Introduction: The ideal treatment for breast cancer should be carried out by a multidisciplinary team, aiming at the global approach of the patient. It is usually a prolonged process and can cause relevant changes in the lives of patients, such as limited shoulder movement, pain, lymphedema, adherence scarring, sensitive changes, phlebitis, among others. Physical therapy intervention plays an essential role and can prevent and minimize complications, besides enabling functional recovery. It is extremely important that the physical therapy service is aware of the prevalence of the main physical and functional complications and the impact of its intervention. Objective: To assess the main physical and functional complications in the post-surgical treatment of breast cancer. Methods: This is a cross-sectional study in which we analyzed the medical records of patients treated in the Physical Therapy Ambulatory from 2011 to 2016. We obtained data about the surgery, prevalence of pain, changes in perimeter, movement amplitude, evaluation of scapulohumeral rhythm and presence of axillary cording. Results: We assessed the data of 703 patients, whose mean age was 56 years; 50.1% were married, and body mass index pointed to overweight (mean: 26.8kg/m²). Sedentary lifestyle was prevalent (73.7%). The main surgery performed was quadrantectomy, followed by mastectomy. Pain was present, however, in extremely low levels (mean of pain in the Visual Analog Scale on the seventh postoperative day was 2.55); movement amplitude restrictions were higher in the beginning, and functionally reversed during follow-up; scapular dysfunction is also little prevalent (9.9%), with high percentage of long term resolution. The incidence of axillary cording was high (87.9%), which requires special attention. Conclusion: The study allowed to observe the main physical and functional complications after breast cancer treatment, and show the importance and necessity of early physical therapy.

Extensive research has demonstrated the factors that influence burnout among social service employees, yet few studies have explored burnout among long-term care staff in Hawaii. This study aimed to examine the impact of job value, job maintenance, and social support on burnout of staff in long-term care settings in Hawaii, USA. This cross-sectional study included 170 long-term care staff, aged 20 to 75 years, in Hawaii. Hierarchical regression was employed to explore the relationships between the key independent variables and burnout. The results indicate that staff with a higher level of perceived job value, those who expressed a willingness to continue working in the same job, and those with strong social support from supervisors or peers are less likely to experience burnout. Interventions aimed at decreasing the level of burnout among long-term care staff in Hawaii may be more effective through culturally tailored programs aimed to increase the levels of job value, job maintenance, and social support.

BACKGROUND Physical activity is a first-line therapy and secures against persistent illnesses. Essential medical care professionals are obviously situated to advance actual action. Active recuperation mediations focused on advancement and upkeep of wellbeing, personal satisfaction, and wellness. There is, notwithstanding, a deficiency of such examination proof in Ethiopia. OBJECTIVE We planned to assess the degree of KAP of nonprofessional local physical therapists' towards wellbeing advancement in Arsi zone of Oromia, Southeast Ethiopia. METHODS Community based cross sectional study was conducted in April 2018 among 45 physical therapists. Data were collected using pre-tested, structured and self-administrative questionnaires. Participants were selected by quota sampling technique. The questionnaire was drafted specifically to test the KAP. Data were double entered and analyzed by SPSS Version 20.0 program. Descriptive statistics were used. The KAP were estimated using proportion. RESULTS In present response rate is 90.0%. Ages of respondents ranged between 24 – 87 years. 57.8% of participants got initial physical therapy knowledge from either of their parents. 53.3% of the respondents hear about health promotion from families and friends. 57.8% of them provided health promotion at their workplace. The overall percentage of all the respondents’ KAP in health promotion was 60.1%. CONCLUSIONS The respondents’ have good KAP towards health promotion. However there is still room for improvement. Also there is a lack of proper guide lines in determining the impact physical therapy.

Abstract Objectives Given the influence of sensory signals on food intake regulation and the subsequent nutritional status, it is important to reduce the impact of commonly observed smell and taste alterations on the pleasure of eating during chemotherapy. This study aimed to examine the effect of various types of flavor enhancement on the appreciation of an appetizer by a group of patients undergoing chemotherapy. Methods Five eggplant appetizers with no seasoning (REF) or enhanced with either salt, lemon, garlic or cumin were developed. In this cross-sectional study, 36 healthy female subjects (age 58 ± 7 y) (control group) and 84 breast cancer patients (age 56 ± 11 y) treated with chemotherapy were recruited. A hierarchical cluster analysis based on patients' self-reported smell and taste abilities classified the patients in two groups: the ‘unaltered, (high sensory abilities) n = 49’ and the ‘altered, (poor sensory abilities), n = 35’ group. Participants tasted and assessed the appreciation of the reference appetizer on a visual analog scale and performed a randomized comparative liking task, by rating the appreciation for each flavor-enhanced appetizer relative to the REF (one-sample t-test). Results There was no significant difference in the appreciation of the REF appetizer between the three groups (P > 0.05). In the comparative liking task, the control group rated similarly the appreciation between the flavor-enhanced and the REF appetizers except for the cumin-enhanced appetizer, which was appreciated significantly more than the REF (P < 0.001). Patients in both ‘altered’ and ‘unaltered’ groups appreciated similarly the cumin appetizer (P > 0.05), more the salt and garlic-enhanced appetizers (P ≤ 0.001, P = 0.005, respectively) and significantly less the lemon-enhanced appetizer relative to the REF (P = 0.029), confirming a tendency for patients to reject acid-flavored foods. Conclusions The salt and garlic enhancements were appreciated by the patients independent of the stage of chemotherapy or the severity of smell and taste alterations. Flavor enhancement seems a good strategy to maintain the pleasure of eating in patients undergoing chemotherapy. Further research is still needed on the appreciation and intake of a complete meal (starter, main course, dessert). Funding Sources APICIL and ELIOR groups.

Abstract Background: Brazil has been going through a period of economic crisis since 2015 and the impact on health indicators and the quality of life of the most recent crisis is unclear. This study sought to establish a relationship between the impact of the economic crisis experienced by Brazil during the period 2013-2016 and the lifestyle of adult populations in the 27 capitals of the Brazilian state.Results: According to the binary logistic regression model, at least completing secondary education increases the probability of individuals presenting higher LS by 4.9% (OR=1,049, 95% CI 1,029–1,070); being involved in some type of relationship decreases the probability of presenting a higher LS by 5.3% (OR=0.947, 95% CI 0.029-0.965); having an adequate body mass index increases LS by 27.3% (OR=1.273, 95% CI 1,245.10.10). Living in a city with a lower unemployment rate increases the probability of having a higher LS score by 3.8%(OR=1,038, 95% CI 1,012-1.064); living in a city with a more basic food basket decreases the probability of having a higher LS score by 5.4% (OR=0.946, 95% CI 0.910-0.982). Presenting higher yields decreases the probability of having a higher LS by 12% (OR=0.880, 95% CI 0.860-0.900). In addition, there was an increase in the percentage of leisure-time PA in 2015 (20.9%), followed by a decrease to 18.1% in 2016, after a stabilization period in 2013 (19.5%) and 2014 (19.6%). The physical exercise showed a steady annual increase, with a score of 49.6% in 2013, followed by 50.9% and 51.9% in 2014 and 2015, respectively, and ending with 54.5% in 2016. The "Watch TV 5x per week" indicator recorded a sharp drop between 2013 (27.9%) and 2015 (22.7%), as well as the consumption of soft drinks, which went from stable values in 2013 (19.5%) and 2014 (19%) 17.3% in 2015.Conclusion: In the case of Brazil, the economic crisis initially started in 2015 had a negative impact on the population over time, but these effects were only observed in the second year of crisis, being preceded by a temporary improvement of indicators.

Abstract Introduction Protecting the wellbeing of staff in the NHS is becoming ever more critical as we progress through this pandemic. We sought to identify the impact of the pandemic and changes in working pattern on wellbeing of doctors in training in a teaching hospital. Method Cross sectional study, with primary data collection using a web-based survey. Questionnaire was designed to identify issues relating to the personal wellbeing before and since the onset of the pandemic (Nov 2019-Feb 2020 vs March 2020- June 2020). Trainees in all surgical specialities were invited to participate. Results Thirty-six doctors responded. Across the timepoints there was a marked increase in reports of the following: constant fatigue (+500%), headaches (250%), loss of motivation (+500%), loss of interest in hobbies/friends (+566%), anger (+500%), loss of focus (+320%) and heightened emotional state (+700%). Almost 70% reported difficulty sleeping in the preceding month, with only 50% feeling they received adequate rest between shifts. Feelings of ‘too tired to drive’ increased by 216% with accidents or near misses increasing by 500%. Use of alcohol as coping mechanism increased by 500%, and comfort eating rose by 244%. 50% considered leaving their post. Conclusions These findings are significant and point to a dangerous trend if left unchecked. The causes of these outcomes are multi-factorial; changes to working patterns, cancellation of leave, redeployment, and anxieties around career progression were noted. The long-term health implications on the workforce should not be overlooked. This requires a collective response and action by employers, training and regulatory bodies and government.

Abstract Purpose Up to 26% of residents in nursing homes (NHs) are affected by cancer. Their care represents a challenge, because NHs are not usually considered a setting focused on oncologic management and care. The aim of this paper is to describe socio-demographic and clinical features of patients with cancer residing in European NHs. Methods Cross-sectional study based on data from the Services and Health for Elderly in Long TERm care (SHELTER) study. Participants were assessed through the interRAI-LTCF, which includes cancer assessment. Results Among 4140 participants (mean age 83.4 years; female 73%), 442 (10.7%) had cancer. Patients with cancer had a higher prevalence of do-not-resuscitate directives compared to those without cancer (21.1% vs 16.5%, p = 0.019). Variables directly associated with cancer were male sex (adj OR 1.67, 95% CI 1.36–2.05), pain (adj OR 1.43, 95% CI 1.16–1.77), fatigue (adj OR 1.25, 95% CI 1.01–1.55), polypharmacy (adj OR 1.59, 95% CI 1.21–2.08) and falls (adj OR. 1.30, 95% CI 1.01–1.67). Dementia was inversely associated with cancer (adj OR 0.74, 95% CI 0.58–0.94). Symptomatic drugs such as opioids (23.5% vs 12.2, p < .001), NSAIDS (7.2% vs 3.9%, p = 0.001), antidepressants (39.1% vs 33.8%, p = 0.026) and benzodiazepines (40.3% vs 34.3, p = 0.012) were all prescribed more in participants with cancer compared to those without cancer. Conclusions Cancer patients are prevalent in European NHs and they show peculiar characteristics. Studies are needed to evaluate the impact of a supportive care approach on the management of NHs residents with cancer throughout all its phases, until the end-of-life care

Diabetes mellitus is a comorbid condition that is commonly seen in Indian population. Breast cancer is the second most common cancer among women. This can occur due to modifiable and non-modifiable risk factors. This study aims to analyses about the impact of diabetes mellitus on breast cancer outcomes. This study is a hospital record based descriptive cross – sectional study. Ethical clearance for the study was obtained from the institutional review board. All the patients diagnosed and on treatment for carcinoma breast in the year 2020 in the hospital was taken into consideration. Details about their age, stage at the time of diagnosis, presence of diabetes, their outcome at the end of study period was collected. The data were analysed with the help of a statistician. Diabetic patients had a slight chance of later stage of presentation as well as outcome in comparison with the non-diabetic counterparts. So, Care should be taken in diagnosing by screening and treating the diabetic patients with breast cancer.
