Abstract Objective To analyze the application of concept nursing of accelerated rehabilitation surgery in orthopedic postoperative recovery. Methods A total of 120 patients who received orthopedic surgery were divided into the control group undergoing routine orthopedic nursing and the observation group undergoing the concept of accelerated rehabilitation surgery nursing. Results Patients in the observation group had shorter in-bed activity time and out-of-bed activity time, average time of hospital stay, and lower total treatment costs. The incidence of incision infection, respiratory system infection, digestive tract infection, urinary tract infection, deep vein thrombosis, and other complications in the observation group was much lower. The recovery scores of joint function in the observation group at 1, 3, 6, and 12 months after the operation were all better, and the recovery rate of joint function within 1 year after the operation was higher. Conclusion Following the concept of accelerated rehabilitation surgery nursing during the perioperative period can improve the quality of postoperative orthopedic recovery.

Objective To observe the therapeutic effect of the combination of Xiaoaiping injection and chemotherapy on advanced esophageal cancer and coagulation function. Methods 100 patients with advanced esophageal carcer were randomly divided into control group and observation group, and each group had 50 cases. The control group was treated with TP chemotherapy, and the observation group, on the basis of the control group’s treatment, was treated with the Xiaoaiping injection, and treatment effects, Karnofsky, adverse drug reactions and INR changes before and after the treatment of the two groups were observed. Results After 2 periods of treatment, the local control rate of solid tumor, Karnofsky score, and stability in the observation group were significantly higher than those in the control group (p<0.05); and the plasma prothrombin time (PT), activated partial coagulation activity time (APTT) and thrombin time (TT) were significantly lower in the observation group than in the control group (p<0.05), and Fibrinogen (FIB) was significantly higher than the control group (p<0.05); and there was no statistically significant difference in the incidence of adverse reactions between the two groups (p>0.05). Conclusion: The therapeutic effect of the combination of Xiaoaiping Injection and chemotherapy on advanced esophageal cancer is obvious, and it can effectively improve the coagulation function, improve the quality of life, and be safe and reliable, so it's worth popularizing and application.

Objectives This study aimed to examine application of fast-track surgery (FTS) in the perioperative period of laparoscopic partial nephrectomy for renal tumors, and to discuss its effects and safety. Methods Eighty patients who received laparoscopic partial nephrectomy in urinary surgery from January 2016 to December 2017 were selected and randomly classified as the observation group (n = 40) and control group (n = 40). Traditional treatments were performed in the control group, while FTS was applied in the observation group. The complication rate after the operation was recorded. Results The duration of the operation and intraoperative blood loss were not different between the groups. The duration of anesthesia and fluid transfusion volume on the day of the operation were significantly less in the observation group than in the control group. The rates of infection of the incisional wound, nausea and vomiting, and anastomotic stomal bleeding were not significantly different between the groups. However, the rates of postoperative urinary tract infection, abdominal distension, thirst, hypothermia, and pulmonary infection were significantly lower in the observation group than in the control group. Conclusion Application of FTS in laparoscopic partial nephrectomy contributes to postoperative recovery and reduction of postoperative complications.

This study aimed to explore the influence of clinical pathway combined with high-quality nursing on endometrial cancer (EC) patients undergoing radical resection. Methods: A total of 116 EC patients who underwent surgery in our hospital from July 2017 to December 2019 were randomized into control group (CG) and observation group (OG). The CG (n=58) took routine nursing mode, while the OG (n=58) received clinical pathway combined with high-quality nursing. The postoperative recovery, psychological emotion and quality of life of the two groups were observed. Results: The postoperative recovery, psychological emotion and quality of life of the OG were better than those of the CG. Conclusion: Clinical pathway combined with high-quality nursing can effectively improve the quality of life and postoperative recovery of EC patients after radical resection.

Abstract Background Because of the indiscriminate use of opioids during the perioperative period, opioid-free anesthesia (OFA) has been increasingly required. Nevertheless, the studies on the detailed techniques and effects of OFA are not sufficient. The Quality of Recovery-40 (QoR-40) questionnaire is a validated assessment tool for measuring recovery from general anesthesia. However, no study has used the QoR-40 to determine if OFA leads to better recovery than standard general anesthesia. Therefore, we aim to perform this study to determine the effects of OFA using dexmedetomidine and lidocaine on the quality of recovery as well as the various postoperative outcomes. Methods The participants (n = 78) will be allocated to one of the two groups; the study group will receive bolus and infusion of dexmedetomidine and lidocaine, and the control group will receive remifentanil infusion during general anesthesia for gynecological laparoscopy. The other processes including anesthetic and postoperative care will be performed similarly in the two groups. Intraoperative hemodynamic, anesthetic, and nociceptive variables will be recorded. Postoperative outcomes such as QoR-40, pain severity, and opioid-related side effects will be assessed. Additionally, an ancillary cytokine study (inflammatory cytokine, stress hormone, and reactive oxygen species) will be performed during the study period. Discussion This will be the first study to determine the effect of OFA, using the combination of dexmedetomidine and lidocaine, on the quality of recovery after gynecological laparoscopy compared with standard general anesthesia using remifentanil. The findings from this study will provide scientific and clinical evidence on the efficacy of OFA. Trial registration ClinicalTrials.gov NCT04409964. Registered on 28 May 2020

To study the effect of rapid rehabilitation nursing on patients’ quality of life and pain level during the perioperative period of radical breast cancer surgery. Methods: 126 patients who were hospitalized in our hospital from January 2018 to February 2020 and underwent radical breast cancer surgery were divided into control group and rapid rehabilitation group according to the perioperative period intervention method. Routine nursing intervention and rapid rehabilitation nursing intervention were used respectively. The general conditions of the two groups were recorded, and the differences of T cell subsets before operation and 3 days after operation were detected. Visual analogue pain (VAS) score was used to evaluate the changes of pain degree at 1 day, 3 days and 5 days after operation and on the day of discharge. Postoperative complications and satisfaction were compared between the two groups. Quality of life (QOL) was assessed at 1 month and 3 months after operation using the CARES-SF score. Results: Anesthesia waking time (26.12 ± 5.77) min, off-bed activity time (14.25 ± 2.87) h and hospital stay (7.82 ± 2.15) d in the rapid rehabilitation group were shorter than those in the control group (P < 0.05). The operation time (92.02 ± 14.78) min and intraoperative blood loss (57.96 ± 13.96) mL in the rapid rehabilitation group were not significantly different from those in the control group (P > 0.05). In the control group, 3d after operation, T lymphocyte subsets were decreased gradually than that before operation (P < 0.05), CD3+ was decreased 3d after operation than that before operation in the rapid rehabilitation group (P < 0.05), but CD4+, CD8+, CD4+ / CD8 + 3d after operation had no significant difference than that before operation (P > 0.05). In rapid rehabilitation group, T lymphocyte subsets at 3d after operation were higher than those in the control group (P < 0.05). The postoperative complication rate of the rapid rehabilitation group was lower than that of the control group, and the satisfaction degree was higher than that of the control group, with significant difference (P < 0.05). After follow-up, it was found that the scores of qualities of life, such as physiology, psychosocial, marriage, sexual life and relationship with medical staff in the rapid rehabilitation group were lower than those in the control group at 1 month and 3 months after operation (P < 0.05). Conclusion: The concept of rapid rehabilitation nursing can stabilize the immune function of patients after radical breast cancer surgery, promote the postoperative rehabilitation and improve the quality of life after operation.

Abstract Objective The aim of this study was to explore the safe and effective method of expectoration in the preoperative period of patients with lung cancer resection and to promote the rehabilitation of patients. Methods A total of 100 cases of lung cancer patients undergoing elective surgery were divided into the observation group and the control group, with 50 cases in each group. The control group was treated with vibration expectoration vest for expectoration during the perioperative period, and the observation group was treated with respiratory function exerciser that has expectoration function in the perioperative period, three times a day, and the effect was evaluated after 5 days. Results The number of patients in the observation group after the first expectoration time was significantly less than that of the control group (P<0.001). Pain score, pulmonary atelectasis, and pulmonary infection rate of the observation group were significantly lower than those of the control group; the hospitalization time was significantly shorter than that of the control group; and the difference was statistically significant (P<0.05). Conclusions Lobectomy for lung cancer patients with perioperative respiratory training for respiratory function exercise, compared with conventional methods, is helpful for postoperative expectoration and to reduce the incidence of adverse events.

Objective: To explore the effect of respiratory rehabilitation training on the quality of life of pneumoconiosis patients. Methods: 76 pneumoconiosis patients who were treated in our hospital from April 2017 to December 2019 were selected as the research object, and randomly divided into 2 groups according to the order of admission by coin tossing, 38 cases in each group. The control group carried out health knowledge education on the basis of conventional treatment, and the observation group combined with respiratory rehabilitation training on the basis of the control group to compare the quality of life and lung function of the two groups of patients. Results: After 2 months of nursing care, scores of GQOLI-74 scale and pulmonary ventilation function indexes in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). Conclusion: Respiratory rehabilitation training can improve pulmonary ventilation function of pneumoconiosis patients, improve the quality of life of patients, has good clinical application value.

Objective: To explore the effectiveness of pain nursing interventions for patients in ENT. Methods: The 200 ENT patients admitted from March 2018 to March 2020 were divided into observation group (n=100) and control group (n=100). The control group was used conventional clinical nursing methods, and the observation group received pain nursing intervention as a comparison with the control group. Results: The observation group had 83 cases, 14 cases and 3 cases of mild, moderate and severe pain, respectively, while the control group are of 43 cases, 43cases and 14 cases. Nursing intervention can significantly reduce the pain of patients after otolaryngology surgery, improve the quality of life of patients, and has high clinical value for application.

Objective: To investigate the clinical efficacy of insulin glargine combined with acarbose in the treatment of elderly patients with diabetes. Methods: One hundred and forty-four elderly patients with diabetes who received treatment between December 2016 and December 2017 in Binzhou People’s Hospital, China, were selected and divided into a control group and an observation group, 72 each, using random number table. The control group was treated with insulin glargine, while the observation group was treated with insulin glargine combined with acarbose. The therapeutic effect, improvement of quality of life and adverse reactions were compared between the two groups. Results: After treatment, fasting blood glucose (FBG), 2h postprandial blood glucose (PBG) and glycosylated hemoglobin (Hb Alc) of the two groups were lower than those before treatment, and the decrease degree of the observation group was significantly larger than that of the control group (P<0.05). The time needed for blood glucose reaching the standard level and daily insulin dosage of the observation group were significantly lower than that of the control group, and the differences were statistically significant (P<0.05). SF-36 scale score of the observation group was significantly better than the control group, and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: The combination of insulin Glargine and Acarbose can significantly control the blood glucose level of elderly patients with diabetes, improve the biochemical indicators, and enhance the quality of life. <p>Objective: to analyze the effect of health education intervention in clinical nursing of patients with skin diseases. Methods: from January 2018 to March 2019, 122 patients with skin diseases were admitted to our hospital and randomly divided into two groups, with 61 cases in each group. The selected patients gave informed consent to this study. The control group was given routine dermatological care, and the observation group was given systematic health education. The treatment effect, mood score and quality of life score of the two groups were compared. Results: the treatment time, recurrence times and outpatient treatment times of the observation group were lower than those of the control group, the difference was statistically significant (P &lt; 0.05); the HAMA score and HAMD score of the observation group after intervention were lower than the control group, the difference was statistically significant (P &lt; 0.05). Conclusion: the implementation of health education for patients with skin diseases during the nursing process can effectively promote the recovery of the condition and reduce the number of treatments and recurrences. At the same time, it can improve the patients’ bad mental mood and improve the quality of life of the patients.</p>
