A 54-year old male developed unexpectedy a severe penicillin allergy after 5 standard erysipelas treatments by intravenous benzylpenicillin and/or oral phenoxymethylpenicillin without any clinical symptoms of allergy during 16 years. During the 6th erysipelas treatment by intravenous benzylpenicillin, the patient experienced an anaphylactic reaction within 30 minutes.

Background: Dental cellulitis management is no longer a simple procedure, as more and more patients are needing long-time hospitalization, several surgeries and intensive care follow-up. This prospective study seeks to highlight criteria that can split patients with severe odontogenic infection into two groups: those with simple evolution and those for whom complex management is necessary. Methods: In this observational study, all patients considered with a severe odontogenic infection (which necessitated hospital admission, intravenous antibiotics and general anaesthesia) were enrolled between January 2004 and December 2014 from Clermont-Ferrand University Hospital (France). They were split into two groups: those who needed one surgical intervention with tooth extraction and collection drainage combined with probabilistic antibiotic to treat infection and those who need several surgeries, intensive care unit follow-up or tracheotomy to achieve healing. Results: 653 patients were included, of which 611 (94%) had one surgery, 42 (6%) had more than one surgery before healing. Penicillin allergy (p < 0.001), psychiatric disorders (p = 0.005), oropharyngeal oedema (p = 0.008), floor oedema (p = 0.004), fever (p = 0.04) and trismus (p = 0.018) on admission were the most relevant predictors of complex evolution. A conditional inference tree (CTREE) illustrated the association of prognostic factors and the need of multiple surgery. Conclusions: Besides clinical symptoms of severity, complications of severe odontogenic infection are predicted by measurables and objectives criteria as penicillin allergy, mandibular molar, C-reactive protein level, psychiatric disorders and alcohol abuse. Their specific association potentialize the risks. IRB number: CE-CIC-GREN-12-08.

Background Anecdotal cases of exophthalmos after acute mechanical thrombectomy have been described. We sought to estimate the incidence in a large cohort of patients with acute anterior circulation stroke treated with mechanical thrombectomy. Secondarily, we aimed to evaluate the underlying mechanism and to differentiate it on imaging from other pathology with similar clinical orbital features. Methods Between November 2016 and November 2018, we performed a retrospective single-center study of 250 patients who underwent anterior circulation mechanical thrombectomy. Development of exophthalmos was independently evaluated by two readers on preprocedure and 24-h postprocedure non-contrast cerebral CT. Results In the mechanical thrombectomy cohort, six individuals (2.4%) developed interval ipsilateral exophthalmos at 24 h. Of these, at least two patients developed clinical symptoms. There was almost perfect agreement between assessments of the two readers (Cohen’s kappa = 0.907 (95% confidence interval: 0.726, 1.000)). In two patients, there was delayed ophthalmic artery filling on digital subtraction angiography. None of the patients had features of a direct carotid-cavernous fistula. Conclusions Exophthalmos is not uncommon after mechanical thrombectomy (2.4%). The underlying mechanism is difficult to confirm, but it is most likely due to orbital ischemia from hypoperfusion or distal emboli.

Teicoplanin is now increasingly used as a first-line prophylactic therapy for major surgical procedures, treatment of methicillin-resistant Staphylococcus aureus infections and for those with reported penicillin allergy. Teicoplanin is rarely associated with anaphylaxis and there is limited information on the prevalence of teicoplanin-induced perioperative anaphylaxis. Here, we describe a case of a 12-year-old child with teicoplanin-induced anaphylaxis peri-operatively.

The review of literature details the issues of genetics, the specific features of inheritance, the clinical picture and treatment of pseudohypoparathyroidisms (PHPT). In practice, clinicians more frequently deal with type 1 PHPT and the diagnosis of this type creates no significant problems. However, despite the low prevalence of the other types of PHPT - 1b, 1c, and 2, the diseases may run with noticeably clinical symptoms and present a significant problem in the context of diagnosis and treatment. This review may be of concern to both clinicians and geneticists who are interested in this problem.

Abstract Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) may affect the central nervous system (CNS) and peripheral nervous system (PNS). Major CNS manifestations of SARS-CoV-2 include seizures, meningitis, meningoencephalitis, ischemic stroke, hemorrhagic stroke, anosmia, hypogeusia, acute disseminated encephalomyelitis, hemorrhagic necrotizing encephalopathy, and nonfocal phenomena including lethargy, agitation, confusion, headache, and ataxia. The reversible splenial lesion syndrome (MERS) was first described in 2004. Although MERS was initially recognized as a benign phenomenon, a second type of MERS was identified in later years, which has a poor prognosis and potentially serious sequela. MERS can be caused by numerous etiologies including viruses. In this report, we present a patient with SARS-CoV-2 who presented with ataxia and dizziness as the clinical symptoms of MERS, which is a rare clinical phenomenon and can be caused by numerous etiologies.

Acute fatty liver of pregnancy (AFLP) is an uncommon life-threatening disorder of pregnancy seen commonly in third trimester. When not diagnosed at right time it can lead to hepatic failure, encephalopathy, coagulopathy, maternal and fetal mortality. The clinical symptoms and signs are nonspecific, and it needs to be identified early in order to prevent poor outcome. The gold standard for diagnosis of AFLP is liver biopsy, which is difficult in an acute setting and in abnormal coagulation profile hence the diagnosis is usually based on clinical criteria   Early termination of pregnancy and good intensive care support are the mainstay of management. The authors here presented a case report where even early delivery and good critical care failed to prevent maternal mortality. Review of literature regarding etiopathogenesis, management and recurrences of AFLP are also discussed.

Fat embolism syndrome (FES) is a constellation of clinical symptoms characterized by a triad of respiratory insufficiency, altered sensorium and petechiae occurring after orthopaedic trauma or following surgical manipulation of long bones and spine. The diagnosis is based on clinical presentation and excluding other possible conditions. Beside histopathology, none of the investigations such as fat macroglobinuria in urine or lipid laden macrophages in bronchoalveolar lavage (BAL) is 100% specific. At times FES can present with atypical presentation such as intra alveolar haemorrhage and only high index of suspicion can help in making diagnosis.

Tracheo-oesophageal fistula (TEF) with oesophageal atresia is a common life threatening congenital malformation. H type TEF is a rare subtype (1.8 to 4.2% of all TEF) with the least association with congenital anomalies. We report two cases of H type TEF, first patient was a 2 month old girl who presented with the classical triad of symptoms, and the second patient was a 2 year old girl who presented with recurrent attacks of pneumonia, choking on feeding and noisy breathing. After radiological confirmation, optimization of pulmonary status and nutritional improvement, both underwent right cervicotomy with disconnection of the H fistula. Both patients are doing well on regular follow up. Detection of H fistula is difficult as compared to TEF since the clinical symptoms are variable, radiological detection difficult and diagnosis is based on a high degree of clinical suspicion. We present these two cases to document the history, presentation, clinical management and surgical procedure done in these patients.

Although the general ultrastructure of Cyanidium caldarium, an acidophilic, thermophilic alga of questionable taxonomic rank, has been extensively studied (see review of literature in reference 1), some peculiar ultrastructural features of the chloroplast of this alga have not been noted by other investigators.Cells were collected and prepared for thin sections at the Yellowstone National Park and were also grown in laboratory cultures (45-52°C; pH 2-5). Fixation (glutaraldehyde-osmium), dehydration (ethanol), and embedding (Epon 812) were accomplished by standard methods. Replicas of frozenfracture d- etched cells were obtained in a Balzers apparatus. In addition, cells were examined after disruption in a French Press.
