PT. GMF Aeroasia, Tbk is a company engaged in aircraft maintenance services. The aircraft maintenance lead time is regulated according to the agreement set forth in the agreement in the form of a work order, when the maintenance process takes place there is almost always a delay when working on the aircraft, both in terms of physical work or delivery of documents. These problems are several forms of waste, where waste is any activity that does not provide added value. Therefore, the purpose of this study is to identify the type of waste and determine the factors that cause waste in the delivery process of aircraft c-check maintenance documents. The method used in this study used a fishbone chart, cause failure mode effect, failure mode and effects analysis and problem identification and corrective action. Fishbone chart and cause failure mode effect are used to find waste that causes delay in document delivery, then analyzed using failure mode and effects analysis, three causes of the highest delay were found, namely stamps on incomplete documents (RPN 16), waiting for customer approval (RPN 16) and material certificates not available (RPN 12). Corrective Action was prepared as a recommendation to improve the delivery process for aircraft c-check maintenance documents. By knowing the types of waste and the factors causing waste in the delivery process of aircraft c-check maintenance documents at PT. GMF Aeroasia Tbk is expected to reduce waste and fulfill customer satisfaction. Keywords: Lead Time, Waste, Delay Delivery of Documents, Cause Failure Mode Effect Method, Corrective Action

A company should pay attention to operational risk to maximise its efficiency and effectivity.
Identification of failure and the cause and the impact of the failure at milling station is needed to be done so that the sugar production process can run well. Here, we employ a failure mode and effect analysis to identify the failure and its effect. We obtain 22 operational risks with two critical risks. The two critical risks are then used as top event in analysing the cause of the failure. The result from fault tree analysis shows that there are 4 basic events, which are the turbine vibrates, schedule part’s change before failure, high rainfall and the schedule and priority of processing

Failure Mode Effect Analysis (FMEA) is popular and versatile approach applicable to risk assessment and safety improvement of a repairable engineering system. This method encompasses various fields such as manufacturing, healthcare, paper mill, thermal power industry, software industry, services, security etc. in terms of its application. In general, FMEA is based on Risk Priority Number (RPN) score which is found by product of probability of Occurrence (O), Severity of failure (S) and Failure Detection (D). As human judgement is approximate in nature, the accuracy of data obtained from FMEA members depend on degree of subjectivity. The subjective knowledge of members not only contains uncertainty but hesitation too which in turn, affect the results. Fuzzy FMEA considers uncertainty and vagueness of the data/ information obtained from experts. In order to take into account, the hesitation of experts and vague concept, in the present work we propose integrated framework based on Intuitionistic Fuzzy- Failure Mode Effect Analysis (IF-FMEA) and IF-Technique for Order Preference by Similarity to Ideal Solution (IF-TOPSIS) techniques to rank the listed failure causes. Failure cause Fibrizer (FR) was found to be the most critical failure cause with RPN score 0.500. IF-TOPSIS has been implemented within IF-FMEA to compare and verify ranking results obtained by both the IF based approaches. The proposed method was presented with its application for examining the risk assessment of cutting system in sugar mill industry situated in western Uttar Pradesh province of India. The result would be useful for the plant maintenance manager to fix the best maintenance schedule for improving availability of cutting system.

AbstractThe central aim of the article is company governance, i.e., researching governance of a company that does not want to be only financially successful but also direct its governance toward socially responsible governance. The article begins with the definition of “theoretical backgrounds,” in which social responsibility in regard to company governance improvement in quality is explained. The article then focuses on the measurement of the quality of company governance; in the research, the selected tool chosen to evaluate the governance of the chosen company regarding social responsibility, i.e., SEECGAN index, is used. Further, the case study of a Slovenian public limited liability company is used. One of the important research findings is the recognition that the addressed part of the SEECGAN index needs to be innovated and further developed. Additional questions for the completion of the index used presents the added value of the article. This article has two limitations: 1) it focuses only on the tool chosen to evaluate the governance of the chosen company regarding social responsibility; 2) the case study is based on publicly accessible data.

Drug services in Public Healthcare are an important component whose availability influences the success of health efforts. District / City Health Office is a regional government work unit in the health sector, one of the tasks and authorities of which is to provide and manage public medicines for the district/city area. Effective pharmaceutical management in health service facilities is very important for patient welfare so risks must be identified and controlled. One of the efforts made by the City Health Office is to procure drugs using other methods that are legal according to government procurement of goods/services besides e-purchasing, which is called non-e-purchasing of drugs. Risks in the pharmaceutical supply chain are related to product discontinuity, product shortages, poor performance, patient safety, expense errors, and technological errors, all of which can result in system disruption. This study aims to determine the suitability of the plan with the realization of e- purchasing of drugs, identify problems that hamper the implementation of e-purchasing of drugs, determine the impact of obstacles on procurement, cost efficiency of drug procurement and determine the potential savings of drug procurement by e-purchasing for the period 2015 to 2019 by using the FMEA method, the e-purchasing data was collected at the Wonokromo Public Health Center in Surabaya. Study Failure Mode Effect (FMEA) in the procurement of drugs and UHC health supplies in Surabaya Wonokromo Health Center for the period of 2015-2019, it can be concluded that the absorption of capitation funds is more considered using the availability variable compared to the uptake of funds based on the rupiah value because the availability of more guarantees the need for treatment in the sustainable health centre. In this case, the perception of procurement actors in the analysis of UHC procurement process obstacles there are 41 obstacles, the role of E-purchasing is a very helpful system even though the implementation has not been perfect.
