Evaluating a theory-based intervention for improving eHealth literacy in older adults: A single group, pretest–posttest design (Preprint)

BACKGROUND The Internet use rate of older adults, who were considered underprivileged in the digital world, is steadily increasing. Specifically, the Internet is considered an important channel for providing health information to older adults, who are the largest consumers of medical services. However, due to factors such as technical difficulties, lack of learning opportunities, and deterioration of eHealth literacy, the Internet is not usually a common factor of delivering health information to older adults. Therefore, we developed an intervention program to improve eHealth literacy in older adults according to the information-motivation-behavioral skills (IMB) theory and Intervention Mapping. OBJECTIVE This study aimed to analyze the effect of developed intervention on information, motivation, behavioral skills, and behaviors related to eHealth information in older adults. METHODS This study applied a single group pretest–posttest design. The participants were 46 older adults over the age of 65 recruited from two senior welfare centers in a city in South Korea. We divided the participants into 4 groups and conducted 4 interventions in the computer room of the Senior Welfare Center from March to December 2019. One intervention was performed once a week (2 hours/1 time) for 5 weeks, and the total lecture time was 10 hours. The instructors were all researchers involved in the development of this intervention, and the instructors for each intervention consisted of one lecture instructor and two assistant instructors who supported the participants in the computer practices. RESULTS Participants’computer/web knowledge (χ²= 60.04, P <.001), perceived ease of use (χ²= 17.86, P <.001), perceived enjoyment (χ²= 23.06, P <.001), and attitude toward eHealth information (χ²= 18.47, P <.001) showed statistically significant increases. The eHealth information literacy efficacy score (χ²= 72.76, P< .001), searching performance score (χ²= 162.57, P < .001), and understanding score (χ²= 60.96, P< .001) were also significantly different from those before and after the intervention. However, there was no significant difference in perceived usefulness (χ²= 5.91, P = .052). CONCLUSIONS Our findings suggest that the application of the current theory based methodology can improve the quality of research in developing eHealth information literacy interventions. Additionally, it is necessary to develop and continuously apply various interventions to improve eHealth information literacy among older adults. Finally, measures to improve the Internet environment for older adults should be considered.

Abstract Nearly 60% of older adults use the internet for health-related reasons. Some studies have demonstrated differences in the frequency at which men and women perform various online activities. However, few studies have investigated gender differences in health-related search behaviors (HRSB). The purpose of this study was to examine differences in self-reported HRSB between older men and women. A total of 47 older adults (M age = 66.6, 55% female, 87% White) completed a survey assessing perceived usefulness and trust in the internet for health-care information, types of websites used, and reasons for looking up health information. Independent samples t-tests revealed that, compared to women, men regard the internet as more useful in helping them make health care decisions (t (45) = 2.715) and as a more trustworthy source (t (45) = 2.24, p's &lt; 0.05). Men were more likely to get health information through sources affiliated with educational institutions (χ2(1) = 3.9) and government agencies (χ2(1) = 8.8), whereas women were more likely to use social media, (χ2(1) = 4.3, p's &lt; 0.05). Lastly, men were more likely to use the internet to learn about information on medical procedures (χ2(1) = 5.1), while women were more likely to learn about alternative treatments (χ2(1) = 4.9, p 's &lt; 0.05) online. As 72.3% of participants indicated the internet as their first source for health information, interventions geared towards enhancing HRSB for older adults are needed, especially for older women whose HRSB may make them particularly vulnerable to misinformation.

Possessing eHealth literacy skills among students has been increasingly appreciated in nursing education. Despite eHealth literacy has been widely studied, little published research is available regarding Filipino nursing students’ eHealth literacy. This study aimed to determine the self-reported eHealth literacy of Filipino nursing students and the relative importance of the criteria they use when selecting health websites. A cross-sectional research design was used involving 66 conveniently chosen nursing students. Self-reported data were collected using adopted questionnaires. Data were analyzed using descriptive statistics, the Mann-Whitney U test, and Spearman’s rho. Findings revealed that nursing students had high eHealth literacy. The utility of information and accuracy of information were the top criteria students consider important when selecting health websites. There were no significant differences in the eHealth literacy of male and female and first-year and second-year nursing students. The extent of Internet use for health information, perceived usefulness of the Internet when making health decisions, and perceived importance to access health resources on the Internet were significantly correlated with eHealth literacy. To derive full benefit from online health information, nursing students’ literacy needs in the Web context and digital environment must be given attention.

This qualitative paper explores how health information mediated by the internet and media is used and made valuable within the life of consumers managing non-crisis health challenges, and how informal information seeking and gathering influences self-positioning within patient-clinician relationships. Findings have implications for health information literacy and collaborative, patient-centred care.Cette étude qualitative explore comment l’information sur la santé relayée par Internet et les médias est utilisée et rendue utile dans le contexte de consommateurs gérant des problèmes médicaux non urgents, et comment la recherche et la collecte d’information informelles influencent l’auto-positionnement dans la relation patient clinicien. Les résultats ont des applications dans la maîtrise de l’information médicale et les soins collaboratifs centrés sur le patient.

Background As a result of demographic changes, the number of people aged 60 years and older has been increasing steadily. Therefore, older adults have become more important as a target group for health communication efforts. Various studies show that online health information sources have gained importance among younger adults, but we know little about the health-related internet use of senior citizens in general and in particular about the variables explaining their online health-related information–seeking behavior. Media use studies indicate that in addition to sociodemographic variables, lifestyle factors might play a role in this context. Objective The aim of this study was to examine older people’s health-related internet use. Our study focused on the explanatory potential of lifestyle types over and above sociodemographic variables to predict older adults’ internet use for health information. Methods A telephone survey was conducted with a random sample of German adults aged 60 years and older (n=701) that was quota-allocated by gender, age, educational status, and degree of urbanity of their place of residence. Results The results revealed that participants used the internet infrequently (mean 1.82 [SD 1.07]), and medical personnel (mean 2.89 [SD 1.11]), family and friends (mean 2.86 [SD 1.21]), and health brochures (mean 2.85 [SD 1.21]) were their main sources of health information. A hierarchical cluster analysis based on values, interests, and leisure time activities revealed three different lifestyle types for adults aged over 60 years: the Sociable Adventurer, the Average Family Person, and the Uninterested Inactive. After adding these types as second-step predictors in a hierarchical regression model with sociodemographic variables (step 1), the explained variance increased significantly (R2=.02, P=.001), indicating that the Average Family Person and the Sociable Adventurer use the internet more often for health information than the Uninterested Inactive, over and above their sociodemographic attributes. Conclusions Our findings indicate that the internet still plays only a minor role in the health information–seeking behavior of older German adults. Nevertheless, there are subgroups including younger, more active, down-to-earth and family-oriented males that may be reached with online health information. Our findings suggest that lifestyle types should be taken into account when predicting health-related internet use behavior.

Using the "think aloud" protocol, which allows for the collection of data in real time, the researcher audio taped comments from 13 white college students from a predominately white university in the Southeastern United States and 15 black students from a predominately black university, as they explained how they searched for HIV/AIDS information on the Internet. A grounded theory analysis of the tapes revealed a three-stage model that students progressed through as they searched for HIV/AIDS information on the Internet. That model also revealed that all of the white students searched for general information about HIV/AIDS on the Internet, while all black students searched for general and specific information about how the disease affected the African-American community. Eighty percent of students regardless of race did not know how to properly search for online health information. The researcher discusses the need for online health information literacy training, the theories that might explain why black students searched the way that they did, and the challenges to providing culturally-sensitive online health information literacy training for African-Americans who have been historically suspicious of the United State's health care system.

BACKGROUND The Internet can increase the accessibility of mental health information and improve the mental health literacy of older adults. The quality of mental health information on the Internet can be inaccurate or biased, leading to misinformation OBJECTIVE This study’s objectives were to evaluate the quality, usability, and readability of websites providing information concerning depression in later life. METHODS Websites were identified through a Google search, and evaluated by assessing quality (DISCERN), usability (Patient Education Materials Assessment Tool; PEMAT) and readability (Simple Measure of Gobbledygook; SMOG). RESULTS The overall quality of late-life depression websites (N = 19) was moderate, usability was low, and readability was poor. No significant relationship was found between quality and readability of websites. CONCLUSIONS Websites can be improved by enhancing information quality, usability, and readability related to late-life depression. The use of high-quality websites may improve mental health literacy and shared treatment decision-making for older adults.

Objective. This study aimed to assess web-based health information seeking and eHealth literacy among Iranian college students. Methods. The study was conducted in five colleges of the Shiraz University of Medical Sciences in Iran during 2018. The data were collected by a researcher-made questionnaire consisting of seven questions on a 4-point Likert-type scale, with scores ranging from 7 to 28. These questions were: ′I know how to use the Internet to answer my questions about health′, ′I think there is enough information about health-related issues on the Internet′, ′I know the vocabulary used in health issues on the Internet′, ′I can tell high-quality health resources from low-quality health resources on the Internet′, ′I know how to use the health information I find on the Internet to help me′, ′I feel confident in using information from the Internet to make health decisions′, and ′Searching for health-related information on the Internet will increase my knowledge in this field′. High eHealth literacy level is defined as above the total mean score and low eHealth literacy level is defined as lower than the total mean score. Results. In all, 386 college students participated in the study. The results showed that the mean score of eHealth literacy was 19.11 out of 28; 205 participants (54.4%) had low eHealth literacy. In addition, the students used the Internet to search for information regarding diseases symptoms (70%), physical illnesses (67.1%), existing treatments (65%), and diagnosis (63.1%). Conclusion. The results showed that participants in this study usually searched for illnesses, symptoms, and treatments after they got sick and paid little attention to other aspects related to integral health.How to cite this article: KHademian F, Roozrokh M, Aslani A. With the Internet continuously being used as a means of providing health education and promotion to the public, consumers are increasingly going online to gather pertinent health information. However, disparities exist with regards to consumers’ ability in finding, evaluating, and applying online health information (collectively referred to as eHealth literacy). Identifying these disparities may elucidate which segments of the population would benefit from targeted eHealth literacy interventions and ways to adapt online health promotion materials. This study uses data from the 2020 CALSPEAKS survey to identify disparities in eHealth literacy among older adults aged 65+ residing in California, USA ( N = 237). eHealth literacy is self-assessed using the previously validated 8-item eHEALS questionnaire. Ordinary least squares regression analyses are performed on individual eHEALS items and on a summed eHealth literacy score, with demographic and technology use-related characteristics as predictors. Results show that the strongest and most consistent predictors of eHealth literacy include education, frequency of Internet use, and breadth of Internet activities regularly performed. Findings suggest that those seeking to increase eHealth literacy specifically among older Californians may benefit from tailoring their interventions and online health promotion materials towards those with less education and those with less Internet experience.
