ABSTRACT Although headgear is rarely used in adult patients, its use in adults is mainly for anchorage control. In the current case report, a 24-year-old patient had a skeletal Class I relationship with a Class II tendency, brachyfacial pattern, significant facial asymmetry, and dental 3/4 cusp Class II molar and canine relationships on both sides. The patient declined surgery, and facial asymmetry was not his concern. The final treatment goal was to achieve a stable Class I dental relationship and normal occlusion without significantly compromising the patient's profile. The patient was compliant with the use of cervical-pull headgear after he refused the options of orthodontic-orthognathic combined treatment, maxillary premolar extraction, or temporary skeletal anchorage mini-implants. A 5-mm maxillary arch distal movement was accomplished without significant distal tipping of the molar crowns. The active treatment duration was 31 months. Proper overbite and overjet, balanced occlusion, and an acceptable facial profile were achieved. The treatment results inspire reconsideration of the possibility of using headgear in dental Class II correction in adult patients.

Association of tongue posture with dental and facial skeletal pattern have been suggested in past. This study was undertaken to assess tongue posture and dimensions in Class I and Class II Dentoskeletal patterns to determine whether any correlation exists between tongue posture and skeletal pattern of an individual. Cephalograms of 150 individuals (aged 18-23 years), taken in Natural Head Position (NHP) and tongue at rest were divided into three groups i.e. Group 1 – Class I Normal occlusion, Group 2 – Class II Division 1 Normodivergent and Group 3 – Class II Division 1 Hypodivergent, consisting of 50 samples each. To ensure the rest position of tongue, patient was asked to relax for 30 seconds after coating the tongue with barium sulphate in midline and then to swallow, and the X-ray was taken at the end of swallowing. Each group was divided into two subgroups according to sex. Groups were constituted according to the Frankfort mandibular plane angle (FMA) angle. The subjects who had skeletal Class II pattern due to mandibular retrusion and not due to maxillary prognathism were only included in the study group. Statistical analysis was done using the software SPSS version 21.0. The statistical tests used were unpaired t-test and One-way ANOVA test with post-hoc bonferroni test. The p-value was considered significant if less than 0.05.The dorsum of the tongue was higher at back and lower in front in Class II Division 1 Hypodivergent group as compared to Class I Normal occlusion group (P<.05). Tongue height and tongue length were significantly reduced in Class II Division 1 Normodivergent and Class II Division 1 Hypodivergent malocclusion groups when compared to Class I control group (P<.05).The study supports the existence of a relationship between posture & dimensions of the tongue with Class I and Class II skeletal patterns.

24 year male patient presented with skeletal class II base with prognathic maxilla and orthognathic mandible. Angles class II division 1 subdivision malocclusion with proclined upper and lower anteriors, increase overjet, increased overbite, spacing with upper and lower anteriors, scissor bite with 35, class I molar and canine relation on right side, end on molar and canine relation on left side. Distalization was planned in maxillary arch to correct end on molar relation on left side and upper incisor proclination. Unilateral Pendulum appliance was used to distalize upper left molar. Post treatment Class I molar relationship was achieved bilaterally within 2-4 months with incisor proclination reduced. The total treatment ended in 18 months.

Introduction: Autism spectrum disorder (ASD) is a group of neurodevelopmental disorders diagnosed by impairments in social interaction and communication with repetitive and restrictive stereotyped behavioral patterns. The Prevalence of autism has been reported to be increased in recent years. This study aimed to assess the prevalence of different types of malocclusion among ASD patients in Isfahan in 2018. Materials & Methods: In a descriptive and cross-sectional trial, 92 ASD patients were studied in the age range of 7-18 years at the center for autism patients in Isfahan. Clinical oral examinations of patients are taken to assess the involved malocclusions (Cl I, Cl II and Cl III malocclusions) and malocclusion traits (deep bite, open bite and cross bite) by an educated dental student under the supervision of an orthodontist under natural light. The data are reported using frequency and percentage indices. Results: Class I malocclusion had the highest prevalence 54.3% (50) among ASD patients and the prevalence of class II and class III were found to be 19.6% (18) and 7.6% (7) respectively. The frequency of malocclusions traits of deep bite, cross bite and the open bite were 27.2% (25), 18.5% (17) and 7.6% (7) respectively. Among of the total patients, 65.2% (60) showed normal bite and 18/5% (17) showed Normal occlusion. Conclusion: ASD patients showed class I, class II and class III malocclusions from the most to least frequency and the most frequent malocclusion traits were also deep bite, cross bite and open bite respectively.

Introduction: Various forms of malocclusion are a matter of serious concern in Nepalese population. This study was carried out to understand the prevalence of malocclusion among the school children of Biratnagar. The objective of this research is to find out the prevalence of malocclusion of children from different schools of Biratnagar visiting the Pedodontics and Orthodontics department of Nobel Medical College and Teaching Hospital, Biratnagar. Materials and Method: A descriptive cross sectional study method was used in this research. Data was collected by using direct observation of the subjects and occlusal assessment was done according to Angle’s classification and Dewey’s modification types of class I, class II and class III malocclusion. Result: Subjects with normal occlusion was found to be 39 % and with malocclusion was found to be 61%. Among them, class I malocclusion (60%) and angles class II div I subjects (88.33%) were in majority of the total study population. Conclusion: The present study helps to determine the prevalence of malocclusion and need of orthodontic treatment for the school children of Biratnagar

The branch of orthodontics has had an interest in the cervical vertebrae wherein cervical spine is used as a reference structure for natural head position, so skeletal age was evaluated by studying variations in the cervical vertebral morphologies. Among all evaluations, very limited data is available wherein comparison between cervical vertebral body volumes between the different malocclusions has been done. This study aimed to compare the differences in the volumes of cervical vertebral bodies of C2, C3, and C4 between skeletal class I and class II malocclusions of both horizontal and vertical growth patterns. In class I the volume was significantly lesser as compared to class II. It was seen that there was statistically no significant difference in the volume between the horizontal and vertical growers. It can be concluded from this study that cervical vertebral body volume has no effect on growth pattern. However, variations in cervical vertebral body volume are seen with different malocclusions.

AIM: The objective of this study was evaluating the prevalence of mandibular asymmetry in skeletal Class I adult patients. MATERIALS AND METHODS: The sample was composed by cone-beam computed tomography images of 250 skeletal Class I patients with age from 18 to 70 years old. The side deviation of mandibular asymmetry was evaluated (right and left), as well as the intensity of this asymmetry. People with gnathic deviation until 2mm were considered as patients with slight asymmetry; deviation between 2 and 5mm was considered moderate asymmetry and those higher than 5mm as severe asymmetries. The error calculation method was performed and there was not significant error in the measurements. To verify the association between the prevalence of mandibular asymmetry and the gender of individuals, the Χ2 was carried out and the significant level adopted was 5% (p<0.05). RESULTS: The results showed that mandibular deviation occurred more frequently to the left side in both genders (male 56.5% e female 58.2%), without statistically significant difference between them (p=0.792). Besides, the prevalence of slight, moderate and severe asymmetries were, respectively 61%, 27% and 12% for males and 54%, 28% and 18% for female; and there was not statistically significant difference (p=0.366) between the gender of the sample evaluated. CONCLUSION: In this study, both deviation and intensity of mandibular asymmetry did not present preference regarding to the gender.

Objetive: This study aimed to compare the anatomical characteristics of the mandible in patients with skeletal class I, II and class III disorders using cone beam computed tomography (CBCT). Material and Methods: CBCT scans of patients between 17 to 40 years taken with NewTom 3G CBCT system with 12-inch field of view (FOV) were selected from the archive. Lateral cephalograms were obtained from CBCT scans of patients, and type of skeletal malocclusion was determined (Class I, II or III). All CBCT scans were evaluated in the sagittal, coronal and axial planes using the N.N.T viewer software. Results: The ramus height and distance from the mandibular foramen to the sigmoid notch in class II patients were significantly different from those in skeletal class I (P < 0.005). Distance from the mandibular canal to the anterior border of ramus in class III individuals was significantly different from that in skeletal class I individuals (P < .005). Conclusion: Length of the body of mandible in skeletal class I was significantly different from that in skeletal class II and III patients. Also, ramus height in skeletal class I was significantly different from that in skeletal class II patients. CBCT had high efficacy for accurate identification of anatomical landmarks.   Keywords Prognathism; Retrognathism; Mandible; Anatomy; Cone beam computed tomography.

Background and Objective: Dental study casts play a vital role in the diagnosis and treatment planning of various orthodontic cases. This study was carried out to compare the tooth widths, arch widths, and arch lengths in Class-I normal dentition to those in Class-I and Class-II crowded dentition in an effort to improve treatment planning and to eventually reduce treatment duration. Methods: Total 170 patients, 12 to 40 years of age with a complete set of permanent teeth till 1st molars; who presented to the Orthodontics Department at Armed Forces Institute of Dentistry (A.F.I.D), Rawalpindi from Sep 2019 to Feb 2020, were included in the study. Non-probability purposive method of sampling was used. The dental casts obtained were used to measure tooth widths, arch widths, and arch lengths. Subjects were classified into Class-I normal and Class-I and Class-II crowded occlusion and comparison of the sum of tooth widths, arch widths, and arch length discrepancies were determined among the three occlusion groups. Data was analyzed in SPSS version 21 and independent samples t-test was used to differentiate the variables of interest. Results: Out of 170 subjects, 73 (42.9%) subjects had Class-I normal occlusion while 97 (57%) had Class-I and Class-II crowded occlusions. No statistical difference was found between the occlusal groups with regard to the sum of tooth widths, inter-canine widths, inter-first premolar widths, inter-second premolar widths and inter-molar widths. However, a remarkable difference was observed between the occlusal groups with respect to arch perimeters and arch length discrepancies (p = 0.000 and 0.000 respectively). Conclusions: Results of the current study indicate that crowding of teeth occurs as a consequence of decreased arch perimeters which may lead to increased arch length discrepancies.
