Objective: To study the morphological features and subpopulation composition of immunocompetent cells of adhesion tissue in women with adhesions of the pelvic organs.Materials and Methods: Th e study was carried out using surgical material obtained from 70 women aged 23 to 40 years. Of these, 50 tissue samples of peritoneal adhesions from patients with adhesions of organs in the small pelvis of I – II degree who underwent adhesiolysis and 20 samples of parietal peritoneum from healthy women who underwent endoscopic sterilization for contraception or completion of generative function. Th e authors used histological, immunohistochemical, and morphometric research methods.Results: Immunological changes in adhesion tissue were characterized by the activation of the T-cell link of immunity. It was confi rmed by a signifi cant increase in the content of CD4+ (p <0.001), CD8+ (p <0.001), a shift in the balance of immunoregulatory subpopulations towards CD8+, a lower indicator of the immunoregulatory index (p = 0.015), and insuffi ciency of the humoral link of immunity, namely, the absence of CD20+ content against the background of a slight increase in the CD138+ pool.Conclusion: To prevent the postoperative adhesion process in the small pelvis in patients of reproductive age, it is necessary to apply immunomodulatory therapy in the early postoperative period, which will improve the results of surgical treatment and is pathogenetically justifi ed.

Introduction. Adhesive process of the pelvic organs is a pressing issue for operative gynecology that does not allow to consider results of therapeutic and preventive measures as sufficient.Aim: to analyze published data regarding contemporary aspects of etiopathogenesis of the adhesive process in pelvic organs as well as methods of its prevention.Materials and Methods. The literature sources retrieved from electronic databases PubMed, Embase, Medline, Ovid HealthSTAR, Cochrane, Google Scholar, eLibrary, CyberLeninka as well as scientific articles published in peer-reviewed open access journals over the last 30 years, including basic research in the field have been analyzed. While searching, the following keywords and their combinations in Russian and English were used: "adhesive process of the pelvic organs", "pathogenesis of the adhesive process", "prevention of the adhesive process", "gynecology", "pelvic adhesions", "pathogenesis of adhesions", "аdhesion prophylaxis", "gynecology".Results. The current aspects of the etiology and pathogenesis for adhesive process have been summarized. Existing adhesion classifications are presented. The proposed methods for preventing formation of peritoneal adhesions are described exerting most prominent effectiveness as well as describing the properties and characteristics according to the application method, the composition of contained substances and drug form. A phenotype profile of peripheral blood and peritoneal fluid lymphocytes from patients with adhesive process remains debated.Conclusion. A need to further examine formation of peritoneal adhesions at molecular and cellular levels for developing a comprehensive pathogenetically substantiated method to prevent and treat adhesions of the pelvic organs is in demand.

The principles of management of all sarcomas that involve the abdominal and pelvic cavity are presented. The anatomic sites for the primary malignancy include retroperitoneal sarcomas, pelvic side-wall sarcomas, sarcomas arising from the abdominal viscera, and sarcomas arising from the pelvic organs. All histologic types of sarcomas may be considered together when therapeutic options are being discussed. This presentation stresses surgical removal with an adequate margin of resection as the principal goal of management. The curative treatment of these cancers places great emphasis on the surgeon's knowledge of anatomy, technical skills, innovation, and surgical courage. Systemic chemotherapy and radiotherapy have not shown reproducible efficacy. Complete resection in the absence of tumor spillage remains the only reliable treatment option. Possible benefits of induction chemotherapy and intraperitoneal chemotherapy using cisplatin and doxorubicin in the early postoperative period are presented.

Abstract Introduction Females of reproductive age with a clinical diagnosis of appendicitis may undergo laparoscopy prior to appendicectomy. This paper advocates laparoscopy after appendicectomy only if the appendix is macroscopically normal in this patient group.  Method A cohort of patients under the care of a single general surgeon is reported. If the appendix was macroscopically normal a laparoscopic port was inserted through the right iliac fossa incision. This single port was used for laparoscopic assessment of the pelvic organs. Results Fourteen females of reproductive age underwent laparoscopy following a macroscopically normal open appendicectomy. No additional laparoscopic ports were required. A benign dermoid ovarian cyst was identified in a 14 year-old which was electively excised following further investigation by a gynaecologist. Two ruptured simple ovarian cysts were also identified. There were no complications reported in this patient group. Conclusions Laparoscopy following a macroscopically normal open appendicectomy allows assessment of the pelvic organs in females of reproductive age and reduces the number of ports required during surgery.

Endometriosis is the commonest gynaecological problem; it affects 7 to 10% women in reproductive age group. Commonest site being the pelvic organs, extrapelvic presentation in almost all parts of the body have been reported. However umbilical endometriosis has been reported in more than one hundred cases. It was first described by Villar in 1886. Umbilical endometriosis accounts for only 0.5% to 1% of all endometriosis cases. Most of the reported cases occurred in the scar following gynaecological and obstetrical surgery. A few cases of primary umbilical endometriosis have been reported. We report such a rare case of primary umbilical endometriosis and the diagnosis was done by FNAC & the patient underwent wide local excision.DOI: http://dx.doi.org/10.3329/cmoshmcj.v13i1.19426

The objective: in this work represented part of materials of department rehabilitation of reproductive function for the examination and treatment of 250 patients with uterine fibrods in order to study and determine in detail the predictors of the disease, develop methods of treatment (conservative and surgical), as well as rehabilitation of the reproductive function, depending on the type and localization of the nodes. Materials and methods. The analysis of the anamnestic data, taking into account both objective and subjective data, analysis of anamnestic data of close relatives (maternal and paternal lines), concomitant genital and extragenital pathology is carried out. Results. The most numerical group of patients with uterine fibroids, according to our data, in the age groups of 26–35 years (32%) is 36–40 years (28.4%), which are the most socially and reproductively active. In this category, women with a high education of 33.6% and social status (30.4% and 22.4%) prevailed. The transferred inflammatory diseases of the genitals were noted in 48% of the examined, weighed gynecological anamnesis (abortions, miscarriages) were noted in 42.4%. Conclusion. Also for this category of patients is characterized by a high percentage of endometrial pathology – 60.8%, as well as concomitant adenomyosis – 34% and adhesion process of pelvic organs – 26.8%. The research aims at studying the effectiveness of plasmapheresis and ozonotherapy in treatment of women of reproductive age in early postoperative period after reconstructive-plastic operations. 110 patients with different factors of infertility who passed surgical laparoscopy were examined. All the patients were rehabilitated applying medicinal plasmapheresis (70 patients) and ozonotherapy (40 patients). The results of the study showed the advantages of these methods of treatment in comparison with traditional therapy (antibacterial and physiotherapy in comparative group, 72 patients). So, early rehabilitation of patients including medicinal plasmapheresis and ozonotherapy makes it possible to increase essentially the course of postoperative period, to stabilize adaptive reactions, to recover reproductive function in considerable amount of infertile women.
