Drop out from newly initiated exercise regimens is problematic. Three treatments intended to support new exercise programs (standard exercise counseling, computer feedback, and behavioral support) were tested to estimate their association with drop out over the initial 3 and 6 mo. Data from a total of 1,336 adults (44% men; Mage = 41.9 yrs., SD = 9.8) initiating exercise programs at 18 community exercise facilities (six in each of the three treatment groups) were aggregated by facility on a measure of drop out and were then contrasted both within and between groups. After a mixed model repeated-measures analysis of variance was significant overall, planned contrasts were conducted using the Tukey-Kramer test. Significantly less drop out was found at Month 3 for both the computer support (27%) and behavioral support (17%) groups, when contrasted with the standard exercise counseling group (38%). Behavioral support had significantly less drop out than computer feedback at Month 3. The behavioral support group showed significantly less drop out at Month 6 (33%) when contrasted with both the standard exercise counseling (58%) and computer feedback (52%) groups, which did not significantly differ from one another. Limitations and the need to evaluate and extend research on interventions for reducing drop out from exercise programs were discussed.

Background. Resistance exercise training can be effective against sarcopenia. We identified predictors of drop-out and compared physical outcomes between men and women after such training.Methods. Subjects (N=236,73.7±5.7years) participated in a 12-week resistance exercise program. Outcome variables were measured at baseline and endpoint.Results. Drop-out was 11.9% and not significantly different between genders. Drop-outs were significantly older and had poorer strength and physical function in comparison to completers. Anthropometrics, QoL, and cognitive function were not related to drop-out. According to multivariate analysis, gait speed and physical activity were the strongest predictors of drop-out. After the training, gains in lean mass or appendicular muscle were significantly higher in men than women; however relative gains in appendicular muscle as well as absolute improvements in strength and function were similar in men and women, respectively.Conclusions. Participants who drop out are older, have poorer physical function, and are less physically active. Old women do not drop out more frequently than men and show meaningful improvements in relevant outcomes similar to men after such a training program. Background: Cancer and its treatment can cause serious health issues that impair physical and mental well-being in children and adolescents. Exercise may be a valid strategy for managing some symptoms, including fatigue. In the light of our experience, we provide further justification for including exercise as part of routine childhood cancer care.Methods: Forty-four children and adolescents who had solid cancers not contraindicating their movement were invited to join an in-hospital 6-week supervised exercise program, and asked afterwards to complete validated quality of life and fatigue scales. The program consisted of personalized workout sessions of aerobic, resistance and flexibility exercises. The results obtained on the scales were compared between 21 patients who engaged in the exercise program (GYM group) and 23 who refused (No-GYM group), examining the different dimensions of health-related quality of life (physical, emotional, cognitive, social) and fatigue (general, sleep/rest, cognitive) in the two groups.Results: Being diagnosed with cancer initially prompted all but one of the respondents to drop-out of previous routine exercise or sports although their continuation had not been contraindicated. After 6 weeks of exercise, the GYM group's scores for quality of life and fatigue showed a statistically significant better perceived emotional functioning, and a trend toward a better social functioning than in the No-GYM group.Conclusion: We suggest that exercise improves the satisfaction of children and adolescents with cancer with their physical, mental and social functioning. We would emphasize the potential benefits of general practitioners discussing and recommending exercise for their young patients with cancer.

Abstract Introduction: Physical exercise when performed regularly brings several health benefits, especially for older adults. However, there are barriers that induce these individuals to withdraw from exercise. Objective: This study investigated the causes of drop out from a supervised physical exercise program specific to older adults. Methods: A semi-structured questionnaire was applied by telephone contact, containing questions about their reasons for leaving the program. Demographic data were analyzed by relative and absolute frequencies, and the Chi-square test was used to verify the associations between the variables collected. A significance level of 5% was considered. Results: Fifty-nine older adults were interviewed and the main reasons for drop out were lack of time (33.9%), emergence or worsening of disease (28.8%), and the need to care for family members with health problems (18.7%). A high percentage of older adults (62.7%) showed a worsening in health status after leaving the program, with changes in muscle strength (80.0%), balance (76.7%), and motor coordination (40.0%). Conclusion: A professional intervention is suggested to enable organizational control of factors related to lack of time, as well as a logistical monitoring service for older adults and/or family members who fall ill, promoting the return to practice as soon as possible.

Attendance at treatment sessions based on cognitive-behavioral and social cognitive theory were significantly correlated with exercise session drop out ( rpb = –.56) and attendance ( rpb = .69) in 130 adults beginning new exercise programs.

Purpose Semi-occluded vocal tract (SOVT) exercises with tubes or straws have been widely used for a variety of voice disorders. Yet, the effects of longer periods of SOVT exercises (lasting for weeks) on the aging voice are not well understood. This study investigated the effects of a 6-week straw phonation in water (SPW) exercise program. Method Thirty-seven elderly subjects with self-perceived voice problems were assigned into two groups: (a) SPW exercises with six weekly sessions and home practice (experimental group) and (b) vocal hygiene education (control group). Before and after intervention (2 weeks after the completion of the exercise program), acoustic analysis, auditory–perceptual evaluation, and self-assessment of vocal impairment were conducted. Results Analysis of covariance revealed significant differences between the two groups in smoothed cepstral peak prominence measures, harmonics-to-noise ratio, the auditory–perceptual parameter of breathiness, and Voice Handicap Index-10 scores postintervention. No significant differences between the two groups were found for other measures. Conclusions Our results supported the positive effects of SOVT exercises for the aging voice, with a 6-week SPW exercise program being a clinical option. Future studies should involve long-term follow-up and additional outcome measures to better understand the efficacy of SOVT exercises, particularly SPW exercises, for the aging voice.

Language development is the foundation for competence in social, emotional, behavioral, and academic performance. Although language impairment (LI) is known to co-occur with behavioral and mental health problems, LI is likely to be overlooked in school-age children with emotional and behavioral disorders (EBD; Hollo, Wehby, & Oliver, in press). Because language deficits may contribute to the problem behavior and poor social development characteristic of children with EBD, the consequences of an undiagnosed language disorder can be devastating. Implications include the need to train school professionals to recognize communication deficits. Further, it is critically important that specialists collaborate to provide linguistic and behavioral support for students with EBD and LI.

Hintergrund: Dieser Artikel beschreibt die Entwicklung eines internetbasierten begleiteten Selbsthilfetrainings (Albtraumcoach) zur Bewältigung von Albträumen, das innerhalb zweier Pilotstudien auf seine Durchführbarkeit und Anwendbarkeit getestet wurde. Methoden: Innerhalb eines neunstufigen Modells wird die Entwicklung der Intervention beschrieben. Dabei wird neben der wissenschaftlichen Evidenz, die die Basis für die Intervention bildet, auf technische, ethische, datenschutzrechtliche und weitere spezifische Maßnahmen innerhalb der Interventionsentwicklung eingegangen. In zwei Pilotstudien evaluierten zudem 10 Personen mit schlechten Träumen in der Vergangenheit die Durchführbarkeit (Studie 1) und 12 Personen mit wiederkehrenden Albträumen die Anwendbarkeit sowie den Nutzen der Intervention (Studie 2). Abhängige Variablen waren die Qualität der einzelnen Sitzungen, die Zufriedenheit mit der Intervention sowie Albtraumfrequenz, Albtraumbelastung, Schlafqualität und Depressivität. Ergebnisse und Diskussion: Die Durchführbarkeit und Anwendbarkeit des Trainings wurden positiv beurteilt. Während die Drop-out-Rate verhältnismäßig hoch war, ergaben sich erste, zurückhaltend zu beurteilende, Hinweise für einen Nutzen der Intervention.
