Objectives: The aim of this study was to evaluate the effect of the precipitate that was formed by combining Sodium Hypochlorite (NaOCl) and Chlorhexidine Gluconate (CHX) on the sealing ability of root canal obturation materials. Materials and Methods: The fluid filtration method was conducted on a total of 100 roots. Samples were randomly divided into two control (n=5) and three experimental groups (n=30). The samples in group 1 were irrigated with 1.5 mL of 2.5% NaOCl, and then the smear layers of the teeth were removed by 17% EDTA, while the specimens of group 2 were irrigated by 1.5 mL of 2.5% NaOCl and 1.5 mL of 2% CHX; after the smear layer removal, a final flush with 1.5 mL of 2.5% NaOCl was performed. The samples of group 3 were irrigated the same as group 1 but after the smear layer removal canals were irrigated again with 1.5 mL of 2.5% NaOCl and then a final flush with 1.5 mL of 2% CHX was performed. Teeth were obturated with gutta-percha and AH26 sealer and after seven days, microleakage was evaluated by the fluid filtration technique. The results were analyzed by the ANOVA and Tukey's test. Results: The samples in group 3 had significantly greater microleakage compared to teeth in group 1, 2 (p<0.05), and the specimens in group 1 showed significantly less amount of microleakage than samples in group 2, 3 (p<0.05). Conclusion: The presence of the precipitate that is formed due to interaction between NaOCl and CHX has negative effect on the sealing ability of gutta-percha and AH26 sealer.

ABSTRACT Objectives: The aim of this study was to evaluate the sealing abilities of three different gutta-percha techniques in experimentally defective roots (EDR) and non-defective roots (NR). Materials and Methods: Sixty canine teeth were divided into six groups of ten; Group 1, NR + cold lateral condensation (CLC); Group 2, EDR + LC; Group 3, NR + BeeFill; Group 4, EDR + BeeFill; Group 5, NR + Thermafil; and Group 6, EDR + Thermafil. Apical leakage was measured using a computerized fluid filtration meter with a laser system. Results: Statistical analysis revealed that the CLC demonstrated more microleakage in the EDR than in the NR (P < 0.01). Thermafil demonstrated more microleakage in the NR than in the EDR (P < 0.01). No statistically significant differences were found between the BeeFill groups (P > 0.05). Conclusions: The results of this study indicate that internal resorptive cavities can affect the apical sealing properties of different root canal filling techniques, with Thermafil ensuring the lowest apical microleakage.

ABSTRACT Aim To evaluate and compare the efficacy, cleaning ability of hand and two rotary systems in root canal retreatment. Methodology Sixty extracted premolars were retreated with following systems: Group 1—ProTaper Universal retreatment files, Group 2—ProFile system, Group 3—H-file. Specimens were split longitudinally and amount of remaining gutta-percha on the canal walls was assessed using direct visual scoring with the aid of stereomicroscope. Results were statistically analyzed using ANOVA test. Results Completely clean root canal walls were not achieved with any of the techniques investigated. However, all three systems proved to be effective for gutta-percha removal. Significant difference was found between ProTaper universal retreatment file and H-file, and also between ProFile and H-file. Conclusion Under the conditions of the present study, ProTaper Universal retreatment files left significantly less guttapercha and sealer than ProFile and H-file. Clinical significance Rotary systems in combination with gutta-percha solvents can perform superiorly as compared to the time tested traditional hand instrumentation in root canal retreatment. Abstract Objective: To compare the sealing ability of three single-cone obturation systems using a glucose leakage model. Materials and Methods: A total of 90 freshly extracted human mandibular incisors with oval canals were selected and their crowns were cut. The root canal of each sample was instrumented using a rotary crown down technique and then divided into four experimental (n = 20 each) and two control groups (n = 5 each). Samples in the experimental groups were filled as follows: Group 1, cold lateral condensation using Gutta-percha/AH Plus; Group 2, single-cone C-Points/Smart-Paste Bio Sealer; Group 3, single cone Bio-Ceramic Impregnated Gutta-Percha/Endo-Sequence Bio-Ceramic Sealer; Group 4, single-cone Resilon/RealSeal SE. After 7 days, the sealing ability of root canal fillings was tested at different time intervals using glucose leakage model. Glucose leakage values were measured using a spectrophotometer and statistically analyzed. Results: The four experimental groups presented significantly different glucose leakage values at all test periods (P < 0.05). At the end of the observation period, the cumulative glucose leakage values of Groups 2 and 3 were significantly lower than those of Groups 1 and 4 (P < 0.05). Conclusion: C-Points/Smart-Paste Bio and Bio-Ceramic Impregnated Gutta-Percha/Endo-Sequence Bio-Ceramic Sealer combinations provided the superior sealing ability over the lateral condensation technique.

AIM: The aim of current research was to find out the sealing ability of two commonly used sealers in the presence and absence of smear layer at different time interval. MATERIAL AND METHODS: Total of 180 single rooted vital teeth were used. Transversal section was made with the help of digital slow speed cutting saw to divide the root and crown part. After removal of crown roots part was instrumented and prepared. The 180 teeth were randomly divided into two equal groups (n=90) 1 and 2. In group 1 the smear layer was kept intact but it was removed from group 2 with EDTA (17%). Group 1 was then divided into two sub-groups, A1, A2. Group 2 was again separated into two sub-groups, B1 and B2. Each sub-group contained of 45 samples. In sub-groups A1 and B1, AH Plus sealer, in sub-groups A2 and B2, Ketac-endo sealer and cold lateral condensation technique was used for obturation with gutta percha. All samples were kept in an incubator at 37°C for 24 hours, with help of nail polish/varnish all samples root surfaces was painted only excluding apical area after words each sub group were further divided in to three groups of 15 to represent immersion periods of 7,15 and 30 days. All samples were then kept in 5 % methylene blue dye solution at 37o c for their respective time periods. After specific time period, the roots of every group were cut longitudinally and evaluate under a stereomicroscopes to evaluate apical micro leakage in millimeter. Data was subjected to Repeated measure ANOVA with post-hoc analysis using Tukey and bonferroni tests. RESULTS: Overall analysis indicated significant reduction in sealing ability canals in with and without smear layer over different time periods (p <0.0001). CONCLUSION: Current study has concluded that AH plus sealer provides significantly better seal in the absence of smear layer than in the presence of smear layer in addition it gave better seal than Ketac-Endo sealer in the absence and presence of smear layer.

AIM: To evaluate the effect of different irrigant solutions employed during removal and replacement of calcium hydroxide paste on the diffusion of hydroxyl ions through root canal dentine in vitro. METHODOLOGY: Thirty-five maxillary and mandibular human canines with straight and fully developed roots were used. After mechanical preparation up to 1mm short of tooth length, 30 canals were filled with calcium hydroxide paste and 5 canals were left empty; all teeth had their coronal accesses properly sealed. Teeth were placed in plastic containers with distilled water, and pH was read after 30 days when the paste from 20 teeth was renewed. After removal of the paste by endodontic instrumentation and irrigation with distilled water, canals were replenished with newly mixed paste in Group 1 and 2. In these groups, final irrigation was conducted with 5 mL of EDTA followed by 5 mL of NaOCl in specimens in Group 1, and 5 mL of NaOCl only in specimens in Group 2. In 10 teeth the paste was not replenished at 30 days (Group 3). All specimens were returned to the containers with fresh distilled water, and the pH was recorded after another 30 days. The differences between the first (30d) and second (60d) pH readings were calculated and submitted to analysis of variance and individual comparisons using the Scheffeé's test. RESULTS: Results of mean analysis on differences of pH readings showed that greater diffusion had occurred on specimens in Group 3. Individual comparisons using Scheffeé's test showed statistical significance between Groups 2 and 3, and equivalence between all other groups. CONCLUSION: It was concluded that the use of EDTA did not enhance diffusion of hydroxyl ions through root canal dentine.

Objective. To compare the efficacy of ProTaper retreatment files in removing three different obturating materials.Study Design. Forty-five human, single-rooted premolars were divided into three experimental groups. Group 1 was obturated with gutta-percha and AH Plus sealer, Group 2 was obturated with gutta-percha and zinc oxide eugenol sealer, and Group 3 was obturated with GuttaFlow. Retreatment was done using the ProTaper universal rotary retreatment files. Root halves were visualized using magnifying loops at 3X magnification and optical stereomicroscope at 10X magnification. Images were analyzed using AutoCAD 2004 software to calculate area of the remaining debris in the canal. For statistical analysis were used variance test and ANOVA.Results. Total debris/canal area ratio between the three groups showed a statistically significant difference (P<0.001).Conclusion. ProTaper retreatment system did not produce completely clean canals in any of the groups. However, it had the best efficacy towards removing silicon based obturating material GuttaFlow.

Abstract This study aimed to evaluate sodium hypochlorite (NaOCl), limewater (LW), and Polymyxin B (PMB) as irrigants over MMP-3, MMP-8 and MMP-9. Thirty-three patients with apical periodontitis of single-rooted teeth were treated according to three-experimental groups (n=11): group-1: 2.5% NaOCl was used as irrigant; group-2: 2.5% NaOCl for the first two files and LW: [0.14% Ca(OH)2] for the last two files; group-3: 2.5% NaOCl for the first two files and PMB for the last two files. The association of Ca(OH)2 and CHX was used as an intracanal medication in all groups. Four root canal samplings (S) were collected: S1) immediately after access cavity; S2) after biomechanical preparation; S3) after EDTA application; and S4) after removal of the intracanal medication. After quantification of MMP-3, MMP-8, and MMP-9, the data were analyzed by Friedman and Kruskal-Wallis tests and completed by Dunn test (5%). Regardless the used irrigant, there was no difference in reducing MMP-3 or MMP-8 (P=0,5273, P=0,7048 respectively). However, in reducing MMP-9 (P=0,0246) the NaOCl group was the most effective followed by NaOCl+LW group and NaOCl+PMB group respectively. The intracanal medication [Ca(OH)2 + CHX] with the NaOCl and NaOCl+LW was effective in reducing MMP-8 (P<0,0001, P=0,0025) and MMP-9 (P=0,0007, P=0,0047) respectively, but not for the group of NaOCl+PMB which was not effective in reducing MMP-8 or MMP-9 (P=0,1718, P=0,1953) respectively. NaOCl and NaOCl+LW were effective in reducing MMP-9 levels, and this effectivity could be improved by the use of the intracanal medication [Ca(OH)2 + CHX] in reducing MMP-8 and MMP-9 levels.

ABSTRACT Forty-eight extracted maxillary anterior teeth instrumented with ProTaper system to apical preparations 0.25, 0.40 and 0.60 mm (n = 16 each) were divided into six groups for filling as following: Group 1: Apical preparation 25/100 + gutta-percha/AH Plus sealer, group 2: Apical preparation 25/100 + Resilon/Real Seal, group 3: Apical preparation 40/100 + gutta-percha/AH plus sealer, group 4: Apical preparation 40/100 + Resilon/Real Seal, group 5: Apical preparation 60/100 + gutta-percha/AH plus, group 6: Apical preparation 60/100 + Resilon/Real Seal. All teeth were filled using the warm vertical technique. Horizontal sections at 1, 2 and 3 mm from the apex were observed by SEM under 200 × magnification. Sections were digitally photographed under a stereomicroscope and the images were transferred to a compatible PC for image analysis. The surface area of voids were calculated and compared at all levels; using the Kruskal-Wallis test with Sidak correction and Mann-Whitney U test (p > 0.05). Comparisons within each group (gutta-percha/AH plus and Resilon/Real Seal) showed the presence of voids but there was no significant difference between any level of sectioning for apical preparations of 25 and 40 (p > 0.05). The only difference was in the groups prepared to size 60 and filled with Resilon/ Real Seal which showed significantly more voids than the group filled with gutta-percha/AH plus and especially at the 3 mm level. Conclusion The system Resilon/Real Seal did not achieve better results in terms of sealing ability in the apical third when compared to the conventional gutta-percha/AH plus sealing system. Aim: The aim of this study was to evaluate the effectiveness of two instrumentation and irrigant techniques in smear-layer removal from root canals. Methodology: Thirty single-rooted teeth were randomly divided into two groups. Group 1 (continuous irrigation) was prepared using Self-adjusting file, and group 2 (manual irrigation; conventional needle irrigation) was prepared using with ProTaper file system. Groups were irrigated using sodium hypochlorite (5%) as an initial irrigant following MTAD in a closed system. Canals were bisected and examined by scanning electron microscopy. Smear layers were evaluated using a five-point scoring system with 2500x magnification. Results: Smear layers were eliminated in the coronal, middle, and apical thirds of the root canal, respectively, in 93%, 80%, and 60% of samples in the group 1, and 67%, 60%, and 27% in the group 2, of the coronal, middle, and apical thirds of the root canals, respectively. Evaluation by SEM showed continuous irrigation differed not-significantly from the manual irrigation group in all areas (P >0.05). Objectives: The purpose of this in vitro quantitative laboratorial study is to compare the ability of three filling techniques to fill simulated lateral canals.Methods: Thirty extracted, single-rooted human teeth were used. After cleaning and shaping, three lateral canals were created, one in each third. The teeth were randomly separated into three groups: continuous wave of condensation (Group 1); thermomechanical compaction (Group 2); and lateral condensation (Group 3). The teeth were cross-sectioned, making the cut through points over the lateral canals; thus, 90 specimens were obtained. Each specimen was immersed in a polyester resin, and the blocks were polished. Images were obtained using a stereoscopic lens (40x). Radiographic analysis was performed, followed by a filling linear measure using the Image Tool 3.0 program (University of Texas). Data were statistically analyzed using SPSS 12.0 for Windows (Kruskal- Wallis test).Results: A greater number of simulated lateral canals were obturated in Groups 1 and 2. Group 2 presented the largest percentage of linear measure of lateral canals filling with gutta-percha and sealer. No statistical differences were found between Group 1 and Group 2 when we analyzed the filling with gutta-percha and sealer or just sealer (P>.05).Conclusions: Thermoplasticized gutta-percha filling techniques (Groups 1 and 2) are better for filling lateral canal with gutta-percha and sealer or with just sealer than lateral condensation (Group 3).
