of spontaneous cerebral hematoma associated with this syndrome in the literature. A literature review of the main aspects of both topics was performed. Conclusions: Although spontaneous posterior fossa epidural hematomas are rare, clinicians should be aware and highly suspicious of this differential diagnosis in certain situations such as coagulopathies and other specific systemic diseases, as fast recognition and assertive treatment are important for better outcomes.
