Background: Paediatric patients with transfusion dependent thalassemia require regular lifelong red cell transfusions. Blood transfusions increase the risk of iron overload, which can lead to cardiac hemosiderosis. Serum ferritin can be a parameter for evaluating systemic hemosiderosis. Objective: To evaluate the correlation between serum ferritin levels and impaired heart function in children with transfusion dependent thalassemia Methods: A cross sectional study was conducted with transfusion-dependent thalassemia who attended a cardiology and hemato-oncology outpatient clinic from January to December 2018 and had undergone more than 10 transfusion periods. Serum ferritin levels were taken from the mean of 3 measurements before echocardiography. Echocardiography examination was performed by a cardiologist using a 3D Doppler tool to evaluate ejection fraction (EF), tricuspid annular plane systolic excursion (TAPSE) and E / A ratio by calculating the initial diastole (E wave) and atrial contraction (wave A). Data analysis used the Spearman correlation with p <0.05. Results: There were 45 children with a median age of 10 (2-17) years and had received a median of 18 (10-51) blood transfusions. The mean serum ferritin level was 4,321 (1,168-15,233) ng / mL. Blood transfusion frequency was associated with an increase in serum ferritin (rho 0.74, P <0.005). From echocardiography examination, approximately 3/45 children had a feature of dilated cardiomyopathy. The mean value of EF 70 (SD 14.32), TAPSE 1.97 (SD 0.57) cm and an E / A ratio 1.68 (SD 0.46). Serum ferritin was negatively correlated with the ejection fraction (rho = -0.78, P <0.001), TAPSE (rho = -0.65, P <0.001) but positively correlated with the E / A ratio (rho = 0.67, P <0.001). Conclusion: Paediatric patients with TDT have decreased cardiac systolic and diastolic function, and serum ferritin correlates with the decreased cardiac function.

Background: Sepsis still causes morbidity and mortality in children admitted to the pediatric intensive care unit (PICU). Sepsis induces myocardial dysfunction and causes a reversible decline in ejection fraction (EF) of ventricles. Many biomarkers have been described for diagnosing sepsis, including serum ferritin and C-reactive protein (CRP). Objectives: This study was conducted to assess the relationship of cardiac dysfunction evaluated using echocardiogram, ferritin, and CRP with negative outcomes of sepsis in the PICU. Methods: A cross-sectional study was conducted on 80 patients aged between one month and six years who fulfilled the following criteria: (1) confirmed diagnosis of sepsis according to the American College of Critical Care Medicine; (2) receiving ventilation for 48 h and/or vasoactive medicines. The CRP and ferritin levels were recorded on the first day (D1) and third day (D3) of hospitalization in the PICU. Participants underwent an echocardiography study to investigate the ejection fraction on D1 and D3. All outcomes were evaluated. Results: Our results showed a highly statistically significant difference between D1 and D3 in ejection fraction (P = 0.001). The serum ferritin level and CRP enhanced significantly from D1 to D3 (P < 0.001). Low left ventricular ejection fraction, and high serum ferritin were associated with unfavorable outcomes (P values < 0.001 and 0.021, respectively), but there was no significant difference in the outcomes regarding CRP. Conclusions: Cardiac dysfunction and high serum ferritin were associated with unfavorable outcomes in children with sepsis admitted to the PICU.

The aim of the study was to determine serum ferritin level in transfusion dependent thalassaemia patients. A total of 64 transfusion dependent thalassaemia (TDT) patients was included in this cross sectional study from April, 2018 to September, 2019 according to selection criteria. Body iron load was estimated by serum ferritin level.  The study sample consisted of 43 male and 21 female TDT patients, with a mean age of 25.5 years. Most of the patients (43.8%) patients had serum ferritin level in between 1000-2500 ng/ml. 12 patients (18.7%) had serum ferritin level >5000 ng/ml and 3 patients (4.7%) had serum ferritin in between 2501-5000 ng/ml. The mean serum ferritin was found 2462.6 ± 2792.7ng/ml with range from 207.0 -11891.2ng/m.

Objective: To evaluate the frequency of use of oral contraceptives and other risk factors among women with ischemic stroke seen at a hospital in the northern region of the state of Rio Grande do Sul. Methods: Prospective, cross-sectional study. The population was characterized by patients admitted with a diagnosis of ischemic stroke during a period from March to August 2019 and the sample by female patients found in the population. The data were collected through an individual interview using a questionnaire developed by the researchers and tabulated for further analysis. All analyzes were performed using the statistical program Bioestat 5.0, considering p = 0.05. Results: In the characterization of the sample, an average age of 68.2 years was observed. The occurrence of gender was 52.4% for women. The mean hospital stay was 12.2 ± 8.91 days. Outcome of hospital discharge was 89.1%. The mean value of body mass index was 25 ± 4.89 kg / m², with an average of 27.4 kg / m² in patients who used oral contraceptives and 25.23 kg / m² in patients who did not use oral contraceptives. The sample had a 34.5%incidence of oral contraceptive use. The average age of those who used oral contraceptives was 62.4 years and of the patients who did not use it was 70.3 years. Conclusion: The study showed a high use of oral contraceptives in the sample, with percentage values very close to other factors evaluated, thus showing itself as a probable risk factor for the development of ischemic stroke.

Objectif : Décrire les aspects épidémiologique, clinique et pronostique associés aux étiologies des hémorragies digestives hautes au CHU Campus de Lomé Patients et méthode: Etude transversale à collecte rétrospective, à visée descriptive et analytique menée du 1er Janvier 2014 au 31 Décembre 2019. Le seuil de significativité était retenu pour p<0,05. Résultats: Deux cent cinquante et un patients avaient été inclus. L’hémorragie était d’origine hypertensive portale chez 69 patients (27,71%) ; ulcéreuse gastro-duodénale chez 100 patients (39,84%). Chez 25 patients (9,96%), la fibroscopie oesogastroduodénale était normale. Une rupture de varices oesophagiennes était retrouvée chez 98,55% des patients présentant une hypertension portale. Les ulcères gastroduodénaux représentaient 54,94% des hémorragies digestives hautes d’origine non hypertensive portale. La valeur moyenne du score de Rockall était de 4(±1) chez les patients présentant une hémorragie d’origine hypertensive et de 3(±1) chez les patients avec hémorragie non hypertensive portale (p<0,001). La valeur moyenne du score de Glasgow-Blatchford était de 10(±3) chez les patients présentant une hémorragie d’origine hypertensive et 9(±3) chez les patients avec hémorragie digestive haute d’origine non hypertensive (p<0,001). La récidive hémorragique et le décès étaient survenus chez les patients présentant un saignement d’origine hypertensive portale dans respectivement 54,84% (p<0,001) et 71,42% (p<0,001). Conclusion: Les lésions inflammatoires aiguës et chroniques représentent la première étiologie des hémorragies digestives hautes dans le service d’Hépato-gastroentérologie du CHU Campus. Ces hémorragies sont associées à la prise de médicaments gastrotoxiques et à un moindre risque de récidive hémorragique et de décès. Objective: To describe epidemiological et prognostic outcomes associated with etiologies upper gastrointestinal bleeding in Campus Teaching Hospital of Lome Patients and method: Cross-sectional study with retrospective collection, descriptive and analytical aim carried out from January 1, 2014 to December 31, 2019. Results: Two hundred and one patients were included. The hemorrhage was of portal hypertensive origin in 69 patients (27.71%); peptic ulcer in 100 patients (39.84%). In 25 patients (9.96%), the oesogastroduodenal fibroscopy was normal. Ruptured esophageal varices were found in 98.55% of patients with portal hypertension. Peptic ulcers accounted for 54.94% of upper GI bleeding of non-portal hypertensive origin. The mean value of the Rockall score was 4(±1) in patients with hemorrhage of hypertensive origin and 3(±1) in patients with non-portal hypertensive hemorrhage (p<0.001). The mean Glasgow-Blatchford score was 10(±3) in patients with hemorrhage of hypertensive origin and 9(±3) in patients with upper GI hemorrhage of nonhypertensive origin (p<0.001). Hemorrhagic recurrence and death occurred in patients with bleeding of hypertensive origin in 54.84% (p<0.001) and 71.42% (p<0.001) respectively. Conclusion: Acute and chronic inflammatory lesions represent the first etiology of upper GI bleeding in the Gastroenterology Department of the Campus Teaching Hospital of Lome. These hemorrhages are associated with the use of gastrotoxic drugs and with a lower risk of recurrence of hemorrhage and death.

Abstract Background: HAART has been reported to be associated with a number of side effects in human immunodeficiency virus patients among which dyslipidemia is a common metabolic disorder. Methods: A facility based comparative cross-sectional study among 228 HIV positive persons was conducted from July to August 2020. Socio-demographic and clinical data were collected using structured questionnaires. Fasting venous blood sample was drawn for Lipid profiles and CD4 cell determination. Anthropometric measurement was done. Data was analyzed using SPSS version 22 for windows. Result: A total of 228 HIV patients were enrolled in the study. Prevalence of dyslipidemia in HAART naive and on HAART HIV positive persons was 61(53.5%) and 84 (73.7%), respectively. The prevalence of TC≥200 mg/dl was 50% and 30%; HLD-c<40 mg/dl was 43.8% and 36%; LDL-c≥130mg/dl was 48.3% and 28.1%; and TG≥150 mg/dl 59.6% and 39% among on HAART and HAART naïve, respectively. Age greater than 40 years (AOR = 3.27, 95% C.I: 1.47 - 7.25), blood pressure ≥ 140/90 (AOR = 16.13, 95% C.I: 5.81 - 44.75), being on HAART (AOR = 2.73, 95 % C.I: 1.35 - 5.53) and body mass index > 25kg/m2 (AOR = 1.92, 95 % C.I: 1.20 - 4.81) were identified as determinants of dyslipidemia. Conclusion: The mean value of lipid profile was significantly higher among HIV positive clients on HAART as compared to those HAART naïve HIV positive clients.

Objective: To assess factors associated with the average number of dental caries in pregnant women. Method: Basic research design: This cross-sectional study was performed between January 2017 and December 2018 in Santa Maria, Brazil. Clinical setting and participants: Multistage random sampling resulted in the recruitment of 256 pregnant women from public health centers across the city. Clinical exams and semi-structured questionnaires including demographic, socioeconomic and behavioral questions were performed by trained interviewers. Main outcome measure: The experience of dental caries was evaluated through the mean value of the Decay, Missing, and Filled Surface Index (DMFS) by 4 trained and calibrated examiners. Multilevel Poisson regression models were used to evaluate the influence of different variables on the average number of dental caries. Rate ratio (RR) and respective 95% confidence intervals were calculated (CI 95%). Results: The prevalence of untreated dental caries was 62.7% in the sample, while the mean DMFS index was 10.27 (± 10.92). Women who smoked during pregnancy had a higher mean DMFS (RR 1.41; 95% CI 1.25-1.57). Furthermore, pregnant women who had poor self-ratings of oral health had a higher average DMFS (RR 1.21; 95% CI 1.10-1.32). Conclusions: The results showed that older age, ethnicity, fewer years in education and the presence of dental plaque resulted in higher means of DMFS.

Abstract The present study aimed to (a) establish the frequency of consumption of red meat and eggs; (b) determine serum ferritin levels (μg/l); and (c) establish the relationship between serum ferritin and the consumption of red meat and eggs. In Colombia during 2014–2018, an analytical study was conducted in 13 243 Colombian children between the ages of 5 and 17 years, based on cross-sectional data compiled by ENSIN-2015 (Encuesta Nacional de la Situación Nutricional en Colombia-2015) on serum ferritin levels and dietary consumption based on a questionnaire of the frequency of consumption. Using simple and multiple linear regression, with the serum ferritin level as the dependent variable and the frequency of consumption as the main explanatory variable, the crude and adjusted partial regression coefficients (β) between serum ferritin levels and consumption were calculated. The frequency of habitual consumption of red meat was 0⋅49 (95 % CI 0⋅47, 0⋅51) times/d. The frequency of habitual egg consumption was 0⋅76 (95 % CI 0⋅74, 0⋅78) times per d. The mean serum ferritin level in men was 41⋅9 (95 % CI 40⋅6, 43⋅1) μg/l and in women, 35⋅7 (95 % CI 34⋅3, 37⋅7) μg/l (P < 0⋅0001). The adjusted β between the consumption of red meat and eggs and serum ferritin levels were β = 3⋅0 (95 % CI 1⋅2, 4⋅7) and β = 2⋅5 (95 % CI 1⋅0, 3⋅9) for red meat and eggs, respectively. In conclusion, red meat and eggs are determinants of serum ferritin levels in Colombia and, therefore, could be considered public policy options to reduce anaemia and Fe deficiency.

Background: The serum concentrations of ferritin and adiponectin are associated with several metabolic disorders and have been used as predictors of insulin resistance and metabolic syndrome. But there have been no reports demonstrating a direct correlation between serum ferritin and adiponectin levels. We performed this study to evaluate the association between serum ferritin and adiponectin concentrations.Subjects and methods: We evaluated a total of 995 subjects from the Korea Rural Genomic Cohort Study in a population-based cross-sectional study. Extensive clinical and laboratory measurements, including adiponectin and ferritin concentrations, were recorded.Results: Univariate analysis revealed that the serum adiponectin level was correlated with age, sex, BMI, diastolic blood pressure, triglyceride, HDL-cholesterol, fasting blood glucose, fasting insulin, HOMA-IR, hs-CRP, and ferritin. Multivariate analysis demonstrated that the adiponectin concentration was correlated with age, BMI, fasting glucose, hs-CRP, and ferritin. The ferritin concentration was the most powerfully associated with serum adiponectin. In non-diabetic subgroups, the adiponectin level was correlated with BMI, triglyceride, HDL-cholesterol, fasting glucose, and ferritin level in multivariate analysis. In diabetic subgroups, the adiponectin level was correlated with BMI, triglyceride, hs-CRP, and ferritin level in multivariate analysis.Conclusions: The serum adiponectin concentration was correlated with conventional clinical variables, but the most powerfully associated factor was the serum ferritin level.

Objectives: To determine the correlation of Vit D3 levels with serum ferritin in patients with anemia. Study Design: Cross Sectional study. Setting: Department of Pathology, Qazi Hussain Ahmed Medical Complex Nowshera. Period: 5th Jan 2019 to 31st Dec 2020. Material & Methods: Data entered in SPSS 25th version. Descriptive statistics was used for numerical variables. Pearson correlation was used for correlation of Vit D3 with gender. Normality of data was checked by Shapiro wilk test. Mann Whitney U test was used to show the difference of Vit D3 and ferritin levels in gender. Results: The total of 192 anemic patients with Hb<10g/dl as per definition of WHO1 were referred for ferritin level estimation with 142(74%) females and 50(26%) males. Out of total, 47 were also advised with Vit D3 level estimation with 29(61.7%) females and 18(38.3%) males. Mean with standard deviation for age was 30+9.6 years. Mean with standard Error of mean of Vit D3 was (Mean-15.5ng/ml, SE 2.19). Mean with standard Error of mean of serum ferritin was (Mean-48.2ng/ml, SE 5.90). We observed 35(74.5%) cases out of 47, as Vit D3 deficient with a count less than 20 ng/ml. We observed that 118 (61.5%) were iron deficient with serum ferritin less than 15ng/ml. Person correlation showed a statistically significant correlation of Vit D3 with ferritin (p= 0.022, r=0.7). Spearman ranked correlation showed a statistically significant correlation between the categories of Vit D3 and ferritin (p=0.022, r=0.7). Mann Whitney U Test showed no significant difference in gender groups for botht the variables (p= 0.86 &p=0.33 respectively) thus retain the null hypotheses. Conclusion: The frequency of deficiency Vit D3<20ng/ml in anemic patients was 74% while that of ferritin<15ng/ml in anemic patients was 61.5%. There is a strong statistically significant correlation of Vit D3 with serum ferritin in anemic patients with Hb<11g/dl.

Background: Subcutaneous Deferoxamineis often not tolerated by patients and its rejection leads to iron overload with its complications. So, other methods with better toleration and reduction of Deferoxamine consumption are necessary. The present study aimed to evaluate the effect of intravenous Deferoxamine infusion during blood transfusion on serum ferritin (SF) in thalassemia major patients.Methods: In a retrospective cross-sectional study, thirty four patients with β-thalassemia major treated with monthly blood transfusion at Bu-Ali hospital in Ardabil city from April 2013 to April 2014, were selected and followed for six months. The mean SF rate and the needed subcutaneous Deferoxamine rates before intervention were considered as baseline. All patients received intravenous Deferoxamine concomitant with their routine monthly blood transfusion for six months. After six months mean values for ferritin, subcutaneous Deferoxamine were compared with baseline values. Collected data were analyzed using t-test and paired t-test by SPSS, version 18.P<0.05 was considered as significant.Results: Compared with baseline, the subcutaneous Deferoxamine rate and ferritin level have been decreased significantly after intervention.Conclusions: Intravenous Deferoxamine concomitant use with routine monthly blood transfusion in thalassemia major patients can lead to decreasing of ferritin level. With this method, patient care could be improved, health care costs and complications of treatment effectively reduced.
