Decompression sickness (DCS) remains a major operational concern for diving operations, submarine escape and high-altitude jumps. Aside from DCS symptoms, venous gas emboli (VGE) detected with ultrasound post-dive are often used as a marker of decompression stress in humans, with a specificity of 100% even though the sensitivity is poor [1]. Being non-invasive, portable and non-ionizing, ultrasound is particularly suited to regular and repeated monitoring. It could help elucidate inter- and intra-subject variability in VGE and DCS susceptibility, but analyzing these recordings remains a cumbersome task [2].

Diving as a method of fishing is used worldwide in small-scale fisheries. However, one of the main causes of morbidity and mortality among fishermen is decompression sickness (DCS). We report the case of a 46-year-old male fisherman diver who presented with chronic inguinal pain that radiated to the lower left limb. Living and working in a fishing port in Yucatan, he had a prior history of DCS. A diagnosis of avascular necrosis in the left femoral head secondary to DCS was made via analysis of clinical and radiological findings. The necrosis was surgically resolved by a total hip arthroplasty. Dysbaric osteonecrosis is a more probable diagnosis. In this region fishermen undergo significant decompression stress in their daily fishing efforts. Further studies regarding prevalence of dysbaric osteonecrosis among small-scale fisheries divers are needed. In a community where DCS is endemic and has become an epidemic, as of late, the perception of this health risk remains low. Furthermore, training and decompression technique are lacking among the fishing communities.

AbstractTheta-burst stimulation (TBS) is a form of non-invasive neuromodulation which is delivered in an intermittent (iTBS) or continuous (cTBS) manner. Although 600 pulses is the most common dose, the goal of these experiments was to evaluate the effect of higher per-dose pulse numbers on cortical excitability. Sixty individuals were recruited for 2 experiments. In Experiment 1, participants received 600, 1200, 1800, or sham (600) iTBS (4 visits, counterbalanced, left motor cortex, 80% active threshold). In Experiment 2, participants received 600, 1200, 1800, 3600, or sham (600) cTBS (5 visits, counterbalanced). Motor evoked potentials (MEP) were measured in 10-min increments for 60 min. For iTBS, there was a significant interaction between dose and time (F = 3.8296, p = 0.01), driven by iTBS (1200) which decreased excitability for up to 50 min (t = 3.1267, p = 0.001). For cTBS, there was no overall interaction between dose and time (F = 1.1513, p = 0.33). Relative to sham, cTBS (3600) increased excitability for up to 60 min (t = 2.0880, p = 0.04). There were no other significant effects of dose relative to sham in either experiment. Secondary analyses revealed high within and between subject variability. These results suggest that iTBS (1200) and cTBS (3600) are, respectively, the most effective doses for decreasing and increasing cortical excitability.

The number of data sources available for academic research on retirement economics and policy has increased rapidly in the past two decades. Data quality and comparability across studies have also improved considerably, with survey questionnaires progressively converging towards common ways of eliciting the same measurable concepts. Probability-based Internet panels have become a more accepted and recognized tool to obtain research data, allowing for fast, flexible, and cost-effective data collection compared to more traditional modes such as in-person and phone interviews. In an era of big data, academic research has also increasingly been able to access administrative records (e.g., Kostøl and Mogstad, 2014; Cesarini et al., 2016), private-sector financial records (e.g., Gelman et al., 2014), and administrative data married with surveys (Ameriks et al., 2020), to answer questions that could not be successfully tackled otherwise.

The ability to manoeuvre at high altitude has become one of the main requirements in the design of modern military aircraft. Ability to manoeuvre enables the fighter pilot to execute tight turns during the attack on a bomber and the bomber pilot to perform evasive manoeuvres and so make the task of the fighter more difficult.Normally the main manoeuvre considered is the application of increased normal acceleration, or g, and if we assume that (a) there is sufficient pitching power available from the longitudinal control surface to increase the wing incidence to give the necessary increase in normal force, and (b) that the new wing incidence is below the stalling incidence of the wing, then the degree of manoeuvrability available is a function of thrust and drag alone. In the past the manoeuvrability required at a given altitude has been expressed in terms of the rate of climb available at that altitude and thus we have such definitions as “ the 1,000 ft./min. ceiling ” and so on.

The United States Army Research Institute of Environmental Medicine (USARIEM) celebrated its 50th anniversary on July 1, 2011. This article reviews its history, evolution, and transition of its research programs as well as its scientific and military accomplishments, emphasizing the past 25 yr. During the 1990s, USARIEM published a series of pocket guides providing guidance for sustaining Warfighter health and performance in Southwest Asia, Somalia, the former Republic of Yugoslavia, Rwanda, and Haiti. Issues identified during Operation Desert Storm elicited research that improved nutritional guidelines for protracted desert operations; safer use of nuclear, chemical, and biological protective clothing; equipment, development, and fielding of efficient microclimate cooling systems; and effective evaluation of pharmaceuticals to protect soldiers from chemical and biological threats. During the first decade of the 21st century, USARIEM and the Department of the Army published official medical/performance doctrines for operations in the heat and cold and at high altitude. The current Global War on Terrorism focused research to improve doctrines for hot, cold, and high-altitude operations, reduce musculoskeletal training injuries, provide improved field nutrition, more efficient planning for operational water requirements, and improve both military clothing and materiel. This article also describes the critically important interactions and communications between USARIEM and deployed units and the benefits to Warfighters from this association. This report presents USARIEM's unique and world-class facilities, organizational changes, scientific and support personnel, and major research accomplishments, including the publication of 2,200 scientific papers over the past 25 yr.

Recreational sport diving is becoming an increasingly popular sport for women. Women now comprise approximately 25% of the diving community according to Divers Alert Network statistics. In the diving literature it has been stated that women are at a greater risk for decompression sickness than men. Most of these statements were derived from high-altitude (hypobaric environment) studies rather than from a scuba diving (hyperbaric) environment. Data from the naval diving and salvage training center were analyzed, and it was found that women are not more susceptible to decompression sickness than men during dives between 4 and 10 atmospheres. More specific studies on sport diving should be completed on factors contributing to underwater decompression sickness in both men and women.

Over the past decade, research interest has risen on the direct effects of temperature on exercise capacity and tolerance, particular in the heat. Two major paradigms have been proposed for how hyperthermia may contribute to voluntary fatigue during exercise in the heat. One suggests that voluntary exhaustion occurs upon the approach or attainment of a critical internal temperature through impairment in a variety of physiological systems. An alternate perspective proposes that thermal inputs modulate the regulation of self-paced workload to minimize heat storage. This review seeks to summarize recent research leading to the development of these two models for hyperthermia and fatigue and explore possible bridges between them. Key areas for future research and development into voluntary exhaustion in the heat include (i) the development of valid and non-invasive means to measure brain temperature, (ii) understanding variability in perception and physiological responses to heat stress across individuals, (iii) extrapolating laboratory studies to field settings, (iv) understanding the failure in behavioural and physiological thermoregulation that leads to exertional heat illness, and (v) the integration of physiological and psychological parameters limiting voluntary exercise in the heat.

Purpose According to the perspective of evolutionary economic theory, the marketplace continuously evolves over time, following the changing needs of both customers and firms. In accordance with the theory, the second-hand apparel market has been rapidly expanding by meeting consumers’ diverse preferences and promoting sustainability since 2014. To understand what changes in consumers’ consumption behaviors regarding used apparel have driven this growth, the purpose of this study is to examine how the second-hand apparel market product types, distribution channels and consumers’ motives have changed over the past five years. Design/methodology/approach This study collected big data from Google through Textom software by extracting all Web-exposed text in 2014, and again in 2019, that contained the keyword “second-hand apparel,” and used the Node XL program to visualize the network patterns of these words through the semantic network analysis. Findings The results indicate that the second-hand apparel market has evolved with various changes over the past five years in terms of consumer motives, product types and distribution channels. Originality/value This study provides a comprehensive understanding of the changing demands of consumers toward used apparel over the past five years, providing insights for retailers as well as future research in this subject area.
