Effective Alarm Management System for Process Safety and Plant Reliability

Poorly managed alarms and improper design of alarm system have contribute to major industrial incident. Some examples are fire explosion on Milford Haven, 24th July 1994 where it flooded by alarms at the rate of one every two or three seconds resulting operator cancelling those alarms due to the nuisance (Health and Safety Executive, 1997), other case was from Texas City Refinery explosion, 23rd March 2005 where the high level alarm was ignore while the secondary high alarm was faulty (BP Executive Summary, 2005), and recent incident of Fatal Gas Well Blowout in Pryor Trust gas well Pittsburgh Country 22nd January 2018 where the entire alarm system was disabled by rig personnel due to nuisance condition (U.S. Chemical Safety and Hazard Investigation Board,2019). Banyu Urip facility have put significant effort on improving alarm effectiveness. This paper describes Banyu Urip alarm management workflow and improvement consisting of data collection, alarm analysis (identify bad actor), short term repair/improvement, alarm review & rationalization, management of change process, re-evaluation of operating envelopes, and high focus for implementation and stewardship. Alarm management tools and dashboard significantly helps the process. There are 4 KPIs that periodically reviewed: number of alarm rate, standing alarm, shelved alarm and critical operating parameter. This paper also provide Banyu Urip plant alarm management journey that started by first cold eyes review found imbalance of 3 panel operator load. Reconfigure asset assignment significantly improve the alarm rate as well as plant stability. The journey continue by multiple approach depend on the case of alarm such as nuisance alarm and standing alarm consisting alarm rationalization, alarm suppression logic, equipment out Of service logic, time delay implementation, control loop tuning, and deadband setting. Other than that, manual shelving process also utilized for nuisance alarms short term action caused by equipment or sensor problem, as long as mitigation exists, communicated on every shift handover and directly followed up to maintenance order list. There are also some cases where known equipment issues waiting for long term resolution causing continuous alarms. Risk screening process help all parties aware on incremental risk during the interim period. Having above alarm management improvement workflow, CPF site team have able to reduce down the alarm rate to stable criteria (<12 / hours) and standing alarm continue to decrease to 10/console. Finally, this paper also describes how alarm management practices can be also utilized as supplementary surveillance dashboard to improve plant reliability

This study aims to explore the financial management practices ofsmall-and-medium-enterprises (SMEs) in the Greater Jakarta (Jabodetabek). We investigate into 3 SME cases by conducting the semi-structured interviews with the owner-managers and using direct observations to know the practices of financial management of SMEs. Through the research, we have found six propositions related to the practice of short-term financial management. They apply bootstraps to ensure availability of working capital. They set aside cash reserves from retained earnings and minimize loans from financial institutions. They have the computerized system to track receivables facilitating working capital needs. They keep theirinventory control efficient to manage working capital. They screen customers using transactional records and reputations to minimize the risk of bad debts.

In Ethiopia, the practice of land management started three decades ago in order to address the problem of land degradation and to further boost agricultural production. However, the impact of land management practices in curbing land degradation problems and improving the productivity of the agricultural sector is insignificant. Various empirical works have previously identified the determinants of the adoption rate of land management practices. However, the sustainability of land management practices after adoption, and the various factors that control the sustainability of implemented land management practices, are not well addressed. This study analyzed the factors affecting the sustainability of land management practices after implementation in Mecha Woreda, northwestern Ethiopia. The study used 378 sample respondents, selected by a systematic random sampling technique. Binary logistic regression was used to analyze the quantitative data, while the qualitative data were qualitatively and concurrently analyzed with the quantitative data. The sustained supply of fodder from the implemented land management practices, as well as improved cattle breed, increases the sustainability of the implemented land management practices. While lack of agreement in the community, lack of enforcing community bylaws, open cattle grazing, lack of benefits of implemented land management practices, acting as barrier for farming practices, poor participation of household heads during planning and decision-making processes, as well as the lack of short-term benefits, reduce the sustainability of the implemented land management practices. Thus, it is better to allow for the full participation of household heads in planning and decision-making processes to bring practical and visible results in land management practices. In addition, recognizing short-term benefits to compensate the land lost in constructing land management structures must be the strategy in land management practices. Finally, reducing the number of cattle and practicing stall feeding is helpful both for the sustainability of land management practices and the productivity of cattle. In line with this, fast-growing fodder grass species have to be introduced for household heads to grow on land management structures and communal grazing fields for stall feeding.

The short- and medium—long-term effects of management and hillside position on soil organic carbon (SOC) changes were studied in a centenary Mediterranean rainfed olive grove. One way to measure these changes is to analyze the soil quality, as it assesses soil degradation degree and attempts to identify management practices for sustainable soil use. In this context, the SOC stratification index (SR-COS) is one of the best indicators of soil quality to assess the degradation degree from SOC content without analyzing other soil properties. The SR-SOC was calculated in soil profiles (horizon-by-horizon) to identify the best soil management practices for sustainable use. The following time periods and soil management combinations were tested: (i) in the medium‒long-term (17 years) from conventional tillage (CT) to no-tillage (NT), (ii) in the short-term (2 years) from CT to no-tillage with cover crops (NT-CC), and (iii) the effect in the short-term (from CT to NT-CC) of different topographic positions along a hillside. The results indicate that the SR-SOC increased with depth for all management practices. The SR-SOC ranged from 1.21 to 1.73 in CT0, from 1.48 to 3.01 in CT1, from 1.15 to 2.48 in CT2, from 1.22 to 2.39 in NT-CC and from 0.98 to 4.16 in NT; therefore, the soil quality from the SR-SOC index was not directly linked to the increase or loss of SOC along the soil profile. This demonstrates the time-variability of SR-SOC and that NT improves soil quality in the long-term.

Abstract A statistical analysis of the foreshock-main shock correlation for a seismically active region in Italy is presented. It is found that the probability that a weak shock be followed within 2 days by a main shock is of the order of 2 per cent, while the probability that a main shock be preceded by a foreshock is of the order of 50 per cent. These results are quite similar to those found by L. Jones (1985) for southern California. The effectiveness of alarm systems based on a pair of short-term earthquake precursors is then analyzed. In particular, the analysis shows under what conditions the precursor, consisting of potential foreshocks, could be combined with another precursor to provide a reasonably effective alarm system.

Occupational Safety and Health Experts in Indonesia have an important role in integrating environmental health and safety factors, including in this regard as human resources assigned to undertake hazardous waste management. Comprehensive knowledge and competence skills need to be carried out responsibly, as an inherent professional occupational safety and health profession. Management leaders should continue to provide training in external agencies responsible for science in the management of toxic waste to enable occupational safety and health experts to improve their performance in the hierarchy of control over the presence of hazardous materials. This paper provides an overview of what strategies and competencies the Occupational Safety and Health expert needs to have in embracing hazardous waste management practices.

In a context of changing demographics and transformations to the world of work, concerns about age management are gradually turning into concerns about knowledge management. The vast experiential knowledge and diverse skills developed by workers to cope with the numerous situations encountered in the course of their work and to protect themselves against risks to their health and safety constitute part of the intangible assets vital to the sustainability of worker expertise and even the survival of the organization. Management practices play an important role in helping safeguard experiential knowledge in organizations. However, the transformations that have been taking place in recent years in response to an unstable economic climate have driven organizations to introduce a number of changes in workplaces. Three case studies, conducted in Quebec, each focused on the study of a specific occupation (film technicians, food service helpers, and homecare nurses), and based on interviews and observations made in the field, will be presented in an effort to describe the impact of some of these changes, namely precarious employment, flexible management practices and work intensification, on knowledge sharing in real work situations. The results suggest that by undermining work teams and increasing the workload of experienced workers, these changes actually hinder the knowledge sharing process. In fact, in such a context, the work teams are continually being reconfigured, which can demotivate experienced workers who constantly have to initiate new recruits despite already having a work overload. Possible avenues for research are proposed with a view to helping organizations cope with these changes in a way that supports the experiential knowledge transfer and sharing process so vital to organizational performance and the preservation of worker health.

Limited information exists on the effects of forest management practices on bryophytes, despite their importance to forest ecosystems. We examined short-term responses of ground-layer bryophytes to logging disturbance and creation of edges in mature Pseudotsuga forests of western Washington (USA). The abundance and richness of species were measured in four 1-ha forest aggregates (patches of intact forest) and in surrounding logged areas before and after structural retention harvests. One year after treatment, species richness, total cover, and frequency of most moss and liverwort taxa declined within harvest areas. Within forest aggregates, mosses did not show significant edge effects; however, richness and abundance of liverworts declined with proximity to the aggregate edge. Our results suggest that, over short time frames, 1-ha-sized aggregates are sufficient to maintain most common mosses through structural retention harvests but are not large enough to prevent declines or losses of liverworts. Thus, current standards for structural retention, which allow for aggregates as small as 0.2 ha, may be inadequate to retain the diversity and abundance of species found in mature, undisturbed forests.Key words: bryophyte, edge effects, forest borders, forest management, logging effects, structural retention harvest.

Seeing the good opportunity to study financial systems and practices of VJ Engineers, it is relatively important to take up assignment on ‘WORKING CAPITAL MANAGEMENT IN VJ ENGINEERS’. During the project work, it is being analyzed the working capital position of this organization. [1],[ 3],[5] Decisions relating to working capital and short term financing are referred to as working capital management. These involve managing the relationship between a firm's short-term assets and its short-term liabilities. The goal of Working capital management is to ensure that the firm is able to continue its operations and that it has sufficient money flow to satisfy both maturing short-term debt and upcoming operational expenses.The study of working capital management is very helpful for the organisation to know its liquidity position. The study is relevant to the organization to know the day to day expenditure. This study is relevant to give an idea to utilise the current assets.This study is also relevant to the student as they can use it as a reference. This report will help in conducting further research. Other researcher can use this project as secondary data uncovering of PDA incorporation in effects on police reports.
