AbstrakSchistosomiasis merupakan suatu penyakit tropik yang disebabkan oleh cacing genus Schistosoma. Spesies yang dapat menginfeksi manusia antara lain Schistosoma mansoni, Schistosoma japonicum, Schistosoma mekongi, Schistosoma haematobium dan Schistosoma intercalatum. Penyakit ini telah menyerang 200 juta orang penduduk di negara berkembang. Penularan pada manusia terjadi dengan cara serkaria menembus kulit sewaktu kontak dengan air yang mengandung serkaria.Respon imun pada penderita schistosomiasis terhadap antigen cacing dan telurnya mempengaruhi perjalanan penyakit dan klinis yang ditimbulkan. Status imunitas menentukan perubahan patologi yang akan terjadi seperti pembentukan granuloma, gangguan terhadap organ atau bahkan melindungi penderita terhadap kejadian infeksi berat. Pada keadaan tertentu cacing schistosoma dapat bertahan selama bertahun – tahun meskipun hospes mempunyai respon imun yang kuat.Gejala schistosomiasis akut dapat berupa demam, malaise, mialgia, batuk, sakit kepala dan nyeri abdomen yang dikenal dengan sindroma Katayama. Gejala akut ini sering muncul pada orang yang mengalami infeksi pertama kali. Pada keadaan kronik, schistosomiasis dapat menimbulkan kerusakan organ berupa fibrosis, striktur dan kalsifikasi.Kata Kunci : schistosimiasis, sindroma Katayama, fibrosis, granulomaAbstractSchistosomiasis is a tropical disease which is caused by helminth of genus schistosoma.Species of schistosoma which can infect human are Schistosoma mansoni, Schistosoma japonicum, Schistosoma mekongi, Schistosoma haematobium and Schistosoma intercalatum. Schistosoma has infected 200 million people in developing countries. It is transmitted to human when the free living cercariae penetrate the skin in contaminated water.Immune response to somatic and egg antigen determine natural history of disease and clinical symptom. Immunity is responsible for pathological changes which formed granuloma, organ disfunction and even able to protect the body from heavy infection. In certain case, schistosomiasis can persist for years in host with strong immunity.TINJAUAN PUSTAKA82Symptoms of acute schistosomiasis also called Katayama syndrome are fever, malaise, myalgia, cough, headache and abdominal pain. The acute symptome frequently occur in first schistosomal infection. Work fatigue is part of a common problem that is often encountered in the workforce. Work fatigue is a symptom related to work efficiency, skills, boredom, and increased anxiety. From the initial survey of interviews and observations conducted, information was obtained that nurses who worked at Permata Hati Duri Hospital often complained of work fatigue The research method used was quantitative with cross sectional design. This research was conducted at Permata Hati Duri Hospital in June-July 2019. The research sample was 66 respondents. Data collection techniques using a questionnaire. The results showed there was no gender relationship (p value = 0.523), there was no relationship between nutritional status (p value 0.523), there was no history of disease relationship (p value 0.940), and there was an age relationship (p value = 0.000), status marriage (p value 0,000), years of service (p value 0.048), work shifts (p value 0,048) with nurse work fatigue. It is recommended that nurses should do sufficient circumstance after work to maintain the physical and mental condition of the body, to reduce fatigue nurses can do refreshing during holidays. For the hospital, management is expected to be able to take part in training related to efforts to prevent work fatigue by minimizing factors that can influence the occurrence of work fatigue.

This review examined the homeostasis of uric acid in human body and analyzed recent studies of the affecting major variables. Normal uric acid concentration in male is 3.5-7.2 mg/dL and in female is 2.6-6 mg/dL. Daily turnover of normal uric acid ranges from 498-1392 mg/day, miscible pool is 767-1650 mg, reabsorption is 8064 mg/day, renal excretion is 262-620 mg/day and intestine 186-313 mg/day. The dynamics of uric acid is influenced by factors of food, drink, age, history of disease, and genetic. High purine dietary consumption increases blood uric acid by 1-2 mg/dL, 213-290 g/day fructose drinks increases 0.52-1.7 mg/dL, 1.5 g/kgBW sucrose increases 0.61 mg/dL, and 10-20 ml/kgBW beer increases 0.50-0.92 mg/dL. The ABCG2 gene plays a role in bringing uric acid out of the body by 114.31-162.73 mg/dL, SLC2A9 of 5.43-20.17 mg/dL, and SLC22A12 of 5.77-6.71 mg/dL. The data described the homeostasis of uric acid and the magnitude of the impact of environmental (consumption of food, beverages, and lifestyle) and genetic factors. Understanding uric acid homeostasis and its disturbances is important in managing diseases as a consequence of hyperuricemia and hypouryscemia

Acute appendicitis is the most common acute surgical infection during pregnancy. Although usually pyogenic in origin, parasitic infections account for a small percentage of cases. Despite the relatively high prevalence of acute appendicitis in our environment, it is not commonly associated with schistosomiasis. We report here the association of pregnancy and appendicitis caused by Schistosoma haematobium. Schistosomiasis is very common complication of pregnancy in hyperendemic areas. Schistosome egg masses can lodge throughout the body and cause acute inflammation of the appendix, liver and spleen. Congestion of pelvic vessels during pregnancy facilitates passage of eggs into the villi and intervillous spaces, causing an inflammatory reaction. Tourism and immigration make this disease a potential challenge for practitioners everywhere.

Background and Importance: Spondylodiscitis is an inflammatory disease of the body of one or more vertebrae and intervertebral disc. The fungal etiology of this disease is rare, particularly in patients without immunodeficiency. Delay in diagnosis and treatment of this disease can lead to complications and even death. Case Presentation: A 63-year-old diabetic female patient, who had a history of spinal surgery and complaining radicular lumbar pain in both lower limbs with a probable diagnosis of spondylodiscitis, underwent partial L2 and complete L3 and L4 corpectomy and fusion. As a result of pathology from tissue biopsy specimen, Aspergillus fungi were observed. There was no evidence of immunodeficiency in the patient. The patient was treated with Itraconazole 100 mg twice a day for two months. Pain, neurological symptom, and laboratory tests improved. Conclusion: The debridement surgery coupled with antifungal drugs can lead to the best therapeutic results.
