Palatally erupted twin mesiodens - A case report with literature review

Mesiodens is the most common supernumerary tooth and is present in the midline between the two central incisors. It occurs mostly due to hyperactivity of the dental lamina. They are usually small, with a cone shaped crown and a short root, may be single or paired, erupted or impacted and occasionally even be inverted. Presence of more than one mesiodens is termed as mesiodentes. Presence of mesiodens may cause impaction or delayed eruption of permanent teeth, malocclusion leading to disturbance in chewing, swallowing and speech, root resorption of the adjacent teeth, impaired dentofacial aesthetics, and sometimes cyst formation. The erupted mesiodens can be easily diagnosed clinically, and the unerupted ones are best diagnosed by clinical and radiological evaluation. Although mesiodens is the most common supernumerary teeth, presence of double mesiodens is uncommon. In this paper we describe a case of palatally erupted double mesiodens and its management in a 20year old girl.

Supernumerary teeth are developmental disorders that can appear in any maxillary región. Whilst their etiology is not well-known, it has been postulated that they are the result of a "doubling" of the dental lamina. Mesiodens can cause several complications, such as the crowding, delayed eruption or displacement of dental organs. Diagnosis is carried out clinically and radiographically. For clinical management is reported in a male patient of 5 years with a mesiodens in the maxilla, who was treated at Hospital Dos de Mayo, odontopediatry service. Intraoral examination revealed a molariform supernumerary tooth eruption, and x-ray analysis showed the presence of an dental organ in the midline. Due to the observed displacement of permanent teeth, it was decided that the supernumerary would be removed in the conventional manner. Se reporta el caso de manejo clínico en un paciente masculino de 5 años de edad con un mesiodens en el maxilar superior, que acudió al Hospital Dos de Mayo, servicio de odontopediatria. A la exploración intraoral se observó un diente supernumerario molariforme semi erupcionado, y el análisis de radiografía mostró la presencia de un órgano dentario en la línea media. Supernumerary teeth may have all characteristics of normal teeth (dens supernumeratus) or, more frequently, appear as rudimentary remains of the tooth bud (dens accessories). Such teeth may cause various complications and the most frequent are orthodontic anomalies and follicular cysts. Among all supernumerary teeth, the most frequent is mesiodens. It may be of different shape, but most often is conical. Untimely diagnosed and treated mesiodens may, in older age, result in complications and compromise prosthodontic rehabilitation. The aim of this study was to present a case where untimely treatment (diagnostics and extraction) of a mesiodens resulted in the formation of a follicular cyst and a partial resorption of the root of an adjacent tooth (central incisor), which had to be extracted. The patient sought a prosthodontic treatment. However, a hazelnut-size tumor was found in the upper anterior region. Clinical and X-ray examination implied the presence of a follicular cyst around a supernumerary tooth, which was situated between upper central incisors. Cystical sac was removed in a usual manner along with the extraction of the impacted mesiodens. Unfortunately, the upper central left incisor had to be extracted as well due to a progressed root resorption.

Objective. This case report presents 3-year follow-up of a case of nonsyndromic multiple supernumerary teeth (NSMST) with 11 supernumerary teeth, 2 of which showed subsequent formation.Case Report. A 10-year-old girl was referred to the dental clinic with the chief complaint of delayed eruption. Radiographic examination showed 9 retained supernumerary teeth. The treatment plan consisted of extraction of the supernumerary teeth and associated primary teeth in order to allow the permanent teeth to erupt. After 2 years of follow-up, 2 additional supernumerary teeth were observed.Conclusion. Regular follow-up for late forming supernumeraries is crucial for NSMST cases.

Internal root resorption (IRR) refers to a slow or rapid progressive resorption process that occurs in the pulp cavity of the tooth or the dentin of the root. IRR occurs as result of odontoclast action; in many cases, the pulp tissue exhibits chronic inflammation, and odontoblasts and predentin do not appear on the dentin wall near the pulp. Exact predisposing factors have not been clearly elucidated; therefore, it is difficult to identify reliable data on the prevalence of IRR because of its scarce occurrence and pathology. Reports have indicated that IRR is more common in the primary than in the permanent teeth. This case report discusses a 17-year-old girl with multiple idiopathic internal root resorptions of anterior permanent teeth in a short period of a time and its management.

In his survey of supernumerary teeth Stafne (1932) saw 131 upper but only 10 lower fourth molars. The following is a report of a single supernumerary tooth in the lower third molar region.

Abstract Background. Supernumerary teeth are described as an excess of the normal teeth number of 20 deciduous and 32 permanent teeth and can occur in any dental region. Material and methods. The authors present the case of a 12-year-old female child with a paramolar in the maxilla, discovered accidentally during an orthopantomogram, emphasising the treatment modality and the complications that can appear. Conclusion. Supernumerary teeth can be present in any region of the oral cavity. Both practitioners and clinicians should be aware of the various types of paramolars and make a treatment plan after an accurate clinical and radiographic examination.

Supernumerary teeth in the premolar region, unlike other supernumeraries, occur more often in the mandible where they are generally of the supplemental type. Occasionally, they are conical or smaller than normal, particularly in the upper premolar regions. They might occur singly or in multiples, be erupted or impacted, but the majority have been found to be unerupted and asymptomatic. The prevalence of supernumerary teeth in the premolar region has been demonstrated to be between 0.01 and 1 percent depending on the population studied. Interestingly, populations from the East or Africa are known to be the most affected. Various theories have been suggested to explain the etiology of supernumerary teeth in general including both genetic and environmental factors. Furthermore, it has been suggested that supernumerary premolar teeth belong to a third (postpermanent) series, developing from extensions of the dental lamina. Several consequences can result from the presence of supernumerary premolars, especially in the mandible, such as cyst formation, transposition, and other clinical scenarios.

Supernumerary teeth can develop in any location of the mandible or maxilla and may have a significant impact on the developing dentition. This paper reviews the prevalence, aetiology and classification of supernumerary teeth. It also describes their clinical characteristics and management options. A case report involving the interdisciplinary management of delayed eruption of central incisors due to the presence of a supernumerary tooth is outlined. CPD/Clinical Relevance: Timely diagnosis and appropriate management may reduce the potential problems associated with supernumerary teeth. General dental practitioners should be aware of the clinical characteristics and management options related to supernumerary teeth.

ABSTRACTSupernumerary teeth occur frequently in permanent dentition, but they are rarely found in primary dentition. Supernumerary teeth of orthodox shape and size that resemble normal dentition are called ‘supplemental teeth’. Supplemental teeth are less common than supernumerary teeth and are often overlooked because of their normal shape and size. Supplemental teeth may cause esthetic problems, delayed eruption and crowding, and they require early diagnosis and treatment to prevent complications. The case reported here is one of bilateral supplemental teeth impeding the eruption of permanent maxillary lateral incisors, and it emphasizes the importance of early diagnosis and treatment during early mixed dentition. Background: Supernumerary teeth are one of the most common anomalies in the human dentition, found most frequently in the maxillary anterior region causing impaction or displacement of the adjacent permanent teeth. Aim :The purpose of this retrospective study was to determine the prevalence and characteristics of supernumerary teeth in orthodontically treated patients. Study design: Pre-treatment facial and intraoral photographs, study models, panoramic and periapical radiographs of 3,000 consecutively treated orthodontic patients (mean age 12.2 years) from the Department of Orthodontics Tel Aviv University, were examined to detect supernumerary teeth in both arches. They were recorded according to gender, age, number, location, position and morphology. Results: Thirty-six patients, 22 (61%) males and 14 (39%) females with 50 supernumerary teeth, of which 42 (84%) were found in the maxillary anterior region, and 8 (16%) in the mandible, presenting a prevalence of 1.2%. Conclusions: A prevalence of 1.2% was found in our study. The most common supernumerary tooth is mesiodens located at the maxillary anterior region. The characteristics of supernumeraries were based on their morphology, location and position. The most frequent complications caused were rotations, displacement and arrested eruption of maxillary incisors.
