Objective To observe the therapeutic effect of the combination of Xiaoaiping injection and chemotherapy on advanced esophageal cancer and coagulation function. Methods 100 patients with advanced esophageal carcer were randomly divided into control group and observation group, and each group had 50 cases. The control group was treated with TP chemotherapy, and the observation group, on the basis of the control group’s treatment, was treated with the Xiaoaiping injection, and treatment effects, Karnofsky, adverse drug reactions and INR changes before and after the treatment of the two groups were observed. Results After 2 periods of treatment, the local control rate of solid tumor, Karnofsky score, and stability in the observation group were significantly higher than those in the control group (p<0.05); and the plasma prothrombin time (PT), activated partial coagulation activity time (APTT) and thrombin time (TT) were significantly lower in the observation group than in the control group (p<0.05), and Fibrinogen (FIB) was significantly higher than the control group (p<0.05); and there was no statistically significant difference in the incidence of adverse reactions between the two groups (p>0.05). Conclusion: The therapeutic effect of the combination of Xiaoaiping Injection and chemotherapy on advanced esophageal cancer is obvious, and it can effectively improve the coagulation function, improve the quality of life, and be safe and reliable, so it's worth popularizing and application.

Abstract Objective To analyze the application of concept nursing of accelerated rehabilitation surgery in orthopedic postoperative recovery. Methods A total of 120 patients who received orthopedic surgery were divided into the control group undergoing routine orthopedic nursing and the observation group undergoing the concept of accelerated rehabilitation surgery nursing. Results Patients in the observation group had shorter in-bed activity time and out-of-bed activity time, average time of hospital stay, and lower total treatment costs. The incidence of incision infection, respiratory system infection, digestive tract infection, urinary tract infection, deep vein thrombosis, and other complications in the observation group was much lower. The recovery scores of joint function in the observation group at 1, 3, 6, and 12 months after the operation were all better, and the recovery rate of joint function within 1 year after the operation was higher. Conclusion Following the concept of accelerated rehabilitation surgery nursing during the perioperative period can improve the quality of postoperative orthopedic recovery.

Objective: To investigate the clinical efficacy of insulin glargine combined with acarbose in the treatment of elderly patients with diabetes. Methods: One hundred and forty-four elderly patients with diabetes who received treatment between December 2016 and December 2017 in Binzhou People’s Hospital, China, were selected and divided into a control group and an observation group, 72 each, using random number table. The control group was treated with insulin glargine, while the observation group was treated with insulin glargine combined with acarbose. The therapeutic effect, improvement of quality of life and adverse reactions were compared between the two groups. Results: After treatment, fasting blood glucose (FBG), 2h postprandial blood glucose (PBG) and glycosylated hemoglobin (Hb Alc) of the two groups were lower than those before treatment, and the decrease degree of the observation group was significantly larger than that of the control group (P<0.05). The time needed for blood glucose reaching the standard level and daily insulin dosage of the observation group were significantly lower than that of the control group, and the differences were statistically significant (P<0.05). SF-36 scale score of the observation group was significantly better than the control group, and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: The combination of insulin Glargine and Acarbose can significantly control the blood glucose level of elderly patients with diabetes, improve the biochemical indicators, and enhance the quality of life. Abstract Objective:The purpose of this study is to retrospectively observe the clinical efficacy and adverse reactions of docetaxel hyperthermic intraperitoneal chemotherapy (HIPEC) combined with endostar in treatment of malignant ascites. Methods:56 cases of malignant ascites admitted to Zhejiang Hospital from July 2019 and October 2020 had received no less than second-line chemotherapy. The observation group (n = 29) was treated with endostar 60mg d1,4,7 Q3W intraperitoneal injection combined with docetaxel 60mg/m2 d4 HIPEC Q3W for 2 cycles. The control group (n = 27) was just treated with docetaxel 60mg/m2 d1 HIPEC Q3W for 2 cycles. The general clinical data and qualitative data of the treatment results were processed by SPSS26.0 using χ2 test, and quantitative data were processed by t test. When P < 0.05, statistical data can be considered statistically significant.Results:There were significant differences between the observation group and the control group in objective response rate (ORR) (65.5% vs 37.0%,P=0.033),in the improvement rate of KPS (48.3% vs 22.2%, P=0.042),also in the median control time (57dvs 45d,P=0.027). The incidence of III to IV adverse reactions was low, and no treatment-related death was observed. There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:Endostar combined with docetaxel HIPEC has good clinical effect on malignant ascites and can improve patients’ quality of life, and the adverse reactions can be tolerated.

Objective: To explore the impact of comprehensive nursing intervention on the quality of intestinal preparation in the process of intestinal preparation for hospitalized patients undergoing colonoscopy. Methods: A total of 320 patients underwent colonoscopy in our department from January to June 2021. They were grouped by random number table. There were 160 cases in the control group and 160 cases in the observation group. The adverse reactions of intestinal preparation and the cleanliness of intestinal preparation were analyzed. Results: Through comprehensive nursing intervention, it can be found that the incidence of adverse reactions in the preparation of colonoscopy in the control group was 24.38%, and that in the observation group was 13.48%. The results showed that the incidence of adverse reactions in the preparation of colonoscopy was significantly lower than that in the control group (P < 0.05), which indicates that the difference was statistically significant. In the control group, 105 people qualified in intestinal cleanliness score (> 5 points) during enteroscopy preparation, and the cleanliness qualification rate was 65.63%. In the observation group, 139 people qualified in intestinal cleanliness score (> 5 points) during enteroscopy preparation, and the cleanliness qualification rate was 86.88%. The cleanliness qualification rate of enteroscopy preparation was significantly higher than that of the control group, and P < 0.05, which indicates that the difference was statistically significant. Conclusion: Comprehensive nursing intervention can promote the quality of preparation of patients for colonoscopy, improve the compliance of patients and improve the accuracy of examination effect.

<b><i>Background:</i></b> Patients with maintenance hemodialysis (MHD) generally have a microinflammatory state. The aim of this study was to investigate the effects of hemodialysis (HD) combined with hemoperfusion (HP) on microinflammatory state in elderly patients with MHD. <b><i>Methods:</i></b> One hundred and fifty elderly patients with MHD were randomly divided into the control group and the observation group. The control group received simple HD treatment, and the observation group received combined HD + HP treatment on the basis of the control group. After 6 months of continuous treatment, the patients were evaluated to compare the quality of life, inflammation, adverse reactions, and nutritional indicators in the 2 groups before and after treatment. <b><i>Results:</i></b> There was no significant difference in the quality of life between the 2 groups before treatment. After treatment, the scores of psychological aspects, physiological aspects, social aspects, environmental aspects, and independent ability in the observation group were higher than those in the control group, with statistical significance (<i>p</i> &#x3c; 0.05). There was no statistical significance in the level of inflammation between 2 groups before treatment. After treatment, the levels of hs-CRP, Hcy, IL-6, and TNF-α in the observation group were significantly lower than those in the control group, with statistical significance (<i>p</i> &#x3c; 0.05). The incidence of dry mouth, skin reaction, neuritis, and subcutaneous tissue fibrosis in the observation group was lower than that in the control group, with statistical significance (<i>p</i> &#x3c; 0.05). There was no statistical significance in nutritional level indexes between 2 groups before treatment (<i>p</i><sub>1</sub> &#x3e; 0.05). After treatment, the levels of hemoglobin, total protein, albumin, and transferrin in the observation group were significantly higher than those in the control group, with statistical significance (<i>p</i> &#x3c; 0.05). <b><i>Conclusion:</i></b> The clinical effect of HD combined with HP in elderly MHD patients is significant, which can effectively reduce the incidence of adverse reactions and inflammation in the patients and improve the quality of life and nutritional indicators of the patients.

Objective: To investigate the effects of different blood transfusion volumes on coagulation function and safety in patients with severe trauma.Patients and Methods: One hundred and ten cases with severe trauma were selected as the research subjects. They were admitted to our hospital from April 2016 to April 2018. Sixty patients who needed massive transfusion (1-1.5 times of their own blood volume level) were assigned to the study group, and another 50 patients who needed small amount of transfusion (less than their own blood volume level) were assigned to the control group. The blood coagulation function index, thrombelastogram index, blood gas analysis index and the influence of adverse reactions were compared between patients in the two groups before transfusion and 24h after transfusion. Results:Twenty fourhoursafter transfusion, prothrombin time (PT), activated partial thrombin time (APTT) and thrombin time (TT) were prolonged in both groups. Both platelet (PLT) and fibrinogen (FIB) decreased(p<0.05). Both response time of blood coagulation factor (R) and fibrin polymerization reaction time (K) decreased and were lowerin the study group than those in the control group, while fibrous protein aggregation function (Angle), platelet aggregation function (MA) elevated and were higherin the study group than those in the control group (p<0.05). pH in both groups decreased, oxygen partial pressure (P02) and carbon dioxide partial pressure (PC02) in both groups increased (p<0.05). PaC02 and Pa02 in the study group were higher than those in the control group, and pH was lower than that in the control group (p<0.05). The incidence of total adverse reactions in the study group was higher than that in the control group (p<0.05). Conclusion:ln conclusion, massive transfusion for patients with severe trauma could reduce the patient’s platelet and affect their coagulation function.

Objective: Therapeutic effect of heat-clearing and dampness-eliminating dredging collaterals combined with papaya antirheumatic pills on treatment of damp-heat obstructive ankylosing spondylitis. Methods: A total of 200 patients with ankylosing spondylitis who were treated in our hospital from January 2018 to October 2019 were selected as the research objects and randomly divided into 2 groups, each with 100 patients. The control group was treated with leflunomide + papaya antirheumatic pills, and the observation group was treated with self-drafted recipe of heat-clearing and dampness-eliminating, the curative effect, visual analogue score (VAS), barthel AS function index (BASFI) score, biochemical indicators and adverse reactions between the two groups were compared. Results: After 12 weeks of treatment, the efficacy of the observation group was higher than that of the control group. The pain score, BASFI score, and biochemical index were lower than those of the control group, and the differences were statistically significant (P<0.05); There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Heat-clearing and dampness-eliminating dredging collaterals combined with papaya antirheumatic pills is effective in the treatment of damp-heat obstructive ankylosing spondylitis, which is beneficial to reduce the degree of pain and inflammation of the affected area, helps patients remain active, and improves activities of daily living.

Objective: To explore the clinical nursing effect of high-quality nursing on sinusitis patients. Methods: Altogether 127 patients with sinusitis treated in our hospital from June 2018 to July 2019 were collected. Among them, 69 patients were treated with high-quality nursing as the observation group (OG) and 58 patients were treated with routine nursing as the control group (CG). The quality of life (QOL), pain, nursing satisfaction, compliance, incidence of adverse reactions and length of stay before and after nursing were compared. Results: The QOL in OG was evidently higher than that in CG, the pain in OG was evidently lower than that in CG, the nursing satisfaction and compliance in OG were evidently higher than that in CG, and the incidence of adverse reactions and length of stay in OG were evidently lower than that in CG. Conclusion: High-quality nursing can reduce postoperative pain of patients with sinusitis surgery, and improve the QOL and nursing satisfaction of patients.

ABSTRACT Introduction: Difficulty falling asleep is connected to the malfunctioning of the sleep and wakefulness mechanism of the human body caused by various reasons. There are a series of adverse reactions resulting from abnormal or poor quality of sleep during sleep per se. This symptom severely affects an individual's physical condition and mental health. Objective: To explore the effect of physical exercise on patients with difficulty falling asleep. Methods: Mathematical statistics were used to analyze 60 patients with difficulty falling asleep. We divided the patients into a sports group and a control group. The patients in the sports group took sports training, while the control group did not. After two weeks of intervention and comparison, we used mathematical statistics to evaluate the groups’ cognitive function. Results: After physical exercise, the patient's sleep quality was higher and sleep disorders were reduced. Conclusion: Physical activity is a simple and popular way of exercising. It is effective in improving the cognitive function of patients with difficulty falling asleep. Level of evidence II; Therapeutic studies - investigation of treatment results.

Objective: To observe the clinical efficacy of sequential butylphthalide therapy combined with dual antiplatelet therapy in the treatment of elderly patients with acute cerebral infarction (ACI). Methods: One hundred and twenty-two elderly patients with ACI who were admitted to the department of neurology of our hospital at May 2016-August 2018 were selected grouped into a control group and an observation group by random number table method, 61 in each group. On the basis of conventional treatment, the patients in the control group were given dual antiplatelet therapy (aspirin enteric-coated tablets + clopidogrel bisulfate tablets), while the patients in the observation group were given sequential butylphthalide therapy on the basis of the control group. The clinical effects of the two groups were compared after four weeks of treatment, and the changes of National Institutes of Health Stroke Scale (NIHSS), ADL score, plasma 3-mercaptopyruvate sulphurtransferase (3-MST) and Amyloid β42 (Aβ42) levels and the occurrence of adverse reactions during treatment were recorded. Results: The clinical efficacy of the observation group was better than that of the control group (P<0.05). There was no significant difference in NIHSS and ADL scores between the two groups before treatment (P>0.05). After treatment, the NIHSS and ADL scores of the observation group were better than those of the control group (P<0.05). There was no significant difference in plasma levels of 3-MST and AB42 between the two groups before treatment (P>0.05). The level of plasma 3-MST in the observation group was higher than that in the control group, and the level of plasma Aβ42 was lower than that in the control group (P<0.05). No serious adverse reactions occurred during the treatment period in both groups. Conclusion: Butylphthalide sequential therapy combined with dual antiplatelet therapy is effective in the treatment of elderly ACI.
