Clinical evaluation of Kasisadi Ghrita local application in Parikartika (Fissure-in-ano)

Background: Parikartika is characterized by sharp cutting pain in anal regionIn Parikartika, Teevrashoola, bleeding is seen, similarly severe pain and slimy blood discharge are seen in Fissure-in-ano. Objective: To assess the efficiency of Kasisadi Ghruta local application in the management of fissure-in-ano. Methods: The patient having classical signs and symptoms of Parikartika (fissure in ano) having Age group 20 to 60 years were recruited for the study. Trial group was treated with Kasisadi Ghruta twice a day local application for 28 days. Results: Average relief observed was 62.5% in most of symptoms. Kasisadi Ghruta is having Shothahara, Vedanasthapana and Ropana properties due to which it helps in healing of fissure in ano. Conclusion: Kasisadi Ghruta local application is having better results in fissure-in-ano.

To test the efficacy of thelocal application of kasisadi ghruta in managing. Classical signs and symptoms of parikartika (fissure in ano) age groups of 20 to 60 years have been recruited for the research. The trial group obtained treatment with kasisadi ghruta local application for 28 days, twice a day. Average relief observed was observed 62.5% for most symptoms shothahara, vedanasthapana, and ropana are in kasisadi ghruta. Properties because of which it helps in healing the ano fissure.

Gridhrasi comes under 80 types of Nanatmaja Vatavyadhi. The cardinal signs and symptoms of Gridhrasi are Ruk (pain), Toda (pricking sensation), Spandana (twitching) in the Sphik, Kati, Uru, Janu, Jangha and Pada in order and Sakthikshepa Nigraha i.e., restricted lifting of the leg associated with Gourava, Aruchi. Kati Basti is widely being practiced throughout country as Bahyaupakrama which has both Snehana and Swedana effects. Matra Basti is Snehana procedure which does Vataanulomana, Vatashamana. Objectives: To evaluate the efficacy of Matrabasti and Katibasti with Dhanvantaram Taila in the management of Gridhrasi. Materials and Methods: Patients presenting with the classical features of Gridhrasi and between the age group of 16 to 50 years irrespective of sex were selected and allotted in Group A, B and C with 15 patients in each group. Group A was administered with Matra basti with Dhanwantram Taila and Group B with Kati basti with Dhanwantram Taila and Group C with both Matra basti and Kati basti with Dhanwantram Taila for 9 days. Results: In combined treatment of Kati Basti and Matra Basti in Group C there was tremendous response in relieving Ruk (73%), Toda (46%), Spandana (60%), Supti (80%), Gourava (73%), Aruchi (66%), Sparshasahyata (73.3%) SLR (60%). In Group A, patients subjected to Matra Basti was effective in relieving Ruk (26.7%), Toda (13.3%), Spandana (26.7%), Supti (66.7%), Gourava (46.7%), Aruchi (53.3%), Sparshasahyata (13.3%) SLR (6.7%). In Group B patients subjected to Kati Basti was effective in relieving Ruk (6.7%), Toda (0%), Spandana (0%), Supti (40%), Gourava (20%), Aruchi (0%), Sparshasahyata (20%) SLR (0%). Conclusion: On the basis of the results of this study, it can be clearly concluded that combined treatment of Matrabasti and Kati basti with Dhanwantaram taila provided significant relief in the signs and symptoms of Gridhrasi than individually performed Matra basti and Kati basti with Dhanwantaram taila.

Fissure means a tear or crack. In other words, fissure-in-anois simply just a tear in the anal canal. Mainly occurs because of constipation or loose motion (frequently passing stool) or as a complication of Virechan & Basti karma. One and most important sign of fissure-in-ano is pain and bleeding during and after defecation. Severity and complication increase along with time, recurrence, and routine habits. Daily lifestyles include dietary habits, working field, personal hygiene, sleep & bowel habits, stress, etc. which directly affects the digestion of an individual. And we all know that our digestion is the direct key to our health. Early-stage or acute fissure can be cured by local application of medicines or by subsiding digestive issues (constipation or loose motions). But chronic fissure or fissure due to spasticity of sphincters is still a point of worry and research. In our study, we included 60 patients having signs and symptoms of Parikartika from OPD, and Arka kshar was used for local application. The patient was clinically assessed based on pain, per rectal bleeding, itching etc. Significant results were observed in spasticity and symptoms of patients.

Allergic conjunctivitis is a common ophthalmic problem predominantly affecting the outdoor workers. The eyes are exposed to different environmental factors. The eye and eyelids are very common sites for allergic reactions. About 50% of conjunctivitis seen by primary physicians is allergic in nature. Vataj Abhishyanda is a clinical entity which can be correlated with allergic conjunctivitis. Triphala Ghrita Aschyotan helps to relieve the symptoms of Vataj Abhishyand w.r.t allergic conjunctivitis. Triphala Ghrita is a Vyadhi Pratyanik Dravya and helpful in topical eye allergies. Aim: To study the role and efficacy of Triphala Ghrita Aschyotan in Vataj Abhishyanda w.r. t. allergic conjunctivitis. Materials and Method: A total 60 patients of the age group 15-60 years presenting with signs and symptoms of Vataj Abhishyanda w.r.t allergic conjunctivitis were selected randomly from OPD of the department of Shalakyatantra, Government Ayurved College, Nanded (M.S.) within inclusion criteria and were treated in two groups. The 30 patients of trial group were treated with Triphala Ghrita Aschyotana and patients of control group in similar number were subjected to Ketotifen Fumarate eye drop. Results: The trial drug Triphala Ghrita is equally effective as compared to Ketotifen eye drop. Trial drug provided more relief in symptoms like Sangharsha (Itching of eyes), which is the parameter of efficacy. Conclusion:Triphala Ghrita Aschyotan is an effective, safe and potent treatment of Vataj Abhishyanda w.r.t allergic conjunctivitis.

Abhishyanda is one of the Sarvagata Vyadhi mentioned by Acharya Sushruta. In modern science, signs and symptoms of Netrabhishyanda may be correlated with conjunctivitis, which is the inflammatory condition of conjunctiva. Kavala Dharana, Dhumrapana, Aschyotana, Sechana Putapaka etc. are various suggested regimens in Ayurvedic texts for the treatment of Netrabhishyanda. Considering this effectiveness of Palasha as mentioned by Acharya Sushruta, it is planned to study the use of Palasha Mula Arka Aschyotana in Pittaja Netrabhishyanda. 60 patients having classical signs and symptoms of Pittaja Abhishyanda (Acute Bacterial Conjunctivitis) as per Ayurvedic and modern texts were selected for the study. After completion of therapy, mean Conjunctival hyperemia was reduced to 0.2167 ± 0.4086 in Control and 0.5667 ± 0.5040 in Trial Group. Palasha Mula Arka was found effective in reduction in conjunctival hyperemia, watering and the number of culture colonies, it was also found effective in burning sensation, discharge, foreign body sensation and photophobia.

Forty children (age group four to 12 years) undergoing adenoidectomy and/or tonsillectomy were subjected to pre- and post-operative polysomnography. Thorough clinical evaluation and X-ray soft tissue nasopharynx lateral view was carried out for all the patients. The tonsils were clinically graded from grade I to IV, whereas the adenoids were measured radiographically (using three different measurements) in all children. Thirty out of 40 (75 per cent) children presented with predominant obstructive symptoms, out of whom 22 (73.3 per cent) were found to have obstructive sleep apnoea (OSA), i.e. apnoea index > five per hour. The remaining 10 (25 per cent) had predominantly inflammatory symptoms on presentation and out of these two (20 per cent) were found to have OSA. Relative adenoid size expressed as a ratio between the distance from the point of maximum thickness of adenoids along a line drawn along a straight part of the basiocciput and distance from the posterior nasal spine to the antero-inferior edge of the spheno-basioccipital synchondrosis, was found to have a highly significant correlation with the grade of OSA. In our study, all patients with this ratio greater than 0.64 were found to have OSA. No correlation between tonsil size and grade of OSA was found. There was a highly significant improvement in polysomnographic scores following surgery in all patients.

Fissure in Ano ( Parikartika) is a vertical tear or an elongated longitudinal ulcer in the long axis of stratified squamous epithelium of the lower anal canal. Ancient text couldn’t give a brief idea about this condition. Mainly it has been described as complication of Bastikarma and Atisar. It is so named in which sharp cutting pain is felt in the anus. For that Modern surgical treatment of choice is again has limitations such as faecal incontinence. Hence, karpoor Ghrita having effective Vranaropan & vedanashamak, tridoshaghna  property i.e. it relieves burning sensation quickly. So we modified a method of Local Application is conceived for this study.
