Nyeri leher merupakan suatu kondisi kesehatan yang memiliki angka kejadian yang tinggi di populasi umum. Melakukan asesmen terhadap tanda dan gejala merupakan hal yang sangat penting dalam proses perencanaan dan evaluasi. Suatu alat ukur harus memenuhi unsur psikometrik dan mampu menyesuaikan dengan karakteristik sosial dan budaya setempat. Tujuan dari penelitian ini adalah untuk menghasilkan alat ukur kuesioner disabilitas Neck Disability Index versi Indonesia dengan metode adaptasi lintas budaya dengan nilai validitas dan reliabilitas yang tinggi. Penelitian ini merupakan penelitian observasional analitik, dimana tiap subjek diminta mengisi kuesioner dua kali dengan jarak waktu tujuh hari. Pengambilan sampel dilakukan dengan metode purposive sampling.  Hasil analisis menggunakan pearson product moment didapatkan nilai p sebesar 0.000 (p<0.05), nilai rhitung>rtabel  dan nilai korelasi koefision diatas 0.61 ( 0.61 < r ? 0.80 ). Hasil analisis menggunakan content validity index didapatkan nilai I/CVI = 1 (I/CVI > 0.79), S-CVI/Ave = 1 dan S-CVI/UA = 1 (S-CVI > 0.80), dan nilai ACP = 97.67% (ACP > 0.90). Hasil analisis menggunakan cronbach’s alpha didapatkan nilai r sebesar 0.895 ( 0.81 < r ? 1.00 ), dan dari hasil analisis interclass correlation coefficient menggunakan spearman correlation coefficient didapatkan nilai r sebesar 0.859 ( 0.81 < r ? 1.00 ). Berdasarkan hasil analisis, dapat disimpulkan bahwa adaptasi lintas budaya kuesioner neck disability index versi bahasa Indonesia memiliki nilai validitas dan reliabilitas yang tinggi.

Background: The Neck Disability Index (NDI) is an important self-assessment tool used extensively worldwide, in clinical practice with implications into scientific research fields. It is used to assess the extent of pain and levels of functional disability associated with neck pain. The NDI consists of 10 items where each item was scored from a scale of 0 to 5 giving the maximum score possible to 50. Though proven to be a reliable instrument in the English-speaking population, the NDI has never been validated and culturally adapted in the Mizo language among the rural north-east Indian region where English is not spoken as means of communication. The aim is to translate and cross culturally adapt the NDI into Mizo tawng (official language of Mizoram) with the objective of establishing reliability and validity of the M-NDI in patients with non-specific neck pain.Methods: A total of 49 subjects participated voluntarily from the rural primary health care, Lunglei district, Mizoram. Subject having chronic non-specific neck pain lasting more than 3 months were included after taking a written formal consent.Results: The internal consistency determined by Cronbach alpha, and the Intraclass Correlation Coefficient (ICC) using the test-retest reliability showed a good and an excellent reliability respectively (α=0.82, ICC=0.97, 95% CI= 0.95-0.98). Construct validity was determined between the variables-Numerical Pain Rating Scale (NPRS) and NDI by Pearson’s correlation coefficient and found to have a good correlation r=0.89 and significant difference at p<0.001.Conclusions: The study results concluded the Mizo version of NDI to be easy to understand, reliable and valid instrument for measuring disability and functional limitations of daily activities in non-specific neck pain in the Mizo speaking population.

Aims The purpose of this retrospective study was to investigate the clinical relevance of increased facet joint distraction as a result of anterior cervical decompression and fusion (ACDF) for trauma. Patients and Methods A total of 155 patients (130 men, 25 women. Mean age 42.7 years; 16 to 87) who had undergone ACDF between 1 January 2001 and 1 January 2016 were included in the study. Outcome measures included the Neck Disability Index (NDI) and visual analogue scale (VAS) for pain. Lateral cervical spine radiographs taken in the immediate postoperative period were reviewed to compare the interfacet distance of the operated segment with those of the facet joints above and below. Results There was a statistically significant relationship between greater facet distraction and increased NDI and VAS pain scores. This was further confirmed by Spearman correlation, which showed evidence of a moderate correlation between both NDI score and facet joint distraction (Spearman correlation coefficient 0.34; p < 0.001) and VAS score and facet distraction (Spearman correlation coefficient 0.52; p < 0.001). Furthermore, there was a discernible transition point between outcome scores. Significantly worse outcomes, in terms of both NDI scores (17.8 vs 8.2; p < 0.001) and VAS scores (4.5 vs 1.3; p < 0.001), were seen with facet distraction of 3 mm or more. Patients who went on to have a posterior fusion also had increased NDI and VAS scores, independent of facet distraction. Conclusion After undergoing ACDF for the treatment of cervical spine injury, patients with facet joint distraction of 3 mm or more have worse NDI and VAS pain scores. Background:Young adults have grown up today with mobile phones as an evident part of their lives. Text neck syndrome and SMS thumb may occur due to repetitive use of hand held devices (HHDs) resulting in repetitive stress injury or an overuse syndrome while using their mobile phones or other electronic devices for prolonged periods of time. Our aim is to assess self reported addiction to smartphone use and correlate its use and musculoskeletal disorders (MSDs) in neck and hand in young healthy adults. Methods:The study examined 100 healthy physiotherapy students of a college in Ahmedabad by random table sampling, in the age group of 20-25 years. Students were asked to fill a proforma with the questionnaires of Smartphone Addiction Scale (SAS), Neck Disability Index (NDI), and Cornell Hand Discomfort Questionnaire (CHDQ) attached. Level of significance was kept at 5%. Spearman correlation coefficient was used to correlate between the SAS and NDI, and SAS and CHDQ respectively. Results:There was a predominance of females over males (females-76, males-24). Mean±SD of SAS, NDI and CHDQ was 102.49±22.15, 30±0.10 and 6.12±8.73 respectively. Spearman correlation coefficient showed a significant moderate positive correlation between both SAS and NDI (r=0.671, p<0.001) and between SAS and CHDQ (r=0.465, p<0.001). Conclusions: The study showed that musculoskeletal problems in neck and hand(predominantly thumb) can be seen in smartphone addicted students which may be short term initially but may later lead to long term disability.

Background: Despite the potential benefits of virtual reality technology in physical rehabilitation, only a few studies have evaluated the efficacy of this type of treatment in patients with neck pain.Objective: The aim of this study was to compare the effects of virtual reality training (VRT) versus conventional proprioceptive training (CPT) in patients with neck pain.Methods: Forty four participants with nonspecific chronic neck pain were randomly assigned to VRT or CPT in this assessor-blinded clinical trial. A novel videogame called Cervigame® was designed for VRT. It comprises of 50 stages divided into unidirectional and two-directional stages ordered from easy to hard. CPT consisted of eye-follow, gaze stability, eye-head coordination and position and movement sense training. Both groups completed 8 training sessions over 4 weeks. Visual analogue scale score, neck disability index and Y-balance test results were recorded at baseline, immediately after and 5 weeks post-intervention. Mixed repeated measure ANOVA was used to analyze differences between mean values for each variable at an alpha level of 0.05.Results: There were significant improvements in all variables in both groups immediately after and 5 weeks after the intervention. Greater improvements were observed in the visual analogue scale and neck disability index scores in VRT group, and the results for all directions in Y-balance test were similar in both groups. No side effects were reported.Conclusion: Improvements in neck pain and disability were greater in VRT than CPT group. Cervigame® is a potentially practical tool for rehabilitation in patients with neck pain.

Abstract Objectives Granisetron and lidocaine injections have been used for the management of myofascial pain syndrome. This study was aimed to compare the efficacy of granisetron and lidocaine injections to trigger points of upper trapezius in the management of myofascial pain syndrome. Methods We performed a double-blind randomized clinical trial in an outpatient clinic of physical medicine and rehabilitation at a teaching hospital. A total of 40 patients aged ≥18 with neck pain due to myofascial pain syndrome were included. They had pain for at least one month with the intensity of at least 30 mm on a 100 mm visual analog scale. Each participant received a single dose of 1 mL lidocaine 2% or 1 mg (in 1 mL) granisetron. The solutions were injected into a maximum of three trigger points of the upper trapezius. We instructed all patients to remain active while avoiding strenuous activity for three or four days, and to perform stretch exercise and massage of their upper trapezius muscles. We assessed the patients before the interventions, and one month and three months post-injection. The primary outcome was the Neck Disability Index and the secondary outcome was the Neck Pain and Disability Scale. Results Both interventions were successful in reducing neck pain and disability (all p-values <0.001). However, the neck pain and disability responded more favorably to lidocaine than granisetron (p=0.001 for Neck Disability Index, and p=0.006 for Neck Pain and Disability Scale). No significant side-effect was recognized for both groups. Conclusions Both lidocaine and granisetron injections to trigger points are effective and safe for the management of the syndrome and the benefits remain at least for three months. However, lidocaine is more effective in reducing pain and disability. The injections are well-tolerated, although a transient pain at the site of injections is a common complaint. One mL of lidocaine 2% is more effective than 1 mg (in 1 mL) granisetron for injecting into the trigger points of the upper trapezius in myofascial pain syndrome.

Background:Rheumatoid arthritis is a common type of autoimmune arthritis characterized by chronic inflammation. Cervical spine is often affected specially in long lasting diseaseObjectives:Evaluate efficacy of Third occipital nerve Radiofrequency under fluoroscopic guidance to treat refractory cervicogenic headache in RA patients.Methods:The current study was revised and approved from the local ethical committee of Faculty of Medicine; Assiut University, then registered in the clinical trials under the number ofNCT03852355. Inclusion criteria included, Patients who fulfilled the American College of Rheumatology (ACR) (2010) criteria for RA and suffering from upper neck pain and/or headache due to bilateral 3rd occipital nerve involvement, excluding other local cervical spine pathologies was confirmed by MRI and previously failed conservative treatment for at least three months prior to enrollment. Sixty adult patients were randomly assigned to one of the two studied groups Group 1 (RF, n = 30), received bilateral Third occipital nerve Radiofrequency under fluoroscopic guidance or Group 2 (control group, n = 30), received oral prednisolone 10 mg/day. The two groups were then followed-up with neck disability index (NDI), nocturnal neck pain VAS score and headache score every two weeks for three months. Sleep disturbance, sleep disability index were reassessed six months post intervention. Post interventional assessment was done by pain physician who were kept blind to the grouping process.Results:Neck disability index (1ry outcome), Nocturnal pain VAS, and severity of headache showed significant differences during the whole post-interventional study period. The patients in RF group demonstrated significant improvement of pain in comparison to baseline value over the whole six months with p-value < 0.001 as regard to the fore-mentioned three parameters. On the other aspect, the control group patients showed significant improvement in comparison to its baseline value after the 2nd, 12th and 24th weeks only as follows: (0.001,0.003, 0.003 for the NDI) (p values of 0.02,0.01, 0.01 for the nocturnal pain VAS), (0.001 0.009, 0.005 for the headache VAS severity.Conclusion:Radiofrequency of 3rd occipital nerve is effective in treatment of refractory cervicogenic headache in RA.Disclosure of Interests: :None declared

Background/aims The Örebro Musculoskeletal Pain Questionnaire is categorised as a ‘yellow flag’ pain-associated psychological distress screening tool, which predicts long-term disability. The aim of this study was to assess the validity and reliability of the Turkish translation of the Örebro Musculoskeletal Pain Questionnaire in patients with acute and subacute neck pain. Methods The test–retest reliability and internal consistency were assessed with the intraclass correlation coefficient and Cronbach's α. Construct validity was assessed with a visual analogue scale, the Neck Disability Index and the Fear Avoidance Belief Questionnaire. Results Cronbach's α value was found to be 0.790, test–retest reliability was 0.99. The intraclass correlation coefficient was 0.999 (95% confidence interval: 0.998–0.999; P=0.000). The Örebro Musculoskeletal Pain Questionnaire showed a moderate positive correlation with the Neck Disability Index (r=0.544; P=0.0001), a weak correlation with the Fear Avoidance Belief Questionnaire (r=0.264; P=0.0001) but no correlation with the visual analogue scale. Conclusions The Turkish version of the Örebro Musculoskeletal Pain Questionnaire is a valid, reliable and acceptable screening tool in patients with acute and subacute neck pain.

Background: "Text neck" is a term coined to describe the posture created by leaning forward for lengthy periods of time, such as when reading and texting on a cellphone which has been linked to stress injuries. Neck pain, upper back discomfort, shoulder pain, frequent headaches, and greater curvature of the spine are all dangerous indications of text neck. According to a survey, 35% of smartphone users suffer from text neck syndrome. People between the ages of 15 and 18 are more likely to have neck pain. This protocol has been created that describes the design of comparative study to evaluate effectiveness of progressive resisted exercise along with conventional exercise and conventional exercise program alone in text neck syndrome. Methods: The participants (n=80) will be recruited in the study suffering from text neck syndrome and meeting the inclusion criteria. Two groups will be formed such that patients in group A will be treated with conventional therapy and group B will be treated with progressive resisted exercise (PRE) along with conventional therapy. The protocol will cover 4 weeks of treatment. In the rehabilitation period, we will evaluate the pain intensity, strength of neck muscles and functional activity. Our outcome measures will be- Numerical pain rating scale (NPRS) and Neck disability index (NDI). Discussion: Efficacy of the intervention will be evaluated by analyzing the pain intensity by using Numerical pain rating scale (NPRS) and level of functional disability by using Neck disability index (NDI). The result of the study will significantly provide affirmation on either using combination therapy of PRE with conventional exercise or conventional exercise alone.
