A Comparative Study on the effect of Padabhyanga with Tila Taila and Nimba Taila in management of Vipadika w.s.r. to Padasputana

Background: Vipadika affects irrespective of age, sex and socioeconomic status. The causes may be excessive/barefoot walking, contact with dust, detergents and improper food habits. Here control over Vatadosha is important which can be achieved through Padabhyanga. Materials and Methods: It was a comparative clinical study of 30 patients, who were selected by random sampling from the OPD of S.M.V.V.S, R.K.M Ayurveda Medical College, Vijayapura, and categorized into 2 groups as 'A' and 'B', each consisting of 15 patients and were advised Tila Taila and Nimba Taila Padabhyanga for 30 days respectively. Follow up was advised on 45th day of treatment. Severities of the symptoms were assessed before, after treatment and after follow up. Results: In group-A 20% of patients showed complete relief and in group-B 0% have showed complete relief. Conclusion: Tila Taila is having Sara, Sukshma Guna and pacifies Vatakapha Dosha. Nimba Taila is Kushtagna, Krimigna. Both Tailas help in reducing Rukshata and Sputana. Vipadika can be correlated with cracked heels. The results were assessed statistically by Pooled chi square test. It was concluded that group-A patients were significantly better than group-B after follow up. Hence Tila Taila Padabhyanga is beneficial in Vipadika.

Objective: To compare the outcomes stability, malunion and intra operative time by using 2.0mm conventional miniplates and 3-D plates in management of the anterior mandibular fracture. Materials and Methods: Total 94 patients were included and equally divided in two groups. Fixation of fracture was done by 2.0mm conventional miniplates in Group-A and a single 3D plate in Group-B. Stability, healing and malunion were assessed on follow-up at 1st week, 6th week, and 12th week using Chi Square Test. Results: Stability was 91.5% in group-A and 93.6% in group-B. Clinically malunion was 12.8% at 1st week, 14.8% at 2nd week, 14.8% at 8th week in group-A while in group-B, malunion was 8.5% at 1st week, 2nd week and at 8th week. Radiographically, malunion was 12.8% at 1st week, 14.8% at 2nd week, 14.8% at 8th week in group-A while in group-B, malunion was 8.5% at 1stweek, 2nd week and at 8th week. Conclusion: The results of the study concluded that 3-D plating system has advantages over conventional 2 mm miniplates.

In the present study follow up kept on immediate post-operative day and at one week to evaluate pain, bleeding and inflammation. Final follow up at two months for assessment of aesthetic score by three independent personnel. Pain and inflammation were more associated with Group B compared to Group A, but there are no statistical significance differences among this groups (P value- 0.074 and 0.136 for pain and inflammation respectively on immediate post-operative day. Chi-square test). Final follow up assessment of aesthetic scores at two months calculated by Chi- square test and comparisons of two groups for aesthetic scores done by Independent t-Test. All three personnel gave higher aesthetic score to Group B but statistically this was not significant (P-value 0.287, 0.129 and 0.400 by Observer 1, 2 and patients respectively. Chi-square test). The mean aesthetic score given by Observer 1, 2 and patient was higher associated to Group B but statistically this was not significant (P=0.526, 0.055 and 0.232 independent t-Test).

Objective: To compare the clean intermittent self-catheterization (CISC) with continuous indwelling catheterization (CIDC) in relieving acute urinary retention (AUR) due to benign enlargement of prostate (BEP). Materials and Methods :A total 60 patients attending in urology department of Dhaka Medical college hospital were included according to inclusion criteria ,Patients were randomized by lottery into two groups namely group –A and group –B for CISC and IDC drainage respectively . Thus total 60 patients 30 in each group completed study. Results : Most men can safely be managed as out-patients after AUR due to BPH. The degree of mucosal congestion and inflammation within the bladder was found to be lower in those using CISC and the bladder capacity in these patients was also found higher.Patients with an IDC had a high incidence of UTIs then that of patients with CISC. During the period of catheterization the incidence of UTI was 43.3% in group B in comparison to 40% in group A; before TURP 36% in group B in comparison to 10% incidence in group A.According to patient’s opinion CISC is better than IDC in the management of AUR. Experiencing bladder spasm, reporting blood in urine, management difficulties, incidence and severity of pain were less in CISC group, and the method of CISC was well accepted by patients as well as their family members. Conclusion: From the current study it may be suggested that CISC is better technique for management of AUR patient due to BPH than IDC. Objective: To evaluate the effectiveness of placing a resorbable collagen barrier in impeding the extrusion of obturation material in primary molars undergoing resorption. Study design: All the 94 canals in 47 mandibular molars were allocated to 2 groups- Group ‘A’- 47 canals with collagen barrier (Test group) and Group ‘B’- 47 canals without collagen barrier (Control group) based on randomization protocol. Pulpectomy was performed and obturation of both test and control canals were radiographically assessed. Pearson’s chi – square test was applied to analyze the results. The significance level was predetermined at p < 0.05. Results: Among the test group, 93.6% of the canals showed no extrusion while, 6.4% showed visible extrusion of the material outside the apex. In the control group, 83% showed no extrusion whereas 17% of the canals showed visible extrusion outside the apex. But no significant difference was noted (p>0.05). Conclusion: The placement of resorbable collagen barrier in the apical third of the canal prevented the extrusion of obturating material beyond the apex in resorbing primary molars.

Katigraha is a condition in which the vitiated Vata is localizing in the Katipradesha and producing stiff-ness and pain. These symptoms are similar to Lumbar Spondylosis. The degenerative changes due to aging or trauma, changing in lifestyle & work pattern, unhealthy food habits causing nutritional deficit etc., are causing Dhatukshaya and Margavarodha. Here Ashtavarga & Gandharvahasthadi Kashayas are taken to evaluate the effect on Katigraha. Totally 40 patients of Katigraha were selected, divided in to two groups equally. Group A treated with Ashtavargamkashaya and Group B treated with Gandharvahasthadi Kashaya for 30 days. In group A, 55% patients got moderate improvement and 40% patients with mild im-provement in group B, 5%got moderate improvement and 60% got mild improvement. It is concluded that in general statistically significant results were seen in both the groups. AshtavargamKashaya showed better efficacy than Gandharvahasthadikashaya.

Due to present day lifestyle, a greater number of people are inclined to desk work and computer usage leading to many disorders. Manyasthamba is one among such disorders where the stiffness of neck with severe pain is the classical symptom which hampers our day to day life. While explaining treatment of Manyasthamba our Acharyas explained Rukshasweda and Nasya Karma as main line of treatment. Here a study was done by taking Gudashunti Yoga explained in Sharangadara Samhita indicated for Nasya and Kolakulattadi Churna indicated in Vatavyadhi explained in Ashtanga Sangraha for Ruksha Churna Sweda. A comparative clinical study of 40 patients suffering from Manyastambha were selected after thorough investigation. Patients were subjected to Nasya Karma in Group A and Ruksha Sweda and Nasya Karma in Group B for 7 days. Patients were assessed based on standard parameters before and after treatment and 7 days follow up. The statistical analysis revealed that there was a significant improvement in parameters like pain and stiffness. Hence proving the efficacy in the condition.

Nowadays, people are more vulnerable to metabolic disorders due to their faulty dietary and behavioural habits. One such disorder is Vatarakta which causes functional impairment due to involvement of Sandhi (joints). It is manifested by Ruk, Toda, Sparsha asahatva, Shopha, Raga, Daha and Stabdhata in Sandhi. Vatarakta can be correlated with Hyperuricaemia or Gout due to similarity in their clinical features. Hyperuricaemia is defined as abnormally high level of uric acid in blood (i.e. >6mg/dl in female and >7mg/dl in male). On the other hand, Gout is an inflammatory response to monosodium urate crystals formed secondary to hyperuricaemia. Aims and objectives: 1. To evaluate the effectiveness of Trikarshika kwatha and lifestyle modification in the management of Vatarakta. 2. To compare the effects of Trikarshika kwatha with and without lifestyle modification in the management of Vatarakta. Materials and methods: Raw herbs of the research formulation were collected after proper identification and Kwatha was prepared for oral administration. For the clinical study, total 60 patients were selected on the basis of selection criteria. Selected patients were randomly divided into two groups. (i) Group A: 30 patients were treated with Trikarshika kwatha. (ii) Group B: 30 patients were treated with Trikarshika kwatha along with Lifestyle modification. Individual patient was treated for 45 days along with follow up at the interval of every 15 days. To assess the effectiveness of treatment, scoring pattern was followed for subjective and objective parameters. They were assessed before and after treatment. The collected data were analysed statistically by using Paired t-test. Results: On the basis of all statistical data, it can be said that patients of Group B showed better results in all parameters in comparison to patients of Group A. Conclusion: Both Trikarshika kwatha and Lifestyle modification are affective but Trikarshika kwatha with Lifestyle modification is more effective than Trikarshika kwatha without Lifestyle modification in the management of Vatarakta.

A chalazion is chronic lipogranulomatous inflammatory lesion caused by blockage of meibomian gland orifices & stagnation of sebaceous secretion. Common practices in treatment of chalazion are intralesional steroid injection, incision & curettage and excision of chalazion. Recurrence rate is high for incision & curettage in case of large chalazion. The aim of the study is to established that excision large chalazion give better result than incision & curettage. The study was carried out at Diabetic Association Medical College & Hospital, Faridpur and General Hospital, Fadidpur. A total 100 cases were selected for study. Technique of operation were incision & curettage through conjunctival surface and excision of chalazion through skin surface. In 50 % cases we performed incision & curettage both in small (size <5mm) and large (size >5mm) chalazion. In 50 % cases of large chalazion we performed excision of chalazion. The follow up period was 3 months to 6 months. In group -A with incision & curettage through conjunctival surface, in case of small chalazion 27 out of 30 patients were cured (90%). In case of large chalazion 14 out of 20 patients were cured (70%). In group B with excision of chalazion through skin surface, in case of large chalazion 49 out of 50 patients were cured (98%). So higher success rate after excision of large chalazion through skin surface.Faridpur Med. Coll. J. 2014;9(2): 68-69

Abstractwas to assess efficacy of Tadalafil alone versus Tadalafil plus Pentoxifylline in the treatment of erectile dysfunction by using self administered IIEF-5 questionnaire.Two hundred and thirty seven patients presenting with ED at andrology OPD were evaluated for ED by a self administered IIEF (International Index of Erectile Function) questionnaire. Patients were systematically randomized by computer generated random table into two groups groups namely, group A: Tadalafil only group, group B: combination of Tadalafil + Pentoxifyl-line. All the patients were re-assessed by IIEF-5 questionnaire after 8 weeks of medical therapy. Statistical analysis was performed using student’s unpaired t-test, paired t-test, chi square test. p-value < 0.05 was considered statistically significant.Two hundred and thirty seven patients were included in the present study, in group A: 92 patients (78.6%) showed improvement in their IIEF score after 8 weeks of tadalafil treatment. While in group B, overall 104 patients(86.6%) showed improvement after combination of Tadalafil and Pentoxifylline. There was a statistically significant difference of percentage change in IIEF score was seen in group B (group A 90.7±15.2%, group B 95.6±13.4%; p value – 0.014). We found this difference even more statistically significant in patients with severe ED (group A 72.7±47.2%, group B 132.3±54.3%; p value – 0.000). There was no significant difference in between the two groups with regards to occur-rence of side effects.Both tadalafil and combination of Tadalafil + Pentoxifylline improve erectile function in patients of ED. Patients with severe ED showed much significant improvement in erectile function with combination therapy.

IntroductionAcute otitis externa (AOE) is a common but preventable ear condition. Tenderness with movement of the tragus or pinna is a classic feature of otitis Externa. Polymyxin B, neomycin, hydrocortisone preparations are the choice for first-line therapy when the tympanic membrane is intact. This study atiempted to compare the efficacy of polymyxin B, neomycin and polymyxin B, neomycin, hydrocortisone in the treatment of otitis Externa.ObjectiveTo compare the efficacy of polymyxin B, neomycin and polymyxin B, neomycin, hydrocortisone in the treatment of otitis Externa.MethodologyTo evaluate the efficacy of polymyxin B, neomycin and polymyxin B, neomycin, hydrocortisone in the treatment of otitis Externa, a hospital based, randomized, prospective study was conducted in Nepal Medical College and Teaching Hospital (NMCTH), Atiarkhel, Kathmandu from August 2012 to May 2014. 70 outpatients suffering from otitis Externa who met the inclusion and exclusion criteria were included. Patients were randomized into group A and group B with lotiery system. Odd number patients were included in group A and even number patients in group B. Group A patients received pack soaked with ribbon gauge in polymyxin B, neomycin ointment and Group B patients received pack soaked with ribbon gauge in polymyxin B, neomycin, hydrocortisone ointment. The patients were called for follow up after 48 hours and 96 hours to assess the improvement on the basis of tragal and circumduction tenderness either present or absent (present 1 or absent 2). A decrease in the clinical signs and symptoms (i.e. tragal and circumduc_on tenderness) was noted. Absence of pain was considered as clinically cured.ResultsIn comparison to polymyxin B, neomycin group, hydrocortisone group exhibited statistically significant effectiveness after 48 hours of treatment (p<0.05), but in cure rates after 96 hours, no statistical significant difference was observed between two groups (p>0.05).ConclusionPolymyxin B, neomycin, hydrocortisone group showed higher and faster cure rates than polymyxin B, neomycin group in the treatment of otitis Externa at 48 hours follow up. Birat Journal of Health SciencesVol.2/No.1/Issue 2/ Jan - April 2017, Page: 162-167
